The prevalence of overweight and obesity has increased with alarming speed over the past twenty years. It has recently been described by the World Health Organisation as a ‘global epidemic’. In the year 2000 more than 300 million people worldwide were obese and it is now projected that by 2025 up to half the population of the United States will be obese if current trends are maintained. The disease is now a major public health problem throughout Europe. In Ireland at the present time 39% of adults are overweight and 18% are obese. Of these, slightly more men than women are obese and there is a higher incidence of the disease in lower socio-economic groups. Most worrying of all is the fact that childhood obesity has reached epidemic proportions in Europe, with body weight now the most prevalent childhood disease. While currently there are no agreed criteria or standards for assessing Irish children for obesity some studies are indicating that the numbers of children who are significantly overweight have trebled over the past decade. Extrapolation from authoritative UK data suggests that these numbers could now amount to more than 300,000 overweight and obese children on the island of Ireland and they are probably rising at a rate of over 10,000 per year. A balance of food intake and physical activity is necessary for a healthy weight. The foods we individually consume and our participation in physical activity are the result of a complex supply and production system. The growing research evidence that energy dense foods promote obesity is impressive and convincing. These are the foods that are high in fat, sugar and starch. Of these potentially the most significant promoter of weight gain is fat and foods from the top shelf of the food pyramid including spreads (butter and margarine), cakes and biscuits, and confectionery, when combined are the greatest contributors to fat intake in the Irish diet. In company with their adult counterparts Irish children are also consuming large amounts of energy dense foods outside the home. A recent survey revealed that slightly over half of these children ate sweets at least once a day and roughly a third of them had fizzy drinks and crisps with the same regularity. Sugar sweetened carbonated drinks are thought to contribute to obesity and for this reason the World Health Organisation has expressed serious concerns at the high and increasing consumption of these drinks by children. Physical activity is an important determinant of body weight. Over recent decades there has been a marked decline in demanding physical work and this has been accompanied by more sedentary lifestyles generally and reduced leisure-time activity. These observable changes, which are supported by data from most European countries and the United States, suggest that physical inactivity has made a significant impact on the increase in overweight and obesity being seen today. It is now widely accepted that adults shoud be involved in 45-60 minutes, and children should be involved in at least 60 minutes per day of moderate physical activity in order to prevent excess weight gain. Being overweight today not only signals increased risk of medical problems but also exposes people to serious psychosocial problems due mainly to widespread prejudice against fat people. Prejudice against obese people seems to border on the socially acceptable in Ireland. It crops up consistently in surveys covering groups such as employers, teachers, medical and healthcare personnel, and the media. It occurs among adolescents and children, even very young children. Because obesity is associated with premature death, excessive morbidity and serious psychosocial problems the damage it causes to the welfare of citizens is extremely serious and for this reason government intervention is necessary and warranted. In economic terms, a figure of approximately â,¬30million has been estimated for in-patient costs alone in 2003 for a number of Irish hospitals. This year about 2,000 premature deaths in Ireland will be attributed to obesity and the numbers are growing relentlessly. Diseases which proportionally more obese people suffer from than the general population include hypertension, type 2 diabetes, angina, heart attack and osteoarthritis. There are indirect costs also such as days lost to the workplace due to illness arising from obesity and output foregone as a result of premature death. Using the accepted EU environmental cost benefit method, these deaths alone may be costing the state as much as â,¬4bn per year. The social determinants of physical activity include factors such as socio-economic status, education level, gender, family and peer group influences as well as individual perceptions of the benefits of physical activity. The environmental determinants include geographic location, time of year, and proximity of facilities such as open spaces, parks and safe recreational areas generally. The environmental factors have not yet been as well studied as the social ones and this research gap needs to be addressed. Clearly there is a public health imperative to ensure that relevant environmental policies maximise opportunities for active transport, recreational physical activity and total physical activity. It is clear that concerted policy initiatives must be put in place if the predominantly negative findings of research regarding the determinants of food consumption and physical activity are to be accepted, and they must surely be accepted by government if the rapid increase in the incidence of obesity with all its negative consequences for citizens is to be reversed. So far actions surrounding nutrition policies have concentrated mostly on actions that are within the remit of the Department of Health and Children such as implementing the dietary guidelines. These are important but government must now look at the totality of policies that influence the type and supply of food that its citizens eat and the range and quality of opportunities that are available to citizens to engage in physical activity. This implies a fundamental examination of existing agricultural, industrial, economic and other policies and a determination to change them if they do not enable people to eat healthily and partake in physical activity. The current crisis in obesity prevalence requires a population health approach for adults and children in addition to effective weight-reduction management for individuals who are severely overweight. This entails addressing the obesogenic environment where people live, creating conditions over time which lead to healthier eating and more active living, and protecting people from the widespread availability of unhealthy food and beverage options in addition to sedentary activities that take up all of their leisure time. People of course have a fundamental right to choose to eat what they want and to be as active as they wish. That is not the issue. What the National Taskforce on Obesity has had to take account of is that many forces are actively impeding change for those well aware of the potential health and well-being consequences to themselves of overweight and obesity. The Taskforce’s social change strategy is to give people meaningful choice. Choice, or the capacity to change (because the strategy is all about change), is facilitated through the development of personal skills and preferences, through supportive and participative environments at work, at school and in the local community, and through a dedicated and clearly communicated public health strategy. High-level cabinet support will be necessary to implement the Taskforce’s recommendations. The approach to implementation must be characterised by joined-up thinking, real practical engagement by the public and private sectors, the avoidance of duplication of effort or crosspurpose approaches, and the harnessing of existing strategies and agencies. The range of government departments with roles to play is considerable. The Taskforce outlines the different contributions that each relevant department can make in driving its strategy forward. It also emphasises its requirement that all phases of the national strategy for healthy eating and physical activity are closely monitored, analysed and evaluated. The vision of the Taskforce is expressed as: An Irish society that enables people through health promotion, prevention and care to achieve and maintain healthy eating and active living throughout their lifespan. Its high-level goals are expressed as follows: Its recommendations, over eighty in all, relate to actions across six broad sectors: high-level government; education; social and community; health; food, commodities, production and supply; and the physical environment. In developing its recommendations the Taskforce has taken account of the complex, multisectoral and multi-faceted determinants of diet and physical activity. This strategy poses challenges for government, within individual departments, inter-departmentally and in developing partnerships with the commercial sector. Equally it challenges the commercial sector to work in partnership with government. The framework required for such initiative has at its core the rights and benefits of the individual. Health promotion is fundamentally about empowerment, whether at the individual, the community or the policy level.

This Study was jointly commissioned by the Department of Health and Children in Ireland and the Department of Health, Social Services and Public Safety in Northern Ireland. The main purpose of the Study is to develop a strategic framework for taking forward on a North-South basis future collaborative work in health and social care and in planning and delivering health and social care services, as may be appropriate. The Study sets out examples of current North-South cooperation and Identifies a number of potential areas within which further collaborative work may benefit people living in each jurisdiction. A participative approach was adopted in conducting this work and the outcome represents the collective views of key professionals and managers in both healthcare systems. While North-South collaboration on health related issues is not new, what this Study provides is a structured approach to its ongoing development, with clearly identified areas for further cooperation and explicit roles and responsibilities for Departments and Agencies in both jurisdictions.   Click here to download PDF 213KB

Le monitoring de la problématique du cannabis en Suisse constitue un ensemble de travaux qui permettent le suivi de la situation au niveau national et qui sont mis en oeuvre par un consortium d'institutions de recherche. Ce monitoring comprend l'étude présentée dans ce rapport, l'étude sentinelle. Celle-ci s'intéresse à l'évolution de la situation en matière de cannabis ainsi qu'à la gestion de cette situation au niveau local. Il s'agit de répondre aux questions suivantes : - quelle est la situation en matière de consommation de cannabis et de marché et quelle est son évolution ? - quels sont les principaux problèmes rencontrés sur le terrain ? - quelles sont les mesures et interventions qui ont été développées dans ce domaine ? Pour y répondre, on a choisi de suivre la situation dans quatre cantons suisses dits "sentinelle" (St-Gall, Tessin, Vaud, Zurich). Les critères de choix de ces cantons font appel à leur taille, au rapport ville/campagne et à la présence de frontière avec des états voisins, à la langue, au type de politique drogue pratiqué. Dans chaque canton on a constitué des panels d'experts formés par des profes-sionnels de terrain dans trois domaines différents (santé et social, école, police et justice). Leurs observations ainsi que les données cantonales disponibles sont récoltées et discutées lors d'un workshop et analysées sur plusieurs années. Le présent rapport fait état des résultats des quatre workshops de suivi (2005, 2006, 2008, 2009). [Résumé, p. 5]

Recently, the booming rural tourism in endemic areas of the state of Minas Gerais was identified as a contributing factor in the dissemination of the infection with Schistosoma mansoni. This article presents data from six holiday resorts in a rural district approximately 100 km distant from Belo Horizonte, MG, Brazil, where a possibly new and until now unperceived way of transmission was observed. The infection takes place in swimming pools and little ponds, which are offered to tourists and the local population for fishing and leisure activities. The health authorities of the district reported cases of schistosomiasis among the local population after visiting these sites. As individuals of the non-immune middle class parts of the society of big urban centers also frequent these resorts, infection of these persons cannot be excluded. A malacological survey revealed the presence of molluscs of the species Biomphalaria glabrata and Biomphalaria straminea at the resorts. The snails (B. glabrata) of one resort tested positive for S. mansoni. In order to resolve this complex problem a multidisciplinary approach including health education, sanitation measures, assistance to the local health services, and evolvement of the local political authorities, the local community, the tourism association, and the owners of the leisure resorts is necessary. This evidence emphasizes the urgent need for a participative strategic plan to develop the local tourism in an organized and well-administered way. Only so this important source of income for the region can be ensured on the long term without disseminating the disease and putting the health of the visitors at risk.

Cet article fait le point sur les réformes de la gestion publique en Suisse. Après avoir montré que ce pays est marqué par une tradition de modernisation qui le rend très réceptif aux nouvelles approches de gestion, l'article analyse en particulier le mouvement de Nouvelle Gestion Publique (NGP) à la mode helvétique. Les principales orientations de la NGP suisse, ainsi que les évaluations qui en ont été faites sous l'angle des autorités politiques, de l'administration et des citoyens, permettent d'esquisser les défis qui attendent le système suisse dans l'ère de l'après-NGP.

Les tumeurs malignes des cavités naso-sinusiennes (CNS) représentent environ 3% des cancers ORL. La poussière de bois est reconnue comme cancérigène pour l'homme (IARC, classe 1A) et la maladie professionnelle formellement identifiée est le CNS ou le cancer de l'ethmoïde. L'adénocarcinome des CNS est une maladie professionnelle admise chez certains travailleurs spécialisés tels les menuisiers et les ébénistes. Selon les enquêtes WOODEX dans les 25 états membres de UE en 2006, environ 3.6 millions de travailleurs sont exposés par voie respiratoire à la poussière de bois, soit 2% de la population active. En France, 307 000 travailleurs sont exposés, et il y a environ 200 cas de cancers sino-nasaux reconnus comme maladie professionnelle par année. La forte proportion de travailleurs du bois développant un adénocarcinome des CNS a suggéré diverses pistes, tel que le tanin pour les bois durs, le formaldéhyde pour les contre-plaqués et le benzo(a)pyrène produit par le bois surchauffé. Il est reconnu que le tanin ne provoque pas de cancer dans l'exposition à la poussière de thé. Le formaldéhyde est un irritant mais il est aussi classé cancérogène. La piste des hydrocarbures aromatiques polycycliques (HAP) cancérogènes provoqués par le bois surchauffé est séduisante. Nous avons étudié leur teneur en fonction des opérations pratiquées sur le bois en cabine d'expérimentation. Les matériaux testés sont du sapin brut, du chêne et chêne imprégné de polyuréthane. La poussière de bois contient des HAP au niveau du µg/g ou ppm. Le bois imprégné de vernis PU produit 100 fois plus de HAP que le bois brut lors des opérations de ponçage.

Le bassin du Rhône à l'amont du Léman peut être sujet à de fortes précipitations en mesure de provoquer des crues significatives. L'objectif du projet MINERVE dans lequel s'inscrit le présent travail consiste à fournir des outils pour la prévision et la gestion des crues par des actions préventives sur les aménagements hydroélectriques à accumulation. Pour satisfaire ce dernier, il est nécessaire de prévoir au mieux les cumuls de précipitations pour les jours suivants. Ceci est actuellement effectué par le modèle numérique de prévision de MétéoSuisse ; mais, en raison des grandes incertitudes liées à la quantification des événements extrêmes, il a été décidé qu'une approche parallèle de nature statistique pourrait compléter l'information disponible. Ainsi, nous avons adapté la méthode des analogues, qui est une technique de prévision statistique des précipitations, au contexte alpin du bassin d'étude. Pour ce faire, plusieurs paramétrisations de la méthode ont été documentées et calibrées. Afin de prendre en main la méthode, nous avons effectué de multiples analyses paramétriques sur les variables synoptiques, mais également sur la constitution de groupements pluviométriques. Une partie conséquente de cette étude a été consacrée à la programmation d'un logiciel de prévision automatique par la méthode des analogues, ainsi qu'à un outil de visualisation des résultats sous forme de cartes et graphiques. Ce logiciel, nommé Atmoswing, permet d'implémenter un grand nombre de méthodes différentes de prévision par analogie. L'outil est opérationnel depuis mi-2011 et nous a permis de confirmer l'intérêt de la prévision par analogie. La méthode étant ici appliquée à un nouveau contexte, un grand nombre de variables synoptiques ont été évaluées. Nous avons alors confirmé l'intérêt des deux niveaux d'analogie sur la circulation atmosphérique et sur le flux d'humidité, tout en apportant des améliorations à celles-ci. Il en résulte des paramétrisations présentant des scores de performance supérieurs aux méthodes de référence considérées. Nous avons également évalué d'autres améliorations, comme l'introduction d'une fenêtre temporelle glissante afin de rechercher de meilleures analogies synoptiques à d'autres heures de la journée, ce qui s'est avéré intéressant, tout comme une prévision infrajournalière à pas de temps de 6 h. Finalement, nous avons introduit une technique d'optimisation globale, les algorithmes génétiques, capable de calibrer la méthode des analogues en considérant tous les paramètres des différents niveaux d'analogie de manière conjointe. Avec cette technique, nous pouvons nous approcher objectivement d'une paramétrisation optimale. Le choix des niveaux atmosphériques et des fenêtres temporelles et spatiales étant automatisé, cette technique peut engendrer un gain de temps, même si elle est relativement exigeante en calculs. Nous avons ainsi pu améliorer la méthode des analogues, et y ajouter de nouveaux degrés de liberté, notamment des fenêtres spatiales et des pondérations différenciées selon les niveaux atmosphériques retenus.

La filière coordonnée "diabaide" a été mise en place à fin 2004 dans l'objectif d'améliorer la prise en charge des patients diabétiques par une organisation des soins fondée sur la collaboration, le partage de l'information et la coordination des prestations, afin de renforcer l'autonomie des patients (éducation et auto-prise en charge), d'améliorer la qualité des soins (recommandations thérapeutiques et protocoles de soins) et de maîtriser les coûts. La filière, à ses débuts, était constituée principalement de la cellule multidisciplinaire "diabaide", qui offrait des consultations ambulatoires et hospitalières par des professionnels spécialisés. Cette évaluation, intermédiaire, avait pour objectif d'estimer si le programme avait atteint ses objectifs après deux années d'activités. [....] Le développement de programmes de prise en charge des maladies chroniques est encore à ses débuts en Suisse et "diabaide" fait image de pionnier dans ce domaine. Après cette évaluation, le programme a été modifié en 2007 et ne correspond plus à la description fournie dans ce document. De nouveaux programmes ont également été mis en place en Suisse depuis 2007 (par exemple makora Diabetes-Disease Management Programm à Zürich). Dans le canton de Vaud, le département de la santé de l'action sociale a créé en 2010 un programme cantonal visant à réduire l'impact du diabète sur la population en agissant sur la prévention et sur l'amélioration de la prise en charge des personnes diabétiques. Le programme cantonal a pour objectif notamment de développer une prise en charge globale, inspirée en partie du programme "diabaide", qui sera stratifiée en fonction de la sévérité de la maladie et des besoins des patients, intégrera l'auto-gestion (self-management), sera organisée en filières interdisciplinaires, et sera fondée sur les preuves. Résumé : Une épidémie de gale sarcoptique (Sarcoptes scabiei) touche le canton de Genève depuis 1996. Nous avons étudié l'impact de cette maladie sur différents caractères comportementaux du renard roux (Vulpes vulpes) : l'écologie alimentaire, les comportements spatiaux et l'activité, ainsi que les contacts sociaux. Nous avons également évalué si la gale pouvait influencer la composition et la transmission de la communauté helminthique intestinale du renard. En effet, cette espèce est l'hôte de parasites qui sont liés à des zoonoses importantes, en particulier le ténia échinocoque du renard (Echinococcus multilocularis) pour lequel Genève est considérée comme une zone de haute endémie. Durant 5 années, des carcasses de renards ont été récoltées sur l'ensemble du canton. Nous les avons disséquées et avons conservé différents échantillons pour des analyses ultérieures. Des données sur le poids, l'âge et le statut reproductif des femelles ont ainsi pu être obtenues. Les contenus stomacaux ont été analysés pour déterminer le régime alimentaire, les intestins pour collecter des helminthes, des échantillons de sang pour faire des sérologies et des échantillons de peau pour récupérer les sarcoptes. Des renards sauvages ont également été capturés et équipés de colliers émetteurs afin de déterminer leur activité et leur utilisation de l'espace. Finalement, nous avons réalisé des affûts sur des terriers et des observations nocturnes à l'aide d'un détecteur thermique afin d'étudier les contacts sociaux. Nous avons ensuite considéré tous ces aspects pour comparer les renards galeux aux individus sains. Trois catégories de gale ont été prises en compte selon l'importance de l'infection. L'épidémie a traversé le canton en 8 ans environ et elle a provoqué une forte diminution des populations de renard. Les animaux malades étaient caractérisés par un poids réduit, ils utilisaient des domaines vitaux réduits et présentaient un rythme d'activité irrégulier. En ce qui concerne le régime alimentaire, les renards galeux avaient souvent des estomacs vides ou contenant peu d'aliments d'origine animale. Cette réduction de l'alimentation ne semble pas seulement être liée à des capacités de prédation réduite, mais également à un désintérêt face à la nourriture. Tous les changements de comportement cités étaient plus marqués chez les animaux soumis à une forte infestation. Dix taxons d'helminthes ont été identifiés dans les intestins des renards analysés. Deux d'entre eux représentent un risque de santé publique: Echinococcus multilocularis et Toxocara carvis. Pour ces helminthes, nous n'avons pas identifié de différences de prévalence entre les renards galeux et les animaux sains, mais la charge parasitaire était significativement supérieure chez les individus galeux, en particulier ceux souffrant d'une infestation importante. Ceci est probablement lié à une susceptibilité accrue des individus qui présentent une condition physique amoindrie et des défenses immunitaire affaiblies. Selon nos résultats, nous pouvons conclure que la gale induit des changements comportementaux importants et que ces changements ont une influence potentielle sur la transmission de la gale elle-même, mais également sur la transmission du reste de la communauté parasitaire de l'hôte. Les individus qui souffrent d'une infestation importante sont susceptibles de provoquer une contamination de l'environnement accrue en ce qui concerne des helminthes pouvant provoquer des zoonoses. La gale apparaît être un facteur à ne pas négliger dans le cadre de la gestion de la faune sauvage, mais également en ce qui concerne des problématiques de santé publique. Summary An epidemic outbreak of sarcoptic mange (Sarcoptes scabiei) has struck the canton of Geneva since 1996. The impact of the disease on various behavioural traits of the main host, the red fox (Vulpes vulpes), was investigated: feeding ecology, spatial behaviour and activity, and social contacts. We also evaluated if mange might have an influence on the composition and transmission of the intestinal helminth community of foxes. Indeed, this species is host of parasites with potential zoonotic importance, particularly the fox tapeworm (Echinococcus multilocularis) for which Geneva is reported as endemic area. During 5 years, red fox carcasses have been collected throughout the canton. They were then dissected and various samples conserved for further analysis. Data on weight, age, and reproductive status of females were obtained. Stomach content were analysed for diet analysis, intestines to recover helminths, blood to proceed to ELISAs and skin samples to check for the presence of Sarcoptes mites. Further, wild foxes were captured and fitted with transmitters in order to determine their activity pattern and space use. Finally, we proceeded to direct observations at dens and using a thermal imaging sensor at night to gain information about social contacts. A comparison between healthy and mangy foxes was made for all these aspects. Three categories of mangy foxes were considered from moderately to severely infected. The epidemic wave crossed the canton in about 8 years and induced a significant reduction in fox densities. Mangy individuals appeared to have reduced body weights, to use more restricted home ranges and exhibited an irregular activity pattern. Regarding food, sick foxes often had empty stomachs and consumed less food items of animal origin. The reduction in food intake appeared to be linked not only to a reduced ability to hunt, but also to a reduced interest in food. The changes observed were particularly pronounced in individuals with severe infestation. Ten helminth taxa were recovered from the intestines on the analysed fox carcasses. Two of them have an importance with regard to human public health: Echinococcus multilocularis and Toxocara cams. The prevalence of these helminths did not differ between healthy and mangy foxes, however the worm burden was significantly higher in mangy foxes, particularly those with severe mange. This is probably linked to an increased susceptibility in individuals with a reduced body condition and weakened immune defences. From our observations, we can conclude that mange induces pronounced behavioural changes in the red fox, and that those changes influence the transmission risks of mange itself, but also of the rest of the parasite community of the host. Individuals with severe mange are for example likely to increase the environment contamination of free living stages of helminths with zoonotic importance. Mange appears thus to represent a factor not to be neglected for the management of wild species and for public health issues.

Le présent mémoire traite de l'implication des hôpitaux universitaires en évaluation des technologies de la santé. Le champ d'étude est vaste : - L'évaluation des technologies de la santé est une discipline dont l'étendue fait appel aux sciences exactes (physique, statistiques, ingénierie, méthodologie de la recherche analytique, etc.) ainsi qu'aux sciences sociales (économie, éthique, etc.) - L'hôpital universitaire est une organisation des plus complexes qui soit. Le "produit final" - l'amélioration de la santé du patient - est totalement non standardisé, il n'existe pas de modèle d'hôpital universitaire type et unique (inclusion ou non d'un centre de recherche, d'une faculté de médecine, d'un comité de gestion politique, etc.) et les corps de métiers qui le composent sont des plus variés et représentés en grand nombre Ainsi, l'étendue du champ relève des domaines interdépendants tels que la technologie (elle-même incluse dans le domaine de l'innovation aujourd'hui en développement), le management (ou administration des soins, du système de santé dans lequel s'inscrit l'hôpital universitaire), le financement de l'institution (mais aussi la rémunération des professionnels) et enfin la stratégie de l'hôpital. Le présent travail ne fait qu'effleurer ce large spectre d'investigation. La thématique de l'évaluation des technologies de la santé n'est guère connue en Suisse, ou plutôt elle n'est que peu développée et reconnue comme une science interdisciplinaire destinée à porter un jugement de valeur afin d'aider la prise de décision, sens profond qui la définit dans d'autres pays tel le Canada et plus particulièrement le Québec. Enfin, il convient de souligner que le présent travail n'a pas comme objectif de comparer les systèmes de santé québécois et suisse ou de réaliser toute autre critique de l'un ou de l'autre. Le but du stage ([à l'Université de Montréal], cf. Préambule) était d'observer des structures reconnues au niveau international et nullement de juger de l'ensemble du système de santé. Si l'on peut s'inspirer des réussites ailleurs, il convient de s'en tenir à l'inspiration de celles-ci pour ensuite, éventuellement, les contextualiser dans d'autres lieux. Ainsi, à chaque fois qu'il sera fait mention du Québec par la suite, ce sera dans l'intention de démontrer l'originalité de la démarche et d'élargir le débat dans le contexte qui nous intéresse. L'analyse narrative permet d'apprécier comment une théologie se dit en narrativité. L'analyse narrative fait réaliser que la construction d'une intrigue, le dispositif d'un réseau de personnages ou la gestion de la temporalité sont autant d'indicateurs de l'intention théologique qu'un énoncé kérygmatique ou une confession de foi. C'est rendre justice aux auteurs bibliques que s'instruire des outils dont ils ont usé pour construire leur récit, quand bien même la théorisation narratologique leur était étrangère. Enfin, l'analyse narrative offre un appareil de lecture et un métalangage simple

Le modèle de l'agence et le phénomène d'agentification du service public correspondant au cours de ces deux dernières décennies s'inscrit à l'origine dans les réformes associées à la Nouvelle Gestion Publique, dans une visée d'amélioration de la performance du secteur public, par une plus grande efficacité managériale (eg Pollitt et al., 2001). Le modèle est basé sur une logique contractuelle: l'autorité politique attribue un mandat à l'agence, l'agence délivre les prestations attendues ; celle-ci est ensuite contrôlée sur ses résultats. Afin de délivrer les prestations de manière efficiente et efficace, l'agence doit bénéficier d'une latitude et flexibilité substantielle dans l'utilisation de ces ressources, c'est-à-dire d'un affranchissement (partiel) des règles et procédures bureaucratiques (eg Bouckaert, 2003).
