QUALIDADE DE VIDA E ESTRATÉGIAS DE ENFRENTAMENTO DE MULHERES COM E SEM LINFEDEMA APÓS CÂNCER DE MAMA

O linfedema no membro superior é uma complicação inerente ao tratamento de câncer de mama. Caracterizado pelo aumento do volume do membro, leva às limitações físicas e funcionais, e impacto negativo no âmbito psicológico e social. O objetivo deste estudo foi investigar a qualidade de vida e seus domínios, as estratégias de enfrentamento frente ao câncer de mama, e a correlação entre essas variáveis. Este estudo foi realizado em um centro de saúde dedicado às mulheres, por quatro meses. Os instrumentos de avaliação foram: questionário de caracterização geral e específico do câncer de mama, perimetria dos membros superiores; questionários de qualidade de vida da Organização Européia de Pesquisa e Tratamento do Câncer, EORTC QLQ-30 e BR-23; e Inventário de Estratégias de Coping. Foram entrevistadas 82 mulheres, idade média de 57,4 anos (DV12,3), submetidas a tratamento cirúrgico de mama unilateral e esvaziamento axilar, sem metástase. O linfedema apresentou-se em 39,03% (32) e parece não interferir muito na qualidade de vida das mulheres pós-câncer de mama, sendo a função social a mais prejudicada. Sintomas relacionados à quimioterapia e a mama incomodam as mulheres de ambos grupos, porém os sintomas relacionados aos braços foram estatisticamente maiores nas portadoras de linfedema. As estratégias mais utilizadas pelas entrevistadas para enfrentar o câncer foram a reavaliação, resolução de problemas, fuga, suporte social e autocontrole, somente o autocontrole foi estatisticamente maior nas mulheres com linfedema. As estratégias de resolução de problemas, autocontrole e baixo suporte social podem ter colaborado para o desencadeamento do linfedema. Conclui-se que o uso de estratégias ativas e positivas para enfrentar o câncer de mama parece resultar na boa adaptação psicossocial

O câncer em crianças até cerca de duas décadas, era considerado uma doença crônica, com prognóstico desfavorável, resultando na maioria dos casos, em morte. Atualmente, tem-se apresentado como uma doença com melhores perspectivas, onde 70% das crianças acometidas por essa doença podem ser curadas, quando diagnosticadas precocemente, e tratadas em centros especializados¹. Este estudo teve como objetivo, avaliar a qualidade de vida e o stress de crianças e adolescentes com câncer, em remissão e recidiva. Trata-se de um estudo correlacional, quali-quantitativo, transversal. Foi desenvolvido no ambulatório de oncologia pediátrica da Faculdade de Medicina do ABC, e na enfermaria do Hospital Mário Covas. Contou com a colaboração de 40 sujeitos, com idades entre 06 a 14 anos, de ambos os sexos. Como instrumento para medir a qualidade de vida, foi utilizado o Child Health Questionnaire (CHQ-PF50), que possui 15 conceitos em saúde, abrangendo aspectos físicos e psicossociais e para medir o stress, a Escala de Stress Infantil (ESI), que tem como objetivo, avaliar o stress da criança, através de reações físicas e psicológicas. Os resultados indicaram que no domínio físico (PhS), as crianças em situação clínica de recidiva e remissão não apresentam diferenças significativas em relação às variáveis: qualidade de vida e stress, porém, no domínio psicossocial (PsS), houve diferença estatisticamente significante, indicando que os meninos apresentam melhor qualidade de vida e menor stress, se comparados com as meninas, mostrando que o emocional interfere nesse resultado.(AU)

O objetivo deste artigo é tecer algumas reflexões acerca das convicções de saúde em pacientes com câncer, participantes de um programa de atendimento multidisciplinar realizado numa universidade da Grande São Paulo. Foram utilizados depoimentos de 06 (seis) pacientes, na forma de entrevistas dirigidas e o conteúdo analisado segundo um modelo de convicção de saúde, o qual preconiza que o paciente terá maiores possibilidades de aderir aos tratamentos, seguindo cinco convicções básicas de saúde, a saber: suscetibilidade, severidade, benefícios, barreiras, eficácia própria. Uma ampliação deste modelo foi proposta por outro estudo que acrescenta mais duas categorias às já existentes: impacto ao diagnóstico e expectativas de futuro.(AU)

The objective was to understand the process of care in the perception of hospitalized children with cancer. This is a descriptive study of qualitative approach. Data were collected between the months of October 2013 and January 2014, through photographic records and semi-structured interview consisting of questions relating to the identification of age, sex, diagnosis and length of stay and a script of questions related to the recorded pictures. Eight children were included aged between six and twelve who were admitted to a pediatric oncology sector, located in the city of Natal / RN. The criteria used in the sample were: being hospitalized for cancer treatment; and present favorable physical conditions for carrying out the data collection. For the treatment of collected material was used content analysis, thematic modality. The study followed the ethical and legal principles governing scientific research with human beings and took place with the approval of the project by the Ethics and Research Committee of the Northern League Riograndense against Cancer, with opinion registered under number 329 015 and CAAE 16097613.9.0000.5293. According to the results it was found that, for the child, the care happens through technical activities, such as making procedures and the use of personal protective equipment, as well as through the dialogic relationship, which favors the establishment of confidence in care professional. Caring also means developing activities that promote well-being, the fun and the social and cognitive development, highlighting thus the playful, during hospitalization, as an auxiliary tool in the care process. During hospitalization, the child identifies two individuals responsible for their care, accompanying family and professional, and nursing professionals the most cited in moments of care. , Also of note, the promotion of care, in the perception of the child related to the infrastructure of the institution, environmental cleaning, personal hygiene, the medicalization and the food. It is concluded that care understood by the child, whilst still maintaining relations with the biomedical model, points to a new perspective that should consider the biological, social and psychological of acquiring cancer without unlink them of the development child. 9 Moreover, we see the child as an active social actor in this process, and therefore needs to be heard and answered their needs

Except the non-melanoma skin tumors, colorectal cancer is the second most common in the Southeastern Region of Brazil, the third most common in the Southern and Central Regions. It is also the forth most common in the Northern Region and it is the fifth one in the Northeastern. To assess pathological and clinical variables of colorectal Cancer is crucial to know the possible conclusions for the survival of patients and point out the characteristics in the progress of tumor, such as the profile of tumor invasion and its angiogenesis. This work focuses on analyzing clinically and pathologically some settings in colorectal cancer patients (CRC) in the city of Natal and its countryside through those variables as parameters of prognosis and determine the level of protein expression, for instance: E-cadherin (E-cad), beta- -catenin (β-cat), galectin-3 (gal-3), matrix metalloproteinases (MMP) 2 and 9 and vascular-endothelial growth factor alpha (α VEGF) in the tumor tissues. A retrospective study was done in colorectal cancer cases in the regions of Rio Grande do Norte state from 1995 to 2005, specifically in Natal city/RN/Brazil. The pathological and clinical variables, such as: age, gender, ethnicity, lifestyle, family history, the location of the primary tumor, level of differentiation, TDM staging, modified Dukes’, treatment and survival were analyzed. The pathological and clinical data were collected from medical records through a specific form and were filed on Excel. A total of 534 patients were selected from the Pathology Department file in this institution, however, 176 patients were excluded. 358 patients were included for Epidemiological analysis and its clinical and pathological correlations were selected. 180 patients were also selected for histological and immunohistochemical studies. The tumor progression of these selected proteins mentioned before were analyzed. The Paraffin blocks of these samples were treated by Microarray Tissue technique and its blades subjected to immunohistochemistry to test the intensity of these proteins in tumor tissues. The results of this analysis were correlated with clinicopathologic variables of patients. Statistical analysis using the chi-frame Pearson test and analysis of midlife by Kaplan-Meier curve was also utilized. P values < 0.05 were considered statistically significant. The average age of our sample was 58.8 years and 51.7 % were female. Alcohol consumption has increased by 1.71 time the risk of death by CCR (p = 0.034) and tobacco consumption increased 2.7 times the chance of developing tumors of high TNM stage (p = 0.001). Cancer patients had a family history of 3,833 times the chance of developing the CCR (p = 0.002). The expression of MMP-2 showed a significant association with tumors of high TNM stage (p <0.046) and mortality (p = 0.041). The α VEGF expression had statistically significant correlation with high TNM stage (p <0.009), degree of cell indifferentiation (p <0.025) and mortality (p <0.035). Expressions of E-cadherin and beta-catetina demonstrated tumor linked to high TNM stage (p = 0.0001) and Dukes› modified (p = 0.05), lesions in the rectum (p = 0.03 and p = 0.007, respectively), smoking (p = 0.05) and indifferentiation (p = 0.001). The expression of Gal-3 showed statistical significance with high TNM stage of patients (p = 0.01), smokers (p = 0.01), alcohol drinking (p = 0.03), indifferentiation (p = 0.0001) and mortality (p = 0.0001). Based on the results, therefore, we could realize that lifestyle and family history had correlation in the CCR prognosis, as well as MMP-2 expression, MMP-9, VEGF alpha, E-cadherin, Beta-catenin and Galectin-3 were important prognostic markers in tumor progression in colorectal cancer.

The purpose of this systematic review was to compare the effectiveness of topical treatments to minimize post-radiotherapy xerostomia. PubMed, Cochrane Library (CENTRAL) and LILACS databases were searched without restriction on date or language until the 6thAugust, 2015. Key-wordsused for searching were radiotherapy, xerostomia and saliva. Two independent reviewers screened titles and abstracts, carried out data extraction and assessed risk of bias. The first search identified 429 articles. From these, 117 studies were selected for full-text reading, from which 18 were included in the qualitative synthesis. From the eighteen articles included, seven were non- controlled clinical trial, one article was controlled clinical trial and ten studies were randomized clinical trials (three clinical trials were placebo controlled and seven were crossover). By the assessment of the quality of the studies included, ten showed high risk of bias, four showed moderate risk of bias and four presented low risk of bias. All interventions were considered effective in treating xerostomia (mucin, polysaccharides, aloe vera, rape oil, linseed oil, carboxymethylcellulose, polyethylene oxide, pilocarpine and systems of care for xerostomia - gel, paste and mouthwash). Meta-analysis could not be performed due to heterogeneity between thestudiesand interventions. This systematic review showed that a single and general protocol for topical treatment of xerostomia post-radiotherapy does not exist and that follow-up visits should be performed to validate the individualized treatment plan.

En nuestro medio, el linfedema más frecuente es el que aparece en el miembro superior tras los tratamientos del cáncer de mama. Existe un interés creciente acerca de su impacto sobre la Calidad de Vida, entendida ésta como el completo estado de bienestar físico, mental y social de la persona. Para poder medir la CV en esta población, se precisan herramientas específicas, dado que los cuestionarios genéricos no recogen las particularidades de esta enfermedad. El cuestionario ULL-27 (Upper limb lymphedema) fue la primera escala específica desarrollada para valorar la calidad de vida de mujeres con linfedema de miembro superior tras cáncer de mama1. Dada la necesidad de disponer de este tipo de instrumentos en nuestro país, y habiendo mostrado el cuestionario francés ULL-27 buenas propiedades psicométricas en su estudio de validación, se planteó como objetivo su traducción al español y adaptación transcultural a nuestro medio, procediendo a continuación a su validación en una muestra de mujeres con linfedema de miembro superior tras cáncer de mama...

El cáncer colorrectal (CCR) es un importante problema de salud dada su elevada incidencia y morbimortalidad asociada. Como causa de muerte por cáncer, ocupa el segundo lugar en la mayoría de los países desarrollados, tanto en varones como en mujeres, y es la primera causa de fallecimiento cuando se consideran ambos sexos conjuntamente1,2 . La supervivencia del CCR depende de manera fundamental del estadio tumoral en el que sea diagnosticado. El coste sanitario total de la asistencia por cáncer colorrectal en España se valora en más de 1.300 millones de euros de forma global, estimándose que el cribado puede disminuir estos costes en un 40%9,10...

Un porcentaje no desdeñable de mujeres jóvenes con cáncer de mama esporádico son portadoras de mutaciones germinales en BRCA1/2. Esta prevalencia está estrechamente condicionada por otras variables como el tipo de población, fenotipo tumoral y edad de diagnóstico de cáncer de mama. La presencia de una estructura familiar no informativa es una variable predictiva de la presencia de mutaciones en BRCA1/2 en estas pacientes, pero este hallazgo no ha sido debidamente validado en cohortes independientes. Existe una fuerte asociación entre las mutaciones en BRCA1 y los tumores mamarios de tipo triple negativo (TN). En el presente trabajo analizamos la utilidad de las variables estructura familiar no informativa y fenotipo TN como variables predictivas de mutaciones germinales en BRCA1/2 en una cohorte de mujeres jóvenes ( 35 años) con cáncer de mama esporádico previamente remitidas (1998-2012) a unidades de asesoramiento genético españolas (n = 341). Retrospectivamente, la estructura familiar fue clasificada en informativa, no informativa o no clasificable mediante la revisión centralizada de los pedigrees disponibles en el momento de realizarse los respectivos estudios genéticos. Asimismo, los tumores de mama fueron clasificados en TN, no-TN y no clasificables mediante la revisión de las historias clínicas. Veinticuatro pacientes (7.12 %) fueron portadoras de mutaciones patogénicas, 14 en BRCA1 y 10 en BRCA2. Tanto la estructura familiar no informativa (OR = 3.61, p = 0.027) como el subtipo TN (OR = 3.14, p = 0.013) fueron variables independientes de la presencia de mutaciones germinales en BRCA1/2. La prevalencia de mutaciones en el subgrupo de mujeres con al menos un factor predictivo (estructura familiar no informativa, subtipo TN o ambos) fue del 13.5 %, frente al 2.8 % en aquellas mujeres con estrucutra familiar informativa y tumores no-TN (OR = 5.31, p = 0.006). Demostramos, con la información disponible en un contexto de práctica clínica rutinaria, la utilidad de la estructura familiar y el fenotipo TN como modelo predictivo de mutaciones germinales en BRCA1/2 en mujeres jóvenes con cáncer de mama esporádico. Nuestro estudio sugiere que restringir el análisis tan solo a aquellas mujeres con al menos un factor predicitivo podría ser una estrategia válida para optimizar la realización de estudios genéticos en este subgrupo de mujeres.

Oncological patients are submitted to invasive exams in order to obtain an accurate diagnosis; these procedures may cause maladaptative reactions (fear, anxiety and pain). Particularly in breast cancer, the most common diagnose technique is the incisional biopsy. Most of the patients are unaware about the procedure and for that reason they may focus their thoughts on possible events such as pain, bleeding, the anesthesia, or the later surgical wound care. Anxiety and pain may provoke physiological, behavioral and emotional complications, and because of this reason, the Behavioral Medicine trained psychologist takes an active role before and after the biopsy. The aim of this study was to evaluate the effect of a cognitive-behavioral program to reduce anxiety in women submitted to incisional biopsy for the first time. There were 10 participants from the Hospital Juárez de México, Oncology service; all of them were treated as external patients. The intervention program focused in psycho-education and passive relaxation training using videos, tape-recorded instructions and pamphlets. Anxiety measures were performed using the IDARE-State inventory, and a visual-analogue scale of anxiety (EEF-A), and the measurement of blood pressure and heart rate). Data were analyzed both intrasubject and intersubject using the Wilcoxon test (p≤0.05). The results show a reduction in anxiety (as in punctuation as in ranges) besides, a reduction in the EEF-A.

This paper introduces the case of a woman with breast cancer who had developed a mixed depressive-anxiety disorder with avoidance behaviors. The patient presented depressive symptoms like listlessness, insomnia, weeping, food disorders and hopelessness though. Also, she exhibited physiological arousal and restlessness feelings. Additionally, the patient had an avoidance patron behavior in relation with all stimulus she believed could hurt her. Based on the Acceptance and Commitment Therapy (ACT) the intervention planted the following objectives: the patient will learn to accept her illness and the emotional distress that she was experiencing, also, the patient will recover the other areas of her life that she had abandoned. The treatment was developed in 14 sessions. The therapist used these techniques: creative hopelessness, disabling verbal functions, values clarification and loss of control over private events. In the results, it was observed a positive change in the behavior of the patient as well as a decrease in emotional distress that was his reason for initial consultation

Objective: To evaluate the relationship between Perceived Psychological well-being, Optimism and Resilience in women survivors of breast cancer.Method: The sample was composed of 30 women diagnosed with breast cancer who were undergoing adjuvant treatment in Oncology Units General Hospital of Jerez de la Frontera and the University Hospital Puerta del Mar (Cadiz). The average age was 47.47 years (SD = 6,356) and the average of months from diagnosis of the disease was 9.93 (SD = 8,541). Scale of Psychological Well-being administered Perceived Ryff (1989), the Life Orientation Test Revised (LOT-R) Scheier, Carver and Bridges (1994) and the Scale of Resilience Wagnild and Young (1993) to assess psychological well-being the dispositional optimism and resilience respectivelyResults: Descriptive analyzes show that women diagnosed with breast cancer have levels of psychological well-being, dispositional optimism and higher than the average values provided by Resilience scales. On the other hand, regression analyses revealed that only some of the dimensions of resilience allowed to explain and predict some dimensions of psychological wellbeing, not dispositional optimism.Conclusions: The results seem to confirm the idea that if the capacity of women with breast moderate negative affect generated by the diagnosis itself and the consequences of their cancer treatment works and to promote their adaptation to the new situation we can contribute to increasing psychological well-being.

Objective: This essay aims at identifying, describing and analyzing possible changes both in the experience of the body and in interpersonal relations of women with breast cancer, which result from their participation in Dance Therapy group meetings.Method: This is a phenomenologically oriented qualitative research using Maria Fux´s dance therapy method for group experiences. Eight meetings are described here, and an analysis of descriptions based primarily on Merleau-Ponty and María Fux is provided.Results: The participants have been able to express pain and sorrow over the circumstances that breast cancer and its relational environments have brought to their lives. They have been able to go through moments of creation and surrender to the rhythmic body sensations and experiential environment with different emotions lived separately and jointly. They have revived memories and sensations of their childhood and adolescence, and finally, they have rediscovered their sensitive body through body resignifications marked by the absence of the breast, and by means of feelings of greater acceptance and integration of lived experiences in new gestalts.Conclusions: This project is still under way, but it is already possible to conclude that the life experiences provided in dance therapy have allowed these women to improve their integration and welfare. Likewise, they have felt positive changes in the perception of their corporality and in their way of being in the world and with other subjects, thus experiencing the body in a new and different way.
