Vitamin D deficiency is highly prevalent worldwide and in our country estimated un 60% in the adult population and closer to 80% in the elderly population and the fractured hip.

Fracturas de escafoides carpiano: estudio retrospectivo en fracturas de polo proximal

Con el objetivo de evaluar los resultados obtenidos en nuestra experiencia con las fracturas de polo proximal de escafoides carpiano nos dispusimos a analizar una serie de variables para constatar los resultados finales en estas fracturas, detectar fallos en nuestros protocolos de actuación y plantear puntos de mejora en su diagnóstico y tratamiento. Para ello se recogieron todos los pacientes diagnosticados de fractura de polo proximal de escafoides entre 2001 y 2007 y de entre ellos analizamos los resultados de aquellos que se intervinieron quirúrgicamente (n=20). Esto incluía tanto aquellos pacientes que presentaban fracturas agudas, así como las pseudoartrosis y los retrasos de consolidación.

Síntesis de nuevos catecoles como precursores de materiales adhesivos bioinspirados

En nuestro grupo de investigación y en colaboración con el Doctor Daniel Ruiz-Molina del Centro de Investigación en Nanociencia y Nanotecnología (CIN2) de este campus universitario, hemos iniciado un proyecto que se plantea lograr biomimetizar ciertas proteínas adhesivas de moluscos, cuya adherencia se debe a la presencia de la funcionalidad catecol. El objetivo es poder desarrollar nuevos materiales adhesivos basados en esa funcionalidad, siendo necesaria por lo tanto el desarrollar protocolos sintéticos para la obtención de compuestos tipo catecol convenientemente sustituidos. Concretamente, en el presente trabajo de Master en Experimentación Química se ha conseguido la síntesis de los nuevos catecoles 1 y 2, que incorporan en su estructura una cadena alquílica acabada con la funcionalidad tiol, con el fin de poder desarrollar más adelante superficies o nanopartículas de oro adhesivas. Para la síntesis del 4-(3’-mercaptopropil)catecol 1 se ha puesto a punto una ruta de obtención partiendo del producto comercial eugenol, consistente en 3 etapas sintéticas y con un rendimiento global cercano al 70%. También se ha desarrollado una ruta sintética para el 4-(7’-mercaptopropil)catecol 2 que parte del 3,4-dimetoxibenzaldehído, y consta de 6 etapas, presentando un rendimiento global del 25%.

Influencia de un programa de actividad física en niños y adolescentes obesos; evaluación del estrés fisiológico mediante compuestos en la saliva; protocolo de estudio.

Different studies4, 5 relate that stress increases in children and adolescents with overweight and obesity, and consequently their salivary physiological responses (AEA salivary alpha-amylase, cortisol, cytokines, leptin), so in this study we relate these two parameters to see their progress through a program of physical activity. If we manage to reduce overweight or obesity, these physiological responses and stress should also be reduced, thus improving the overall health status of these children and adolescents. The overall objective of the study was to determine the influence of physical activity in obese children and adolescents in perceived stress. An observational, descriptive, prospective and longitudinal study will be carried out. The universe is made up of 60 overweight / obese children and adolescents aged between 10 and 18 years. The assessment will take place from September 2012 to September 2013. To collect saliva samples, the ELISA8 method will be used. Variables such as BMI, lifestyle and diet will also be collected. Among the expected results are to lower overweight and obesity in children through physical activity program. To reduce stress and to normalize physiological salivary parameters.

Correcting non cephalic presentation with moxibustion: study protocol for a multi-centre randomised controlled trial in general practice.

BACKGROUND Non cephalic presentation in childbirth involves various risks to both the mother and the foetus. The incidence in Spain is 3.8% of all full-term pregnancies. The most common technique used to end the gestation in cases of non cephalic presentation is that of caesarian section, and although it provokes a lower rate of morbi-mortality than does vaginal delivery in such situations, there remains the possibility of traumatic injury to the foetal head and neck, while maternal morbidity is also increased. The application of heat (moxibustion) to an acupuncture point, in order to correct non cephalic presentation, has been practised in China since ancient times, but as yet there is insufficient evidence of its real effectiveness. METHODS/DESIGN The experimental design consists of a multi-centre randomised controlled trial with three parallel arms, used to compare real moxibustion, sham moxibustion and the natural course of events, among pregnant women with a non cephalic presentation and a gestational duration of 33-35 weeks (estimated by echography). The participants in the trial will be blinded to both interventions. The results obtained will be analyzed by professionals, blinded with respect to the allocation to the different types of intervention. In addition, we intend to carry out a economic analysis. DISCUSSION This trial will contribute to the development of evidence concerning moxibustion in the correction of non cephalic presentations. The primary outcome variable is the proportion of cephalic presentations at term. As secondary outcomes, we will evaluate the proportion of cephalic presentations at week 38 of gestation, determined by echography, together with the safety of the technique, the specificity of moxibustion and the control of the blinding process. Individualized Therapy with Ranibizumab in Wet Age-Related Macular Degeneration.

Individualized treatment regimens may reduce patient burden with satisfactory patient outcomes in neovascular age-related macular degeneration. Intravitreal anti-VEGF drugs are the current gold standard. Fixed monthly injections offer the best visual outcome but this regimen is not commonly followed outside clinical trials. A PRN regimen requires monthly visits where the patient is treated in the presence of signs of lesion activity. Therefore, an early detection of reactivation of the disease with immediate retreatment is crucial to prevent visual acuity loss. Several trials suggest that "treat and extend" and other proactive regimens provide a reasonable approach. The rationale of the proactive regimens is to perform treatment anticipating relapses or recurrences and therefore avoid drops in vision while individualizing patient followup. Treat and extend study results in significant direct medical cost savings from fewer treatments and office visits compared to monthly treatment. Current data suggest that, for one year, PRN is less expensive, but treat and extend regimen would likely be less expensive for subsequent years. Once a patient is not a candidate to continue with treatment, he/she should be sent to an outpatient unit with adequate resources to follow nAMD patients in order to reduce the burden of specialized ophthalmologist services.

Combined vemurafenib and cobimetinib in BRAF-mutated melanoma.

BACKGROUND The combined inhibition of BRAF and MEK is hypothesized to improve clinical outcomes in patients with melanoma by preventing or delaying the onset of resistance observed with BRAF inhibitors alone. This randomized phase 3 study evaluated the combination of the BRAF inhibitor vemurafenib and the MEK inhibitor cobimetinib. METHODS We randomly assigned 495 patients with previously untreated unresectable locally advanced or metastatic BRAF V600 mutation-positive melanoma to receive vemurafenib and cobimetinib (combination group) or vemurafenib and placebo (control group). The primary end point was investigator-assessed progression-free survival. RESULTS The median progression-free survival was 9.9 months in the combination group and 6.2 months in the control group (hazard ratio for death or disease progression, 0.51; 95% confidence interval [CI], 0.39 to 0.68; P<0.001). The rate of complete or partial response in the combination group was 68%, as compared with 45% in the control group (P<0.001), including rates of complete response of 10% in the combination group and 4% in the control group. Progression-free survival as assessed by independent review was similar to investigator-assessed progression-free survival. Interim analyses of overall survival showed 9-month survival rates of 81% (95% CI, 75 to 87) in the combination group and 73% (95% CI, 65 to 80) in the control group. Vemurafenib and cobimetinib was associated with a nonsignificantly higher incidence of adverse events of grade 3 or higher, as compared with vemurafenib and placebo (65% vs. 59%), and there was no significant difference in the rate of study-drug discontinuation. The number of secondary cutaneous cancers decreased with the combination therapy. CONCLUSIONS The addition of cobimetinib to vemurafenib was associated with a significant improvement in progression-free survival among patients with BRAF V600-mutated metastatic melanoma, at the cost of some increase in toxicity. Modelatge i simulació de quatre centres de processament de dades connectats per WAN

Aquest projecte conté l'estudi de la comunicació entre quatre Centres de Processament de Dades (CPD) per via WAN. L'estudi consisteix en simular aquestes comunicacions en un entorn de proves físic i també en entorn de proves virtual on s'han extret resultats dels protocols de seguretat, d'enrutament, a més dels temps de resposta i limitacions dels equips físics i virtuals.

Essências florais: intervenção vibracional de possibilidades diagnósticas e terapêuticas

Este estudo objetivou conhecer à luz da Teoria do Imaginário de Gilbert Durand, por meio do teste AT.9 e sessões de atendimento com essências florais, a eventual ação diagnóstica e terapêutica das mesmas. Realizado com 30 sujeitos, que se tratavam com as essências florais, num consultório particular na cidade de São Paulo. Os instrumentos de análise foram 60 protocolos de AT.9 preenchidos pelos 30 indivíduos em dois momentos e 60 formulações de essências florais. Analisaram-se os traços comuns, afinados e dissonantes, das relações estabelecidas entre o AT.9 e as formulações florais, trazendo evidências da sua capacidade diagnóstica e da sua ação terapêutica, com redução de oito indivíduos desestruturados para somente um. As duas essências que traduziram o tom característico dessa população foram Califórnia Wild Rose e Evening Primrose.
