Background and Objective: Oral mucositis is a dose-limiting and painful side effect of radiotherapy (RT) and/or chemotherapy in cancer patients. The purpose of the present study was to analyze the effect of different protocols of laser phototherapy (LPT) on the grade of mucositis and degree of pain in patients under RT. Patients and Methods: Thirty-nine patients were divided into three groups: G1, where the irradiations were done three times a week using low power laser; G2, where combined high and low power lasers were used three time a week; and G3, where patients received low power laser irradiation once a week. The low power LPT was done using an InGaAlP laser (660 nm/40 mW/6 J cm(-2)/0.24 J per point). In the combined protocol, the high power LPT was done using a GaAlAs laser (808 nm, 1 W/cm(2)). Oral mucositis was assessed at each LPT session in accordance to the oral-mucositis scale of the National Institute of the Cancer-Common Toxicity criteria (NIC-CTC). The patient self-assessed pain was measured by means of the visual analogue scale. Results: All protocols of LPT led to the maintenance of oral mucositis scores in the same levels until the last RT session. Moreover, LPT three times a week also maintained the pain levels. However, the patients submitted to the once a week LPT had significant pain increase; and the association of low/high LPT led to increased healing time. Conclusions: These findings are desired when dealing with oncologic patients under RT avoiding unplanned radiation treatment breaks and additional hospital costs. Lasers Surg.Med. 41:264-270,2009. The cystine knot structural motif is present in peptides and proteins from a variety of species, including fungi, plants, marine molluscs. insects and spiders. It comprises an embedded ring formed by two disulfide bonds and their connecting backbone segments which is threaded by a third disulfide bond. It is invariably associated with nearby beta-sheet structure and appears to be a highly efficient motif for structure stabilization. Because of this stability it makes an ideal framework for molecular engineering applications. In this review we summarize the main structural features of the cystine knot motif, focussing on toxin molecules containing either the inhibitor cystine knot or the cyclic cystine knot. Peptides containing these motifs are 26-48 residues long and include ion channel blockers, haemolytic agents, as well as molecules having antiviral and antibacterial activities. The stability of peptide toxins containing the cystine knot motif, their range of bioactivities and their unique structural scaffold can be harnessed for molecular engineering applications and in drug design. 1. Recent findings have suggested a significant involvement of the immune system in the control of pain. Immune cells contain opioid peptides that are released within inflamed tissue and act at opioid receptors on peripheral sensory nerve endings. It is also apparent that different types of lymphocytes contain P-endorphin, memory T cells containing more beta -endorphin than naive cells. 2. These findings highlight an integral link between immune cell migration and inflammatory pain, The present review highlights immune system involvement in the site-directed control of inflammatory pain. 3. Full-length mRNA transcripts for opioid precursor proteins are expressed in immune cells. Increased expression of pro-opiomelanocortin mRNA and beta -endorphin has been demonstrated in stimulated lymphocytes and lymphocytes from animals with inflammation. 4. Cytokines and corticotropin-releasing factor (CRF) release opioids from immune cells, Potent peripheral analgesia due to direct injection of CRF can be blocked by antagonists to CRF, antibodies to opioid peptides, antisense to CRF and opioid receptor-specific antagonists. The release of opioid peptides from lymphocytes is calcium dependent and opioid receptor specific. Furthermore, endogenous sources of opioid peptides produce potent analgesia when implanted into the spinal cord. 5. Activated immune cells migrate directly to inflamed tissue using cell adhesion molecules to adhere to the epithelial surface of the vasculature in inflamed tissue. Lymphocytes that have been activated can express opioid peptides, Memory type T cells that contain opioid peptides are present within inflamed tissue; naive cells are not present in inflamed tissue and do not contain opioid peptides, Inhibiting the migration of memory type T cells into inflamed tissue by blocking selectins results in reduced numbers of beta -endorphin containing cells, a reduced quantity of beta -endorphin in inflamed paws and reduced stress- and CRF-induced peripheral analgesia. 6. Immunosuppression is associated with increased pain in patients. Moreover, immunosuppression results in decreased lymphocyte numbers as well as decreased analgesia in animal models.

Tennis elbow or lateral epicondylalgia (LE) is a challenging musculoskeletal condition to treat. This is largely due to the lack of research-based evidence of the clinical efficacy of the myriad of treatment approaches espoused in the literature. In view of this, successful rehabilitation of LE is based on choosing treatments that address the physical impairments found during clinical examination. The primary physical impairment in LE is a deficit in grip strength predominately due to pain and its consequences on motor function. Hence the mainstay of successful management of this condition is therapeutic exercise, providing it is not pain provocative. Adjunctive procedures such as manipulative therapy and sports taping techniques have recently been shown to provide substantial initial pain relief. Early relief of pain in the rehabilitation program helps accelerate recovery and most importantly motivates the client to persist with the therapeutic exercise program. The manipulative therapy and taping treatments presented in this masterclass warrant consideration in the clinical best practice management of LE, and serve as a model for other similar musculoskeletal conditions. Previous studies have demonstrated that the initial hypoalgesic effect of spinal manipulative therapy was not antagonized by naloxone and did not exhibit tolerance with repeated applications. The implication is that endogenous opioid mechanisms of pain relief are probably not at play in spinal manipulative therapy. The role of endogenous opioid peptides in manipulation of the peripheral joints has not been investigated. The aim of this study was to evaluate whether the initial hypoalgesic effect of a peripheral manipulative technique (mobilization-with-movement treatment for the elbow) demonstrated a tolerance to repeated applications (ie, reduction in magnitude of effect over repeated applications). Twenty-four participants with unilateral chronic lateral epicondylalgia participated in the study. A repeated measures study was conducted to examine the effect of repeated applications of the mobilization-with-movement treatment for the elbow on 6 separate treatment occasions at least 2 days apart. Pain-free grip strength and pressure pain threshold were chosen as the pain-related outcome measures. Changes in the percent maximum possible effect scores of measures of hypoalgesia were evaluated across the 6 treatment sessions by using linear trend analysis. The results showed no significant difference for the hypoalgesic effect of the treatment technique between sessions (P >.05). This peripheral manipulative therapy treatment technique appeared to have a similar effect profile to previously studied spinal manipulative therapy techniques, thereby contributing to the body of knowledge that indicates that manipulative therapy most likely induces a predominant non-opioid form of analgesia.

The work of Michel Foucault sees modern penal technology its ann expression of power that operates through and is motivated by a dry instrumental reason. This article draws upon Durkheim and Bakhtin to advance a radically alternative approach. It is suggested that such technology is invested with sacred and profane symbolism and is understood via emotionally charged, dramatically compelling narrative frames. Tensions between official and unauthorized discourses can be understood through a center/periphery model of culture. In an extended case study of the guillotine, it is shown dial the apparatus was initially legitimated as an expression of a sacred revolutionary code. Such a discourse was subsequently destabilized by popular medical debates that raised the specter of pain after decapitation. While inconclusive, these new motifs mobilized Gothic and grotesque themes that confronted the rationalist aesthetics of the guillotine. A situation of Bakhtinian hetoroglossia eventuated. Uncertainty, the uncanny and fable entered a discursive field of increasing complexity.

A transitory increase in blood pressure (BP) is observed following upper airway surgery for obstructive sleep apnea syndrome but the mechanisms implicated are not yet well understood. The objective of the present study was to evaluate changes in BP and heart rate (HR) and putative factors after uvulopalatopharyngoplasty and septoplasty in normotensive snorers. Patients (N = 10) were instrumented for 24-h ambulatory BP monitoring, nocturnal respiratory monitoring and urinary catecholamine level evaluation one day before surgery and on the day of surgery. The influence of postsurgery pain was prevented by analgesic therapy as confirmed using a visual analog scale of pain. Compared with preoperative values, there was a significant (P < 0.05) increase in nighttime but not daytime systolic BP (119 ± 5 vs 107 ± 3 mmHg), diastolic BP (72 ± 4 vs 67 ± 2 mmHg), HR (67 ± 4 vs 57 ± 2 bpm), respiratory disturbance index (RDI) characterized by apnea-hypopnea (30 ± 10 vs 13 ± 4 events/h of sleep) and norepinephrine levels (22.0 ± 4.7 vs 11.0 ± 1.3 µg l-1 12 h-1) after surgery. A positive correlation was found between individual variations of BP and individual variations of RDI (r = 0.81, P < 0.01) but not between BP or RDI and catecholamines. The visual analog scale of pain showed similar stress levels on the day before and after surgery (6.0 ± 0.8 vs 5.0 ± 0.9 cm, respectively). These data strongly suggest that the cardiovascular changes observed in patients who underwent uvulopalatopharyngoplasty and septoplasty were due to the increased postoperative RDI.

The aim of this study was to evaluate the response to the implantation of synthetic hydroxyapatite 30% (HAP-91®) in different physical states as dermal filler. Eighteen New Zealand rabbits were used, distributed randomly into two equal groups and then divided into three groups according to the postoperative period at 8, 21 and 49 days. One mL of HAP-91®, fluid and viscous, was implanted in the subcutaneous tissue, 1 cm proximal to the cranial crest of the right scapula. The thickness of the skin was measured before and after implantation and for the following 15 days. Pain sensitivity assessment was conducted, assigning the following scores: 0 - when the animal allowed the touch of the implant area and expressed no signs of pain; 1 - when the animal allowed the touch, but pain reaction occurred, like increase of the respiratory rate or attempt to escape; 2 - when the animal did not allow the touch to the implanted area. At 8, 21 and 49 days, biopsy of the implanted area was performed. No difference was observed between the thickness of the skin (p>0.05) and all animals received a score 0 for soreness. Histological analysis did not reveal any obvious inflammatory process, showing a predominance of mononuclear cells in samples of eight days and tissue organization around the biomaterial with a tendency to encapsulation. The results indicate that HAP-91®, both viscous and fluid, is biocompatible and suitable for dermal filling.

Objectivo: Identificar os factores psicossociais que influenciam a percepção da dor pós-operatória em doentes submetidos a cirurgia de revascularização do miocárdio (CRM). Material e Métodos: Estudo exploratório correlacional de 91 doentes (71 homens e 20 mulheres) submetidos a CRM (pontagem aortocoronária) por esternotomia). A idade média era de 63,8 ± 9,6 anos (entre 39 e 84). Foram utilizados os seguintes instrumentos: Escala Analógica Visual às 24, 48 e 96 horas do pós-operatório; Questionário de Caracterização Demográfica; Mental Health Inventory de 5 itens; Percepção de Saúde Geral (SF-36); Escala de Expectativas de Dor; Escala de Percepção de Apoio; Escala de Expectativas de Auto-eficácia; Satisfação com o tratamento, médicos e enfermeiros (American Pain Society Questionnaire) aplicados às 96 horas após a cirurgia. Resultados: Os doentes que apresentaram expectativas elevadas de dor, percepcionaram maior apoio, apresentaram níveis elevados de auto-eficácia para lidar com a dor ou, se pertenciam ao sexo masculino, sentiram menos dor. De igual modo, os doentes que apresentaram melhor saúde mental, percepcionaram a sua saúde como boa e os doentes que expressaram maior satisfação com o tratamento sentiram menos dor. A dor não foi influenciada pela idade, grau de escolaridade ou pela satisfação com a conduta de médicos e enfermeiros. Conclusão: Após as primeiras 48 horas do pós-operatório, a experiência de dor é influenciada por factores psicossociais, em oarticular pela expectativa de dor, expectativa de auto-eficácia, apoio percebido, percepção da saúde geral, percepção de saúde mental e satisfação com o tratamento para a dor. Perante os resultados, evidencia-se a necessidade de conjugar conhecimentos no sentido de dar respostas mais alargadas e de carácter multidisciplinar no tratamento da dor pós-operatória em CRM devendo, a par de outros aspectos, focar-se na gestão das expectativas dos doentes. ABSTRACT - Objective: To identify the psychological factors that influence post-surgery pain perception in patients undergoing coronary artery bypass graft surgery (CABG). Methods: This was an exploratory correlational study of 91 patients (71 men and 20 women) who underwent CABG (aortocoronary anastomosis) by sternotomy. Mean age was 63.8 ± 9.6 years (between 39 and 84). The following instruments were used: visual analogical scales at 24, 48 and 96 hours of post-surgery; demographic characteristics survey; pain expectations scale; perceived support scale; self-efficacy scale, Mental Inventory (5 items); health perception scale; and satisfaction with treatment, doctors and nurses (American Pain Society questionnaire) at 96 hours after surgery. Results: Patients who had presented high expectations of pain, perceived more support, presented high levels of self-efficacy to deal with pain or were male, felt less pain. Furthermore, patients who presented better mental health, perceived their general health as being good, or expressed greater satisfaction with treatment, felt less pain. Pain was not influenced by age, level of education or satisfaction with doctors and nurses. Conclusion: After the first 48 hours following surgery, the pain experience is influenced by psychosocial factors, in particular by expectation of pain and of self-efficacy, perceived support, perception of general and mental health, and satisfaction with pain treatment. The results confirm the need to bring together different kinds of knowledge for a broad, multidisciplinary approach to postoperative CABG pain treatment, focusing, along with other aspects, on management of patients’ expectations.

Objectivo: O objectivo deste estudo foi avaliar a implementação de um projecto comunitário na sintomatologia neuro-músculo-esquelética de domésticas. Metodologia: Realizou-se um estudo experimental que incluiu 30 domésticas (20 no grupo experimental e 10 no grupo de controlo) aleatoriamente seleccionadas. Foram utilizados como instrumentos de avaliação o Questionário de Avaliação de Risco, a Escala de Borg da Percepção Subjectiva do Esforço e o Questionário Nórdico Músculo-Esquelético. O projecto comunitário englobou uma acção de educação para a saúde e um programa de exercícios específicos. Resultados: A implementação do projecto comunitário diminuiu significativamente (p<0,05) a sintomatologia neuro-músculo-esquelética, a percepção subjectiva de esforço e a intensidade média de dor reportada pelas domésticas durante a realização de várias tarefas. As domésticas melhoraram significativamente (p<0,05) os seus conhecimentos em relação aos factores de risco e modificaram significativamente (p<0,05) os seus comportamentos, adoptando posturas mais adequadas. Conclusões: O serviço doméstico propicia o aparecimento de sintomatologia neuro-músculo-esquelética e, como tal, a implementação de uma acção de educação para a saúde e um programa de exercícios específicos tornam-se eficazes como projecto de promoção de saúde.

Introdução – A reconstrução mamária, no processo pós-cancro da mama, poderá ser levada a cabo através da lipoaspiração cujas consequências poderão ser ultrapassadas através da fisioterapia. Objetivo – Verificar, num estudo de caso, os resultados da drenagem linfática manual (DLM) e das bandas neuromusculares (BNM) na dor, edema, hematoma e pigmentação do abdómen e mamas após lipoaspiração para reconstrução mamária. Metodologia – Uma paciente foi sujeita a 10 sessões de fisioterapia com aplicação de DLM e BNM. Em cada sessão foram avaliados, nas mamas e no abdómen, a dor, o edema, o hematoma e a pigmentação. Resultados – Eliminação da dor, desaparecimento do hematoma abdominal, normalização da pigmentação mamária e diminuição do edema. Conclusão – Neste estudo de caso, a DLM e as BNM melhoram as consequências no pós-cirúrgico de lipoaspiração para reconstrução mamária. ABSTRACT - Introduction – Breast reconstruction, post breast cancer, can be carried out through liposuction which consequences may be overcomed through physiotherapy. Objective – Check, in a study case, the results of manual lymphatic drainage (MLD) and neuromuscular bandages (NMB) in pain, edema, bruising and pigmentation on abdomen and breasts, after liposuction for breast reconstruction. Methodology – A patient underwent 10 sessions of physiotherapy with application of MLD and NMB. In each session were evaluated breast and abdomen pain, edema, bruising and pigmentation. Results – Elimination of pain, disappearance of abdominal hematoma, normalization of breast pigmentation and reduction of edema. Conclusion – In this study case, MLD and NMB improve the consequences of post-surgical liposuction for breast reconstruction.

A dor espinal em crianças e jovens constitui um problema cujas dimensões e prevalência tem vindo aumentar nas últimas décadas. Esta problemática parece ser tanto mais significativa se for tido em consideração que os episódios de dor espinal na idade escolar se perpetuam e estão relacionados com os que acontecem na idade adulta. Desta forma, os investigadores alertam para a necessidade de identificar potenciais factores de risco para o desenvolvimento de dor espinal em crianças e jovens de idade escolar. Assim, o presente estudo pretende identificar qual a importância assumida pelo contexto familiar na presença de queixas dolorosas espinais nas crianças em idade escolar do concelho da Maia. A amostra é constituída pelos pais de 1017 crianças do 1º ciclo do Ensino Básico do Concelho da Maia que foram avaliadas relativamente à postura, numa fase anterior do estudo. Desta população constituiu amostra 636 pais que concordaram participar no estudo, tendo obtido resposta de 226, obtendo uma percentagem de adesão de 35,5%. Para a obtenção dos dados qualitativos foi utilizado um método de amostragem estratégica de forma a seleccionar dez pais representativos de todas as categorias possíveis (pais com e sem dor e filhos com e sem dor). Esta amostra foi seleccionada tendo em conta os dados obtidos no questionário Parental Pain e Questionário de Bournemouth, sendo, posteriormente aplicado uma entrevista não estruturada. Os principais resultados obtidos permitiram concluir que o contexto tem uma influência determinante na criança com dor. De acordo com os dados extraídos das entrevistas, os comportamentos de manifestação de dor por parte das crianças podem ser um reflexo de comportamentos apreendidos através da observação do comportamento dos adultos e, consequentemente mimetizados, opinião esta igualmente expressa por alguns dos pais. Mas nem sempre os pais apontam a mimetização como causa para a dor referida pelos filhos, referindo igualmente o transporte da mochila, as posturas assumidas no dia-a-dia, o mobiliário escolar e as actividades de lazer realizadas pelas crianças.

Objectivo: O objectivo deste estudo foi avaliar a eficácia de um plano de rotatividade de postos de trabalho, específico, na prevalência de sintomatologia músculo-esquelética auto-referida, de operadoras de costura. Metodologia: Realizou-se um estudo quantitativo, observacional e analítico longitudinal, onde se avaliou o efeito de uma intervenção, em 25 operadoras que se voluntariaram para o estudo, de acordo com o interesse da empresa. Foram utilizados como instrumentos de avaliação o Risk Filter, o Questionário Nórdico Músculo-Esquelético e o Rapid Upper Limbs Assessment (RULA). O projecto englobou uma análise de factores de risco e classificação dos respectivos postos de trabalho de modo a estruturar uma (re)organização do trabalho (Plano de rotatividade de postos de trabalho). Resultados: A implementação do projecto reduziu significativamente (p<0,05) a sintomatologia músculo-esquelética auto-referida, sobretudo ao nível da cervical, tornozelos e pés, punhos e mãos, tendo sido estas últimas as áreas de maior prevalência referida. Verificou-se também uma diminuição da média da intensidade de dor reportada, pelas operadoras. Conclusões: O resultado deste estudo reforça a recomendação e implementação da variação do gesto (variação biomecânica da exposição), através da implementação de um plano de rotatividade de postos de trabalho, específico, tornando-se eficaz como intervenção preventiva e promotora de saúde, no que refere às lesões musculesqueléticas relacionadas com o trabalho.

A crioterapia é prática comum na medicina esportiva, pela praticidade, facilidade de acesso e baixo custo, possuindo vantajoso uso na Saúde Pública. No entanto, os efeitos analgésicos do gelo tem pouca base objetiva e sistematizada em termos de técnicas, duração e frequência. O objetivo deste estudo foi sintetizar através da revisão sistemática (RS) as evidências relativas à efetividade da crioterapia para o tratamento das entorses de tornozelo de atletas. A RS é um método de pesquisa observacional e retrospectivo, pelo qual se tratam artigos, preferencialmente Ensaios Clínicos Aleatórios – ECA, como sujeitos da investigação, com rigorosos critérios de inclusão e exclusão e, quando possível, realiza-se uma macro estatística dos resultados – metaanálise. No presente RS, foram consultados cinco bancos de dados - Medline, Embase, Cochrane, Lilacs e PEDro para buscar ECA sobre crioterapia com os desfechos dor, edema, rigidez e função. Resultados: 289 estudos foram identificados inicialmente, dos quais nove com tratamentos isolados ou associados à crioterapia, porém apenas um preencheu aos critérios de inclusão, cujo N era 121 atletas, dos quais 64 receberam a crioterapia (funcional) e 57 no grupo controle (imobilização). Maior probabilidade para o evento dor foi observada no grupo controle, após 3 e 12 meses. A RS revelou uma lacuna em ECA dentro do tema, mas não encontrou efeito adverso na prática da crioterapia, sendo um princípio analgésico importante, sobretudo em lesões de tecidos moles.
