A inovação desempenha um diferencial na resposta às necessidades globais de saúde, pois representa a mudança que incita a evolução das organizações e conduz à maior eficiência de seus processos por meio da alocação apropriada de recursos. O artigo tem como contribuição destacar em nível acadêmico o debate sobre a qualidade de serviços, as ações de saúde e a inovação, estimulando a compreensão das relações entre eles. Em nível organizacional, a contribuição apresenta um diagnóstico local da inovação no contexto das ações de atenção à saúde, demonstrado pela relação das variáveis a serem gerenciadas, para o desenvolvimento adequado dos recursos sanitários locais e a promoção da qualidade de vida para a população adscrita.

The aim of my research is to answer the question: How is Portugal seen by non-Portuguese fictionists? The main reason why I chose this research line is the following: Portuguese essayists like Eduardo Lourenço and José Gil (2005) focus their attention on the image or representation of Portugal as conceived by the Portuguese; indeed there is a tendency in Portuguese cultural studies (and, to a certain extent, also in Portuguese philosophical studies) to focus on studying the so-called ‗portugalidade‘ (portugueseness), i.e., the essence of being Portuguese. In my view, the problem with the studies I have been referring to is that everything is self-referential, and if ‗portugueseness‘ is an issue, then it might be useful, when dealing with it, to separate subject from object of observation. That is the reason why we, in the CEI (Centro de Estudos Interculturais), decided to start this research line, which is an inversion in the current tendency of the studies about ‗portugueseness‘: instead of studying the image or representation of Portugal by the Portuguese, my task is to study the image or representation of Portugal by the non-Portuguese, in this case, in non-Portuguese fiction. For the present paper I selected three writers of the 20th century: the German Hermann Hesse and the North-Americans Philip Roth and Paul Auster

A comparative study concerning the robustness of a novel, Fixed Point Transformations/Singular Value Decomposition (FPT/SVD)-based adaptive controller and the Slotine-Li (S&L) approach is given by numerical simulations using a three degree of freedom paradigm of typical Classical Mechanical systems, the cart + double pendulum. The effects of the imprecision of the available dynamical model, presence of dynamic friction at the axles of the drives, and the existence of external disturbance forces unknown and not modeled by the controller are considered. While the Slotine-Li approach tries to identify the parameters of the formally precise, available analytical model of the controlled system with the implicit assumption that the generalized forces are precisely known, the novel one makes do with a very rough, affine form and a formally more precise approximate model of that system, and uses temporal observations of its desired vs. realized responses. Furthermore, it does not assume the lack of unknown perturbations caused either by internal friction and/or external disturbances. Its another advantage is that it needs the execution of the SVD as a relatively time-consuming operation on a grid of a rough system-model only one time, before the commencement of the control cycle within which it works only with simple computations. The simulation examples exemplify the superiority of the FPT/SVD-based control that otherwise has the deficiency that it can get out of the region of its convergence. Therefore its design and use needs preliminary simulation investigations. However, the simulations also exemplify that its convergence can be guaranteed for various practical purposes.

Hepatitis B markers were determined in 397 individuals from Niterói and 680 from Nova Iguaçu and prevalences of 9.1% (1.0% of HBsAg and 8.1% of anti HBs) and 11.1% (1.8% of HBsAg and 9.3% of antiHBs) were found, respectively. The comparative prevalence of both markers in relation to age showed a higher prevalence of HBsAg in the group 21-50 years old. Considering the antiHBs antibody, it was demostrated a gradual increase with age, reaching 14.9% in Niterói and 29.1% in Nova Iguaçu in individuals over 51 years old. For hepatitis A, in 259 samples from Niterói, equally distributed by age groups, an overall prevalence of 74.5% of anti-HAV antibodies was found. This prevalence increases gradually reaching 90.0% at age over thirty. In 254 samples from Nova Iguaçu analysed, a prevalence of 90.5% of antibodies was encountered when the same criteria of distribution of samples were used. This level of prevalence reached 90.0% already in the age over ten years old. The tests were performed by enzyme immunoassay with reagents prepared in our laboratory.

The presence of antibodies against rotavirus was investigated by enzyme immunosorbent assay (ELISA) in two distinct groups of children living in a shanty town in Rio de Janeiro. One hundred and thirty six plasma samples were randomly collected from children of 0 to 33 months (first group) and 255 serum samples were collected from other 85 children at ages of 2, 6 and 9 months (second group). A high percentage of antibodies were found in the newborn children and this rate decreased progressively until the age of 11 months, after which it increased again. At the age of 7 months, geometric mean antibody titers increased indicating that infection had occurred.

An evaluation of the IgM antibody immune response against yellow fever using strain 17D was carried out by MAC-ELISA and PRNT. The results showed an agreement of 97% between both tests and the authors conclude that MAC-ELISA can be used as a specific and sensitive asssay to replace the PRNT for detecting yellow fever antibodies in human sera, after vaccination programs.

Laboratorial studies were carried out on 3178 patients with signs and symptoms suggestive of dengue infection from April 1986 to December 1987 in the State of Rio de Janeiro, Brazil. The epidemic had two peaks following the first virus isolation and affected the inhabitants of 17 counties. Both sex and all age groups were affected. Dengue virus type 1 was isolated from 1039 sera and the number of confirmed cases was increased to 1874 (59%) by MAC-ELISA. Isolation rate confirmed cases reached 80% in the specimens obtained until the 4th day after the onset of disease and viraemia ranged from 10 3.0 to 10(8.5) TCID50/ml.

During March 1994 cases of a exanthematic acute disease were reported in the municipalities of Itagemirim, Eunápolis and Belmonte, state of Bahia. Dengue fever was confirmed by serology (MAC-ELISA) and by dengue virus type 2 isolation, genotype Jamaica. Signs and symptoms of classic dengue fever were observed with a high percentual of rash (73.8%) and pruritus (50.5%). Major haemorrhagic manifestations were unfrequent and only bleeding gum was reported. Dengue virus activity spreaded rapidly to important tourism counties like Porto Seguro, Ilhéus, Santa Cruz de Cabrália, Prado, Alcobaça and others, representing a risk for the spreading of dengue virus into the country and abroad.

A seroepidemiologic survey was carried out in schoolchildren from public schools of the Niterói municipality, state of Rio de Janeiro, Brazil, after a period of sequential epidemics by dengue virus type 1 and 2 (DEN-1 and DEN-2). 450 blood samples were obtained by fingertip puncture and collected on filter paper discs. The hemagglutination inhibition (HAI) test was carried out using DEN-1 and DEN-2 antigens. HAI titres were demonstrated in 66% (297/450) of the sera and the geometric means of the titres were 1/182 and 1/71 for DEN-1 and DEN-2, respectively. Secondary infections were observed in 61% (181/297) of positive cases. Among these, 75% (135/181) were under fifteen years old. No dengue haemorrhagic fever (DHF) was reported in these children. Asymptomatic or oligosymptomatic infections were detected in 56% of the studied population. The absolute and relative frequencies of positive tests by age group and sex did not evidence statistically significant difference. The number of individuals infected probably produced a immunologic barrier responsible for the non occurrence of dengue epidemic in the latter years.

Strain typing is a critical tool for molecular epidemiological analysis and can provide important information about the spread of dengue viruses. Here, we performed a molecular characterization of DEN-2 viruses isolated in Brazil during 1990-2000 from geographically and temporally distinct areas in order to investigate the genetic distribution of this serotype circulating in the country. Restriction site-specific polymerase chain reaction (RSS)-PCR presented the same pattern for all 52 Brazilian samples, showing the circulation of just one DEN-2 variant. Phylogenetic analysis using progressive pairwise alignments from 240-nucleotide sequences of the E/NS1 junction in 15 isolates showed that they belong to genotype III (Jamaica genotype).

To compare sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) of self-diagnosis for head lice infestation with visual inspection, we conducted a study in an urban slum in Brazil. Individuals were asked about active head lice infestation (self-diagnosis); we performed visual inspection and thereafter wet combing (gold standard). Of the 175 individuals included, 77 (44%) had an active head lice infestation. For self-diagnosis, sensitivity (80.5%), specificity (91.8%), PPV (88.6%) and NPV (85.7%) were high. Sensitivity of visual inspection was 35.1%. Public health professionals can use self-diagnosis as a diagnostic tool, to estimate accurately prevalence of pediculosis in a community, and to monitor ongoing intervention strategies.

Inibidores contra as amostras de poliovírus do tipo 1, Mahoney e CHAT, foram estudados no sistema de gel de hidróxido de alumínio. O inibidor comportou-se como uma macroglobulina do tipo 19 S (IgM), através filtração em gel (Sephadex G-200), partição em DEAE-Celulose e sedimentação por ultra-centrifugação em gradiente de sacarose. O inibidor foi ainda destruído através o tratamento com 2-Mercapto-Etanol. O inibidor une-se à superfície do vírus na ausência de células. O complexo formado pelo vírus e o inibidor pode ser precipitado por um sôro preparado contra a globulina bovina; o mesmo complexo dissocia-se em idêntico valor de pH ao do complexo vírus-anticorpo 19 S. A chamada infecciosidade residual (cêrca de 10%) que permanece após a neutralização de poliovírus com sôro bovino representa uma dissociação parcial do inibidor da partícula de vírus, quando em presença das células em cultura. Foram obtidas amostras de vírus resistentes à ação específica de soros bovinos. Estas amostras permanecem sensíveis à ação de outros soros bovinos ativos, bem como à ação de anticorpos 7 S e 19 S. Através experiências de adsorção de atividade inibidora, foi possível mostrar a existência de inibidores monoespecíficos, com diferentes especificidades contra as amostras CHAT e Mahoney, respectivamente.

Os autores apresentam os resultados de isolamento de poliovírus de crianças com suspeita de poliomielite paralítica no Isolamento do Hospital Estadual Jesus do Estado da Guanabara. Nas 188 amostras computadas apresentaram-se 79 positivas, destas 73,5% de poliovírus tipo I, 25,3% do tipo II e 1,2% do tipo III. Os autores mostram no Gráfico 1, um aumento da incidência da poliomielite a partir dos últimos meses no ano de 1967. Chamam a atenção para o grande número de casos no grupo etário de 0-3 anos (Gráfico 2); colocam ainda em questão o melhor equacionamento do problema das vacinações (Tabela 1), com estudos laboratoriais de avaliação da vacina Sabin. O estudo da procedência e sexo das crianças não permitiu maiores conclusões, sendo levantada também a relação da idade com a incidência da poliomielite (Tabela 2), bem como a necessidade de se realizar estudos para outros enterovírus, presentes na população infantil da área estudada.

Sôros coletados no Território Federal do Amapá, localidade de Ferreira Gomes, foram, testados para anticorpos neutralizantes de enterovírus Poliovírus 1, 2 e 3 e Coxsackie B1 a B6, em população autóctone. Os resultados apresentados na Tabela 1, indicam alta circulação do vírus da poliomielite na região atingindo-se níveis tão elevados como em escolares do Estado da Guanabara. Em relação a Coxsackie B, alcançaram-se também resultados semelhantes nas duas populações exceto com Coxsackie B., o qual apresentou-se em valôres bem mais altos nos sôros coletados na região norte. Os autores chamam atenção da necessidade de vacinação contra a poliomielite de grandes segmentos da população susceptível em curto prazo, tendo em vista a ampla disseminação das infecções por poliovírus em tôdas as regiões do país onde forem pesquisadas.

196 soros de crianças provenientes de dois educandários da Guanabara foram ensaiados quanto à presença de anticorpos para rubéola. através do teste de inibição de hemaglutinação. Apenas 24 crianças sem anticorpos foram encontradas e destas crianças susceptíveis um grupo foi vacinado com amostra Cendehill liofilizada do Lab. R.I.T. (Genval, Bélgica), sendo o outro grupo deixado como controle. Observou-se 100% de formação de anticorpos para rubéola no grupo vacinado. O alto nivel de anticorpos pré-vacinais encontrado sugere alta circulação do vírus cla rubéola em nosso meio.
