In the field of regenerative medicine, nanoscale physical cuing is clearly becoming a compelling determinant of cell behavior. Developing effective methods for making nanostructured surfaces with well-defined physicochemical properties is thus mandatory for the rational design of functional biomaterials. Here, we demonstrate the versatility of simple chemical oxidative patterning to create unique nanotopographical surfaces that influence the behavior of various cell types, modulate the expression of key determinants of cell activity, and offer the potential of harnessing the power of stem cells. These findings promise to lead to a new generation of improved metal implants with intelligent surfaces that can control biological response at the site of healing.

This paper presents the multi-threading and internet message communication capabilities of Qu-Prolog. Message addresses are symbolic and the communications package provides high-level support that completely hides details of IP addresses and port numbers as well as the underlying TCP/IP transport layer. The combination of the multi-threads and the high level inter-thread message communications provide simple, powerful support for implementing internet distributed intelligent applications.

Design of liquid retaining structures involves many decisions to be made by the designer based on rules of thumb, heuristics, judgment, code of practice and previous experience. Various design parameters to be chosen include configuration, material, loading, etc. A novice engineer may face many difficulties in the design process. Recent developments in artificial intelligence and emerging field of knowledge-based system (KBS) have made widespread applications in different fields. However, no attempt has been made to apply this intelligent system to the design of liquid retaining structures. The objective of this study is, thus, to develop a KBS that has the ability to assist engineers in the preliminary design of liquid retaining structures. Moreover, it can provide expert advice to the user in selection of design criteria, design parameters and optimum configuration based on minimum cost. The development of a prototype KBS for the design of liquid retaining structures (LIQUID), using blackboard architecture with hybrid knowledge representation techniques including production rule system and object-oriented approach, is presented in this paper. Background Diastolic dysfunction induced by ischemia may alter transmitral blood flow, but this reflects global ventricular function, and pseudonormalization may occur with increased preload. Tissue Doppler may assess regional diastolic function and is relatively load-independent, but limited data exist regarding its application to stress testing. We sought to examine the stress response of regional diastolic parameters to dobutomine echocardiography (DbE). Methods Sixty-three patients underwent study with DbE: 20 with low probability of coronary artery disease (CAD) and 43 with CAD who underwent angiography. A standard DbE protocol was used, and segments were categorized as ischemic, scar, or normal. Color tissue Doppler was acquired at baseline and peak stress, and waveforms in the basal and mid segments were used to measure early filling (Em), late filling (Am), and E deceleration time. Significant CAD was defined by stenoses >50% vessel diameter. Results Diastolic parameters had limited feasibility because of merging of Em and Am waves at high heart rates and limited reproducibility. Nonetheless, compared with normal segments, segments subtended with significant stenoses showed a lower Em velocity at rest (6.2 +/- 2.6 cm/s vs 4.8 +/- 2.2 cm/s, P < .0001) and peak (7.5 +/- 4.2 cm/s vs 5.1 +/- 3.6 cm/s, P < .0001), Abnormal segments also showed a shorter E deceleration time (51 +/- 27 ms vs 41 +/- 27 ms, P = .0001) at base and peak. No changes were documented in Am. The same pattern was seen with segments identified as ischemic with wall motion score. However, in the absence of ischemia, segments of patients with left ventricular hypertrophy showed a lower Em velocity, with blunted Em responses to stress. Conclusion Regional diastolic function is sensitive to ischemia. However, a number of practical limitations limit the applicability of diastolic parameters for the quantification of stress echocardiography.

OBJECTIVE - This study sought to determine whether stress echocardiography using exercise (when feasible) or dobutamine echo could be used to predict mortality in patients with diabetes. RESEARCH DESIGN AND METHODS - Stress echo was performed in 937 patients with diabetes (aged 59 +/- 13 years, 529 men) for symptom evaluation (42%) and follow-up of known coronary artery disease (CAD) (58%). Stress echocardiography using exercise was performed in 333 patients able to exercise maximally, and dobutamine echo using a standard dobutamine stress was used in 604 patients. Patients were followed for less than or equal to9 years (mean 3.9 +/- 2.3) for all-cause mortality. RESULTS - Normal studies were obtained in 567 (60%) patients; 29% had resting left ventricular (LV) dysfunction, and 25% had ischemia. Abnormalities were confined to one territory in 183 (20%) patients and to multiple territories in 187 (20%) patients. Death (in 275 [29%] patients) was predicted by referral for pharmacologic stress (hazard ratio [HR] 3.94, P < 0.0001), ischemia (1.77, P <0.0001), age (1.02, P = 0.002), and heart failure (1.54, P = 0.01). The risk of death in patients With a normal scan was 4% per year, and this was associated with age and selection for pharmacologic stress testing. In stepwise models replicating the sequence of clinical evaluation, the predictive power of independent clinical predictors (age, selection for pharmacologic stress, previous infarction, and heart failure; model chi(2) = 104.8) was significantly enhanced by addition of stress echo data (model chi(2) = 122.9). CONCLUSIONS - The results of stress echo are independent predictors of death in diabetic patients with known or suspected CAD.. Ischemia adds risk that is incremental to clinical risks and LV dysfunction.

Quantification of stress echocardiography may overcome the training requirements and subjective nature of visual wall motion score (WMS) assessment, but quantitative approaches may be difficult to apply and require significant time for image processing. The integral of long-axis myocardial velocity is displacement, which may be represented as a color map over the left ventricular myocardium. This study was designed to explore the feasibility and accuracy of measuring long-axis myocardial displacement, derived from tissue Doppler, for the detection of coronary artery disease (CAD) during dobutamine stress echocardiography (DBE). One hundred thirty patients underwent standard DBE, including 30 patients at low risk of CAD, 30 patients with normal coronary angiography (both groups studied to define normal ranges of displacement), and 70 patients who underwent coronary angiography in whom the accuracy of normal ranges was tested. Regional myocardial displacement was obtained by analysis of color tissue Doppler apical images acquired at peak stress. Displacement was compared with WMS, and with the presence of CAD by angiography. The analysis time was 3.2 +/- 1.5 minutes per patient. Segmental displacement was correlated with wall motion (normal 7.4 +/- 3.2 mm, ischemia 5.8 +/- 4.2 mm, viability 4.6 +/- 3.0 mm, scar 4.5 +/- 3.5 mm, p <0.001). Reversal of normal base-apex displacement was an insensitive (19%) but specific (90%) marker of CAD. The sum of displacements within each vascular territory had a sensitivity and specificity of 89% and 79%, respectively, for prediction of significant CAD, compared with 86% and 78%, respectively, for WMS (p = NS). The displacements in the basal segments had a sensitivity and specificity of 83% and 78%, respectively (p = NS). OBJECTIVES The goal of this study was to determine whether the cardiostimulant effects of the endogenous beta(1)-adrenergic receptor (AR) agonist, (-)-norepinephrine are modified by polymorphic (Serine49Glycine [Ser49Gly], Glycine389Arginine [Gly389Arg]) variants of beta(1)-ARs in the nonfailing adult human heart. BACKGROUND Human heart beta(1)-ARs perform a crucial role in mediating the cardiostimulant effects of (-)-norepinephrine. An understanding of the significance of Ser49Gly and Gly389Arg polymorphisms in the human heart is beginning to emerge, but not as yet in adult patients who have coronary artery disease (CAD). METHODS The potency and maximal effects of (-)-norepinephrine at beta(1)-ARs (in the presence of beta(2)-AR blockade with 50 nM ICI 118,551 [erythro-DL-1(7-methylindan-4-yloxy)-3-isopropylamino-butan-2-ol]) for changes in contractile force and shortening of contractile cycle duration were determined in human right atrium in vitro from 87 patients undergoing coronary artery bypass grafting who were taking beta-blockers before surgery. A smaller sample of patients (n = 20) not taking beta-blockers was also investigated. Genotyping for two beta(1)-AR polymorphisms (Ser49Gly and Gly389Arg) was determined from a sample of blood taken at the time of surgery. RESULTS (-)-Norepinephrine caused concentration-dependent increases in contractile force and reductions in time to reach peak force and time to reach 50% relaxation. There were no differences in the potency or maximal effects of (-)-norepinephrine in the right atrium from patients with different Ser49Gly and Gly389Arg polymorphisms. CONCLUSIONS The cardiostimulant effects of (-)-norepinephrine at beta(1)-ARs were conserved across Ser49Gly and Gly389Arg polymorphisms in the right atrium of nonfailing hearts from patients with CAD managed with or without beta-blockers. Background. Regional left ventricular (LV) dysfunction may occur in patients with coronary artery disease (CAD) in the absence of infarction, but the causes of this phenomenon are unclear. We sought to identify whether changes in regional LV function were related to stenosis severity, using sensitive new ultrasound markers of function. Methods: We studied 67 individuals with no history of infarction and with normal LV systolic function: 49 patients with CAD and 18 control subjects without CAD. All patients underwent color Doppler tissue imaging, integrated backscatter (IB), anatomic M-mode echocardiography, and strain rate imaging to detect changes in structure and function. Peak early and late diastolic myocardial velocity, cyclic variation of IB, wall thickness, and percent wall thickening were measured in each basal and mid segment. Strain rate and peak systolic strain were calculated in each wall. CAD was defined as greater than or equal to 50% diameter stenosis. Normokinetic segments (n = 354) subtended by CAD were divided according to stenosis severity into 3 groups: group 1 (subtended by 50%-69% stenosis); group 2 (subtended by 70%-98% stenosis); and group 3 (subtended by greater than or equal to99% stenosis). Each parameter in each group was compared with that in 216 segments from control subjects. Results: Segments subtended by significant CAD showed lower peak early and late diastolic myocardial velocity compared with control segments. Background Exercise testing has limited efficacy for identifying coronary artery disease (CAD) in the absence of anginal. symptoms. Exercise echocardiography is more accurate than standard exercise testing, but its efficacy in this situation has not been defined. We sought to identify whether the Duke treadmill. score or exercise echocardiography (ExE) could be used to identify risk in patients without anginal symptoms. Methods We studied 1859 patients without typical or atypical angina, heart failure, or a history or ECG evidence of infarction or CAD, who were referred for ExE, of whom 1832 (age 51 15 years, 944 men) were followed for up to 10 years. The presence and extent of ischaemia and scar were interpreted by expert reviewers at the time of the original study. Results Exercise provoked significant (>0.1 mV) ST segment depression in 215 patients (12%), and wall motion abnormalities in 137 (8%). Recentemente, a expans??o do n??mero de munic??pios conveniados com a FNS fez aumentar a demanda por novos projetos de melhoria dos sistemas de ??gua e esgotos. Ao mesmo tempo, a FNS enfrentava car??ncia de pessoal t??cnico especializado, dificuldades de admiss??o de novos profissionais e limita????o de recursos para a contrata????o de projetos atrav??s de empresas privadas, inviabilizando o atendimento das demandas pela forma tradicional. Diante disso, servidores da FNS desenvolveram um software para projetos de redes de esgotos sanit??rios, utilizando os recursos modernos da computa????o gr??fica (CAD ??? Computer Aided Design). Acoplado ao CAD, prop??s-se a cria????o de um m??dulo de dimensionamento, resultando numa ferramenta completa de elabora????o de projetos, em ambiente computacional Windows, contemplando as v??rias etapas t??picas de um trabalho na ??rea, desde a digitaliza????o do levantamento topogr??fico, lan??amento da rede, c??lculos, planilhas, desenho e montagem final das pranchas geradas. Este sistema foi denominado Sancad. A utiliza????o do Sancad permitiu uma redu????o m??dia de 60 a 70% no tempo de elabora????o de projetos, significando uma forma de atender a um n??mero maior de demandas e de suprir a car??ncia de profissionais na FNS. Al??m disso, a utiliza????o do Sancad tem mostrado ganhos em termos de qualidade, corre????o e confiabilidade do produto final obtido. As previs??es iniciais quanto ?? abrang??ncia de utiliza????o do Sancad foram amplamente superadas, tendo o mesmo se tornado um padr??o para a ??rea de projetos de saneamento da FNS, estando instalado em 12 unidades da Funda????o em todo o Brasil e em processo de instala????o em outras. O Sancad tornou-se objeto de interesse de t??cnicos de saneamento e de estudantes de engenharia e arquitetura de institui????es p??blicas e privadas, que receberam treinamento a fim de conhec??-lo e utiliz??-lo. Finalmente, o sucesso da experi??ncia tem contribu??do para a dissemina????o da cultura de computa????o gr??fica e a ades??o dos profissionais da FNS e das autarquias municipais conveniadas ?? nova tecnologia. Neste mesmo sentido, ressalta-se a utiliza????o de teodolitos eletr??nicos ??? conhecidos como Esta????o Total ??? para a gera????o de plantas topogr??ficas j?? em formato digital, para serem usadas pelo Sancad, com novos ganhos de qualidade e produtividade na elabora????o dos projetos de saneamento

Este artigo busca revelar o processo de desenvolvimento da categoria dos auditores fiscais da Receita Federal do Brasil (AFRFBs), a partir das mudanças no contexto social e profissional, e a forma como essas mutações concorreram para a construção de uma identidade profissional própria desses servidores. O que se pretende é entender como o contexto político-econômico vem alterando as percepções que esses profissionais têm de si próprios e como as reformas transformaram o modo de exercerem suas funções. A pesquisa, que se dá a partir de uma proposta de transação "quanti-quali", permitiu explorar, na trilha teórica de Dubar, aspectos relevantes do contínuo processo de construção das identidades profissionais ou de perfis identitários desses servidores. Os conhecimentos sobre o trabalho e as formas de identificação profissional na categoria dos auditores fiscais da Receita Federal do Brasil podem contribuir para a reflexão sobre relações de trabalho e processos de gestão pública no Brasil.

Os mecanismos responsáveis pelo desenvolvimento dos seios paranasais ainda são pouco conhecidos, o fluxo aéreo nasal segundo uma das teorias propostas seria fundamental ao crescimento e desenvolvimento saudável dos seios paranasais. OBJETIVO: Estudar comparativamente o desenvolvimento dos seios maxilares e a presença de sinusopatia, em um mesmo modelo, na ausência e presença de fluxo aéreo nasal posterior. MATERIAL E MÉTODOS: Estudo retrospectivo transversal de uma série de casos; os exames tomográficos pré-operatórios de 7 pacientes com atresia coanal unilateral, idade média 16.28 anos (± 5,024). Este estudo realizado em um hospital terciário, com pacientes que passaram neste serviço entre os anos de 1994 e 2004. A área dos seios maxilares foi medida com auxílio do programa Auto-Cad. Utilizou-se o teste de Kruskal-Wallis para análise estatística. RESULTADOS: Nesse estudo observou-se seios maxilares simétricos e até mesmo maiores no lado da atresia coanal em 85,71% dos casos, não houve diferença estatística significativa entre os dois lados comparados. Não se observou sinais de tomográficos de sinusopatia neste grupo de pacientes. CONCLUSÃO: Os achados aqui apresentados contrariam a teoria difusamente aceita sobre o papel do fluxo aéreo nasal na saúde e no desenvolvimento das cavidades paranasais.

Cognitive impaired population face with innumerable problems in their daily life. Surprisingly, they are not provided with any help to perform those tasks for which they have difficulties. As a consequence, it is necessary to develop systems that allow those people to live independently and autonomously. Living in a technological era, people could take advantage of the available technology, being provided with some solutions to their needs. This paper presents a platform that assists users with remembering where their possessions are. Mainly, an object recognition process together with an intelligent scheduling applications are integrated in an Ambient Assisted Living (AAL) environment.
