The Government Decision1 on the Structural, Organisational, Financial Management and Systems Reform of the Health Sector of June, 2003 acknowledged that in order to increase the effectiveness of the health service generally, and its capacity to deliver the reform agenda, it was important that the service was fully concentrated on addressing its core health objectives. The Minister for Health and Children and Minister for Finance felt that there could be scope to transfer certain functions out of the health service and locate them more appropriately within other functional areas of Government. As part of the overall decision, it was agreed that a working group would be established, to include the Departments of Health and Children, Finance and An Taoiseach, to examine the scope for transfer of certain activities to other, more appropriate, Departments and agencies and that on completion of this review, the Minister for Health and Children would bring proposals to Government. The Minister for Health and Children established the Task Force on Sudden Cardiac Death (SCD) in the Autumn of 2004, with the following terms of reference:1) Define SCD and describe its incidence and underlying causes in Ireland.2) Advise on the detection and assessment of those at high risk of SCD and their relatives.3) Advise on the systematic assessment of those engaged in sports and exercise for risk of SCD.4) Advise on maximizing access to basic life support (BLS) and automated external defibrillators (AEDs) and on:- appropriate levels of training in BLS and use of AEDs, and on the maintenance of that training- priority individuals and priority groups for such training- geographic areas and functional locations of greatest need- best practice models of first responder scheme and public access defibrillation, and- integration of such training services.5) Advise on the establishment and maintenance of surveillance systems, including a registry of SCD and information systems to monitor risk assessment, and training and equipment programmes.6) Advise and make recommendations on other priority issues relevant to SCD in Ireland.7) Outline a plan for implementation and advise on monitoring the implementation of recommendations made in the Task Force’s report. In undertaking its work the Task Force was mindful of national health policy, relevant national strategies and of the recently reformed structures for health service delivery in Ireland. RATIONALE AND OBJECTIVES: Dose reduction may compromise patients because of a decrease of image quality. Therefore, the amount of dose savings in new dose-reduction techniques needs to be thoroughly assessed. To avoid repeated studies in one patient, chest computed tomography (CT) scans with different dose levels were performed in corpses comparing model-based iterative reconstruction (MBIR) as a tool to enhance image quality with current standard full-dose imaging. MATERIALS AND METHODS: Twenty-five human cadavers were scanned (CT HD750) after contrast medium injection at different, decreasing dose levels D0-D5 and respectively reconstructed with MBIR. The data at full-dose level, D0, have been additionally reconstructed with standard adaptive statistical iterative reconstruction (ASIR), which represented the full-dose baseline reference (FDBR). Two radiologists independently compared image quality (IQ) in 3-mm multiplanar reformations for soft-tissue evaluation of D0-D5 to FDBR (-2, diagnostically inferior; -1, inferior; 0, equal; +1, superior; and +2, diagnostically superior). For statistical analysis, the intraclass correlation coefficient (ICC) and the Wilcoxon test were used. RESULTS: Mean CT dose index values (mGy) were as follows: D0/FDBR = 10.1 ± 1.7, D1 = 6.2 ± 2.8, D2 = 5.7 ± 2.7, D3 = 3.5 ± 1.9, D4 = 1.8 ± 1.0, and D5 = 0.9 ± 0.5. Mean IQ ratings were as follows: D0 = +1.8 ± 0.2, D1 = +1.5 ± 0.3, D2 = +1.1 ± 0.3, D3 = +0.7 ± 0.5, D4 = +0.1 ± 0.5, and D5 = -1.2 ± 0.5. All values demonstrated a significant difference to baseline (P < .05), except mean IQ for D4 (P = .61). ICC was 0.91. CONCLUSIONS: Compared to ASIR, MBIR allowed for a significant dose reduction of 82% without impairment of IQ. This resulted in a calculated mean effective dose below 1 mSv.

Dental Treatment Services Scheme (DTSS) – Service Utilisation and Treatment Need (Lot 9) Since 2002, following agreement between the Department of Health and Children and the GMS Payments Board, data from the DTSS payments database has been transferred routinely to the Oral Health Services Research Centre (OHSRC), in University College, Cork for analysis. Queries have been developed in the OHSRC to provide individual level data on the treatments provided to patients, such as examinations, extractions, fillings, scale and polish, removal/amputation of roots, root treatment (upper and lower anterior teeth), radiographs, partial dentures and full dentures, and also data on the characteristics of the dentist providing services. Introduction: L'utilisation de marqueurs de l'inflammation comme la CRP pour le diagnostic, le pronostic et le suivi lors de pneumonie acquise dans la communauté (PAC) est une méthode rapide et bon marché. Cependant son utilité reste controversée etles avis d'expert divergent. Au CHUV, la CRP est dosée systématiquement chez les patients hospitalisés avec une PAC. L'objectif de ce travail est double: 1) effectuer une revue systématique de la littérature; 2) mieux comprendre l'usage et la perception de l'utilité clinique de la CRP en analysant la pratique des médecins du CHUV. Méthode: Nous avons envoyé un questionnaire par email à 2 reprises à tous les médecins assistants et chefs de clinique du service de médecine interne (SMI) et du centre interdisciplinaire des urgences (CIU) du CHUV. Les questions portaient sur la pratique clinique et sur la perception de l'utilité de la CRP dans le diagnostic, le pronostic et le suivi de patients hospitalisés pour une PAC. Résultats: Les résultats de la revue systématique sont présentés et analysés en intégrant les réponses issues du questionnaire. Nous avons reçu 53 réponses sur les 125 questionnaires envoyés. 62% des médecins interrogés demandent un dosage de la CRP dans >90% des cas lors d'une suspicion de PAC basée sur la clinique contre 45% si en plus la radiologie parle en faveur d'une PAC. 75% des médecins répètent ce dosage 1 à 4 fois durant le suivi. La raison principale évoquée est la suspicion d'une évolution défavorable et, plus rarement, la présence de comorbidités. Quand on questionne les médecins sur l'utilité de la CRP,on observe que seulement 30% pensent que la CRP est utile pour le diagnostic de PAC et 28% pour exclure une PAC lors de suspicion clinique. De même, 25% des médecins pensent qu'elle est utile dans le suivi des patients et 9% pour évaluer le pronostic de la PAC. On ne trouve pas d'association entre le niveau de formation et l'usage ou l'appréciation de l'utilité de la CRP. Conclusion: Les médecins assistants et les chefs de clinique du SMI et du CIU du CHUV dosent très souvent la CRP chez les patients hospitalisés pour une PAC. Leur motivation n'est ni liée à l'éventuelle valeur diagnostique de cet examen biologique,ni à son hypothétique valeur pour le pronostic et le suivi.
