Serve o presente documento como registo descritivo da prossecução da componente não letiva do mestrado, por via de um estágio curricular realizado no Instituto Diplomático do Ministério dos Negócios Estrangeiros, cujo relatório aqui se apresenta. O presente relatório de estágio destina-se então não só a descrever as atividades desenvolvidas ao longo do estágio como também - após a definição dos seus conceitos estruturantes - a desenvolver a ideia de que a formação diplomática é ministrada como instrumento de diplomacia pública e não como ferramenta de soft power. Pretende-se, a propósito da I Reunião dos Diretores de Formação Diplomática no âmbito da Iniciativa 5+5, entender em que assenta esta nova parceria global euro-mediterrânica e os seus objetivos essenciais, as vantagens que poderão advir desta parceria para Portugal e especialmente para algumas unidades do Ministério dos Negócios Estrangeiro, nomeadamente o IDI, no que respeita à formação diplomática, procurando ainda perceber o papel do IDI na área da diplomacia pública.

The use of wastes and industrial by-products as building materials is an important issue in order to decrease costs with waste management and the embodied energy of building products. Scrap tire rubber has been studied as aggregate for cementitious materials. Natural hydraulic limes are natural binders with particular characteristics of both air and hydraulic binders. Their specifications became stricter with the last version of EN 459-1:2010. In this study scrap tire rubber was used as additional aggregate of mortars, based on NHL3.5 and natural sand. Different particle size fractions and proportions of scrap tire rubber were used: a mix obtained almost directly from industry (only after sieving for preparation of particle sizes similar to mortar aggregate) and separated fine, medium and coarse fractions; 0%, 18%, 36% and 54% weight of binder, corresponding to 2.5%, 5% and 7.5% weight of sand. The influence of the rubbers´ additions on the mortars´ fresh state, mechanical and physical performance is presented, namely by flow table consistency, water retention, fresh bulk density, dynamic elasticity modulus, flexural and compressive strength, open porosity and bulk density, capillary absorption, drying and thermal conductivity. The use of the rubber mix coming from the waste tire industry seems advantageous and may open possibilities for use as raw material by the mortars industry.

Interleukin 8 (CXCL8) is an autocrine chemokine specific for the chemoattraction and activation of granulocytes, NKT cells and T lymphocytes. Patients with tuberculosis and latent Mycobacterium tuberculosis infection were assessed for the spontaneous expression of CXCR1 (CD128) and CXCR2 on lymphocytes and monocytes. Compared with ex vivo profiles, increased spontaneous CXCR2 expression and normal CXCR1 expression were found on lymphocytes in two out of 59 individuals. Monocytes showed normal ex vivo profiles for both receptors. After stimulation with purified protein derivative, the in vitro levels of CXCL8 were below the median levels of all patients with prior tuberculosis. Spontaneous CXCR2 modulation did not cause notable variation in the in vitro levels of CXCL8.

We present a case of a 4.5-month-old boy from Turkey with hemophagocytic lymphohistiocytosis (HLH) associated with H1N1 virus and Leishmania spp. coinfection. Because visceral leishmaniasis can mimic hematologic disorders like HLH, it is important to rule out this clinical condition before starting immunosuppressive therapy. In our case, treatment with liposomal amphotericin B resulted in a dramatic resolution of clinical and laboratory abnormalities.

Introduction The relationship between severe clinical manifestations of visceral leishmaniasis (VL) and immune response profiles has not yet been clarified, despite numerous studies on the subject. This study aimed to investigate the relationship between cytokine profiles and the presence of immunological markers associated with clinical manifestations and, particularly, signs of severity, as defined in a protocol drafted by the Ministry of Health (Brazil). Methods We conducted a prospective, descriptive study between May 2008 and December 2009. This study was based on an assessment of all pediatric patients with VL who were observed in a reference hospital in Maranhão. Results Among 27 children, 55.5% presented with more than one sign of severity or warning sign. Patients without signs of severity or warning signs and patients with only one warning sign had the highest interferon-gamma (IFN-γ) levels, although their interleukin 10 (IL-10) levels were also elevated. In contrast, patients with the features of severe disease had the lowest IFN-γ levels. Three patients who presented with more than two signs of severe disease died; these patients had undetectable interleukin 2 (IL-2) and IFN-γ levels and low IL-10 levels, which varied between 0 and 36.8pg/mL. Conclusions Our results showed that disease severity was associated with low IFN-γ levels and elevated IL-10 levels. However, further studies with larger samples are needed to better characterize the relationship between disease severity and cytokine levels, with the aim of identifying immunological markers of active-disease severity.

Introduction. The genera Enterococcus, Staphylococcus and Streptococcus are recognized as important Gram-positive human pathogens. The aim of this study was to evaluate the performance of Vitek 2 in identifying Gram-positive cocci and their antimicrobial susceptibilities. Methods. One hundred four isolates were analyzed to determine the accuracy of the automated system for identifying the bacteria and their susceptibility to oxacillin and vancomycin. Results. The system correctly identified 77.9% and 97.1% of the isolates at the species and genus levels, respectively. Additionally, 81.8% of the Vitek 2 results agreed with the known antimicrobial susceptibility profiles. Conclusion. Vitek 2 correctly identified the commonly isolated strains; however, the limitations of the method may lead to ambiguous findings.

IntroductionPurpureocillium lilacinum is emerging as a causal agent of hyalohyphomycosis that is refractory to antifungal drugs; however, the pathogenic mechanisms underlying P. lilacinum infection are not understood. In this study, we investigated the interaction of P. lilacinum conidia with human macrophages and dendritic cells in vitro.MethodsSpores of a P. lilacinum clinical isolate were obtained by chill-heat shock. Mononuclear cells were isolated from eight healthy individuals. Monocytes were separated by cold aggregation and differentiated into macrophages by incubation for 7 to 10 days at 37°C or into dendritic cells by the addition of the cytokines human granulocyte-macrophage colony stimulating factor and interleukin-4. Conidial suspension was added to the human cells at 1:1, 2:1, and 5:1 (conidia:cells) ratios for 1h, 6h, and 24h, and the infection was evaluated by Giemsa staining and light microscopy.ResultsAfter 1h interaction, P. lilacinum conidia were internalized by human cells and after 6h contact, some conidia became inflated. After 24h interaction, the conidia produced germ tubes and hyphae, leading to the disruption of macrophage and dendritic cell membranes. The infection rate analyzed after 6h incubation of P. lilacinumconidia with cells at 2:1 and 1:1 ratios was 76.5% and 25.5%, respectively, for macrophages and 54.3% and 19.5%, respectively, for cultured dendritic cells.ConclusionsP. lilacinum conidia are capable of infecting and destroying both macrophages and dendritic cells, clearly demonstrating the ability of this pathogenic fungus to invade human phagocytic cells.

ABSTRACTINTRODUCTION:While no single factor is sufficient to guarantee the success of influenza vaccine programs, knowledge of the levels of immunity in local populations is critical. Here, we analyzed influenza immunity in a population from Southern Brazil, a region with weather conditions that are distinct from those in the rest of country, where influenza infections are endemic, and where greater than 50% of the population is vaccinated annually.METHODS:Peripheral blood mononuclear cells were isolated from 40 individuals. Of these, 20 had received the H1N1 vaccine, while the remaining 20 were unvaccinated against the disease. Cells were stimulated in vitro with the trivalent post-pandemic influenza vaccine or with conserved major histocompatibility complex I (MHC I) peptides derived from hemagglutinin and neuraminidase. Cell viability was then analyzed by [3-(4,5-dimethylthiazol-2-yl)-2,5- diphenyltetrazolium bromide)]-based colorimetric assay (MTT), and culture supernatants were assayed for helper T type 1 (Th1) and Th2-specific cytokine levels.RESULTS:Peripheral blood lymphocytes from vaccinated, but not unvaccinated, individuals exhibited significant proliferation in vitro in the presence of a cognate influenza antigen. After culturing with vaccine antigens, cells from vaccinated individuals produced similar levels of interleukin (IL)-10 and interferon (IFN)-γ, while those from unvaccinated individuals produced higher levels of IFN-γ than of IL-10.CONCLUSIONS:Our data indicate that peripheral blood lymphocytes from vaccinated individuals are stimulated upon encountering a cognate antigen, but did not support the hypothesis that cross-reactive responses related to previous infections can ameliorate the immune response. Moreover, monitoring IL-10 production in vaccinated individuals could comprise a valuable tool for predicting disease evolution.

Foreign body aspiration (FBA) is one of leading causes of death in children, especially among those younger than 3 years of age. The inhalation of a foreign body may cause a wide variety of symptoms, and early diagnosis is highly associated with the successful removal of the inhaled foreign material. Despite the great advances in endoscopic procedures and anesthesia, a large number of difficulties and complications still result from foreign body aspiration. We describe 5 cases of serious acute complications following aspiration of foreign bodies that became lodged in the tracheobronchial tree, including pneumomediastinum, pneumothorax, total atelectasis, foreign body dislodgment, and need for thoracotomy in children admitted into our intensive care unit in 1999 and 2000; these were all situations that could have been prevented with early recognition and prompt therapeutic intervention.

A presente dissertação enquadra-se num estudo que esteve inserido nos projetos FCT-METACAL e FCT-LIMECONTECH, e cujos resultados foram já parcialmente publicados por GRILO et al. (2014a e 2014b). Nesse estudo produziram-se e caraterizaram-se, aos 28, 90 e 180 dias, argamassas de cal hidráulica natural, NHL3.5 produzida em Portugal, com substituições percentuais de ligante por metacaulino (Mk). Outro fator estudado foi a influência do tipo de cura, estando as amostras das argamassas sujeitas a 3 tipos de cura diferentes: standard, marítima e húmida. Aos 180 dias de cura, as argamassas com 10% de substituição de Mk apresentaram melhorias nas resistências mecânicas e melhorias evidentes no comportamento em relação à absorção de água e secagem. No geral as características das argamassas indiciavam a sua adequação para a conservação de fachadas de edifícios antigos e aplicação na reabilitação de edifícios com suportes mais resistentes ou mesmo em construção nova. No presente estudo foram caracterizadas as mesmas argamassas, desta feita com 3 anos de idade, e avaliada a evolução das suas caraterísticas até esta idade em cura marítima e húmida. Ao contrário do previsto, as resistências mecânicas das argamassas com substituição de metacaulino tiveram reduções nos seus valores, chegando, em alguns casos, a ficar abaixo dos valores das argamassas sem Mk. Pela caracterização química e mineralógica constata-se que essa redução foi motivada pela instabilidade dos compostos pozolânicos formados pelo Mk, embora esses compostos tenham sido os responsáveis pelas elevadas resistências mecânicas registadas a idades jovens. As argamassas com Mk apresentaram um melhor comportamento à água, devido ao refinamento da sua estrutura porosa. Dos ensaios realizados apenas o índice de secagem e o ensaio de resistência aos sais revelaram resultados piores nas argamassas com Mk, quando em comparação com as argamassas de referência, só com NHL. Em relação ao tipo de cura, verificou-se não existirem grandes diferenças entre a cura húmida e a marítima.

The aim of this report is to describe the anatomic-pathologic findings from necropsies of 5 drug addicts with titanium pigment in several organs after chronic intravenous injection of crushed propoxyphene hydrochloride tablets. Samples from liver, spleen, lungs, lymph nodes, and bone marrow were obtained, and after being grossly studied, they were submitted to evaluation using common light and polarized microscopy. In all 5 cases, a pigment with characteristics of titanium dioxide was found within tissue samples of the organs studied. Our findings suggest that research concerning titanium pigment within body tissues should be enhanced, considering the potential contribution of this morphologic data to forensic pathology.

Introdução: Os países de clima subtropical, tropical e húmido, onde as populações têm acesso limitado a água potável, saneamento básico efectivo, educação e cuidados de saúde, apresentam as condições favoráveis para o desenvolvimento de doenças transmissíveis, nomeadamente as parasitoses intestinais, cuja prevalência é elevada. A infecção por parasitas intestinais patogénicos pode provocar diversas alterações patológicas tais como anemia, desnutrição e outras patologias orgânicas, afectando principalmente as crianças. Objectivos: Avaliar a prevalência das parasitoses intestinais e determinar a sua relação com o estado nutricional, em crianças dos 0 aos 5 anos de São Tomé e Príncipe. Material e Métodos: Estudo observacional, transversal e analítico, com colheita de dados antropométricos e de fezes, entre Fevereiro e Março de 2011, nos distritos de Água Grande, Mé-Zochi e Lobata, em São Tomé e Príncipe, numa amostra de 390 crianças. A detecção microscópica dos parasitas intestinais foi executada no Instituto de Higiene e Medicina Tropical. O estado nutricional de cada criança foi avaliado através dos z-scores do peso para a idade (PIZ), da estatura para a idade (EIZ), do peso para altura (PEZ) e do IMC para a idade (IMCZ). A relação entre a infecção por parasitas intestinais e a desnutrição foi analisada através de métodos estatísticos. Resultados: A amostra foi constituída maioritariamente por crianças com idade inferior a 24 meses (58%). Foram detectados parasitas intestinais patogénicos em 38,7% das crianças, tendo-se observado uma probabilidade mais alta de infecção em crianças com idade superior a 24 meses. Destacaram-se as infecções simples ou mistas por G. lamblia (27,4%), A. lumbricoides (12,8%), T. trichiura (2,6%), Hymenolepis nana (1%), E. histolytica (0,5%), ancilostomideos (0,5%), Schistosoma intercalatum (0,3%) e Taenia spp. (0,3%). No que se refere à desnutrição, 35,6% de crianças tinham baixo peso, 33,6% apresentavam desnutrição crónica, 27,7% e 27,4% desnutrição aguda (PEZ e IMCZ), destacando-se o grau ligeiro em cada uma. Verificou-se uma associação estatisticamente significativa entre a infecção por parasitas intestinais patogénicos e a desnutrição crónica. Discussão e Conclusão: Este estudo contribuiu para um melhor conhecimento das prevalências e relação das parasitoses intestinais patogénicas com a desnutrição em crianças menores de 5 anos em São Tomé e Príncipe, nomeadamente da associação entre desnutrição crónica e a infecção por parasitas intestinais patogénicos. Sugere-se a execução de estudos semelhantes a este nas restantes regiões do país para fundamentar a adopção de medidas adequadas de combate às infecções por parasitas intestinais patogénicos. Considerando o número de crianças infectadas com G. lamblia encontrada neste estudo, seria recomendável medidas específicas de combate à transmissão de protozoários e a realização de exames anuais das fezes de todas as crianças seguida de tratamento dos casos positivos.

The aim of this work was to study the self-assembly process of C3-symmetric molecules. To accomplish this objective 1,3,5 – benzentricarboxamides (BTA) derivatives were obtained. Five C3-symmetric molecules were synthesized in moderate to good yields (39-72%) using azo-benzene, aniline, benzylamine, tryptophan and tyrosine. The aggregation behavior of the BTA derivatives was probed with 1H-NMR spectroscopy, 1H-1H 2D Nuclear Overhauser Effect Spectroscopy (NOESY) and Diffusion Ordered Spectroscopy (DOSY). These experiments allowed to study the influence of H-bonding groups, aromatic rings, unsaturated bonds and the overall geometry in the molecular self-assembly associated with the different structural patterns present on these molecules. The stacking and large molecule behavior where observed in BTA 1, aniline derivative, BTA 4, tyrosine derivative or BTA 5, tryptophan derivative, with several of those discussed functional groups such as unsaturated bonds and H-bonding groups. BTA 5 was used in a few preliminary interaction studies with glucose and ammonium chloride showing interaction with the ammonium ion.
