A colocação em Acolhimento Familiar e a cessação da estadia constituem transições ecológicas, pelas alterações que acarretam e pelos papéis que atribuem aos seus protagonistas. A preparação destas transições visa não eliminar os obstáculos, uma utopia que o grau de mudança não permite, mas diminui-los, para um nível que facilite o desenvolvimento da criança e a integração na fase seguinte da sua vida. Este artigo revê o trabalho de alguns autores que, no Reino Unido, se têm debruçado sobre a preparação da colocação e da cessação do Acolhimento Familiar. O modelo ecológico é utilizado como quadro teórico orientador daquela medida de protecção e identifica um conjunto de factores que potencializam o sucesso das transições.

The importance of hands in the transmission of nosocomial infection has been world wide admitted. However, it is difficult to induce this behavior in health-care workers. The aim of the present work was to point out the importance of hand bacteria colonization, the influence of hand washing and of patient physical examination. One hundred health-care workers were randomly divided in two groups: Group A without hand washing previous to patient physical examination or handling (PPE); group B with hand washing previous to PPE. Direct fingerprint samples in Columbia agar before and after PPE were obtained. The colonies were counted and identified by conventional techniques, and antibiograms according to NCCLS were performed. Before PPE group A participants showed a high number of bacteria regarding group B participants (73.9 Vs 20.7; p < 0.001); 44 out of 50 participants were carriers of potentially pathogen bacteria. No group B participants were carriers of potential pathogen bacteria before PPE. The latter group showed an increase in number of bacteria after PPE (20.7 CFU (before) Vs 115.9 CFU (after); p < 0.001). Sixteen group B participants were contaminated after PPE with potential pathogens such as S. aureus (50% of them meticillin resistant); Escherichia coli, Pseudomonas aeruginosa and Enterococcus faecalis, half of them multiresistant. We can conclude on the importance of these results to implement educational programs and to provide the health-care workers with the proper commodities to fulfill this practice.

RESUMO: Introdução: As doenças cardiovasculares (DCV) são a principal causa de morbilidade, e mortalidade prematura em Portugal e na Europa. A sua causa é multifactorial e a maior parte dos casos resultam de factores de risco modificáveis. O cálculo do RCVG, pretende ser uma estimativa da probabilidade de desenvolver DCV. Este estudo pretende identificar e caracterizar indivíduos em risco de desenvolver doença isquémica coronária e respectivos factores de risco modificáveis, no Concelho de Faro e determinar a sua prevalência e calcular SCORE global de risco cardiovascular Material e métodos: Estudo, observacional transversal, que incluiu um total de 601 individuos, com idades entre os 40-64 anos de idade, residentes no concelho de Faro, inscritos no Centro de Saúde de Faro e que consistiu da avaliação da prevalência dos factores de risco modificáveis da doença isquémica coronária pela utilização de instrumentos específicos. Resultados e discussão: Foram incluídos 601 individuos de ambos os géneros. Trata-se de uma população potencialmente menos literada e num contexto social, cultural, familiar e profissional que, eventualmente, condicionará o estilo de vida e opções em saúde. 55,9% dos indivíduos tem HTA; 42,4 % dos indivíduos tem valor de colesterol total elevado, compatível com dislipidemia (cut off de 200mg/dl); 50,9 % (cut off 190 mg/dl); 8,7% tem valores de glicemia compatíveis com diabetes; 19,8 são fumadores; 72,7% apresenta excesso de peso/obesidade; 37,6% dos indivíduos tem baixo nível de actividade física; 20,1% dos indivíduos apresenta evidência de stress, ansiedade ou depressão, pela avaliação utilizando a EADS. Parece existir evidência de maior proporção de factores de risco entre os homens e uma proporção significativa de indivíduos com importantes factores de risco modificáveis em simultâneo. Mais de metade dos indivíduos tem 3 ou mais factores de risco em simultâneo, com o excesso de peso/obesidade, hipertensão e dislipidemia a serem os com mais comummente associados. 9,7% dos indivíduos tem risco entre 5-9% e apenas 29,3% tem SCORE inferior a 1%. Conclusões: A elevada prevalência dos factores de risco modificáveis e a elevada proporção de indivíduos identificados com esses factores, e com risco moderado e alto de DCV, parece justificar uma atenção redobrada a esse nível e um planeamento dos cuidados de saúde ajustados e específicos para esta realidade.----------------------- ABSTRACT: Introduction: Cardiovascular diseases are the leading cause of morbidity and premature mortality in Portugal and in Europe. The cause is multifactorial and in most cases results from modifiable risk factors. The global cardiovascular risk calculation is intended as an estimate of the likelihood of developing cardiovascular disease. This study aims to identify and characterize individuals at risk of developing ischemic heart disease and their modifiable risk factors in Faro, and determine its prevalence as well as to calculate global cardiovascular risk SCORE. Methodology: This was a cross sectional observational study, which included a total of 601 individuals, aged 40-64 years of age living in Faro, and enrolled atthe Health Centre of Faro. The prevalence of modifiable risk factors of disease ischemic heart was assessed once, after letter invitation, by the use of the following instruments (acrescentar os instrumentos). Data was analysed with (incluir testes estatisticos utilizados) Results and Discussion: We included 601 subjects of both genders. This is a potentially less literate population with a social, cultural and family contextwhich, may eventually constrain lifestyle and health choices. Almost sixty percent (55.9%) of the individuals have hypertension, 42,4% have high total cholesterol value, compatible with dyslipidemia, 8,7 % had blood glucose values compatible with diabetes, 19,8% are smokers, 72,7% are overweight or obese, 37,6% of individuals have low levels of physical activity, 20,1% of individual show evidence of stress, anxiety or depression, as assessed by using the DASS. There seems to be evidence of a greater proportion of risk factors among men and a significant proportion of individuals with major modifiable risk factores simultaneously. More than half of the studied individuals have three or more risk factors simultaneously, being excess weight / obesity, hypertension and dyslipidemia, the most commonly associated. Almost ten percent (9,7%) of the individuals have a risk between 5-9% and only 29,3% has a SCORE less than 1%. Conclusion: The high prevalence of modifiable risk factors and the high proportion of individuals identified with these factors, and with moderate and high risk of CVD, appear to justify further attention at this level and planning of health care interventions specifically adjusted to this reality.

Evidence indicates that exposure to high levels of noise adversely affects human health, and these effects are dependent upon various factors. In hospitals, there are many sources of noise, and high levels exert an impact on patients and staff, increasing both recovery time and stress, respectively. The goal of this pilot study was to develop, implement and evaluate the effectiveness of a training program (TP) on noise reduction in a Neonatal Intensive Care Units (NICU) by comparing the noise levels before and after the implementation of the program. A total of 79 health professionals participated in the study. The measurements of sound pressure levels took into account the layout of the unit and location of the main sources of noise. General results indicated that LAeq levels before implementation of the training program were often excessive, ranging from 48.7 ± 2.94 dBA to 71.7 ± 4.74 dBA, exceeding international guidelines. Similarly following implementation of the training program noise levels remained unchanged (54.5 ± 0.49 dBA to 63.9 ± 4.37 dBA), despite a decrease in some locations. There was no significant difference before and after the implementation of TP. However a significant difference was found for Lp, Cpeak, before and after training staff, suggesting greater care by healthcare professionals performing their tasks. Even recognizing that a TP is quite important to change behaviors, this needs to be considered in a broader context to effectively control noise in the NICU.

RESUMO - O Inquérito Nacional de Saúde é um instrumento de observação em saúde promovido pelo Ministério da Saúde desde 1987, em resposta às necessidades crescentes de informação de saúde ligadas ao estabelecimento e às exigências de planeamento e avaliação decorrentes da instituição do Serviço Nacional de Saúde. O Inquérito Nacional de Saúde conta, até agora, com quatro edições. Os dados provêm através do estudo transversal de amostras probabilísticas da população portuguesa, através de entrevistas no domicílio, utilizando instrumentos e métodos válidos e estáveis. Os indicadores gerados descrevem o estado de saúde, utilização de cuidados de saúde e determinantes de saúde da população portuguesa residente em unidades de alojamento familiar e tem representatividade a nível nacional e regional. Além de útil para a investigação dos padrões e tendências na área da saúde da população, esta informação é importante como elemento de planeamento, na identificação de necessidades em saúde e na avaliação dos eventuais efeitos e impactes das intervenções realizadas na saúde da população. A evolução do Inquérito Nacional de Saúde depende da definição das necessidades de informação epidemiológica por parte do Estado e da sua integração no sistema europeu de inquéritos de saúde. --------------------------ABSTRACT – The National Health Interview Survey is an instrument of observation in health promoted by the Ministry of Health since 1987, in response to the growing demand for health information, derived from the institution of the National Health Service, as well as its planning and evaluation activities. The National Health Interview Survey has had four editions so far. Data are obtained through the cross-sectional study of probabilistic samples of the Portuguese population, through interviews at home, using valid and stable instruments and methods. Obtained indicators describe the state of health, use of health care and health determinants of the Portuguese population living in familiar dwellings, and are representative at national and regional levels. Besides its usefulness for the study of patterns and trends, this information is important for planning purposes, in the identification of health needs and for the evaluation of the eventual effects of the planned health interventions. The future of the National Health Interview Survey depends on the State’s definition of the needs of epidemiological information and also on its participation in the European system of health surveys. This work describes a novel use for the polymeric film, poly(o-aminophenol) (PAP) that was made responsive to a specific protein. This was achieved through templated electropolymerization of aminophenol (AP) in the presence of protein. The procedure involved adsorbing protein on the electrode surface and thereafter electroploymerizing the aminophenol. Proteins embedded at the outer surface of the polymeric film were digested by proteinase K and then washed away thereby creating vacant sites. The capacity of the template film to specifically rebind protein was tested with myoglobin (Myo), a cardiac biomarker for ischemia. The films acted as biomimetic artificial antibodies and were produced on a gold (Au) screen printed electrode (SPE), as a step towards disposable sensors to enable point-of-care applications. Raman spectroscopy was used to follow the surface modification of the Au-SPE. The ability of the material to rebind Myo was measured by electrochemical techniques, namely electrochemical impedance spectroscopy (EIS) and square wave voltammetry (SWV). The devices displayed linear responses to Myo in EIS and SWV assays down to 4.0 and 3.5 μg/mL, respectively, with detection limits of 1.5 and 0.8 μg/mL. Good selectivity was observed in the presence of troponin T (TnT) and creatine kinase (CKMB) in SWV assays, and accurate results were obtained in applications to spiked serum. The sensor described in this work is a potential tool for screening Myo in point-of-care due to the simplicity of fabrication, disposability, short time response, low cost, good sensitivity and selectivity.

A gold screen printed electrode (Au-SPE) was modified by merging Molecular Imprinting and Self-Assembly Monolayer techniques for fast screening cardiac biomarkers in point-of-care (POC). For this purpose, Myoglobin (Myo) was selected as target analyte and its plastic antibody imprinted over a glutaraldehyde (Glu)/cysteamine (Cys) layer on the gold-surface. The imprinting effect was produced by growing a reticulated polymer of acrylamide (AAM) and N,N′-methylenebisacrylamide (NNMBA) around the Myo template, covalently attached to the biosensing surface. Electrochemical impedance spectroscopy (EIS) and cyclic voltammetry (CV) studies were carried out in all chemical modification steps to confirm the surface changes in the Au-SPE. The analytical features of the resulting biosensor were studied by different electrochemical techniques, including EIS, square wave voltammetry (SWV) and potentiometry. The limits of detection ranged from 0.13 to 8 μg/mL. Only potentiometry assays showed limits of detection including the cut-off Myo levels. Quantitative information was also produced for Myo concentrations ≥0.2 μg/mL. The linear response of the biosensing device showed an anionic slope of ~70 mV per decade molar concentration up to 0.3 μg/mL. The interference of coexisting species was tested and good selectivity was observed. The biosensor was successfully applied to biological fluids.

Recent studies have shown differences in the epidemiology of invasive infections caused by Candida species worldwide. In the period comprising August 2002 to August 2003, we performed a study in Santa Casa Complexo Hospitalar, Brazil, to determine Candida species distribution associated with candidemia and their antifungal susceptibility profiles to amphotericin B, fluconazole and itraconazole. Antifungal susceptibility was tested according to the broth microdilution method described in the NCCLS (M27A-2 method). Only one sample from each patient was analyzed (the first isolate). Most of the episodes had been caused by species other than C. albicans (51.6%), including C. parapsilosis (25.8%), C. tropicalis (13.3%), C. glabrata (3.3%), C. krusei (1.7%), and others (7.5%). Dose-dependent susceptibility to itraconazole was observed in 14.2% of strains, and dose-dependent susceptibility to fluconazole was found in 1.6%. Antifungal resistance was not found, probably related to low use of fluconazole. Further epidemiological surveillance is needed.

The protozoan Cryptosporidium sp. has been frequently detected in faeces from children with persistent diarrhoea. This work achieved to investigate an outbreak of cryptosporidiosis, in a day care center, attending children of high socio-economic level, between 0 and six years old. The outbreak was detected through the network of public health, when stool samples, not diarrhoeic, were examined at the Parasitology Service of the Adolfo Lutz Institute. Among the 64 examined children, 13 (20.3%) showed oocysts of Cryptosporidium sp. in the faeces examined by Kinyoun technique: seven children one year old, three, two years old and three, three years old. Among the 23 examined adults, only a 22 years old woman, possibly having an immunocomprometiment, was positive. Clinical and epidemiological aspects were investigated by questionnaires, highlighting the occurrence of the outbreak in a very dry period.
