That we live in a time of unprecedented and ever-increasing change is both a shibboleth of our age and the more-or-less explicit justification for all manner of “strategic” actions. The seldom, if ever, questioned assumption is that our now is more ephemeral, more evanescent, than any that preceded it. In this essay, we subject this assumption to some critical scrutiny, utilizing a range of empirical detail. In the face of this assay we find the assumption to be considerably wanting. We suggest that what we are actually witnessing is mere acceleration, which we distinguish as intensification along a preexisting trajectory, parading as more substantive and radical movement away from a preexisting trajectory. Deploying Deleuze's (2004) terms we are, we suggest, in thrall to representation of the same at the expense of repetition of difference. Our consumption by acceleration, we argue, both occludes the lack of substantive change actually occurring while simultaneously delimiting possibilities of thinking of and enacting the truly radical. We also consider how this setup is maintained, thus attempting to shed some light on why we are seemingly running to stand still. As the Red Queen said, “it's necessary to run faster even to stay in the one place.”

BACKGROUND: Less than 1% of severely obese US adults undergo bariatric surgery annually. It is critical to understand the factors that contribute to its utilization. OBJECTIVES: To understand how primary care physicians (PCPs) make decisions regarding severe obesity treatment and bariatric surgery referral. SETTING: Focus groups with PCPs practicing in small, medium, and large cities in Wisconsin. METHODS: PCPs were asked to discuss prioritization of treatment for a severely obese patient with multiple co-morbidities and considerations regarding bariatric surgery referral. Focus group sessions were analyzed by using a directed approach to content analysis. A taxonomy of consensus codes was developed. Code summaries were created and representative quotes identified. RESULTS: Sixteen PCPs participated in 3 focus groups. Four treatment prioritization approaches were identified: (1) treat the disease that is easiest to address; (2) treat the disease that is perceived as the most dangerous; (3) let the patient set the agenda; and (4) address obesity first because it is the common denominator underlying other co-morbid conditions. Only the latter approach placed emphasis on obesity treatment. Five factors made PCPs hesitate to refer patients for bariatric surgery: (1) wanting to "do no harm"; (2) questioning the long-term effectiveness of bariatric surgery; (3) limited knowledge about bariatric surgery; (4) not wanting to recommend bariatric surgery too early; and (5) not knowing if insurance would cover bariatric surgery. CONCLUSION: Decision making by PCPs for severely obese patients seems to underprioritize obesity treatment and overestimate bariatric surgery risks. This could be addressed with PCP education and improvements in communication between PCPs and bariatric surgeons.

Despite major improvements in access to liver transplantation (LT), disparities remain. Little is known about how distrust in medical care, patient preferences, and the origins shaping those preferences contribute to differences surrounding access. We performed a single-center, cross-sectional survey of adults with end-stage liver disease and compared responses between LT listed and nonlisted patients as well as by race. Questionnaires were administered to 109 patients (72 nonlisted; 37 listed) to assess demographics, health care system distrust (HCSD), religiosity, and factors influencing LT and organ donation (OD). We found that neither HCSD nor religiosity explained differences in access to LT in our population. Listed patients attained higher education levels and were more likely to be insured privately. This was also the case for white versus black patients. All patients reported wanting LT if recommended. However, nonlisted patients were significantly less likely to have discussed LT with their physician or to be referred to a transplant center. They were also much less likely to understand the process of LT. Fewer blacks were referred (44.4% versus 69.7%; P = 0.03) or went to the transplant center if referred (44.4% versus 71.1%; P = 0.02). Fewer black patients felt that minorities had as equal access to LT as whites (29.6% versus 57.3%; P < 0.001). For OD, there were more significant differences in preferences by race than listing status. More whites indicated OD status on their driver's license, and more blacks were likely to become an organ donor if approached by someone of the same cultural or ethnic background (P < 0.01). In conclusion, our analysis demonstrates persistent barriers to LT and OD. With improved patient and provider education and communication, many of these disparities could be successfully overcome. This paper develops a simple model of the post-secondary education system in Canada that provides a useful basis for thinking about issues of capacity and access. It uses a supply-demand framework, where demand comes on the part of individuals wanting places in the system, and supply is determined not only by various directives and agreements between educational ministries and institutions (and other factors), but also the money available to universities and colleges through tuition fees. The supply and demand curves are then put together with a stylised tuition-setting rule to describe the “market” of post-secondary schooling. This market determines the number of students in the system, and their characteristics, especially as they relate to “ability” and family background, the latter being especially relevant to access issues. The manner in which various changes in the system – including tuition fees, student financial aid, government support for institutions, and the returns to schooling – are then discussed in terms of how they affect the number of students and their characteristics, or capacity and access.

It is argued in this study that current investigations of the role of conflict in shared leadership teams and, thus, teams in which all members have the opportunity to participate in its decision-making process are insufficient as they have focused on the downsides of these conflicts. This study demonstrates that task conflict is beneficial in that it can have positive effects on innovation in teams. It shows that particularly in shared leadership management consultant teams task conflict can stimulate innovation. Therefore, this research investigates the relationships among shared leadership, conflict and innovation. The research develops and empirically tests a conceptual model which demonstrates the relationships between these concepts and for which the inclusion of multiple research methods was essential. The sequential explanatory approach included a combination of quantitative and qualitative methods, the order of which can be adapted for other domains of application. The conceptual model was first tested with a sample of 329 management consultants. This was followed by 25, in-depth, face-to-face interviews conducted with individual survey respondents. In addition, weekly meetings of a management consultant team in action were video recorded over several months. This allowed for an in-depth explanation of the findings from the survey by providing an understanding of the underlying processes. The inclusion of observational methods provided a validating role and explained how and why conflicts contributed to the development of team innovation, through the analysis of subtleties and fleeting disagreements in a real-life management consultant team. The results deliver an assessment of the theoretical model and demonstrate that task conflict can allow for additional innovation in management consultant teams operating under a shared leadership structure. A practical model and guidelines for management consultant teams wanting to enhance their innovatory capacities are provided. In addition, a novel-user methodology which includes video observations is developed, with recommendations and steps aiding researchers aiming to employ a similar combination of methods. An original contribution to knowledge is made regarding the positive effects that task conflict can have towards innovation in shared leadership teams. Collaboration and trust are identified as important mediators between shared leadership and task conflict and significant regarding the development of innovation. The effectiveness of shared leadership in reducing negative relationship conflict and the benefits of both shared leadership and task conflict in enhancing innovation are demonstrated.

The two major varieties of English are American and British English. In Swedish compulsory school both varieties are taught. This essay examines the preferences and attitudes of pupils in 9th grades towards the two varieties, and if there are any gender differences. Previous research has shown that pupils are becoming more and more positive towards American English than they have been in earlier studies. This essay is based on a quantitative study carried out through questionnaires handed-out to 84 9th graders. The results show that the pupils prefer American English in speaking and for educational purposes. However, British English is viewed as more intelligent, professional, beautiful and correct. These findings mostly correspond to previous research but also show some contrasting features such as pupils wanting to speak British English rather than American English.

Keynote speaker for a one day symposium in Fine Art and Photography at De Montfort University, Leicester. The symposium looked at research as a rigorous process of investigation leading to new knowledge. It was aimed at visual arts practitioners wanting to account for their practices as forms of research. Two key questions were addressed: 1. How can arts practitioners account for the rigour of the methods they use in practice? 2. To what kinds of knowledge can their practices lead? Coutts' keynote looked at the concept of 'Unlearning' used by Marie Luisa Knott to explore what Hannah Arendt first had to do in order to rethink the atrocities carried out by the Nazis in the 20th Century. To be able to think anew means also to find a means for undoing what has previously been thought. Coutts looked to Arendt amongst other thinkers and artists, and used examples from within her own practice, to reflect on rigour in research and methods for legitimising a range of visual practices.

Background The culture of current clinical practice calls for collaboration between therapists and patients, sharing power and responsibility. This paper reports on the findings of a qualitative study of exercise prescription for patients with NSCLBP, taking into account issues such as decision making and how this accords with patient preferences and experiences. Objective To understand the treatment decision making experiences, information and decision support needs of patients with NSCLBP who have been offered exercise as part of their management plan. Design A qualitative study using a philosophical hermeneutic approach. Methods Semi-structured interviews with eight patients (including use of brief patient vignettes) was undertaken to explore their personal experiences of receiving exercise as part of the management of their NSCLBP, and their involvement in decisions regarding their care. Findings The findings provide a detailed insight into patients’ perceptions and experiences of receiving exercise-based management strategies. Four themes were formed from the texts: (1) patients’ expectations and patients’ needs are not synonymous, (2) information is necessary but often not sufficient, (3) not all decisions need to be shared, and (4) wanting to be treated as an individual. Conclusions Shared decision making did not appear to happen in physiotherapy clinical practice, but equally may not be what every patient wants. The overall feeling of the patients was that the therapist was dominant in structuring the interactions, leaving the patients feeling disempowered to question and contribute to the decision making.

Ce mémoire présente mon projet de maîtrise dont l’objectif était d’étudier les associations entre la récompense alimentaire à l’égard des aliments riches en gras et en sucres, le statut pondéral et les comportements alimentaires. Cent cinquante femmes de poids normal ou obèses ont été invitées à compléter deux questionnaires : un sur la récompense alimentaire (plaisir explicite, motivation explicite et motivation implicite) et l’autre sur les comportements alimentaires [restriction cognitive (totale, flexible et rigide), désinhibition (totale, habituelle, émotionnelle et situationnelle) et susceptibilité à la faim (totale, interne et externe)]. Aucune association n’a été observée entre la récompense alimentaire et le statut pondéral. Toutefois, un plaisir et une motivation explicites plus importants pour les aliments en général ont été notés chez les femmes avec un score plus élevé de restriction flexible. De plus, une motivation implicite plus importante pour les aliments riches en gras tendait à être associée avec un score plus élevé de susceptibilité émotionnelle à la désinhibition et a été associée significativement avec un score plus élevé de faim externe. Ces résultats suggèrent que les comportements alimentaires ont une plus grande influence sur la récompense alimentaire que le statut pondéral.

Objetivou-se conhecer as implicações do cuidado à criança e ao adolescente vítimas de queimaduras para a prática da enfermagem. Realizou-se uma pesquisa descritiva e exploratória com abordagem qualitativa. Participaram dez profissionais da equipe de enfermagem de um Centro de Referência a Pacientes Queimados do sul do Brasil. Os dados foram coletados no segundo semestre de 2012 por meio de entrevistas semiestruturadas e analisados pela técnica de Análise de Conteúdo. Em relação aos sentimentos frente ao cuidado verificou-se que esses vivenciam ansiedade e tensão frente à dor do paciente, têm a sensação de doação querendo “fazer mais”, tristeza e abalo, sensação de utilidade e de competência ao ver os efeitos do cuidado, impotência por não terem controle sobre a situação vivenciada, revolta e raiva por não compreenderem o porquê este acidente aconteceu e pena dos pacientes e de seus familiares devido o seu sofrimento. Como facilidades para o cuidado referiram a ajuda mútua entre os membros da equipe aliada ao tempo de atuação no setor, o desenvolvimento de um bom relacionamento com a família da criança / adolescente, a sinceridade da criança ao manifestar seus sentimentos, uma identificação e afinidade maior para cuidar crianças e adolescentes e o adolescente ser mais aberto e entender com facilidade a linguagem utilizada no setor. Referiram como dificuldades à falta de preparo e a pouca habilidade para cuidar de crianças/ adolescentes com dor, o desconhecimento acerca do paciente, a falta de habilidades técnicas para realizar procedimentos em crianças/ adolescentes, lidar com o familiar, lidar com a necessidade de manipular o corpo do adolescente, comunicar-se com crianças que não sabem expressar-se, pacientes que não falam o português e adolescentes que possuem linguagem própria, explicar para o paciente a magnitude do trauma sofrido e conversar com esses acerca das sequelas, deformidades e limitações com as quais terão que (con)viver. Quanto às estratégias para se instrumentalizar para o cuidado utilizam a leitura sobre queimaduras e curativos, leituras de materiais de outras áreas da saúde, uso de técnicas de abordagem e interação com pacientes e familiares, a prática diária no setor e a busca de apoio na equipe e na instituição, realizando atividades de educação continuada. Quanto às estratégias utilizadas para cuidar referiram o estabelecimento de vínculo e de uma relação dialógica, o uso de brincadeiras e atividades lúdicas, o fornecimento de apoio, a introdução da família no processo de cuidado, o uso da criatividade, a valorização do aspecto psicológico do paciente, a adaptação do cuidado de acordo com a faixa etária do paciente e o uso da escuta atenta e sensível. A partir dos dados concluiu-se que o cuidado de enfermagem a crianças e adolescentes vítimas de queimaduras é complexo bem como causador de impacto para os profissionais atuantes em Centros de Queimados. Acredita-se que o estudo possibilitará discutir e refletir acerca da prática profissional da enfermagem no Centro de Queimados frente ao cuidado à criança e ao Adolescente vítima de queimaduras.

Emigration is now both a solution and a problem. To us, the ones wanting and seeking a better life, to emigrate is a solution but to the countries we choose to go, we are (or can be) little more than a problem: they even decide to shut borders or to limit the number of people allowed in and when this is not enough they set requirements: one must speak the language of the chosen country, possess awareness of the cultural background and current politics. It is true that other EU members economy is nowhere near as bad as ours but is equally true that they want it to stay like this

Assessing and addressing odour impacts from poultry production is extremely difficult and subjective because the odorants involved and their dynamics over time and space are poorly understood. This knowledge gap is due, in part, to the lack of suitable analytical tools for measuring and monitoring odorants in the field. The emergence of Selected Ion Flow Tube – Mass Spectrometry (SIFT–MS) and similar instruments is changing that. These tools can rapidly quantify targeted odorants in ambient air in real time, even at very low concentrations. Such data is essential for developing better odour abatement strategies, assessment methods and odour dispersion models. This project trialled a SIFT–MS to determine its suitability for assessing the odorants in meat chicken shed emissions over time and space. This report details evaluations in New Zealand and Australia to determine the potential of SIFT–MS as a tool for the chicken meat industry, including odour measurement (as a proxy for dynamic olfactometry). The report is specifically targeted at those funding and conducting poultry odour research. It will be of interest to those involved with environmental odour monitoring and assessment in general. The high upfront cost of SIFT–MS will lead to potential users wanting compelling evidence that SIFT–MS will meet their needs before they invest in one.

[Der Beitrag nähert sich der Frage], ob die aktuelle JeKi-Praxis günstige Voraussetzungen für die erweiterte Einführung inklusiver Settings bietet und wie die bestehenden Ansätze eines inklusiven JeKi-Unterrichts in Grundschulen mit gemeinsamem Unterricht eingeschätzt werden können. [...] Für jede dieser Ebenen wurden in der Studie „JeKi und gemeinsamer Unterricht" ausgewählte Fragestellungen untersucht, in der vorliegenden Darstellung sollen zwei Aspekte im Vordergrund stehen, die der innerpsychischen und der institutionellen Ebene zugeordnet werden können.
