The home support services are a social response in order to improve the quality of life directed predominantly for the elderly and for people with varying degrees of disability and dependence. Examples of those services are hygiene and personal comfort, medication, housekeeping and cleaning, preparation and monitoring of the meals; the dressing, etc. It is necessary to make society aware of the importance of these services to all those who need them. The general objective was to understand the most important relationships among informal caregivers, those who are care and home support services providers. Material and Methods. Data were collected through a questionnaire, using the various dimensions of the construct Quality SERVPERF model of service that matches the 22 items of SERVQUAL model. The various items used to assess the perception of care individuals and informal caregivers about the quality of home care services. 82 individuals participated providers of informal care, to receive home support services, and exclusion criteria, the fact of having a diagnosed psychiatric illness or psychological factors that prevent them from responding. The analysis was performed with SPSS and SEM-PLS for the estimation of the proposed structural model. Written consent was obtained, free and clear of each subject. Results and Conclusions. The results showed that the relationships with healthcare professionals are the most important positive effects on satisfaction. This research emphasizes the need to work closely with health professionals to improve the relationship between technicians and patients. Although current constructs appear to explain much of the satisfaction, it is recommended that the future researches exploit new variables, to get a better understanding of the effects of public health policies on the quality of life of these patients.

As the physiological impact of chronic stress is difficult to study in humans, naturalistic stressors are invaluable sources of information in this area. This review systematically evaluates the research literature examining biomarkers of chronic stress, including neurocognition, in informal dementia caregivers. We identified 151 papers for inclusion in the final review, including papers examining differences between caregivers and controls as well as interventions aimed at counteracting the biological burden of chronic caregiving stress. Results indicate that cortisol was increased in caregivers in a majority of studies examining this biomarker. There was mixed evidence for differences in epinephrine, norepinephrine and other cardiovascular markers. There was a high level of heterogeneity in immune system measures. Caregivers performed more poorly on attention and executive functioning tests. There was mixed evidence for memory performance. Interventions to reduce stress improved cognition but had mixed effects on cortisol. Risk of bias was generally low to moderate. Given the rising need for family caregivers worldwide, the implications of these findings can no longer be neglected.

Older people have been identified as being at risk of social exclusion. However, despite the fact that care is commonly required in later life and the majority of that care provided by informal carers, a connection between social exclusion and informal care-receipt has rarely been considered. The aim of this study was to examine how informal care-receipt is related to social exclusion. A face-to-face questionnaire survey on social exclusion and informal care-receipt was carried out among older people (n=1255) living in Barnsley, United Kingdom. Multivariable analyses examined the association between social exclusion and categories of informal care-receipt: care receiver; assurance receiver; non-receiver with no need; non-receiver with need. Compared to being a non-receiver with no need participants were more likely to be a care receiver or assurance receiver if they had higher levels of social exclusion. The highest level of social exclusion, however, was found in non-receivers with need. Despite a lack of informal care and support, formal practical support and personal care was also low in this latter group. Findings are discussed in relation to the conceptualisation of care-receipt and how contact with medical services could be an opportunity for identification and appropriate referral of non-receivers with need.

The general aim of this dissertation is to describe and analyse patterns of informal care and support for carers in Sweden. One specific aim is to study patterns of informal care from a broad population perspective in terms of types of care and types of carer. A typology of four different care categories based on what carers do revealed that women were much more likely than men to be involved at the ‘heavy end’ of caring, i.e. providing personal care in combination with a variety of other caring tasks. Men were more likely than women to provide some kind of practical help (Study I). Another aim is to investigate which support services are received by which types of informal caregiver. Relatively few informal caregivers in any care category were found to be receiving any kind of support from municipalities or voluntary organizations, for example training or financial assistance (Study II). The same study also examines which kinds of help care recipients receive in addition to that provided by informal carers. It appears that people in receipt of personal care from an informal caregiver quite often also receive help from the public care system, in this case mostly municipal services. However, the majority of those receiving personal, informal care did not receive any help from the public care system or from voluntary organizations or for-profit agencies (Study II). The empirical material in studies I and II comprises survey data from telephone interviews with a random sample of residents in the County of Stockholm aged between 18 and 84. In a number of countries there is a growing interest among social scientists and social policymakers in examining the types of support services that might be needed by people who provide informal care for older people and others. A further aim of the present dissertation is therefore to describe and analyse the carer support that is provided by municipalities and voluntary organizations in Sweden. The dissertation examines whether this support is aimed directly or indirectly at caregivers and discusses whether the Swedish government’s special financial investment in help for carers actually led to any changes in the support provided by municipalities and voluntary organisations. The main types of carer support offered by the municipalities were payment for care-giving, relief services and day care. The chief forms of carer support provided by the voluntary organizations were support groups, training groups, and a number of services aimed primarily at the elderly care recipients (Study III). Patterns of change in municipal carer support could be discerned fairly soon. The Swedish government’s special allocation to municipalities and voluntary organisations appears to have led to an increase in the number of municipalities providing direct support for carers, such as training, information material and professional caregiver consultants. On the other hand, only minor changes could be discerned in the pattern of carer support services provided by the voluntary organizations. This demonstrates stability and the relatively low impact that policy initiatives seem to have on voluntary organizations as providers (Study IV). In studies III and IV the empirical material consists of survey data from mail questionnaires sent to municipalities and voluntary organizations in the County of Stockholm. In the fields of social planning and social work there appears to be a need to clarify the aims of support services for informal carers. Should the support be direct or indirect? Should it be used to supplement or substitute caregivers? In this process of reappraisal it will be important to take the needs of both caregivers and care recipients into account when developing existing and new forms of support. How informal caregivers and care recipients interact with the care system as a whole is undeniably a fertile field for further research.

This PhD was driven by an interest for inclusive and participatory approaches. The methodology that bridges science and society is known as 'citizen science' and is experiencing a huge upsurge worldwide, in the scientific and humanities fields. In this thesis, I have focused on three topics: i) assessing the reliability of data collected by volunteers; ii) evaluating the impact of environmental education activities in tourist facilities; and iii) monitoring marine biodiversity through citizen science. In addition to these topics, during my research stay abroad, I developed a questionnaire to investigate people's perceptions of natural areas to promote the implementation of co-management. The results showed that volunteers are not only able to collect sufficiently reliable data, but that during their participation in this type of project, they can also increase their knowledge of marine biology and ecology and their awareness of the impact of human behaviour on the environment. The short-term analysis has shown that volunteers are able to retain what they have learned. In the long term, knowledge is usually forgotten, but awareness is retained. Increased awareness could lead to a change in behaviour and in this case a more environmentally friendly attitude. This aspect could be of interest for the development of environmental education projects in tourism facilities to reduce the impact of tourism on the environment while adding a valuable service to the tourism offer. We also found that nature experiences in childhood are important to connect to nature in adulthood. The results also suggest that membership or volunteering in an environmental education association could be a predictor of people's interest in more participatory approaches to nature management. In most cases, the COVID -19 pandemic had not changed participants' perceptions of the natural environment.

In questo elaborato viene trattata l’analisi del problema di soft labeling applicato alla multi-document summarization, in particolare vengono testate varie tecniche per estrarre frasi rilevanti dai documenti presi in dettaglio, al fine di fornire al modello di summarization quelle di maggior rilievo e più informative per il riassunto da generare. Questo problema nasce per far fronte ai limiti che presentano i modelli di summarization attualmente a disposizione, che possono processare un numero limitato di frasi; sorge quindi la necessità di filtrare le informazioni più rilevanti quando il lavoro si applica a documenti lunghi. Al fine di scandire la metrica di importanza, vengono presi come riferimento metodi sintattici, semantici e basati su rappresentazione a grafi AMR. Il dataset preso come riferimento è Multi-LexSum, che include tre granularità di summarization di testi legali. L’analisi in questione si compone quindi della fase di estrazione delle frasi dai documenti, della misurazione delle metriche stabilite e del passaggio al modello stato dell’arte PRIMERA per l’elaborazione del riassunto. Il testo ottenuto viene poi confrontato con il riassunto target già fornito, considerato come ottimale; lavorando in queste condizioni l’obiettivo è di definire soglie ottimali di upper-bound per l’accuratezza delle metriche, che potrebbero ampliare il lavoro ad analisi più dettagliate qualora queste superino lo stato dell’arte attuale.

Este trabalho apresenta uma reflexão sobre as "Ações do Programa de Saúde da Família (PSF) direcionadas ao cuidador no contexto domiciliar", fruto de uma pesquisa feita durante o período em que atuei neste Programa com idosos dependentes, no bairro Serra Verde, em Belo Horizonte. O estudo discute a importância do papel do cuidador familiar de idosos dependentes e sua relação com a equipe que atua no PSF, e analisa as ações de suporte técnico e emocional de que ele necessita. A partir de uma análise da literatura brasileira sobre o assunto, publicada no período de 2000 a 2010, observou-se que a visita domiciliar consiste no principal instrumento de abordagem das equipes de PSF, seguida, com expressão menor, pelos grupos operativos e ações educativas. O grande desafio que se apresenta consiste em se realizar estratégias eficazes direcionadas para o suporte ao familiar cuidador - que se ressente de uma sobrecarga emocional, não raro afetando o próprio idoso que acaba se assumindo como um "peso" ou um "rejeitado" - aliadas à necessidade crescente de um programa de cursos e pesquisas voltado para os profissionais dessa área possibilitando o conhecimento de técnicas mais eficazes e um maior envolvimento com a realidade dominante nos domicílios. Cabe um programa de educação continuada, em que os profissionais se sintam valorizados e responsáveis, com oportunidade para uma reflexão imprescindível sobre sua própria velhice. Estudos sobre as formas de se atuar dentro deste contexto são necessários para o auxílio na formulação de estratégias e de políticas direcionadas a esta temática.

Este estudo faz uma reflexão sobre o cuidador informal de idosos a partir de uma revisão de literatura cujo objetivo foi compreender as necessidades do cuidador informal de idosos, que, em razão das dificuldades decorrentes do ato de cuidar, acaba sendo inserido em um quadro de vulnerabilidade às doenças. Como cuidador informal considerou-se aquela pessoa, da família ou não, que assume total responsabilidade em prestar assistência à pessoa idosa que apresenta dependência. Foram utilizadas as bases de dados LILACS (Literatura Latino-Americana em Ciências de Saúde), MEDLINE (Medical Literature Analysis and Retrieval Sistem on-line) e SCIELO (Scientific Eletronic Library On-line), a partir dos seguintes descritores: idosos, cuidador informal, assistência ao cuidador; sendo publicadas no idioma português, no período de 1993 a agosto de 2010. Os temas destacados para análise foram a escolha do cuidador, problemas mais comuns encontrados pelo cuidador e suporte ao cuidador informal de idosos. Constatou-se que os cuidadores informais de idosos não estão preparados para assumir tal função. Observa-se a urgência em se criar, no âmbito do sistema público de saúde, programas de atenção ao cuidador informal de idosos, mais especificamente no contexto da Estratégia de Saúde da Família, já que esta, reafirma e incorpora os princípios do Sistema Único de Saúde, sendo possível, por meio da observância de suas diretrizes e preceitos, o estabelecimento de meios para que se possa oferecer um suporte adequado aos cuidadores, idosos e familiares, de modo a lhes garantir qualidade de vida e dignidade.

Acidente de trabalho é o evento súbito ocorrido no exercício de atividade profissional, independentemente da situação empregatícia e previdenciária do trabalhador acidentado, e que acarreta dano à saúde, potencial ou imediato, provocando lesão corporal ou perturbação funcional que causa direta ou indiretamente a morte, ou a perda ou redução, permanente ou temporária, da capacidade para o trabalho. Os acidentes de trabalho têm expressiva morbimortalidade constituindo-se em importante problema de Saúde Pública. O objetivo deste estudo foi descrever o perfil produtivo do trabalho formal e informal do município de Ibirité-MG e refletir sobre a subnotificação de acidentes de trabalho neste município, com vistas embasar a definição de estratégias para a produção de informações mais fidedignas em saúde do trabalhador. Utilizou-se a base de dados Bireme, Lilacs, SciELO e de Sistemas de Informação em Saúde, como o Sistema de Informação dos Agravos de Notificação (SINAN) e o Sistema de Informação da Atenção Básica (SIAB), para obtenção de dados sobre o município em estudo. Os resultados apontaram que no município de Ibirité, a falta de conhecimento sobre o perfil produtivo formal e informal e a ocorrência de subnotificação dos acidentes de trabalho são problemas que interferem no planejamento e desenvolvimento das ações de saúde do trabalhador no âmbito do SUS. Com o conhecimento adquirido neste estudo, propomos ações de mobilização dos profissionais da saúde responsáveis por notificar os acidentes de trabalho no município, enfatizando sobre a importância da notificação através de capacitações, cartilhas explicativas e sensibilização dos administrativos das unidades notificadoras para assim poderem cobrar a notificação desses profissionais notificadores.

Crohn's disease (CD) is associated with complex pathogenic pathways involving defects in apoptosis mechanisms. Recently, mesenteric adipose tissue (MAT) has been associated with CD ethiopathology, since adipose thickening is detected close to the affected intestinal area. However, the potential role of altered apoptosis in MAT of CD has not been addressed. To evaluate apoptosis in the intestinal mucosa and MAT of patients with CD. Samples of intestinal mucosa and MAT from patients with ileocecal CD and from non-inflammatory bowel diseases patients (controls) were studied. Apoptosis was assessed by TUNEL assay and correlated with the adipocytes histological morphometric analysis. The transcriptional and protein analysis of selected genes and proteins related to apoptosis were determined. TUNEL assay showed fewer apoptotic cells in CD, when compared to the control groups, both in the intestinal mucosa and in MAT. In addition, the number of apoptotic cells (TUNEL) correlated significantly with the area and perimeter of the adipose cells in MAT. Transcriptomic and proteomic analysis reveal a significantly lower transcript and protein levels of Bax in the intestinal mucosa of CD, compared to the controls; low protein levels of Bax were found localized in the lamina propria and not in the epithelium of this tissue. Furthermore, higher level of Bcl-2 and low level of Caspase 3 were seen in the MAT of CD patients. The defective apoptosis in MAT may explain the singular morphological characteristics of this tissue in CD, which may be implicated in the pathophysiology of the disease.

Previous studies from our group have demonstrated the protective effect of S-nitroso-N-acetylcysteine (SNAC) on the cardiovascular system in dyslipidemic LDLr-/- mice that develop atheroma and left ventricular hypertrophy after 15 days on a high fat diet. We have shown that SNAC treatment attenuates plaque development via the suppression of vascular oxidative stress and protects the heart from structural and functional myocardial alterations, such as heart arrhythmia, by reducing cardiomyocyte sensitivity to catecholamines. Here we investigate the ability of SNAC to modulate oxidative stress and cell survival in cardiomyocytes during remodeling and correlation with β₂-AR signaling in mediating this protection. Ventricular superoxide (O₂⁻) and hydrogen peroxide (H₂O₂) generation was measured by HPLC methods to allow quantification of dihydroethidium (DHE) products. Ventricular histological sections were stained using terminal dUTP nick-end labeling (TUNEL) to identify nuclei with DNA degradation (apoptosis) and this was confirmed by Western blot for cleaved caspase-3 and caspase-7 protein expression. The findings show that O₂⁻ and H₂O₂ production and also cell apoptosis were increased during left ventricular hypertrophy (LVH). SNAC treatment reduced oxidative stress during on cardiac remodeling, measured by decreased H₂O₂ and O₂⁻ production (65% and 52%, respectively), and a decrease in the ratio of p-Ser1177 eNOS/total eNOS. Left ventricle (LV) from SNAC-treated mice revealed a 4-fold increase in β₂-AR expression associated with coupling change to Gi; β₂-ARs-S-nitrosation (β₂-AR-SNO) increased 61%, while apoptosis decreased by 70%. These results suggest that the cardio-protective effect of SNAC treatment is primarily through its anti-oxidant role and is associated with β₂-ARs overexpression and β₂-AR-SNO via an anti-apoptotic pathway.

The presynaptic action of Bothriopsis bilineata smaragdina (forest viper) venom and Bbil-TX, an Asp49 PLA2 from this venom, was examined in detail in mouse phrenic nerve-muscle (PND) preparations in vitro and in a neuroblastoma cell line (SK-N-SH) in order to gain a better insight into the mechanism of action of the venom and associated Asp49 PLA2. In low Ca(2+) solution, venom (3μg/ml) caused a quadriphasic response in PND twitch height whilst at 10μg/ml the venom additionally induced an abrupt and marked initial contracture followed by neuromuscular facilitation, rhythmic oscillations of nerve-evoked twitches, alterations in baseline and progressive blockade. The venom slowed the relaxation phase of muscle twitches. In low Ca(2+), Bbil-TX [210nM (3μg/ml)] caused a progressive increase in PND twitch amplitude but no change in the decay time constant. Venom (10μg/ml) and Bbil-TX (210nM) caused minor changes in the compound action potential (CAP) amplitude recorded from sciatic nerve preparations, with no significant effect on rise time and latency; tetrodotoxin (3.1nM) blocked the CAP at the end of the experiments. In mouse triangularis sterni nerve-muscle (TSn-m) preparations, venom (10μg/ml) and Bbil-TX (210nM) significantly reduced the perineural waveform associated with the outward K(+) current while the amplitude of the inward Na(+) current was not significantly affected. Bbil-TX (210nM) caused a progressive increase in the quantal content of TSn-m preparations maintained in low Ca(2+) solution. Venom (3μg/ml) and toxin (210nM) increased the calcium fluorescence in SK-N-SH neuroblastoma cells loaded with Fluo3 AM and maintained in low or normal Ca(2+) solution. In normal Ca(2+), the increase in fluorescence amplitude was accompanied by irregular and frequent calcium transients. In TSn-m preparations loaded with Fluo4 AM, venom (10μg/ml) caused an immediate increase in intracellular Ca(2+) followed by oscillations in fluorescence and muscle contracture; Bbil-TX did not change the calcium fluorescence in TSn-m preparations. Immunohistochemical analysis of toxin-treated PND preparations revealed labeling of junctional ACh receptors but a loss of the presynaptic proteins synaptophysin and SNAP25. Together, these data confirm the presynaptic action of Bbil-TX and show that it involves modulation of K(+) channel activity and presynaptic protein expression.
