Previous research demonstrated that the sequence of informational cues and the level of distraction have an impact on the judgment of a product’s quality. This study investigates the influence of the force behind the processing of these cues, working memory (WM). The results indicate that without distraction, consumers with low and high WM capacity (WMC) equally base their product evaluation on the first sequential cue. In the presence of a distractor, however, low WM individuals are no longer able to recall the initial cue, and thus derive their product judgment from the final cue. Moreover, evidence of intercultural differences in the perception of product related cues, and their aptitude for signaling a favorable product quality is provided.

INTRODUCTION AND HYPOTHESIS: This study aims to estimate fecal, urinary incontinence, and sexual function 6 years after an obstetrical anal sphincter tear. METHODS: Among 13,213 women who had a vaginal delivery of a cephalic singleton at term, 196 women sustained an anal sphincter tear. They were matched to 588 controls. Validated questionnaires grading fecal and urinary incontinence, and sexual dysfunction were completed by the participants. RESULTS: Severe fecal incontinence was more frequently reported by women who had sustained an anal sphincter tear compared to the controls. Women with an anal sphincter tear had no increased risk of urinary incontinence, but reported significantly more pain, difficulty with vaginal lubrication, and difficulty achieving orgasm compared to the controls. A fetal occiput posterior position during childbirth was an independent risk factor for both severe urinary incontinence and severe sexual dysfunction. CONCLUSIONS: Fecal incontinence is strongly associated with an anal sphincter tear. A fetal occiput posterior position represents a risk factor for urinary incontinence and sexual dysfunction.

Comme d'autres institutions académiques européennes, les universités suisses connaissent des changements importants et « l'excellence académique » est devenu un critère central lors de la sélection des dossiers pour les postes de rang professoral. Dans cet article, nous analyserons les critères d'excellence tels qu'ils apparaissent dans le programme Professeur.e.s boursiers.ières du FNS et les confronterons aux résultats d'une recherche sur les trajectoires de la relève académique que nous avons menée à l'Université de Lausanne. Nous montrerons comment la définition de l'excellence académique obéit à des logiques genrées qui rendent l'accession à un poste professoral plus difficile pour les femmes que pour les hommes. Nous verrons pourquoi les critères d'évaluation des sciences dures, qui sont peu à peu transférés vers les sciences humaines, renforcent les mécanismes d'exclusions des femmes des mondes académiques.

BACKGROUND: To date, there is no quality assurance program that correlates patient outcome to perfusion service provided during cardiopulmonary bypass (CPB). A score was devised, incorporating objective parameters that would reflect the likelihood to influence patient outcome. The purpose was to create a new method for evaluating the quality of care the perfusionist provides during CPB procedures and to deduce whether it predicts patient morbidity and mortality. METHODS: We analysed 295 consecutive elective patients. We chose 10 parameters: fluid balance, blood transfused, Hct, ACT, PaO2, PaCO2, pH, BE, potassium and CPB time. Distribution analysis was performed using the Shapiro-Wilcoxon test. This made up the PerfSCORE and we tried to find a correlation to mortality rate, patient stay in the ICU and length of mechanical ventilation. Univariate analysis (UA) using linear regression was established for each parameter. Statistical significance was established when p < 0.05. Multivariate analysis (MA) was performed with the same parameters. RESULTS: The mean age was 63.8 +/- 12.6 years with 70% males. There were 180 CABG, 88 valves, and 27 combined CABG/valve procedures. The PerfSCORE of 6.6 +/- 2.4 (0-20), mortality of 2.7% (8/295), CPB time 100 +/- 41 min (19-313), ICU stay 52 +/- 62 hrs (7-564) and mechanical ventilation of 10.5 +/- 14.8 hrs (0-564) was calculated. CPB time, fluid balance, PaO2, PerfSCORE and blood transfused were significantly correlated to mortality (UA, p < 0.05). Also, CPB time, blood transfused and PaO2 were parameters predicting mortality (MA, p < 0.01). Only pH was significantly correlated for predicting ICU stay (UA). Ultrafiltration (UF) and CPB time were significantly correlated (UA, p < 0.01) while UF (p < 0.05) was the only parameter predicting mechanical ventilation duration (MA). CONCLUSIONS: CPB time, blood transfused and PaO2 are independent risk factors of mortality. Fluid balance, blood transfusion, PaO2, PerfSCORE and CPB time are independent parameters for predicting morbidity. PerfSCORE is a quality of perfusion measure that objectively quantifies perfusion performance.

QUESTION UNDER STUDY: Domestic accidents are an important problem in paediatric medicine. This study was designed to gain a better understanding of burn mechanisms and target prevention. METHODS: Children treated for burn lesions in the Department of Paediatric Surgery between August 2004 and August 2005 were included in this prospective study. The burn mechanisms, the children's ages and the circumstances in which children were burned as well as their home environment variables were analyzed. RESULTS: The current study included eighty-nine patients, aged between 2 months and 15 years. Seventy-eight percent were less than 5 years old. More than half were boys. Hot liquid scalding was the most frequent mechanism. There does not seem to be an increased risk in the immigrant population or in low economic status families. In most cases, an adult person was present at time of injury. CONCLUSIONS: If we were to describe the highest "at risk" candidate for a burn in our region, it would be a boy aged 15 months to 5 years who is burned by a cup of hot liquid on his hand, at home, around mealtime, in the presence of one or both parents. Reduced attention in the safe domestic setting is probably responsible.

BACKGROUND AND OBJECTIVES: Obstructive sleep apnea is associated with significantly increased cardiovascular morbidity and mortality. Fluid overload may promote obstructive sleep apnea in patients with ESRD through an overnight fluid shift from the legs to the neck soft tissues. Body fluid shift and severity of obstructive sleep apnea before and after hemodialysis were compared in patients with ESRD. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: Seventeen patients with hemodialysis and moderate to severe obstructive sleep apnea were included. Polysomnographies were performed the night before and after hemodialysis to assess obstructive sleep apnea, and bioimpedance was used to measure fluid overload and leg fluid volume. RESULTS: The mean overnight rostral fluid shift was 1.27±0.41 L prehemodialysis; it correlated positively with fluid overload volume (r=0.39; P=0.02) and was significantly lower posthemodialysis (0.78±0.38 L; P<0.001). There was no significant difference in the mean obstructive apnea-hypopnea index before and after hemodialysis (46.8±22.0 versus 42.1±18.6 per hour; P=0.21), but obstructive apnea-hypopnea index was significantly lower posthemodialysis (-10.1±10.8 per hour) in the group of 12 patients, with a concomitant reduction of fluid overload compared with participants without change in fluid overload (obstructive apnea-hypopnea index +8.2±16.1 per hour; P<0.01). A lower fluid overload after hemodialysis was significantly correlated (r=0.49; P=0.04) with a lower obstructive apnea-hypopnea index. Fluid overload-assessed by bioimpedance-was the best predictor of the change in obstructive apnea-hypopnea index observed after hemodialysis (standardized r=-0.68; P=0.01) in multivariate regression analysis. CONCLUSIONS: Fluid overload influences overnight rostral fluid shift and obstructive sleep apnea severity in patients with ESRD undergoing intermittent hemodialysis. Although no benefit of hemodialysis on obstructive sleep apnea severity was observed in the whole group, the change in obstructive apnea-hypopnea index was significantly correlated with the change in fluid overload after hemodialysis. Moreover, the subgroup with lower fluid overload posthemodialysis showed a significantly lower obstructive sleep apnea severity, which provides a strong incentive to further study whether optimizing fluid status in patients with obstructive sleep apnea and ESRD will improve the obstructive apnea-hypopnea index.

L’expansion de l’organisation de la production des biens et services sous forme de réseau, découlant de la recherche de flexibilité face à une concurrence économique accrue, soulève des enjeux importants pour les relations industrielles. Notre thèse s’intéresse au processus de précarisation des emplois dans les segments externalisés des réseaux de production. Elle se base sur trois études de cas portant sur les réseaux de services publics et privés d’aide à domicile au Québec. Les études en droit critique ont montré que le droit du travail, fondé sur le modèle de l’entreprise intégrée et de la relation d’emploi bipartite, ne parvient pas à identifier comme employeurs certaines organisations dont l’action a pourtant un effet déterminant sur les conditions de travail dans les réseaux. Notre thèse visait donc à mettre en lumière la réalité empirique des relations de pouvoir qui traversent les réseaux et leurs effets et ce à partir d’un modèle analytique transdisciplinaire intégrant les dimensions juridiques et sociologiques. Les résultats nous amènent d’abord à constater que l’employeur juridique des travailleuses employées par les prestataires privés intégrés aux réseaux – une majorité de femmes – est le plus souvent une agence de location de personnel, une entreprise d’économie sociale ou même l’usager-ère des services d’aide à domicile. Cet employeur est rarement identifié aux organismes publics, donc sous l’égide de l’État, qui exercent un pouvoir prépondérant dans ces réseaux. Le décalage entre la régulation juridique du travail et la réalité empirique du pouvoir intra-réseau entraîne une précarisation de la protection sociale et des conditions de travail de ces travailleuses. Ainsi, la faible protection de leurs droits de représentation et de négociation collectives facilite l’imposition de la « disponibilité permanente juste-à-temps », combinant les logiques du lean et du travail domestique, ainsi qu’une déqualification sexuée et racialisée de leurs emplois par rapport à la situation dans le secteur public. Notre étude montre néanmoins que certaines pratiques innovatrices d’action collective réticulaire ont pu influencer la dynamique de pouvoir, en particulier celles portées par des associations locales de personnes handicapées recourant aux services et par un syndicat de travailleuses d’agence.

Unter dem Schlagwort “(Neo-)Extraktivismus” ist in den vergangenen Jahren eine rege Debatte über die Wirtschafts- und Sozialpolitik der neuen progressiven Regierungen in Lateinamerika entstanden. Kern des Konzepts ist die Kritik an einer hauptsächlich auf Rohstoffausbeutung basierenden Wachstums- und Entwicklungsstrategie, die, so die Vertreter der Debatte, für weite Teile der Region Lateinamerikas und der Karibik (nach wie vor) als vorherrschend betrachtet werden kann. Die geführte Auseinandersetzung basiert jedoch bis dato auf einer schmalen empirischen Grundlage. Der Gegenstand des Working Papers ist die Frage nach der quantitativen Dimension des (Neo-)Extraktivismus in Lateinamerika. Das Paper greift hierzu zentrale Annahmen der Debatte auf und unterzieht sie einer systematischen Analyse. Diese umfasst eine Länderauswahl von fünfzehn Staaten bestehend aus Ländern der Andengemeinschaft, des Cono Sur-Raums und Mittelamerikas. Die zentrale These des Working Papers lautet, dass auf dem Subkontinent nicht von einem einheitlichen Extraktivismus gesprochen werden kann, da sich unterschiedliche Ausprägungen des Phänomens belegen lassen.
