Introduction: Particularly in elderly patients, the brain responds to a systemic inflammatory response with an increased production of inflammatory mediators. This has hypothetically been linked to the development of postoperative cognitive dysfunction (POCD). Methods: We investigated 31 patients aged >65 yrs undergoing elective major surgery under standardized general anaesthesia (thiopental, sevoflurane, fentanyl, atracurium). Cognitive function was measured preoperatively and 7 days postoperatively using the extended version of the Consortium to Establish a Registry for Alzheimer's Disease - Neuropsychological Assessment Battery (CERAD-NAB, validated German version) for which we developed a diagnostic cut-off in healthy elderly volunteers. Systemic C-reactive protein (CRP) and interleukin 6 (IL-6) were measured preoperatively, 2 days postoperatively, and 7 days postoperatively. Values for CRP, IL-6, operative characteristics and hospital length of stay in patients with POCD and without POCD were compared using the Mann- Whitney U test and are shown as median [range]. Results: Fourteen patients (45%) developed POCD. Values for CRP were not statistically different in patients with POCD and without POCD but tended to be higher in patients with POCD 2 days postoperatively. Patients with POCD had significantly higher IL-6 values on postoperative days 2 and 7 (table 1). These patients also had a significantly longer duration of anaesthesia (305 [195-620] vs.190 [150-560] min, p = 0.034), larger intraoperative blood loss (425 [0-1600] vs. 100 [0-1500] ml, p = 0.018) and longer hospital stays (15 [8-45] vs. 8 [4-40] days, p = 0.008). In patients with POCD, duration of anaesthesia was significantly longer, and intraoperative blood losses were larger. These risk factors will need to be confirmed in a larger group of patients. The difference in length of stay may be indicative of postoperative complications, which have been linked to POCD earlier.

A identificação de genótipos com elevada acumulação de biomassa e nutrientes, mas com baixo índice de colheita de nutrientes (razão entre conteúdo de nutrientes nos grãos e na parte aérea), pode reduzir a remoção pelas colheitas e aumentar a sustentabilidade agrícola. Evidências de reduzida variabilidade nos índices de colheita em feijoeiro (Phaseolus vulgaris L.) demandam a avaliação de uma ampla gama de genótipos. Este trabalho teve como objetivo avaliar a variabilidade dos índices de colheita de nutrientes e sua relação com a produção de grãos em genótipos de feijoeiro. Foi conduzido um experimento de campo em Seropédica - RJ, com 64 genótipos de feijoeiro em quatro repetições, incluindo 41 cultivares, 12 linhagens de melhoramento e 10 cultivares locais da região Sul. Os índices de colheita foram mensurados a partir das quantidades acumuladas de biomassa e nutrientes nos grãos, caules e palha de vagens produzidos após trilhagem dos grãos. A média da produção de grãos foi de 205 g m-2, com índices de colheita médios de biomassa, N, P, K, Ca e Mg de 0,62, 0,83, 0,89, 0,58, 0,33 e 0,51 g g-1, respectivamente, denotando intensa translocação de N e P para os grãos, sendo relativamente estreita a variabilidade desses índices. A massa seca de resíduos após trilhagem dos grãos foi em média de 107 g m-2, contendo 2,0, 0,15, 3,6, 2,8 e 1,3 g m-2 de N, P, K, Ca e Mg, respectivamente, o que indica que esses materiais podem restituir quantidades relativamente elevadas de nutrientes em comparação com as demandas do cultivo. Foram obtidas elevadas correlações fenotípicas e genéticas (p < 0,001): positivas entre produção de grãos e índices de colheita de biomassa e N e negativas entre produção e teores de N e P nos grãos. Como as correlações fenotípicas e genéticas entre rendimento e índice de colheita de P foram menos significativas (p < 0,01), foi possível identificar alguns genótipos com baixo índice de colheita de P e bom rendimento. A seleção de genótipos de feijoeiro para maior rendimento de grãos pode resultar em maiores índices de colheita de biomassa e de N, assim como em maiores quantidades de nutrientes nos grãos e menores teores de N e P nos grãos.

Hepatic and extrahepatic insulin sensitivity was assessed in six healthy humans from the insulin infusion required to maintain an 8 mmol/l glucose concentration during hyperglycemic pancreatic clamp with or without infusion of 16.7 micromol. kg(-1). min(-1) fructose. Glucose rate of disappearance (GR(d)), net endogenous glucose production (NEGP), total glucose output (TGO), and glucose cycling (GC) were measured with [6,6-(2)H(2)]- and [2-(2)H(1)]glucose. Hepatic glycogen synthesis was estimated from uridine diphosphoglucose (UDPG) kinetics as assessed with [1-(13)C]galactose and acetaminophen. Fructose infusion increased insulin requirements 2.3-fold to maintain blood glucose. Fructose infusion doubled UDPG turnover, but there was no effect on TGO, GC, NEGP, or GR(d) under hyperglycemic pancreatic clamp protocol conditions. When insulin concentrations were matched during a second hyperglycemic pancreatic clamp protocol, fructose administration was associated with an 11.1 micromol. kg(-1). min(-1) increase in TGO, a 7.8 micromol. kg(-1). min(-1) increase in NEGP, a 2.2 micromol. kg(-1). min(-1) increase in GC, and a 7.2 micromol. kg(-1). min(-1) decrease in GR(d) (P < 0. 05). These results indicate that fructose infusion induces hepatic and extrahepatic insulin resistance in humans.

OBJECTIVE: To compare the effects of sodium bicarbonate and lactate for continuous veno-venous hemodiafiltration (CVVHDF) in critically ill patients. DESIGN AND SETTINGS: Prospective crossed-over controlled trial in the surgical and medical ICUs of a university hospital. PATIENTS: Eight patients with multiple organ dysfunction syndrome (MODS) requiring CVVHDF. INTERVENTION: Each patient received the two buffers in a randomized sequence over two consecutive days. MEASUREMENTS AND RESULTS: The following variables were determined: acid-base parameters, lactate production and utilization ((13)C lactate infusion), glucose turnover (6,6(2)H(2)-glucose), gas exchange (indirect calorimetry). No side effect was observed during lactate administration. Baseline arterial acid-base variables were equal with the two buffers. Arterial lactate (2.9 versus 1.5 mmol/l), glycemia (+18%) and glucose turnover (+23%) were higher in the lactate period. Bicarbonate and glucose losses in CVVHDF were substantial, but not lactate elimination. Infusing (13)C lactate increased plasma lactate levels equally with the two buffers. Lactate clearance (7.8+/-0.8 vs 7.5+/-0.8 ml/kg per min in the bicarbonate and lactate periods) and endogenous production rates (14.0+/-2.6 vs 13.6+/-2.6 mmol/kg per min) were similar. (13)C lactate was used as a metabolic substrate, as shown by (13)CO(2) excretion. Glycemia and metabolic rate increased significantly and similarly during the two periods during lactate infusion. CONCLUSION: Lactate was rapidly cleared from the blood of critically ill patients without acute liver failure requiring CVVHDF, being transformed into glucose or oxidized. Lactate did not exert undesirable effects, except moderate hyperglycemia, and achieved comparable effects on acid-base balance to bicarbonate.

ABSTRACT: Pharmacogenetic tests and therapeutic drug monitoring may considerably improve the pharmacotherapy of depression. The aim of this study was to evaluate the relationship between the efficacy of mirtazapine (MIR) and the steady-state plasma concentrations of its enantiomers and metabolites in moderately to severely depressed patients, taking their pharmacogenetic status into account. Inpatients and outpatients (n = 45; mean age, 51 years; range, 19-79 years) with major depressive episode received MIR for 8 weeks (30 mg/d on days 1-14 and 30-45 mg/d on days 15-56). Mirtazapine treatment resulted in a significant improvement in mean Hamilton Depression Rating Scale total score at the end of the study (P < 0.0001). There was no evidence for a significant plasma concentration-clinical effectiveness relationship regarding any pharmacokinetic parameter. The enantiomers of MIR and its hydroxylated (OH-MIR) and demethylated (DMIR) metabolites in plasma samples on days 14 and 56 were influenced by sex and age. Nonsmokers (n = 28) had higher mean MIR plasma levels than smokers (n = 17): S(+)-enantiomer of MIR, 9.4 (SD, 3.9) versus 6.2 (SD, 5.5) ng/mL (P = 0.005); R(-)-enantiomer of MIR, 24.4 (SD, 6.5) versus 18.5 (SD, 4.1) ng/mL (P = 0.003). Only in nonsmokers, plasma levels of S(+)-enantiomer of MIR and metabolites depended on the CYP2D6 genotype. Therefore, high CYP1A2 activity seen in smokers seems to mask the influence of the CYP2D6 genotype. In patients presenting the CYP2B6 *6/*6 genotype (n = 8), S-OH-MIR concentrations were higher those in the other patients (n = 37). Although it is not known if S-OH-MIR is associated with the therapeutic effect of MIR, the reduction of the Hamilton scores was significantly (P = 0.016) more pronounced in the CYP2B6 *6/*6-genotyped patients at the end of the study. The role of CYP2B6 in the metabolism and effectiveness of MIR should be further investigated.

The rates of energy expenditure and wholebody protein turnover were determined during a 9-h period in a group of seven men while they received hourly isocaloric meals of high-protein (HP) or high-carbohydrate (HC) content. Their responses to feeding were compared with those to a short period of fasting (15-24 h). The 9-h thermic response to the repeated feeding of HP meals was found to be greater than that to the HC meals (9.6 +/- 0.6% vs 5.7 +/- 0.4% of the energy intake, respectively, means +/- SEM, p less than 0.01). The rate of whole-body nitrogen turnover over 9 h increased from 17.6 +/- 2.2 g on the fasting day to 27.4 +/- 1.4 g during HC feeding (NS) and there was a further increase to 58.2 +/- 5.3 g resulting from HP feeding (p less than 0.001). By using theoretical estimates (based upon ATP requirements) of the metabolic cost of protein synthesis, 36 +/- 9% of the thermic response to HC feeding and 68 +/- 3% of the response to HP feeding could be accounted for by the increases in protein synthesis compared with the fasting state.

Introduction: Tout praticien est confronté quotidiennement à des urgences. Parmi celles-ci, les urgences vitales peuvent avoir des conséquences majeures pour le patient et pour le praticien. Le but de cette analyse est de déterminer la fréquence de survenue des urgences vitales au cabinet médical qui motivent l'intervention d'une ambulance, avec ou sans médicalisation par un SMUR (Service Mobile d'Urgence et de Réanimation) et d'en évaluer des conséquences en terme de mise en place de procédures, d'équipements ainsi que de formations post-graduée ou continue spécifiques. Matériel et méthode: Etude rétrospective des fiches d'intervention pré-hospitalière des services d'ambulances et des missions des SMUR du canton de Vaud (650'000 habitants) entre 2003 et 2006 pour les missions dont la prise en charge d'un patient a eu lieu dans le cadre d'un cabinet médical. Résultats: Entre 2003 et 2006, 2'224 interventions avec ambulances ont eu lieu dans un des 1'655 cabinet médical vaudois (= 2,3% de l'ensemble des missions) et, dans >90% des cas, dans un délai de 20 minutes. Parmi les interventions, on relève les urgences suivantes (n =, % des interventions): cardio-vasculaires: 755 (= 33,9%), dont 17 arrêts cardio-respiratoires (ACR); respiratoires: 165 (= 7,4%); neurologiques: 138 (= 6,2%); psychiatriques: 129 (= 5,8%); traumatologiques: 475 (= 21,4%), dont 261 (= 54,9%) concernent les extrémités; diverses: 205 (= 9,5%); autres: 359 (= 16,1%). Sur ces interventions, 634 (= 28,5%) ont bénéficié d'une médicalisation par un SMUR, dont 440 (= 70% des missions SMUR au cabinet) pour des urgences cardio-vasculaires. Il y a eu 6 cas de décès au cabinet. Discussion: Les urgences cardio-vasculaires au cabinet représentent un tiers des interventions faisant appel à une ambulance, mais plus des deux tiers des interventions nécessitant une médicalisation, soit plus que pour les autres sites d'interventions médicalisées dans la communauté (46%). Conclusions: Les urgences vitales au cabinet médical ne sont pas négligeable, peuvent avoir des conséquences lourdes (ACR, décès) et perturber significativement son fonctionnement. Dès lors, une formation appropriée tant pour le médecin que pour son personnel, ainsi qu'un équipement adéquat (par ex. salle équipée avec défibrillateur, appareil d'aérosol, attelles pour les extrémités) devraient être encouragés et généralisés auprès du corps médical.

BACKGROUND Few population studies have described the heritability and intrafamilial concordance of the retinal microvessels, or the genetic or environmental correlations of the phenotypes of these vessels. METHODS We randomly selected 413 participants from 70 families (mean age, 51.5 years; 50.1% women) from a Flemish population. We postprocessed retinal images using IVAN software to generate the central retinal arteriole equivalent (CRAE), central retinal venule equivalent (CRVE), and arteriole-to-venule-ratio (AVR) from these images. We used SAGE version 6.2 and SAS version 9.2 to compute multivariate-adjusted estimates of heritability and intrafamilial correlations of the CRAE, CRVE, and AVR of the retinal microvessels in the images. RESULTS Sex, age, mean arterial pressure, and smoking explained up to 12.7% of the variance of the phenotypes of the retinal microvessels of the study participants. With adjustments applied for these covariates, the heritability estimates of CRAE, CRVE, and AVR were 0.213 (P = 0.044), 0.339 (P = 0.010), and 0.272 (P = 0.004), respectively. The parent-offspring correlations for CRAE, CRVE, and AVR were 0.118 (NS), 0.225 (P < 0.01), and 0.215 (P < 0.05), respectively. The corresponding values were 0.222 (P < 0.05), 0.213 (P < 0.05), and 0.390 (P < 0.001) for sib-sib correlations, respectively. The genetic and environmental correlations between CRAE and CRVE were 0.360 and 0.545 (P < 0.001 for both). CONCLUSION Our study showed moderate heritability for CRAE, CRVE, and AVR, and a significant genetic correlation of CRAE with CRVE in the Flemish population of our study. These findings suggest that genetic factors influence the diameter of the retinal microvessels, and that CRAE and CRVE share some genetic determinants.

1.1 Objectifs Le syndrome de bas débit cardiaque est une appréciation clinique (Hoffman et al.) dont les signes sont peu sensibles, peu spécifiques, souvent tardifs et dont la mesure chez l'enfant est rendue impossible en clinique pour des raisons techniques et de fiabilités des systèmes de mesures. Aucun score n'est applicable à l'heure actuelle chez l'enfant. C'est pourquoi, ces 10 dernières années, de nombreuses équipes se sont penchée activement sur ce problème afin de définir des marqueurs performants prédisant la survenue du LCOS après chirurgie cardiaque chez l'enfant. Cette étude s'est attachée à réunir ses marqueurs cardiaques, de les combiner et de les inclure dans un score de bas débit cardiaque. 1.2 Méthode Enfants et nouveau-nés ayant subit une chirurgie cardiaque au CHUV, après malformation cardiaque congénitale, entre janvier 2010 et octobre 2011 (N=48). Age : 8 jours à 13 ans (médiane : 16.3 mois). Deux scores développés. Soumission à l'aveugle de la liste des patients à un comité d'expert pour identifier les patients en LCOS à 48h post-chirurgie, puis comparaison avec le résultat du score. Les paramètres du premier score (SCORE 1), sont agendées de manière ordinales, alors que dans le deuxième score (SCORE 2) elles le sont de manière dichotomiques. Valeurs cut-off supérieures et inférieures des scores choisies selon une recherche extensive dans la littérature. Les valeurs cut-off intermédiaires (SCORE 1) ont été choisies au hasard. 1.3 Résultats La régression logistique multivariée pour la prédiction d'un LCOS à 48h, démontre que seul le score d'amine durant les 24 premières heures et un prédicteur indépendant de LCOS (OR 16.6 [2.6- 105.5] p<0.0001). Ce paramètre est bien corrélé avec le résultat des experts avec un coefficient de corrélation r=0.57 (p<0.0001). Les spécificités des deux scores (AUC=0.78 (p<0.0001) respectivement AUC=0.81 (p<0.0001)) sont de 71% respectivement 93.5%, les sensibilités de 70.6% respectivement 41.2 %, VPP de 57.1% respectivement 77.8%, VPN de 81.5 % respectivement 74.4%. Les tests du khi2 valent 7.7 (p=0.006) respectivement 8.69 (p=003), rejettent l'hypothèse nulle d'indépendance entre le résultat des experts et celui prédit par le score. 1.4 Conclusions Les scores développés dans le cadre de cette étude ne montrent pas une corrélation significative avec l'apparition d'un bas débit cardiaque. Même si le choix des paramètres permettant de quantifier l'apparition d'un bas débit cardiaque à 48h fût réalisé selon une recherche extensive dans la littérature, le design rétrospectif de l'étude, n'a pas permit de vérifier efficacement la relation entre l'apparition d'un bas débit cardiaque et le score de bas débit cardiaque.

Introduction Swallowing difficulties, or dysphagia, can occur in anyage group, although it is most common among elderly people. It canaffect patients' ability to take solid oral dosage forms, thus compromisingmedication adherence. Although literature is poor, availabledata show that prevalence in the general population ranges from 25 to60%. Prevalence in community pharmacies needs to be explored.Materials & Methods Community pharmacies were recruited from arandom selection in three Swiss states: Basel-Stadt (BS), Basel-Landschaft (BL) and Lausanne (LA). Patients' ability to swallowsolid oral medications was enquired with a semi-structured interview;the interviewer spent 4 h in each included pharmacy. Each consecutivepatient (18 years and older) entering the pharmacy with aprescription for at least 3 different solid oral forms was enrolled.Study was approved by the Lausanne ethics committee.Results Sixty pharmacies took part in the study (20 in BS, 10 in BL,30 in LA) between March and May 2010. Patient inclusion rate was77.8% (410/527). Prevalence of swallowing disorders was 22.4% (92/410). Patients with swallowing disorders were older (mean age: 67.5± 16 years vs. 63.0 ± 14 years, range 19-96; p = 0.03) and moreoften women (69.6% vs. 59.1%; Chi2 = 3.3, p = 0.04) than patientswithout swallowing disorders. They had on average 4.6 ± 2.7 drugswith a mean number of 5.5 ± 3.3 tablets or capsules to take daily,which didn't differ from the number of drugs taken by patientswithout swallowing difficulties (4.9 ± 2.5 drugs and 5.9 ± 3.5 tablets;n.s.). The difficulty was mainly related to the big size (63%) orthe quality of pill coating (rough, sticky, 14%). Twenty-one patients(37.5%) stated that their swallowing disorders resulted in nonadherence, rated as rarely (12 patients), sometimes (6 patients), veryoften (1 patient) or always (2 patients). According to patients, nopharmacist and only 2 physicians enquired about patients' swallowingissue.Discussion & Conclusion Swallowing difficulties are frequent amongpatients in community pharmacies in Switzerland with an estimatedprevalence of 22%. The problem resulted in non adherence or partialadherence in at least 35% of these patients. However, pharmacists andphysicians did not routinely inquire about the disorder. Guidelinesshould be developed for promoting systematic approaches of patientsin community pharmacies.

The Organization of the Thesis The remainder of the thesis comprises five chapters and a conclusion. The next chapter formalizes the envisioned theory into a tractable model. Section 2.2 presents a formal description of the model economy: the individual heterogeneity, the individual objective, the UI setting, the population dynamics and the equilibrium. The welfare and efficiency criteria for qualifying various equilibrium outcomes are proposed in section 2.3. The fourth section shows how the model-generated information can be computed. Chapter 3 transposes the model from chapter 2 in conditions that enable its use in the analysis of individual labor market strategies and their implications for the labor market equilibrium. In section 3.2 the Swiss labor market data sets, stylized facts, and the UI system are presented. The third section outlines and motivates the parameterization method. In section 3.4 the model's replication ability is evaluated and some aspects of the parameter choice are discussed. Numerical solution issues can be found in the appendix. Chapter 4 examines the determinants of search-strategic behavior in the model economy and its implications for the labor market aggregates. In section 4.2, the unemployment duration distribution is examined and related to search strategies. Section 4.3 shows how the search- strategic behavior is influenced by the UI eligibility and section 4.4 how it is determined by individual heterogeneity. The composition effects generated by search strategies in labor market aggregates are examined in section 4.5. The last section evaluates the model's replication of empirical unemployment escape frequencies reported in Sheldon [67]. Chapter 5 applies the model economy to examine the effects on the labor market equilibrium of shocks to the labor market risk structure, to the deep underlying labor market structure and to the UI setting. Section 5.2 examines the effects of the labor market risk structure on the labor market equilibrium and the labor market strategic behavior. The effects of alterations in the labor market deep economic structural parameters, i.e. individual preferences and production technology, are shown in Section 5.3. Finally, the UI setting impacts on the labor market are studied in Section 5.4. This section also evaluates the role of the UI authority monitoring and the differences in the Way changes in the replacement rate and the UI benefit duration affect the labor market. In chapter 6 the model economy is applied in counterfactual experiments to assess several aspects of the Swiss labor market movements in the nineties. Section 6.2 examines the two equilibria characterizing the Swiss labor market in the nineties, the " growth" equilibrium with a "moderate" UI regime and the "recession" equilibrium with a more "generous" UI. Section 6.3 evaluates the isolated effects of the structural shocks, while the isolated effects of the UI reforms are analyzed in section 6.4. Particular dimensions of the UI reforms, the duration, replacement rate and the tax rate effects, are studied in section 6.5, while labor market equilibria without benefits are evaluated in section 6.6. In section 6.7 the structural and institutional interactions that may act as unemployment amplifiers are discussed in view of the obtained results. A welfare analysis based on individual welfare in different structural and UI settings is presented in the eighth section. Finally, the results are related to more favorable unemployment trends after 1997. The conclusion evaluates the features embodied in the model economy with respect to the resulting model dynamics to derive lessons from the model design." The thesis ends by proposing guidelines for future improvements of the model and directions for further research.
