The study provides an evaluation of health and social services from the perspective of older people themselves and provides an opportunity for older people to express their lifelong care preferences. The National Council on Ageing and Older People strongly endorses the principle that older people should be involved in the development, planning and evaluation of their health and social services. Issued jointly by the Health and Social Care Board and Public Health AgencyThe Health and Social Care Board and the Public Health Agency have today launched, for public consultation, a new Community Development Strategy.The consultation period will run for 12 weeks from Friday 10 June until Friday 2 September 2011.The Board and Agency want to see strong, resilient communities where everyone has good health and wellbeing, places where people look out for each other and have community pride in where they live.Residents from deprived areas in Northern Ireland experience;lower life expectancy;higher rates of emergency admission to hospital;higher rates of lung cancer;higher rates of suicide; andhigher rates of smoking and alcohol related deaths.The kinds of health and social care issues which can be improved by community development approaches include depression; isolation; falls amongst elderly people; child protection; teenage pregnancy; childhood asthma; postnatal depression; drug and alcohol abuse; and ultimately also long term conditions such as obesity, diabetes and cancer.The Board and Agency seek a number of benefits from implementing this strategy such as; a reduction in health and wellbeing inequalities, which also means addressing the social factors that affect health; strengthening partnership working with service users, the community and voluntary sectors and other organisations; strengthening families and communities; supporting volunteering and making best use of our resources.John Compton, Chief Executive of the Health and Social Care Board said: "Community development is an important way to improving health and wellbeing - driving a message that 'prevention is better than cure' between different groups and communities, and helping to ensure the most effective use of the health and social care budget."Now more than ever we need to work in partnership with families and communities to achieve better health and wellbeing for those living in Northern Ireland.No one organisation can meet this challenge on its own and strong partnerships are needed. The Official Food Safety laboratories have a critical role in ensuring food safety and public health for the whole population of the Republic of Ireland. These public health laboratories are made up of 7 microbiological testing laboratories and 3 chemical or Public Analyst’s laboratories. The laboratories are regionally based and offer an accredited (INAB) service to 10 health boards thus spanning the country. The role of the laboratories is to test food for compliance with the relevant legislation and guidelines, identify food-borne hazards and disease outbreaks, provide essential risk assessment information for national and international needs, provide a food testing service for consumers and a water testing service on a national basis. They also participate in dedicated National and EU surveys under the auspices of the Food Safety Authority of Ireland (FSAI). There has been significant investment and development in food-related public health protection in Ireland in recent years. However, there are still a number of issues that have the potential to impact on these laboratories in delivering a fully effective public health service in a cost efficient manner. Building on what has been achieved to date, this strategic review identifies those issues to be addressed in order to ensure (1) a cost effective national co-ordinated food safety laboratory service, (2) that future laboratory service needs are accounted for in the delivery of their Public Health role, and (3) that this Service meets both national and international requirements and standards.

The key element of the HSE South’s Programme is to enhance and develop community mental health services in Carlow, Kilkenny and South Tipperary, to enable the service user to remain in the community to the greatest extent possible. HSE South has prioritised the implementation of the change programme and has allocated more than €20m capital funding and over €1.75m revenue funding to support this comprehensive development programme. Speaking at the briefings Mr. Pat Healy, Regional Director of Operations, HSE South said, “When this plan is delivered, clients will have access to the highest standards of services in all three counties, which should significantly improve these clients’ treatment programmes and quality of life. The National Service Users Executive are supporting the change programme, which is of immense importance to HSE South. The programme heralds the enhancement and development of community mental health services, the closure of old long stay institutions, the separation of North and South Tipperary acute inpatient mental health services and development of appropriate acute inpatient services, for the extended catchment area, in line with the national strategy for mental health “A Vision for Change”. This publication outlines a model of delivery for introductory level Cognitive Behavioural Therapy (CBT) training in the Mental Health Service, HSE South (Carlow, Kilkenny, South Tipperary, Waterford, Wexford). This model has proved useful in guiding the development of four introductory programmes during 2009 and 2010. As a result of this experience, we have amended and updated our programme delivery strategies. We see this process as organic and ever changing, thus these reflections are a snap shot of our current thinking which we have no doubt will evolve as we proceed with future programmes. This booklet will act as a guide for our upcoming programmes in 2010 and 2011 and we believe it may also offer guidance to others who will be involved in the delivery of CBT training within the Irish Mental Health Service.This resource was contributed by The National Documentation Centre on Drug Use.

This document details the HSE's Palliative Care Services - Five Year Medium Term Development Framework (2009 - 2013). It has been developed using a holistic, system-wide, approach to addressing the level of need identified by both the Baseline Study on the Provision of Hospice / Specialist Palliative Care Services in Ireland (2006) as well as the HSE Audit of Palliative Care Service Provision (2007). The HSE Audit was conducted by each of the four HSE Administrative Areas, in conjunction with Area Development Committees, against the recommendations of the Report of the National Advisory Committee on Palliative Care 2001 (the NACPC Report). Therefore, this document is grounded in the recommendations of the NACPC Report, (e.g. palliative care definition, palliative care service areas, staffing and bed number ratios), and informed by the findings of the HSE Audit. The purpose of this document is not to replace the recommendations of the NACPC Report from a policy context, nor does it represent a new national strategy for palliative care. This document details the required actions and initiatives necessary to address the gaps in palliative care service provision, against the recommendations set out in the NACPC Report.

The project objectives were to: - Review the current situation for the population of the East Midlands, across the Tier 4 services and for young people in Out of Area Placements. - Examine current commissioning arrangements across the region and across the agencies of health, social care and education. - Consider the development of a shared framework within which future Tier 4 services could be planned and delivered. - Identify care pathways across the continuum of CAMHS services and highlight any gaps within CAMHS Tier 4 provision.

The Office of the Minister for Integration (OMI), in conjunction with the Department of Education and Science (DES), commissioned an independent review to assist in the development of a national English Language policy and framework for legally-resident adult immigrants. Horwath Consulting Ireland, in association with Rambll Management and Matrix Knowledge Group, were awarded the contract to undertake this assignment. The terms of reference for the assignment state that: “proposed future developments will be governed by a clear strategy which reflects the importance of English language tuition in overall integration objectives and which addresses key coordination, technical, funding and service-delivery issues."

The PHA, through the Regional HSC Personal and Public Involvement Forum, have led on the development of an agreed regional policy to ensure that services users and carers are not out of pocket when involved in helping us to plan, deliver, review and evaluate HSC services. Staff are asked to familiarise themselves with the policy and apply it when involving services users and carers in their work.���

BACKGROUND: Three different burnout types have been described: The "frenetic" type describes involved and ambitious subjects who sacrifice their health and personal lives for their jobs; the "underchallenged" type describes indifferent and bored workers who fail to find personal development in their jobs, and the "worn-out" in type describes neglectful subjects who feel they have little control over results and whose efforts go unacknowledged. The study aimed to describe the possible associations between burnout types and general sociodemographic and occupational characteristics. METHODS: A cross-sectional study was carried out on a multi-occupational sample of randomly selected university employees (n = 409). The presence of burnout types was assessed by means of the "Burnout Clinical Subtype Questionnaire (BCSQ-36)", and the degree of association between variables was assessed using an adjusted odds ratio (OR) obtained from multivariate logistic regression models. RESULTS: Individuals working more than 40 hours per week presented with the greatest risk for "frenetic" burnout compared to those working fewer than 35 hours (adjusted OR = 5.69; 95% CI = 2.52-12.82; p < 0.001). Administration and service personnel presented the greatest risk of "underchallenged" burnout compared to teaching and research staff (adjusted OR = 2.85; 95% CI = 1.16-7.01; p = 0.023). Employees with more than sixteen years of service in the organisation presented the greatest risk of "worn-out" burnout compared to those with less than four years of service (adjusted OR = 4.56; 95% CI = 1.47-14.16; p = 0.009). CONCLUSIONS: This study is the first to our knowledge that suggests the existence of associations between the different burnout subtypes (classified according to the degree of dedication to work) and the different sociodemographic and occupational characteristics that are congruent with the definition of each of the subtypes. These results are consistent with the clinical profile definitions of burnout syndrome. In addition, they assist the recognition of distinct profiles and reinforce the idea of differential characterisation of the syndrome for more effective treatment.

The presentation will focus on the reasons for deploying an e-reader loan service at a virtual university library as a part of an e-learning support system to aid user mobility, concentration of documentary and electronic resources, and ICT skills acquisition, using the example of the UOC pilot project and its subsequent consolidation. E-reader devices at the UOC are an extension of the Virtual Campus. They are offered as a tool to aid user mobility, access to documentary and electronic resources, and development of information and IT skills. The e-reader loan service began as a pilot project in 2009 and was consolidated in 2010. The UOC Library piloted the e-reader loan service from October to December 2009. The pilot project was carried out with 15 devices and involved 37 loans. The project was extended into 2010 with the same number of devices and 218 loans (October 2010). In 2011 the e-reader loan service is to involve 190 devices, thus offering an improved service. The reasons for deploying an e-reader loan service at the UOC are the following: a) to offer library users access to the many kinds of learning materials available at the UOC through a single device that facilitates student study and learning; b) to enhance access to and use of the e-book collections subscribed to by the UOC Library; c) to align with UOC strategy on the development of learning materials in multiple formats, and promote e-devices as an extension of the UOC Virtual Campus, and d) to increase UOC Library visibility within and beyond the institution. The presentation will conclude with an analysis of the key issues to be taken into account at a university library: the e-reader market, the unclear business and license model for e-book contents, and the library's role in promoting new reading formats to increase use of e-collections.

ABSTRACT Innovation is essential for improving organizational performance in both the private and public sectors. This article describes and analyzes the 323 innovation experiences of the Brazilian federal public service that received prizes during the 16 annual competitions (from 1995 to 2012) of the Award for Innovation in Federal Public Management held by the Brazilian National School of Public Administration (ENAP). It is a qualitative and quantitative study in which were employed as categories for analysis the four types of innovation defined in the Copenhagen Manual: product, process, organizational and communication. The survey results allow us to affirm that there is innovation in the public sector, in spite of the skepticism of some researchers and the incipient state of theoretical research on the subject. It was possible to observe that organizational innovation was the one with the highest number of award- -winning experience, followed respectively by process, communication and product innovation, with citizen services and improvement of work processes being the main highlights. The results showed that, although the high incidence of innovation occurs at the national level, a significant number of innovations also occur at the local level, probably because many organizations of the federal government have their actions spread only at this level of government. Concerning the innovative area, health and education predominate, with almost 33% of initiatives, which can be explained by capillarity of these areas and the fact that both maintain a strong interaction with the user. The contributions of this work include the use of theoretical model of innovation analysis in the public sector in Brazil still upcoming, and the systematization of knowledge in empirical basis for this innovation. In this sense, it also contributes to the development of the theory with the presentation of evidence that the characteristics, determinants and consequences of innovation in the public sector differ not only from innovation in the industry, but also from innovation in services in the private sector.

Abstract : Breast cancer incidence rates have increased over the past hundred years, in particular, in Western industrial countries and they continue to rise worldwide. Breast cancer risk has been linked to life exposure to endogenous and exogenous estrogens, and there is increasing concern that exposure to endocrine disruptors which are increasingly accumulating in our environment may also have a role. Using the mouse as model, I have analyzed the physiological role of estrogen signaling in mammary gland development. I have shown that estrogen signaling through the estrogen receptor alpha (ERα) in the mammary epithelium is required for ductal morphogenesis during puberty. Moreover, I have demonstrated that estrogens induce proliferation of mammary epithelial cells through a paracrine mechanism. The presence of estrogen signaling is essential cell intrinsically via ERα or ERβ for the terminal differentiation into milk secreting cells during pregnancy. Furthermore, I have examined how perinatal exposure to the estrogenic plasticizer bisphenol A (BPA) found ubiquitously in consumer goods such as baby bottles formula and beverage containers affects the normal mammary gland development and possibly predispose the mammary gland to tumorigenesis. I have found that C57b16 mice that were exposed, via their drinking water, to several BPA doses ranging from 0.025µg/kg/day to 250µg/kg/day exhibits delayed terminal end bud formation and consequently the ductal outgrowth. Later in life, the mice that were exposed in utero to BPA displayed an increased number of mammary epithelial cells. Acute exposure of 3-week-old mice to BPA can alter gene expression levels of an important estrogen target gene, amphiregulin. Taken together these data are compatible with a scenario in which perinatal BPA exposure may alter mammary gland development by affecting developmental signaling pathways. Résumé : Les taux d'incidence des cancers du sein ont augmenté au cours des cent dernières années en particulier dans les pays industriels occidentaux et ils continuent d'augmenter dans le monde entier. Le risque du cancer du sein a été corrélé à l'exposition au cours de la vie aux oestrogènes endogènes et exogènes. Il y a une préoccupation croissante concernant l'exposition aux perturbateurs endocriniens qui ne cessent de s'accumulent dans notre environnement et qui peuvent également avoir un rôle dans l'augmentation des cancers du sein. En utilisant le modèle de souris, j'ai analysé le rôle physiologique de la voie de signalisation à l'oestrogène dans le développement mammaire. J'ai prouvé que l'oestrogène par l'intermédiaire de son récepteur alpha (ERα) est indispensable dans l'épithélium pour la morphogénèse du système canalaire pendant la puberté. De plus, j'ai démontré que les oestrogènes induisent la prolifération des cellules épithéliales mammaires par un mécanisme paracrine. La présence de la voie de signalisation à l'oestrogène est essentielle de manière intrinsèque à la cellule par l'intermédiaire d'ERα ou ERβ pour la différentiation terminale des cellules épithéliales en cellules sécrétrices de lait pendant la grossesse. En outre, j'ai examiné comment l'exposition périnatale au bisphénol A (BPA), un plastifiant présentant des propriétés ostrogéniques et omniprésent dans divers produits d'usage courant tels que les biberons des bébés et les récipients en plastique, affecte le développement de la glande mammaire et prédispose probablement celle-ci à la tumorigénèse. J'ai constaté que l'exposition périnatale à BPA retarde la formation des bourgeons terminaux et par conséquent la croissance du système canalaire. Plus tard dans la vie, les souris qui ont été exposées dans l'utérus au BPA ont montré un plus grand nombre de cellules épithéliales mammaires. L'exposition aiguë de souris âgées de 3 semaines au BPA perturbe le niveau d'expression d'un gène cible important de l'oestrogène, l'amphiregulin. Ces données sont compatibles avec un scénario dans lequel l'exposition périnatale au BPA peut changer le développement de la glande mammaire en affectant des voies de signalisation développementales.
