Study Objectives: Sleep apnea is common in patients with congestive heart failure, and may contribute to the progression of underlying heart diseae. Cardiovascular and metabolic complications of sleep apnea have been attributed to intermittent hypoxia. Elevated free fatty acids (FFA) are also associated with the progression of metabolic, vascular, and cardiac dysfunction. The objective of this study was to determine the effect of intermittent hypoxia on FFA levels during sleep in patients with heart failure. Design and interventions: During sleep, frequent blood samples were examined for FFA in patients with stable heart (ejection fraction < 40%). In patients with severe sleep apnea (apnea-hypopnea index = 15.4 +/- 3.7 events/h; average low SpO(2) = 93.6%). In patients with severe sleep apnea, supplemental oxygen at 2-4 liters/min was administered on a subsequent night to eliminate hypoxemia. Measurements and Results: Prior to sleep onset, controls and patients with severe apnea exhibited a similar FFA level. After sleep onset, patients with severe sleep apnea exhibited a marked and rapid increase in FFA relative to control subjects. This increase persisted throughout NREM and REM sleep exceeding serum FFA levels in control subjects by 0.134 mmol/L (P = 0.0038) Supplemental oxygen normalized the FFA profile without affecting sleep architecture or respiratory arousal frequency. Conclusion: In patients with heart failure, severe sleep apnea causes surges in nocturnal FFA that may contribute to the accelerated progression of underlying heart disease. Supplemental oxygen prevents that FFA elevation.

Background Although significant associations of childhood adversities with adult mental disorders are widely documented, most studies focus on single childhood adversities predicting single disorders. Aims To examine joint associations of 12 childhood adversities with first onset of 20 DSM-IV disorders in World Mental Health (WMH) Surveys in 21 countries. Method Nationally or regionally representative surveys of 51 945 adults assessed childhood adversities and lifetime DSM-IV disorders with the WHO Composite International Diagnostic Interview (CIDI). Results Childhood adversities were highly prevalent and interrelated. Childhood adversities associated with maladaptive family functioning (e.g. parental mental illness, child abuse, neglect) were the strongest predictors of disorders. Co-occurring childhood adversities associated with maladaptive family functioning had significant subadditive predictive associations and little specificity across disorders. Childhood adversities account for 29.8% of all disorders across countries. Conclusions Childhood adversities have strong associations with all classes of disorders at all life-course stages in all groups of WMH countries. Long-term associations imply the existence of as-yet undetermined mediators.

Background Burden-of-illness data, which are often used in setting healthcare policy-spending priorities, are unavailable for mental disorders in most countries. Aims To examine one central aspect of illness burden, the association of serious mental illness with earnings, in the World Health Organization (WHO) World Mental Health (WMH) Surveys. Method The WMH Surveys were carried out in 10 high-income and 9 low- and middle-income countries. The associations of personal earnings with serious mental illness were estimated. Results Respondents with serious mental illness earned on average a third less than median earnings, with no significant between-country differences (chi(2)(9)=5.5-8.1, P=0.5-0.79). These losses are equivalent to 0.3-0.8% of total national earnings. Reduced earnings among those with earnings and the increased probability of not earning are both important components of these associations: Conclusions These results add to a growing body of evidence that mental disorders have high societal costs. Decisions about healthcare resource allocation should take these costs into consideration.

Background Suicide is a leading cause of death worldwide; however, little information is available about the treatment of suicidal people, or about barriers to treatment. Aims To examine the receipt of mental health treatment and barriers to care among suicidal people around the world. Method Twenty-one nationally representative samples worldwide (n=55 302; age 18 years and over) from the World Health Organization`s World Mental Health Surveys were interviewed regarding past-year suicidal behaviour and past-year healthcare use. Suicidal respondents who had not used services in the past year were asked why they had not sought care. Results Two-fifths of the suicidal respondents had received treatment (from 17% in low-income countries to 56% in high-income countries), mostly from a general medical practitioner (22%), psychiatrist (15%) or non-psychiatrist (15%). Those who had actually attempted suicide were more likely to receive care. Low perceived need was the most important reason for not seeking help (58%), followed by attitudinal barriers such as the wish to handle the problem alone (40%) and structural barriers such as financial concerns (15%). Only 7% of respondents endorsed stigma as a reason for not seeking treatment. Conclusions Most people with suicide ideation, plans and attempts receive no treatment. This is a consistent and pervasive finding, especially in low-income countries. Improving the receipt of treatment worldwide will have to take into account culture-specific factors that may influence the process of help-seeking.

Sports Exerc., Vol. 41, No. 1,. pp. 237-242, 2009. Purpose: Nonpharmacological interventions may lead to an improvement in sleep quality. The objective of our study was to evaluate the effects of acute intensive exercise and chronic exercise on sleep patterns in patients with periodic leg movements (PLM). Methods: The study involved acute and chronic exercise. The acute intensive exercise group consisted of 22 volunteers who underwent a maximum effort test and a polysomnography (PSG) on the same night. The chronic exercise group included. 11 patients who performed 72 physical training sessions undergoing three PSG studies on the night of sessions 1, 36, and 72. Blood samples were collected from both acute and chronic groups for beta-endorphin dosage. Results: Our results showed that both forms of physical exercise lowered PLM levels. The acute physical exercise increased sleep efficiency, rapid eye movement (REM) sleep, and reduced wake after sleep onset, whereas the chronic physical exercise increased sleep efficiency, REM sleep, and reduced sleep latency. We also found a significant negative correlation between beta-endorphin release after acute intensive exercise and PLM levels (r = -0.63). Conclusion: Physical exercise may improve sleep patterns and reduce PLM levels. The correlation between beta-endorphin release after acute intensive exercise and PLM levels might be associated with the impact physical exercise has on the opiodergic system. We suggest that physical exercise may be a useful nonpharmacological treatment for PLM.

In the 1980s the development of the doubly labelled water (DLW) technique made it possible to determine the validity of dietary assessment methods using external, independent markers of intake in free-living populations. Since then, the accuracy of self-reported energy intake (EI) has been questioned on a number of occasions as under-reporting has been found to be prevalent in many different populations. This paper is a review of investigations using the DLW technique in conjunction with self-reported EI measures in groups including adults, children and adolescents, obese persons, athletes, military personnel and trekking explorers. In studies where a person other than the subject is responsible for recording dietary intake, such as parents of young children, EI generally corresponds to DLW determined energy expenditure. However, in instances where the subjects themselves report their intake, EI is generally under-reported when compared with energy expenditure. It was originally believed that this phenomenon of under-reporting was linked to increased adiposity and body size, however, it is now apparent that other factors, such as dietary restraint and socio-economic status, are also involved. This paper therefore aims to present a more comprehensive picture of under-reporting by tying in the findings of many DLW studies with other studies focusing particularly on the characteristics and mechanisms for under-reporting. Awareness of these characteristics and mechanisms will enable researchers to obtain more accurate self-reports of EI using all dietary recording techniques.

Objective: We examined the relationship between self-reported calcium (Cal intake and bone mineral content (BMC) in children and adolescents. We hypothesized that an expression of Ca adjusted for energy intake (El), i.e., Ca density, would be a better predictor of BMC than unadjusted Ca because of underreporting of EI. Methods: Data were obtained on dietary intakes (repeated 24-hour recalls) and BMC (by DEXA) in a cross-section of 227 children aged 8 to 17 years. Bivariate and multivariate analyses were used to examine die relationship between Ca, Ca density, and the dependent variables total body BMC and lumbar spine BMC. Covariates included were height, weight, bone area, maturity age, activity score and El. Results: Reported El compared to estimated basal metabolic rate suggested underreporting of El. Total body and lumbar spine BMC were significantly associated with El, but not Ca or Ca density, in bivariate analyses. After controlling for size and maturity, multiple linear regression analysis revealed unadjusted Ca to be a predictor of BMC in males in the total body (p = 0.08) and lumbar spine (p = 0.01). Unadjusted Ca was not a predictor of BMC at either site in females. Ca density was not a better predictor of BMC at either site in males or females. Conclusions: The relationship observed in male adolescents in this study between Ca intake and BMC is similar to that seen in clinical trials. Ca density did not enable us to see a relationship between Ca intake and BMC in females, which may reflect systematic reporting errors or that diet is not a limiting factor in this group of healthy adolescents.

Purpose: The training program undertaken by many athletes will affect directly the total, habitual energy requirements of that individual. Unless that energy requirement is met via the diet and or supplementation, chronic negative energy balance will ensue, which will have both short-term and long-term effects not only on performance but also on general health. The aim of this research was therefore to determine the energy expenditure (EE) and hence energy requirements of lightweight female rowers and, further, to compare this with their self-reported energy intake (EI). Methods: The El of seven lightweight female rowers was measured using a self-reported 4-d weighed dietary record. EE was determined using the doubly labeled water (DLW) technique over a 14-d period. Results: The mean (+/-SD) age, height, and weight of the subjects was 20 (+/-1.1) yr, 168.8 (+/-4.7) cm, and 60.9 (+/-23) kg, respectively. The rowers self-reported El was 2214 (+/-313) kcal.d(-1) and their total EE was 3957 (+/-1219) kcal.d(-1). After adjusting total EE for changes in body weight (mean (+/-SD) - 1.2 (+/-1.2) kg), the comparison between adjusted El and reported showed a bias to underreporting of 1133 (+/-1539) kcal.d(-1) or 34%. The bias was not consistent across adjusted El, and two of the seven subjects overreported their intake. Conclusions: Due to the underreporting of EI, diet recording may not be an appropriate way of assessing energy requirements in lightweight female rowers. A benefit of accurately determining energy requirements, as with DLW, is that female lightweight rowers will be able to successfully manipulate their EI and achieve the set weight cut-off for participation without compromising their health or performance.

Respiration is altered during different stages of the sleep-wake cycle. We review the contribution of cholinergic systems to this alteration, with particular reference to the role of muscarinic acetylcholine receptors (MAchRs) during rapid eye movement (REM) sleep. Available evidence demonstrates that MAchRs have potent excitatory effects on medullary respiratory neurones and respiratory motoneurones, and are likely to contribute to changes in central chemosensitive drive to the respiratory control system. These effects are likely to be most prominent during REM sleep, when cholinergic brainstem neurones show peak activity levels. It is possible that MAchR dysfunction is involved in sleep-disordered breathing, Such as obstructive sleep apnea. (C) 2002 Elsevier Science B.V. All rights reserved.

Purpose : Myelomeningocele is a complex disease often complicated by obesity for reasons not well understood. The objectives of this study were to determine body composition and energy expenditure of children with MMC. Methods : Resting energy expenditure (REE), body composition and anthropometry were measured in 19 children with MMC (12 M, 7 F). Total energy expenditure (TEE) was estimated using a 3-day activity record. Energy intake (EI) was measured in seven children (5 M, 2 F) with MMC. Data were then compared with predicted values. Results : Mean REE ( n = 19) was 4680 ±1452 kJ/day (96.1 ±18.1% of predicted REE). The range was large (45.8-125.7% of predicted REE). TEE ( n = 7) was 4344 ±2376 kJ/day, hence only 73 34% of predicted TEE. EI ( n = 7) was 6560 ±1329 kJ/day, approximating a normal energy requirement. Overall, BCM was lower than expected values. Conclusions : REE in children with MMC is variable when compared to predicted values. TEE was found to be lower in children with MMC than predicted values and EI was similar to predicted values in this group of seven children. BCM is reduced in children with MMC when compared to expected values.

Este estudo focaliza a constituição do trabalho docente na Educação Infantil (EI), tomando como referência o cenário da EI articulado ao campo profissional, vinculado às especificidades da EI, na indissociabilidade do educar e do cuidar, no contexto de transformações do sistema educativo e da expansão da oferta da EI. Perspectiva compreender os sentidos que emergem do/circulam no trabalho docente na EI das auxiliares de creche e professoras que atuam com crianças de zero a três anos, quando mediadas por um processo de formação. É sustentado pelos pressupostos teórico-metodológicos bakhtinianos vinculados aos referenciais do trabalho docente e da formação no campo da educação infantil, com base numa configuração dialógica da compreensão. Articula essa ancoragem à pesquisa de abordagem qualitativa por meio dos procedimentos metodológicos de observação participante e entrevistas, tendo como campo de estudo a experiência de uma instituição de Educação Infantil. Os resultados demonstram tensionamentos entre formação continuada e vivência profissional, estendendo-se às profissionais docentes em situação funcional díspar diante da perspectiva pedagógica da indissociabilidade do educar e do cuidar. Os sentidos que emergiram e circularam, a partir das rodas de conversa propostas nesta pesquisa, abrangem a complexidade do trabalho docente na EI, configurado por lógicas hierárquicas no exercício da docência. Tais fundamentos incidem na diluição de um trabalho educativo pautado no compromisso pedagógico e na formação das crianças, pois, da forma como vêm sendo ampliadas e flexibilizadas as contratações, intensificam a fragilidade da função docente, no que tange à formação mínima exigida em lei para atuar na EI quanto na formação continuada.

O Transtorno do pânico (TP) é um transtorno mental comum que afeta até 5% da população em algum momento da vida, sendo caracterizada pela presença de ataques de pânico (AP) recorrentes. Constitui uma psicopatologia que pode ser afetada pela privação do sono (PS), relação que ainda é pouco compreendida. Neste contexto, modelos experimentais de AP e de PS são ferramentas úteis na investigação dessa possível correlação, especialmente motivado pela crescente condição de privação de sono, que tem se tornado cada vez mais frequente na sociedade moderna. Assim, este estudo avaliou os efeitos da privação de sono paradoxal (PSP) sobre os limiares dos comportamentos defensivos induzidos por estimulação intracraniana (EI) da MCPD e CS de ratos num modelo experimental de AP, assim como verificou a influência da corticosterona sobre esses limiares. Foram utilizados 160 ratos Wistar machos (300g), organizados em 4 grupos com 40 animais cada, como se segue: Grupo Controle (CTR) submetido à EI, porém sem PSP; Grupo Privação (PRV), submetido à EI e privado por 96 horas; Grupo Privação + Bloqueio da corticosterona (PRB), submetido ao tratamento com metirapona, EI, e privado por 96 horas, e Grupo Controle + Bloqueio da corticosterona (CTB), submetido ao tratamento com metirapona e EI, porém sem privação de sono. Após 10 dias do implante cirúrgico intracraniano de eletrodo na MCPD e CS, os animais passaram por 5 sessões de estimulação, como se segue: 1ª (TRI) considerada triagem - imediatamente antes da privação, 2ª (P48) após 48h de privação, 3ª (P96) após 96h de privação, 4ª (R48) após 48h de retirada da privação e 5ª (R96) após 96h de retirada da privação. As curvas de limiares das respostas individuais de defesa obtidas nas várias sessões de estimulação da MCPD e CS (TRI, P48, P96, R48 e R96) dos ratos foram comparadas entre si, bem como as curvas de limiares de uma dada resposta nos diferentes grupos (CTR, PRV, CTB e PRB). Além disso, os níveis de corticosterona (CORT) foram dosados nas diferentes sessões de EI, e comparadas num mesmo grupo, bem como nos diferentes grupos. No grupo CTR, todos os comportamentos foram iguais em todas as sessões quando comparados à TRI, entretanto, nos animais privados (PRV), o limiar do galope (GLP) reduziu significativamente em R48 e R96, não ocorrendo xix alterações nos demais comportamentos. Em contraste, no grupo PRB, o Trote (TRT) aumentou a partir de P48, enquanto o GLP não foi alterado em nenhuma sessão de EI. Na comparação entre os grupos, em Salto (SLT), Micção (MIC), Exoftalmia (EXO), Imobilidade (IMO), Defecação (DEF), TRT e GLP, não sofreram alterações decorrentes da CORT produzida decorrente da PSP, sugerindo que a corticosterona não altera os comportamentos defensivos característicos do Ataque de Pânico. Em adição, tais resultados sugerem que os efeitos tardios da PSP sobre os limiares de GLP possivelmente se devam a mecanismos neuroquímicos tempo-dependente.

A prevalência de SAOS em crianças é de 0,7-3%, com pico de incidência nos pré-escolares. Fatores anatômicos (obstrução nasal severa, más-formações craniofaciais, hipertrofia do tecido linfático da faringe, anomalias laríngeas, etc.) e funcionais (doenças neuromusculares) predispõem à SAOS na infância. A principal causa da SAOS em crianças é a hipertrofia adenotonsilar. As manifestações clínicas mais comuns são: ronco noturno, pausas respiratórias, sono agitado e respiração bucal. A oximetria de pulso noturna, a gravação em áudio ou vídeo dos ruídos respiratórios noturnos e a polissonografia breve diurna são métodos úteis para triagem dos casos suspeitos de SAOS em crianças, e o padrão-ouro para diagnóstico é a polissonografia em laboratório de sono durante uma noite inteira. Ao contrário dos adultos com SAOS, as crianças costumam apresentar: menos despertares associados aos eventos de apnéia, maior número de apnéias/hipopnéias durante o sono REM e dessaturação mais acentuada da oxihemoglobina mesmo nas apnéias de curta duração. O tratamento da SAOS pode ser cirúrgico (adenotonsilectomia, correção de anomalias craniofaciais, traqueostomia) ou clínico (higiene do sono, pressão positiva contínua nas vias aéreas - CPAP).

Em Pernambuco, a disponibilidade de P é avaliada sem se considerarem características físicas, químicas, ou, mineralógicas dos solos. Este trabalho teve como objetivos determinar os níveis críticos de P em diferentes solos, utilizando-se diferentes extratores químicos, considerando-se a quantidade e a qualidade da fração argila e relacionar as características físicas, químicas e mineralógicas dos solos com os níveis críticos de P. Os solos foram classificados como: Argissolo Amarelo distrocoeso (PAdx); Latossolo Amarelo distrófico (LAd); Argissolo Amarelo distrófico (PAd); Gleissolo háplico eutrófico (GXve) e Espodossolo cárbico duripânico (EKo). Os tratamentos consistiram em sete doses de P, determinadas de acordo com a capacidade máxima de adsorção de P (CMAP). O delineamento experimental foi em blocos ao acaso, com quatro repetições. Foram coletadas amostras de solo na área útil das parcelas. Essa amostragem foi realizada 30 dias após o plantio e aplicação das doses de P, numa profundidade de 0,0-0,2 m. Nessas amostras, determinaram-se o teor de P pelos extratores Mehlich-1, Mehlich-3, Bray-1 e Resina de Troca Iônica (RTA). Com os teores de P no solo e a produção agrícola, determinaram-se os níveis críticos de P no solo. A RTA extraiu mais P independentemente do tipo de solo. Os níveis críticos de P no solo foram dependentes da CMAP, da quantidade de argila e do P remanescente (P-rem), quando o extrator para avaliação do P disponível foi o Mehlich-1. Os níveis críticos no solo determinados pela RTA não foram dependentes de nenhuma característica química e, ou, física dos solos.

O regime da Propriedade Horizontal soluciona muitos dos problemas relacionados com a necessidade de construção/alojamento. O que caracteriza este regime é o fato de as fracções, embora independentes, fazerem parte de um edifício de estrutura unitária, o que leva ao estabelecimento de especiais relações de interdependência entre os condóminos. A Propriedade Horizontal, enquanto meio de organização e de satisfação de necessidades preeminente, é um instituto jurídico com uma acentuada importância económica e social. Cada condómino é proprietário exclusivo da fracção que lhe pertence e com proprietário das partes comuns do edifício. Assim, cada condómino a fazê-lo voluntariamente. Se isto não suceder, está ao seu alcance cobrá-las recorrendo á via judicial. Neste âmbito, a questão de se saber se o adquirente de uma fracção autónoma é responsável por prestações de condomínio já vencidas á data da aquisição assume particular importância. Tal questão divide a doutrina e a jurisprudência, quanto ao sujeito passivo de uma possível acção, sendo que os Condomínios, alheios á problemática, apenas pretendem receber o dinheiro indispensável á prevenção e ,manutenção das partes comuns. Existem, fundamentalmente, duas orientações diversas, partindo ambas de um pressuposto comum: as obrigações em causa são propter rem, ou seja, são obrigações cujo devedor se determina pela titularidade do direito real. A controvérsia surge quando se pretende atribuir apenas a algumas dessas obrigações a ambulatoriedade. Face ás disposições em vigor o nosso ordenamento, comparando regimes legais de outros países, e aprofundando a natureza das obrigações em causa, procede-se a uma analise da contenda, concluindo-se pela transmissibilidade da divida e consequentemente pela responsabilidade de novo proprietário pelo pagamento dos valores anteriores á aquisição que estejam em divida para com o condomínio.
