Las disputas en torno a determinados aspectos del dinero, como su neutralidad y el carácter endógeno o exógeno de la oferta monetaria, han sido permanentes entre las distintas escuelas de pensamiento y autores, estando su origen, probablemente, en la época de desarrollo del pensamiento escolástico. En este artículo pretendemos, en primer lugar, realizar un recorrido cronológico e histórico sobre el tratamiento científico económico del dinero, para, en segundo lugar, poner sobre la mesa la macroeconomía ortodoxa a la que han dado lugar las interpretaciones al respecto, así como los enfoques alternativos frente a este pensamiento dominante. Finalmente, intentamos poner en valor los desarrollos monetarios post-keynesianos, integrados en lo que denominan “Economía Monetaria de Producción”, confrontándolos con la llamada Nueva Síntesis Neoclásica.

BACKGROUND: Prostate cancer might have high radiation-fraction sensitivity that would give a therapeutic advantage to hypofractionated treatment. We present a pre-planned analysis of the efficacy and side-effects of a randomised trial comparing conventional and hypofractionated radiotherapy after 5 years follow-up.
METHODS: CHHiP is a randomised, phase 3, non-inferiority trial that recruited men with localised prostate cancer (pT1b-T3aN0M0). Patients were randomly assigned (1:1:1) to conventional (74 Gy delivered in 37 fractions over 7·4 weeks) or one of two hypofractionated schedules (60 Gy in 20 fractions over 4 weeks or 57 Gy in 19 fractions over 3·8 weeks) all delivered with intensity-modulated techniques. Most patients were given radiotherapy with 3-6 months of neoadjuvant and concurrent androgen suppression. Randomisation was by computer-generated random permuted blocks, stratified by National Comprehensive Cancer Network (NCCN) risk group and radiotherapy treatment centre, and treatment allocation was not masked. The primary endpoint was time to biochemical or clinical failure; the critical hazard ratio (HR) for non-inferiority was 1·208. Analysis was by intention to treat. Long-term follow-up continues. The CHHiP trial is registered as an International Standard Randomised Controlled Trial, number ISRCTN97182923.
FINDINGS: Between Oct 18, 2002, and June 17, 2011, 3216 men were enrolled from 71 centres and randomly assigned (74 Gy group, 1065 patients; 60 Gy group, 1074 patients; 57 Gy group, 1077 patients). Median follow-up was 62·4 months (IQR 53·9-77·0). The proportion of patients who were biochemical or clinical failure free at 5 years was 88·3% (95% CI 86·0-90·2) in the 74 Gy group, 90·6% (88·5-92·3) in the 60 Gy group, and 85·9% (83·4-88·0) in the 57 Gy group. 60 Gy was non-inferior to 74 Gy (HR 0·84 [90% CI 0·68-1·03], pNI=0·0018) but non-inferiority could not be claimed for 57 Gy compared with 74 Gy (HR 1·20 [0·99-1·46], pNI=0·48). Long-term side-effects were similar in the hypofractionated groups compared with the conventional group. There were no significant differences in either the proportion or cumulative incidence of side-effects 5 years after treatment using three clinician-reported as well as patient-reported outcome measures. The estimated cumulative 5 year incidence of Radiation Therapy Oncology Group (RTOG) grade 2 or worse bowel and bladder adverse events was 13·7% (111 events) and 9·1% (66 events) in the 74 Gy group, 11·9% (105 events) and 11·7% (88 events) in the 60 Gy group, 11·3% (95 events) and 6·6% (57 events) in the 57 Gy group, respectively. No treatment-related deaths were reported.
INTERPRETATION: Hypofractionated radiotherapy using 60 Gy in 20 fractions is non-inferior to conventional fractionation using 74 Gy in 37 fractions and is recommended as a new standard of care for external-beam radiotherapy of localised prostate cancer.

FERNANDES, M. de Fátima. A expressão metafórica do sentido de existir na Literatura Cabo-verdiana contemporânea – João Varela, Corsino Fortes e José Luís Tavares.2013. 274 f. Tese (Doutorado) FFLCH – Faculdade de Filosofia, Letras e Ciências Humanas, Universidade de São Paulo, São Paulo, 2013. A produção literária assinada pelos autores cabo-verdianos João Vário (heterônimo de João Varela), Corsino Fortes e José Luís Tavares constitui o objeto central desta análise desenvolvida a partir do estudo teórico do conceito de Identidade, visando enquadrar teoricamente a produção literária cabo-verdiana contemporânea, identificar as principais linhas de sua orientação (conteúdos, temática, estilos, retórica), relevando a receptividade da intercomunicação cultural e literária de/entre eles, bem como apresentar uma fundamentação teórica e metodológica para a leitura de suas obras. Aliando conteúdos teóricos aos contextos de produção poética pré e pós-independência, analisamos poemas das obras Exemplos (1966-1998), de João Vário, A cabeça calva de Deus (2001), de Corsino Fortes, e Agreste matéria mundo (2004), de José Luís Tavares – para verificar o posicionamento dos sujeitos literários na (re)construção da(s) identidade(s) na Literatura Cabo-Verdiana a partir dos anos 1960 do século XX à atualidade. O tratamento do tema visa caracterizar uma expressão metafórica do sentido de existir na Literatura Cabo-verdiana contemporânea, consubstanciada essencialmente no cruzamento de orientações dos estudos literários pós-estruturalistas, pós-coloniais, Estudos Comparados e Teoria da Literatura, e ainda com base na exploração de conteúdos literários com uma forte dimensão filosófica da contemporaneidade, conforme concebida por teóricos como Stuart Hall, Zigmut Bauman, Antonny Giddens, Michel Foucault, Jacques Derrida, Edward Said, Hassan Zaoual e Benedict Anderson, Milton Santos, entre outros.

This report presents a metric called FRESH (for Foodservice Impact Rating for Environmentally Sustainable Hospitality Events). FRESH can be used to evaluate the performance of any foodservice meal period or event in the hospitality sector with regards to its sustainability, based on seven measurements. These measures are: a (post-consumer) food-waste indicator, a no-show indicator (when unexpectedly few people show up), an over-show measure (when too many people show up), a planning indicator (measuring intentional overproduction), a portion-size indicator (measuring per-guest consumption against expectations), an economies of scale indicator, and a post-event indicator (which depends on disposal approaches). FRESH can help managers, authorities, and potential guests evaluate the sustainability of food production in any establishment.

Although anti−cancer immuno−based combinatorial therapeutic approaches have shown promising results, efficient tumour eradication demands further intensification of anti−tumour immune response. With the emerging field of nanovaccinology, multi−walled carbon nanotubes (MWNTs) have manifested prominent potentials as tumour antigen nanocarriers. Nevertheless, the utilization of MWNTs in co−delivering antigen along with different types of immunoadjuvants to antigen presenting cells (APCs) has not been investigated yet. We hypothesized that harnessing MWNT for concurrent delivery of cytosine−phosphate−guanine oligodeoxynucleotide (CpG) and anti-CD40 Ig (αCD40), as immunoadjuvants, along with the model antigen ovalbumin (OVA) could potentiate immune response induced against OVA−expressing tumour cells. We initially investigated the effective method to co−deliver OVA and CpG using MWNT to the APC. Covalent conjugation of OVA and CpG prior to loading onto MWNTs markedly augmented the CpG−mediated adjuvanticity, as demonstrated by the significantly increased OVA−specific T cell responses in vitro and in C57BL/6 mice. αCD40 was then included as a second immunoadjuvant to further intensify the immune response. Immune response elicited in vitro and in vivo by OVA, CpG and αCD40 was significantly potentiated by their co−incorporation onto the MWNTs. Furthermore, MWNT remarkably improved the ability of co−loaded OVA, CpG and αCD40 in inhibiting the growth of OVA−expressing B16F10 melanoma cells in subcutaneous or lung pseudo−metastatic tumour models. Therefore, this study suggests that the utilization of MWNTs for the co−delivery of tumour−derived antigen, CpG and αCD40 could be a competent approach for efficient tumours eradication.

D’abord, nous présentons les principes physiques nous permettant de modéliser et comprendre le phénomène de propagation linéaire des impulsions lumineuses dans un milieu homogène, dans les guides d’ondes planaires et enfin dans les fibres optiques microstructurées. Ensuite, nous faisons une analyse mathématique rigoureuse des équations linéaires de propagation et posons le problème comme celui de la recherche de valeurs propres d’opérateurs auto-adjoints dans un espace de Hilbert. On verra que ces résultats théoriques s’appliquent aux équations simulées dans le logiciel Comsol Multiphysics. Enfin, nous recensons et proposons différentes façons de prédire les valeurs de dispersion chromatique et d’atténuation dans les fibres microstructurées à coeur suspendu en utilisant les notions et équations discutés dans les deux premiers chapitres. Le choix de la géométrie, du matériau et de la longueur d’onde de la lumière transmise sont parmi les variables étudiées numériquement. Nous ferons également un exemple détaillé d’utilisation du logiciel Comsol Multiphysics pour construire un modèle de fibre optique microstructurée.

Computer-based tools for assessing micro-longitudinal patterns of cognitive function in older adults

Patterns of cognitive change over micro-longitudinal timescales (i.e., ranging from hours to days) are associated with a wide range of age-related health and functional outcomes. However, practical issues with conducting high-frequency assessments make investigations of micro-longitudinal cognition costly and burdensome to run. One way of addressing this is to develop cognitive assessments that can be performed by older adults, in their own homes, without a researcher being present. Here, we address the question of whether reliable and valid cognitive data can be collected over micro-longitudinal timescales using unsupervised cognitive tests.In study 1, 48 older adults completed two touchscreen cognitive tests, on three occasions, in controlled conditions, alongside a battery of standard tests of cognitive functions. In study 2, 40 older adults completed the same two computerized tasks on multiple occasions, over three separate week-long periods, in their own homes, without a researcher present. Here, the tasks were incorporated into a wider touchscreen system (Novel Assessment of Nutrition and Ageing (NANA)) developed to assess multiple domains of health and behavior. Standard tests of cognitive function were also administered prior to participants using the NANA system.Performance on the two “NANA” cognitive tasks showed convergent validity with, and similar levels of reliability to, the standard cognitive battery in both studies. Completion and accuracy rates were also very high. These results show that reliable and valid cognitive data can be collected from older adults using unsupervised computerized tests, thus affording new opportunities for the investigation of cognitive function.

The role of computer modeling has grown recently to integrate itself as an inseparable tool to experimental studies for the optimization of automotive engines and the development of future fuels. Traditionally, computer models rely on simplified global reaction steps to simulate the combustion and pollutant formation inside the internal combustion engine. With the current interest in advanced combustion modes and injection strategies, this approach depends on arbitrary adjustment of model parameters that could reduce credibility of the predictions. The purpose of this study is to enhance the combustion model of KIVA, a computational fluid dynamics code, by coupling its fluid mechanics solution with detailed kinetic reactions solved by the chemistry solver, CHEMKIN. As a result, an engine-friendly reaction mechanism for n-heptane was selected to simulate diesel oxidation. Each cell in the computational domain is considered as a perfectly-stirred reactor which undergoes adiabatic constant- volume combustion. The model was applied to an ideally-prepared homogeneous- charge compression-ignition combustion (HCCI) and direct injection (DI) diesel combustion. Ignition and combustion results show that the code successfully simulates the premixed HCCI scenario when compared to traditional combustion models. Direct injection cases, on the other hand, do not offer a reliable prediction mainly due to the lack of turbulent-mixing model, inherent in the perfectly-stirred reactor formulation. In addition, the model is sensitive to intake conditions and experimental uncertainties which require implementation of enhanced predictive tools. It is recommended that future improvements consider turbulent-mixing effects as well as optimization techniques to accurately simulate actual in-cylinder process with reduced computational cost. Furthermore, the model requires the extension of existing fuel oxidation mechanisms to include pollutant formation kinetics for emission control studies.

Les coopératives en milieu scolaire oeuvrent dans le domaine de la librairie depuis très longtemps. Comme toute organisation, elles ont des forces et des faiblesses. Sur le marché de la librairie en plein concentration, elles occupent, plus de la moitié du marché dans les librairies de campus, et cette proportion est en croissance. En 1993, 99 coopératives en milieu scolaire étaient déclarées dont 58 emploient 534 employés(es); elles ont réalisé un chiffre d'affaire d'environ 70 millions $ et possèdent un actif d'une valeur de 15,6 millions $ en servant 690.800 membres. Les coopératives scolaires se sont regroupées en fédération en 1983 et ce afin de promouvoir leurs intérêts économiques et sociaux. L'instauration de la loi 51 « loi sur le développement des entreprises Québécoises dans le domaine du livre scolaire » présente beaucoup d'obstacles pour les coopératives étant donné qu'elle n'est pas claire et qu'elle est facilement contournable. En plus de cette loi, les coopératives en milieu scolaire ont des difficultés à contrôler leur frais d'approvisionnement, elles n'ont pas de pouvoir de négociation avec les fournisseurs qui leur imputent les frais de manutention aux frais de transport; elles sont perçues par les éditeurs comme des librairies de seconde zone; elles ont un coût de transport très élevé; elles ne peuvent pas profiter des économies d'échelle et elles n'ont aucun avantage dû à la taille dans la négociation avec les fournisseurs.

Hepatitis C virus (HCV) is emerging as one of the leading causes of morbidity and mortality in individuals infected with HIV and has overtaken AIDS-defining illnesses as a cause of death in HIV patient populations who have access to highly active antiretroviral therapy. For many years, the clonal analysis was the reference method for investigating viral diversity. In this thesis, a next generation sequencing (NGS) approach was developed using 454 pyrosequencing and Illumina-based technology. A sequencing pipeline was developed using two different NGS approaches, nested PCR, and metagenomics. The pipeline was used to study the viral populations in the sera of HCV-infected patients from a unique cohort of 160 HIV-positive patients with early HCV infection. These pipelines resulted in an improved understanding of HCV quasispecies dynamics, especially regarding studying response to treatment. Low viral diversity at baseline correlated with sustained virological response (SVR) while high viral diversity at baseline was associated with treatment failure. The emergence of new viral strains following treatment failure was most commonly associated with emerging dominance of pre-existing minority variants rather than re-infection. In the new era of direct-acting antivirals, next generation sequencing technologies are the most promising tool for identifying minority variants present in the HCV quasispecies populations at baseline. In this cohort, several mutations conferring resistance were detected in genotype 1a treatment-naïve patients. Further research into the impact of baseline HCV variants on SVR rates should be carried out in this population. A clearer understanding of the properties of viral quasispecies would enable clinicians to make improved treatment choices for their patients.

Thoracic aortic dissection (TAD) has a very high mortality rate and is often missed due to the atypical presentation of patients. We present a case of a man with chronic hypertension, atrial fibrillation (AF) (on regular warfarin) and a previous endovascular aneurysm repair (EVAR), who presented with dyspepsia and was incidentally found to have a ruptured distal thoracic aneurysm on imaging with no obvious clinical signs on examination, nor abnormalities on admission chest x-ray (CXR).

La pregunta sobre el humanismo está íntimamente ligada con las metáforas que las distintas sociedades en diferentes momentos han utilizado no sólo para comprender las relaciones con su entorno, sino también para estructurar su visión sobre sí mismas.En la Edad Media, el topos del microcosmos y el macrocosmos no sólo servía para reconocer en el mundo los principios de la analogía y la similitud como articuladores de las «realidades» naturales y divinas, sino que permitían una vía de interpretación de esas dos instancias en una visión totalizadora en la que la Trinidad se presentaba como metáfora por excelencia. Por otro lado, el humanismo renacentista aliviado ya de la metáfora anterior, y entusiasmado con una nueva -el hombre- se aventuró a explorar y a inventar bordes y límites por los que transitara su renovada humanidad.
