A síndrome do estalido ou do ressalto ocorre durante a movimentação de várias articulações e pode ser acompanhada de dor local e de crepitação ou de estalido audível. Nem sempre o estalido audível ou a crepitação articular à palpação têm significado patológico ou implicam necessidade de tratamento. Esta síndrome tem causas variadas intra-articulares e extra-articulares e os achados clínicos podem ser pouco precisos, com dificuldade para definir o melhor método de imagem que confirme o diagnóstico. O objetivo deste trabalho é discutir as causas mais comuns da síndrome do estalido em cada articulação e enfatizar a indicação e a limitação de cada um dos diferentes métodos diagnósticos em situações específicas da prática clínica.

A síndrome do impacto do tornozelo é uma condição dolorosa causada por atrito de tecidos articulares, que é tanto causa quanto consequência de uma biomecânica alterada desta articulação. A sua principal causa são as lesões pós-traumáticas, principalmente lesões ligamentares, resultando em dor crônica no tornozelo. Do ponto de vista anatômico e clínico, estas síndromes são classificadas em: ântero-lateral, anterior, ântero-medial, póstero-medial e posterior. A ressonância magnética é um ótimo método diagnóstico para demonstrar as alterações ósseas e as partes moles dos vários tipos de impacto do tornozelo, fornecendo dados que auxiliam não só na comprovação desse diagnóstico, como na diferenciação com outras causas de dor articular. Os autores objetivam ilustrar os principais achados de ressonância magnética na síndrome do impacto do tornozelo.

O lúpus eritematoso sistêmico é uma doença autoimune que envolve múltiplos sistemas orgânicos. O acometimento musculoesquelético é uma das manifestações mais comuns da doença, com envolvimento ósseo, articular, muscular, tendíneo e ligamentar, tanto primário como relacionado ao tratamento instituído. Neste artigo revisamos e ilustramos as alterações articulares e complicações musculoesqueléticas mais comuns relacionadas a esta doença.

Partial-thickness tears of the supraspinatus tendon frequently occur at its insertion on the greater tubercule of the humerus, causing pain and reduced strength and range of motion. The goal of this work was to quantify the loss of loading capacity due to tendon tears at the insertion area. A finite element model of the supraspinatus tendon was developed using in vivo magnetic resonance images data. The tendon was represented by an anisotropic hyperelastic constitutive law identified with experimental measurements. A failure criterion was proposed and calibrated with experimental data. A partial-thickness tear was gradually increased, starting from the deep articular-sided fibres. For different values of tendon tear thickness, the tendon was mechanically loaded up to failure. The numerical model predicted a loss in loading capacity of the tendon as the tear thickness progressed. Tendon failure was more likely when the tendon tear exceeded 20%. The predictions of the model were consistent with experimental studies. Partial-thickness tears below 40% tear are sufficiently stable to persist physiotherapeutic exercises. Above 60% tear surgery should be considered to restore shoulder strength.

OBJETIVO: Avaliar a sensibilidade e a especificidade do exame ultrassonográfico de alta resolução para a avaliação dos distúrbios intracapsulares temporomandibulares. MATERIAIS E MÉTODOS: Estudamos 38 pacientes (76 articulações) com queixas de distúrbios temporomandibulares. Todos os pacientes realizaram exames de ultrassonografia e ressonância magnética (padrão ouro para a avaliação) e os resultados obtidos foram comparados. RESULTADOS: De 24 articulações evidenciando deslocamento discal com o paciente em repouso na ressonância magnética, 7 foram confirmados pela ultrassonografia, em 13 não foram visualizados os discos e 4 estavam tópicos na ultrassonografia. Em 48 articulações, o disco articular não foi visualizado na ultrassonografia com o paciente em repouso. Destes, 41 apresentavam posicionamento normal na ressonância magnética e 7 apresentavam deslocamento anterior. Alterações morfológicas do côndilo mandibular foram visualizadas pela ressonância magnética em 13 articulações, identificadas pela ultrassonografia em 2 delas. CONCLUSÃO: Podemos concluir, no estudo, que o exame de ultrassonografia apresenta alta sensibilidade e especificidade para o diagnóstico da localização do disco articular com o paciente em repouso, tanto para a análise de seu posicionamento anatômico como nos casos de deslocamentos, não apresentando resultados significativos para a análise dos discos com o paciente com a boca aberta e para a análise de alterações morfológicas discais e condilares.

OBJECTIVES: Basic calcium phosphate (BCP) crystal and interleukin 6 (IL-6) have been implicated in osteoarthritis (OA). We hypothesise that these two factors may be linked in a reciprocal amplification loop which leads to OA. METHODS: Primary murine chondrocytes and human cartilage explants were incubated with hydroxyapatite (HA) crystals, a form of BCP, and the modulation of cytokines and matrix-degrading enzymes assayed. The ability of IL-6 to stimulate chondrocyte calcification was assessed in vitro. The mechanisms underlying the effects of HA on chondrocytes were investigated using chemical inhibitors, and the pathways mediating IL-6-induced calcification characterised by quantifying the expression of genes involved in chondrocyte mineralisation. The role of calcification in vivo was studied in the meniscectomy model of murine OA (MNX), and the link between IL-6 and cartilage degradation investigated by histology. RESULTS: In chondrocytes, BCP crystals stimulated IL-6 secretion, further amplified in an autocrine loop, through signalling pathways involving Syk and PI3 kinases, Jak2 and Stat3 molecules. Exogenous IL-6 promoted calcium-containing crystal formation and upregulation of genes involved in calcification: the pyrophosphate channel Ank, the calcium channel Annexin5 and the sodium/phosphate cotransporter Pit-1. Treatment of chondrocytes with IL-6 inhibitors significantly inhibited IL-6-induced crystal formation. In meniscectomised mice, increasing deposits of BCP crystals were observed around the joint and correlated with cartilage degradation and IL-6 expression. Finally, BCP crystals induced proteoglycan loss and IL-6 expression in human cartilage explants, which were reduced by an IL-6 inhibitor. CONCLUSIONS: BCP crystals and IL-6 form a positive feedback loop leading to OA. Targeting calcium-containing crystal formation and/or IL-6 are promising therapeutic strategies in OA.

OBJETIVO: Descrever os aspectos clínicos e de imagem que podem auxiliar no diagnóstico correto do osteoma osteoide no cotovelo. MATERIAIS E MÉTODOS: Realizado estudo retrospectivo de sete pacientes com diagnóstico de osteoma osteoide no cotovelo confirmado histologicamente por biópsia óssea. Eles tiveram seus prontuários médicos e exames de imagem revisados. RESULTADOS: Do total de 142 pacientes identificados em nossos arquivos, 4,9% apresentavam a lesão no cotovelo, com predomínio no sexo masculino (2,5:1) e idade média de 25 anos. Dor e limitação de movimento foram os sintomas mais comuns. Cinco pacientes (71,4%) receberam outro diagnóstico clínico prévio. A duração média dos sintomas foi de 21 meses. As radiografias não demonstraram o nidus em 42,8% dos casos. A tomografia computadorizada e a ressonância magnética mostraram claramente o nidus. Derrame articular foi um achado constante. O aspecto histológico observado foi o usual. A ressecção cirúrgica promoveu alívio dos sintomas e/ou melhora funcional em todos os casos. CONCLUSÃO: É importante considerar a possibilidade de osteoma osteoide em paciente adulto jovem com dor, limitação do movimento e sinais de sinovite no cotovelo, refratária ao tratamento conservador. Esclerose óssea, espessamento cortical e/ou reação periosteal detectados na radiografia permitem direcionar a tomografia computadorizada para a visualização precisa do nidus.

Nuestras unidades analíticas, están constituidas por dos familias dedicadas al comercio cuyo recorrido nos permite analizar un arco temporal amplio de prácticas y estrategias de racionalidad empresaria durante el siglo XIX argentino. El debate que guía el trabajo puede resumirse en la preponderancia de los lazos de parentesco o en el predominio de estrategias de tipo corporativo a la hora de articular la lógica mercantil. El desafío de nuestra propuesta es plantear el funcionamiento corporativo, las prácticas empresariales, y la integración político/institucional como variables fundamentales que compensaban la debilidad de los vínculos de parentesco. Los dos casos contrastados exudan similar funcionamiento en cuanto al origen inmigratorio de sus responsables y a las estrategias utilizadas para integrarse en ciudades portuarias de reciente importancia pero con una ambición común: convertirse en centros neurálgicos de la vida mercantil. El primer caso atañe a un comerciante catalán llegado en la segunda mitad del siglo XVIII, que construyó su patrimonio y su casa mercantil a través de un complejo entramado de redes sociales. El segundo ejemplo corresponde a un grupo empresario, llegado cien años después y dispuesto a condensar lazos identitarios con la ocupación de lugares claves de la vida mercantil e industrial regional. Ambos casos comparten la ausencia de una base familiar que garantizara la supervivencia patrimonial, una carencia que permite rediscutir el rol de la familia.

La crisis financiera y la crisis de la deuda soberana han tenido un gran impacto en el Estado autonómico y han polarizado las posiciones entre los partidarios de una mayor centralización y los que consideran agotado el modelo autonómico y defienden la necesidad de articular nuevas vías para alcanzar y garantizar el nivel de autogobierno. En este análisis se argumenta que a pesar de que la crisis ha tenido un impacto negativo en el debate sobre la viabilidad del Estado autonómico y en la percepción ciudadana sobre el nivel de gasto y de competencias de las Comunidades Autónomas este debate es anterior a la crisis económica. Además la crisis ha tenido un mayor impacto en estados de estructura unitaria o poco descentralizada, y menor en estados de estructura federal, por lo que no parece que exista una relación directa entre descentralización y crisis económica.

Because of technical principles, samples to be observed with electron microscopy need to be fixed in a chemical process and exposed to vacuum conditions that can produce some changes in the morphology of the specimen. The aim of this work was to obtain high-resolution images of the fresh articular cartilage surface with an environmental scanning electron microscope (ESEM), which is an instrument that permits examination of biological specimens without fixation methods in a 10 Torr chamber pressure, thus minimizing the risk of creating artifacts in the structure. Samples from weight-bearing areas of femoral condyles of New Zealand white rabbits were collected and photographed using an ESEM. Images were analyzed using a categorization based in the Jurvelin classification system modified by Hong and Henderson. Appearance of the observed elevations and depressions as described in the classification were observed, but no fractures or splits of cartilage surface, thought to be artifacts, were detected. The ESEM is a useful tool to obtain images of fresh articular cartilage surface appearance without either employing fixation methods or exposing the specimen to extreme vacuum conditions, reducing the risk of introducing artifacts within the specimen. For all these reasons it could become a useful tool for quality control of the preservation process of osteochondral allografting in a bank of musculoskeletal tissues.

The calcified tissues, comprising bone and cartilage, are metabolically active tissues that bind and release calcium, bicarbonate and other substances according to systemic needs. Understanding the regulation of cellular metabolism in bone and cartilage is an important issue, since a link between the metabolism and diseases of these tissues is clear. An essential element in the function of bone-resorbing osteoclasts, namely regulation of bicarbonate transport, has not yet been thoroughly studied. Another example of an important but at the same time fairly unexplored subject of interest in this field is cartilage degeneration, an important determinant for development of osteoarthritis. The link between this and oxidative metabolism has rarely been studied. In this study, we have investigated the significance of bicarbonate transport in osteoclasts. We found that osteoclasts possess several potential proteins for bicarbonate transport, including carbonic anhydrase IV and XIV, and an electroneutral bicarbonate co-transporter NBCn1. We have also shown that inhibiting the function of these proteins has a significant impact on bone resorption and osteoclast morphology. Furthermore, we have explored oxidative metabolism in chondrocytes and found that carbonic anhydrase III (CA III), a protein linked to the prevention of protein oxidation in muscle cells, is also present in mouse chondrocytes, where its expression correlates with the presence of reactive oxygen species. Thus, our study provides novel information on the regulation of cellular metabolism in calcified tissues.

Chondrogenesis is a co-ordinated differentiation process in which mesenchymal cells condensate, differentiate into chondrocytes and begin to secrete molecules that form the extracellular matrix. It is regulated in a spatio-temporal manner by cellular interactions and growth and differentiation factors that modulate cellular signalling pathways and transcription of specific genes. Moreover, post-transcriptional regulation by microRNAs (miRNAs) has appeared to play a central role in diverse biological processes, but their role in skeletal development is not fully understood. Mesenchymal stromal cells (MSCs) are multipotent cells present in a variety of adult tissues, including bone marrow and adipose tissue. They can be isolated, expanded and, under defined conditions, induced to differentiate into multiple cell lineages including chondrocytes, osteoblasts and adipocytes in vitro and in vivo. Owing to their intrinsic capability to self-renew and differentiate into functional cell types, MSCs provide a promising source for cell-based therapeutic strategies for various degenerative diseases, such as osteoarthritis (OA). Due to the potential therapeutic applications, it is of importance to better understand the MSC biology and the regulatory mechanisms of their differentiation. In this study, an in vitro assay for chondrogenic differentiation of mouse MSCs (mMSCs) was developed for the screening of various factors for their chondrogenic potential. Conditions were optimized for pellet cultures by inducing mMSC with different bone morphogenetic proteins (BMPs) that were selected based on their known chondrogenic relevance. Characterization of the surface epitope profile, differentiation capacity and molecular signature of mMSCs illustrated the importance of cell population composition and the interaction between different populations in the cell fate determination and differentiation of MSCs. Regulation of Wnt signalling activity by Wnt antagonist sFRP-1 was elucidated as a potential modulator of lineage commitment. Delta-like 1 (dlk1), a factor regulating adipogenesis and osteogenesis, was shown to exhibit stage-specific expression during embryonic chondrogenesis and identified as a novel regulator of chondrogenesis, possibly through mediating the effect of TGF-beta1. Moreover, miRNA profiling demonstrated that MSCs differentiating into a certain lineage exhibit a specific miRNA expression profile. The complex regulatory network between miRNAs and transcription factors is suggested to play a crucial role in fine-tuning the differentiation of MSCs. These results demonstrate that commitment of mesenchymal stromal cells and further differentiation into specific lineages is regulated by interactions between MSCs, various growth and transcription factors, and miRNA-mediated translational repression of lineage-specific genes.

Radiostereometric analysis (RSA) is a highly accurate method for the measurement of in vivo micromotion of orthopaedic implants. Validation of the RSA method is a prerequisite for performing clinical RSA studies. Only a limited number of studies have utilised the RSA method in the evaluation of migration and inducible micromotion during fracture healing. Volar plate fixation of distal radial fractures has increased in popularity. There is still very little prospective randomised evidence supporting the use of these implants over other treatments. The aim of this study was to investigate the precision, accuracy, and feasibility of using RSA in the evaluation of healing in distal radius fractures treated with a volar fixed-angle plate. A physical phantom model was used to validate the RSA method for simple distal radius fractures. A computer simulation model was then used to validate the RSA method for more complex interfragmentary motion in intra-articular fractures. A separate pre-clinical investigation was performed in order to evaluate the possibility of using novel resorbable markers for RSA. Based on the validation studies, a prospective RSA cohort study of fifteen patients with plated AO type-C distal radius fractures with a 1-year follow-up was performed. RSA was shown to be highly accurate and precise in the measurement of fracture micromotion using both physical and computer simulated models of distal radius fractures. Resorbable RSA markers demonstrated potential for use in RSA. The RSA method was found to have a high clinical precision. The fractures underwent significant translational and rotational migration during the first two weeks after surgery, but not thereafter. Maximal grip caused significant translational and rotational interfragmentary micromotion. This inducible micromotion was detectable up to eighteen weeks, even after the achievement of radiographic union. The application of RSA in the measurement of fracture fragment migration and inducible interfragmentary micromotion in AO type-C distal radius fractures is feasible but technically demanding. RSA may be a unique tool in defining the progress of fracture union.
