Gottfried Leibniz generalized the derivation and integration, extending the operators from integer up to real, or even complex, orders. It is presently recognized that the resulting models capture long term memory effects difficult to describe by classical tools. Leon Chua generalized the set of lumped electrical elements that provide the building blocks in mathematical models. His proposal of the memristor and of higher order elements broadened the scope of variables and relationships embedded in the development of models. This paper follows the two directions and proposes a new logical step, by generalizing the concept of junction. Classical junctions interconnect system elements using simple algebraic restrictions. Nevertheless, this simplistic approach may be misleading in the presence of unexpected dynamical phenomena and requires including additional “parasitic” elements. The novel γ-junction includes, as special cases, the standard series and parallel connections and allows a new degree of freedom when building models. The proposal motivates the search for experimental and real world manifestations of the abstract conjectures.

O termo necrose intestinal traduz exclusivamente um conceito anatomopatológico e clínco, e implica sempre um isquémia instestinal oclusiva ou não. A enterocolite necrosante, em sentido lato, implica uma isquémia intestinal não oclusiva associada a um mecanismo infeccioso. O factor desencadeante da necrose é, por vezes, difícil de determinar. A enterocolite necrosante ocorre em 90% dos casos em recém-nascidos prematuros, sendo mesmo frequente no recém-nascido de termo e rara na crança mais velha e no adulto. Apresentam-se os casos clínicos de duas crianças; uma de sete anos com antecedentes de neutropénia crónica e outra de onze anos com défice cognitivo grave, dismorfia e alterações de comportamento, ambas internadas por quadro de abdómen agudo e choque. Foi efectuada ressecção do segmento jejunal num dos casos e ressecção subtotal do cólon no outro, por necrose. Apesar da cirurgia e da terapêutica médica de suporte, ambas acabaram por morrer em falência multiorgânica, respectivamente três horas após a cirurgia e ao 14º dia de internamento, após segunda intervenção com ressecção do segmento necrosado íleon. A necrópsia de ambos os casos revelou necrose extensa de todo o restante intestino. Os dois casos clínicos comportaram-se como enterocolite necrosante da criança, sendo o diagnóstico de exclusão numa delas, de enterocolite neutropénica. Efectua-se uma revisão dos mecanismos etiopatogénicos da necrose intestinal da criança, desde os tromboembólicos, obstrutivos extrínsecos ou endoluminais, inflamatórios, isquémicos não oclusivos, até aos infecciosos. Para além das medidas gerais de terapêutica, salienta-se a necessidade de uma intervenção cirúrgica precose e de meios diagnósticos terapêuticos, como o doseamento, no sangue e na urina, da proteína de ligação aos ácidos gordos intestinais e a arteriografia selectiva.

Considering that there are some studies with autopsies from AIDS describing only malignant neoplasias and that changes can occur after the introduction of Highly Active Antiretroviral Therapy (HAART), our objectives were to analyze the frequency of benign and malignant neoplasms in AIDS patients in the periods of both pre- and post-HAART. This is a retrospective study with 261 autopsies of HIV-positive patients between 1989 and 2008 in Uberaba, Brazil. Sixty-six neoplasms were found (39 benign, 21 malignant and six premalignant) in 58 patients. The most frequent malignant neoplasms were lymphoid, in 2.7% (four Non-Hodgkin lymphoma, one Hodgkin, one multiple myeloma and one plasmablastic plasmacytoma), and Kaposi's Sarcoma, in 2.3% (six cases). The most frequent benign neoplasms were hepatic hemangiomas in 11 (4.2%) of 261 cases and uterine leiomyoma in 11 (15.7%) of 70 woman. In the pre-HAART period eight (9.8%) benign neoplasias and four (4.9%) malignant occurred in 82 patients; in the post-HAART period, 29 (16.2%) benign and 17 (9.5%) malignant were present; however, the differences were not significant. We conclude that the introduction of HAART in our region doesn't look to have modified the frequency of neoplasms occurring in patients with HIV.

Involvement of the digestive system in patients with acquired immunodeficiency syndrome (AIDS) is frequent and many changes in these patients are diagnosed only at autopsy. There are few studies of autopsy with detailed analysis of this system and only one was conducted in Brazil. We evaluated each segment of the digestive system in 93 consecutive autopsies of patients infected with human immunodeficiency virus (HIV) and the importance of these lesions to death. Of these, 90 (96.8%) patients had AIDS. We reviewed medical records, autopsy reports and histological sections from tongue to rectum stained with hematoxylin-eosin. When necessary, we analyzed special stains and immunohistochemistry to investigate infections. There was damage to the digestive system in 73 (78.5%) cases. The most common infections were candidiasis (42%), cytomegalovirus (29%), histoplasmosis (11.8%), toxoplasmosis (9.7%) and mycobacterial infection (9.7%). Malignancies were rare, present in four (4.3%) cases (two Kaposi's sarcoma, one adenocarcinoma and one metastatic embryonal carcinoma). All segments showed lesions: tongue (48.6%), esophagus (44.8%), stomach (44.7%), colon (43.2%) and small intestine (28.9%). The lesions found were immediate cause of death in five (5.4%) cases. In another 36 (38.7%) cases the basic disease was systemic and also compromised the digestive system.

Chronic leg ulcers are persistent conditions that might be a diagnostic and therapeutic challenge, with great impact in health care costs and patients’ quality of life. We report a case of a 60-year-old woman, with long-lasting recalcitrant leg ulcers, which led to left leg amputation 10 years ago. Several attempts to heal the right leg were made, including skin grafting in three different occasions and several surgical debridements, all with unsatisfactory outcome. Some months before the ulcers began, the patient had been diagnosed with undifferentiated connective tissue disease because of arthralgia and positive antinuclear antibodies, therefore low dose systemic corticosteroids and azathioprine were prescribed. For the last 4 years she has been followed in our department and since then no evidence of clinical or laboratorial criteria for autoimmune diseases was found, thus the immunosuppressive therapy was stopped. She maintained ever since a high rheumatoid factor but without other evidence of autoimmune disease. Medical history was otherwise irrelevant. Several cutaneous biopsies were performed, with no evidence of malignancy or vasculitis. Recently, cryoglobulins became positive, with type 2b cryoglobulin identification on immunofluorescence. Serology for Hepatitis C virus was consistently negative, hence an Essential type 2 Cryoglobulinemia diagnosis was established. No renal impairment, vascular purpura, arthralgia or arthritis was found. The authors emphasize the importance of considering less common etiologies for chronic leg wounds, even in the absence of other suggestive symptomatology, as well as the pertinence of reconsidering diagnosis in highly suspect cases.

O Linfoma de Burkitt (LB) gastrintestinal é um subtipo de linfoma não Hodgkin (LNH) que surge com maior frequência no íleon e no cego, sobretudo na sua forma esporádica e na criança. O LB do estômago e do duodeno é particularmente raro e pode manifestar-se de diversas formas. Os autores apresentam dois casos clínicos de LB do estômago e do duodeno, em idade pediátrica.

Relata-se um resumo epidemiológico do surto epizoótico de febre amarela ocorrido no ano de 1973. São postas em evidência a sua incidência, a distribuição por idades, a distribuição por sexo e a distribuição geográfica. Em seguida, são apresentados os resultados obtidos pela revisão dos casos de febre amarela, que vêm sendo estudados, recolhidos e arquivados na Coleção do Serviço de Viscerotomia da Seção de Histopatologia da Febre Amarela do Instituto Oswaldo Cruz, desde 1931. O trabalho está documentado com tabelas e gráficos que registram os dados acima indicados. Os autores ressaltam os seguintes pontos: 1) A Febre amarela, urbana e silvestre, apresenta quadros epidemiológicos inteiramente diferentes; 2) A incidência da febre amarela silvestre ocorre principalmente no sexo masculino, de acordo com os dados estatisticamente significativos; 3) A idade mais atingida pela forma silvestre é a entre 20 e 30 anos; 4) A febre amarela silvestre continua em áreas enzoóticas, ao longo dos cursos d'água; 5) Parece existir uma relação entre as condições climáticas e a incidência da febre amarela; 6) Os surtos de febre amarela ocorrem aproximadamente de 7 em 7 anos; 7) É do máximo valor o programa de vacinação e o serviço de viscerotomia da SUCAM.

Ao curso de um surto epizoótico de febre amarela acontecido no ano de 1973 nas regiões norte e centro-oeste do Brasil, foram feitos exames histopatológicos de material de fígado de casos suspeitos, retirados por viscerotomia e necropsias. Foram diagnosticados 57 casos positivos. Os exames histopatológicos demonstraram que não existe diferença da doença entre os casos da África e da América do Sul. O diagnóstico histopatológico continua baseado no fígado, na lesão de Councilman, na hiperplasia e hipertrofia das células de Kupfer, nas inclusões nucleares de Magarino Torres, no sinal de Eudoro Villela nos casos de longa duração. Não houve diferença histológica entre os casos descritos nas epidemias de F.A. urbana e os surtos epidêmicos e enzoóticos de F.A. silvestre nos últimos 43 anos.

Philander frenata and Didelphis marsupialis harbor parasitism by Trypanosoma cruzi without developing any apparent disease and on the contrary to D. marsupialis, P. frenata maintains parasitism by T. cruzi II subpopulations. Here we compared the humoral immune response of the two didelphids naturally and experimentally infected with T. cruzi II group, employing SDS-PAGE/Western blot techniques and by an Indirect immunofluorescence assay. We also studied the histopathological pattern of naturally and experimentally infected P. frenata with T. cruzi. P. frenata sera recognized more antigens than D. marsupialis, and the recognition pattern did not show any change over the course of the follow up of both didelphid species. Polypeptides of 66 and 90kDa were the most prominent antigens recognized by both species in the soluble and enriched membrane fractions. P. frenata recognized intensely also a 45kDa antigen. Our findings indicate that: 1) there were no quantitative or qualitative differences in the patent or subpatent phases in the recognition pattern of P. frenata; 2) the significant differences in the recognition pattern of parasitic antigens by P. frenata and D. marsupialis sera suggest that they probably "learned" to live in harmony with T. cruzi by different strategies; 3) although P. frenata do not display apparent disease, tissular lesions tended to be more severe than has been described in D. marsupialis; and 4) Both didelphids probably acquired infection by T. cruzi after their evolutionary divergence.
Presença extra-intestinal de cistos unizóicos de Isospora belli em paciente com SIDA: relato de caso

Descreve-se a presença de cistos unizóicos de Isospora belli em linfonodos mesentéricos e de gametócitos no epitélio da vesícula biliar de um paciente brasileiro de 26 anos de idade, com a Síndrome da Imunodeficiência Adquirida que recebeu tratamento, por diversas vezes, com sulfametoxazol-trimetoprim. Discute-se a importância dos cistos teciduais de I. belli como possíveis focos de resistência do parasita e a associação destes a episódios de recidivas da infecção mesmo após tratamento com medicação anticoccídios.

Hepatitis C virus (HCV) infection has quite high prevalence in the prison system, reaching rates of up to 40%. This survey aimed to estimate the prevalence of HCV infection and evaluate risk factors for this exposure among male inmates at the Ribeirão Preto Prison, State of São Paulo, Brazil, between May and August 2003. A total of 333 participants were interviewed using a standardized questionnaire and underwent immunoenzymatic assaying to investigate anti-HCV. The prevalence of HCV infection among the inmates was 8.7% (95% CI: 5.7-11.7). The participants'mean age was 30.1 years, and the prevalence was predominantly among individuals over 30 years of age. Multivariate analysis showed that the variables that were independently associated with HCV infection were age > 30 years, tattooing, history of previous hepatitis, previous injection drug use and previous needle-sharing.

Breast cancer can be perceived as a traumatic event with disturbing effects on psychological domains such as depression, anxiety, and Posttraumatic Stress Disorder. In contrast, growing evidence has shown that posttraumatic growth can occur as a result of coping with breast cancer. Challenging the assumptive world, deliberate rumination, and emotional disclosure are recognized as strong predictors of posttraumatic growth. Group interventions may also increase social support, distress disclosure, and posttraumatic growth. The aim of this study is to evaluate how group-based interventions can facilitate posttraumatic growth and promote improved psychosocial adjustment to breast cancer. This article describes the study protocol and the applied research methods.

Background: Despite the consensus regarding the existence of a relationship between “impacts on oral health” and “health-related quality of life”, this relationship, considering the latent nature of these variables, is still poorly investigated. Thus, we performed this study in order to determine the magnitude of the impacts of oral health, demographic and symptom/clinical variables on the health-related quality of life in a Brazilian sample of dental patients. Methods: A total of 1,007 adult subjects enrolled in the School of Dentistry of São Paulo State University (UNESP) - Araraquara Campus for dentistry care between September/2012 and April/2013, participated. 72.4 % were female. The mean age was 45.7 (SD = 12.5) years. The Oral Health Impact Profile (OHIP-14) and the Short Form Health Survey (SF-36) were used. The demographic and symptom/clinical variables collected were gender, age, economic status, presence of pain and chronic disease. The impact of studied variables on health-related quality of life were evaluated with a structural equation model, considering the factor “Health” as the central construct. The fit of the model was first analyzed by the evaluation of the goodness of fit indices (χ 2 /df ≤ 2.0, CFI and TLI ≥ 0.90 and RMSEA < 0.10) and the evaluation of the variables’ impact over health-related quality of life was based on the statistical significance of causal paths (β), evaluated by z tests, for a significance level of 5 %. Results: We observed adequate fit of the model to the data (χ 2 /df = 3.55; CFI = 0.95; TLI = 0.94; RMSEA = 0.05). The impacts on oral health explained 28.0 % of the variability of the health-related quality of life construct, while the total variance explained of the model was 39.0 %. For the demographic and symptom/clinical variables, only age, presence of pain and chronic disease showed significant impacts (p < 0.05). Conclusion: The oral health, age, presence of pain and chronic disease of individuals had significant influence on health-related quality of life.

Oral dirofilariasis is very rare with non-specific clinical manifestations. Here, we report the case of a 65-year-old South American woman with a submucosal nodule on her right buccal mucosa. The nodule was slightly tender and painful. Differential diagnoses included mesenchymal (lipoma or fibrolipoma, solitary fibrous tumor, and neurofibroma) or glandular benign tumors (pleomorphic adenoma) with secondary infections. We performed excisional biopsy. A histopathological examination revealed a dense fibrous capsule and a single female filarial worm showing double uterus appearance, neural plaque, well-developed musculature and intestinal apparatus. Dirofilariasis was diagnosed, and the patient was followed-up for 12 months without recurrence.
