Introducción: La artritis séptica neonatal es un padecimiento poco común que se ha relacionado con recién nacidos internados en la unidad de cuidados intensivos neonatal, cuyo principal factor de riesgo es el uso previo de catéter central. Objetivo: establecer la prevalencia de la artritis séptica neonatal en nuestro Hospital. Material y método: estudio observacional descriptivo de prevalencia. Se incluyeron todos los recién nacidos con diagnostico de artritis séptica neonatal internados un la unidad de cuidados intermedios e intensivos neonatal del Hospital Universitario Dr. José Eleuterio González del año 2003 al 2007. Se revisaron los expedientes y se registraron variables generales, factores de riesgo, bacteriología y mortalidad. Resultados: Se diagnosticaron 26 casos de artritis séptica neonatal durante el periodo de tiempo estudiado, solo 20 casos lograron criterios de inclusión: la prevalencia fue de 1.2 casos por cada 1,000 nacidos. La edad gestacional fue 39.2 ± 1.3 semanas, con peso 3193 ± 709 g. El factor de riesgo que se presento con mas frecuencia fue el antecedente de uso previo de catéter en 75% de los casos. Se aisló microorganismo en 80% de los casos, los bacterias grampositivas se aislaron en 80% de los casos; el Staphylococcus aureus fue el microorganismo aislado predominantemente en 75% de los casos. La presentación poliarticular fue en 40% de los casos. La mortalidad fue de 0%. Conclusiones: La prevalencia fue menor a lo reportado en la literatura. Los microorganismos grampositivos continúan siendo los microorganismos predominantes en la artritis séptica neonatal. La mortalidad es baja.

La neosporosis bovina es una enfermedad parasitaria causada por el protozoo Neospora caninum (NC). Se considera una de las principales causas de aborto en la especie bovina, especialmente en el ganado lechero y está relacionada con pérdidas productivas y trastornos de la fertilidad. Su prevalencia es variable alrededor del mundo así como su comportamiento epidemiológico. El objetivo de este trabajo fue determinar la relación entre la seropositividad producto de la transmisión horizontal y distintos problemas reproductivos identificados en vacas Holstein de la sierra sur del Ecuador.

Background False-positive blood cultures findings may lead to a falsely increased morbidity and increased hospital costs. Method The survey was conducted as retrospective - prospective study and included 239 preterm infants (born before 37 weeks of gestation) who were treated in Neonatal Intensive Care Unit (NICU) in Institute for Child and Youth Health Care of Vojvodina during one year (January 1st, 2012 to December 31st, 2012). The retrospective part of the study focused on examination of incidence of neonatal sepsis and determination of risk factors. In the prospective part of the study infants were sub-divided into two groups: Group 1- infants hospitalized in NICU during the first 6 months of the study; blood cultures were taken by the ‘’clean technique’’ and checklists for this procedure were not taken. Group 2- neonates hospitalized in NICU during last 6 months of the study; blood cultures were taken by ‘’sterile technique’’ and checklists for this procedure were taken. Results The main risk factors for sepsis were prelabor rupture of membranes, low gestational age, low birth weight, mechanical ventilation, umbilical venous catheter placement, and abdominal drainage. Staphylococcus aureus and coagulase negative Staphylococcus were the most frequently isolated microorganisms in false-positive blood samples. Conclusions Education of employees, use of checklists and sterile sets for blood sampling, permanent control of false positive blood cultures, as well as regular and routine monthly reports are crucial for successful reduction of contamination rates.

Introduction Jaundice is the yellowish pigmentation of the skin, sclera, and mucous membranes resulting from bilirubin deposition. Children born to mothers with HIV are more likely to be born premature, with low birth weight, and to become septic—all risk factors for neonatal jaundice. Further, there has been a change in the prevention of mother-to-child transmission (PMTCT) of HIV guidelines from single-dose nevirapine to a six-week course, all of which theoretically put HIV-exposed newborns at greater risk of developing neonatal jaundice. Aim We carried out a study to determine the incidence of severe and clinical neonatal jaundice in HIV-exposed neonates admitted to the Chatinkha Nursery (CN) neonatal unit at Queen Elizabeth Central Hospital (QECH) in Blantyre. Methods Over a period of four weeks, the incidence among non-exposed neonates was also determined for comparison between the two groups of infants. Clinical jaundice was defined as transcutaneous bilirubin levels greater than 5 mg/dL and severe jaundice as bilirubin levels above the age-specific treatment threshold according the QECH guidelines. Case notes of babies admitted were retrieved and information on birth date, gestational age, birth weight, HIV status of mother, type of feeding, mode of delivery, VDRL status of mother, serum bilirubin, duration of stay in CN, and outcome were extracted. Results Of the 149 neonates who were recruited, 17 (11.4%) were HIV-exposed. One (5.88%) of the 17 HIV-exposed and 19 (14.4%) of 132 HIVnon- exposed infants developed severe jaundice requiring therapeutic intervention (p = 0.378). Eight (47%) of the HIV-exposed and 107 (81%) of the non-exposed neonates had clinical jaundice of bilirubin levels greater than 5 mg/dL (p < 0.001). Conclusions The study showed a significant difference in the incidence of clinical jaundice between the HIV-exposed and HIV-non-exposed neonates. Contrary to our hypothesis, however, the incidence was greater in HIVnon- exposed than in HIV-exposed infants.

Se realizó un estudio observacional en recién nacidos sépticos ocurridos en la Unidad de Neonatología de la Fundación Humanitaria Pablo Jaramillo, durante el período de enero de 2009 a diciembre de 2011, con el objetivo de analizar la incidencia de antecedentes maternos y parámetros propios del recién nacido como posibles factores de riesgo para la sepsis, los parámetros de laboratorio que contribuyen a diagnosticar procesos infecciosos en el recién nacido y el desenlace que tuvieron los mismos. Durante este tiempo ingresaron 164 casos con riesgo de sepsis, sospecha de sepsis y sepsis bacteriana (42.62 /1000 nacidos vivos) y la mortalidad fue del 3.04%. Predominó la sepsis de inicio precoz sobre la tardía. Los principales antecedentes maternos fueron la ruptura prematura de membranas seguido de la infección de vías urinarias. En cuanto a los factores del recién nacido prevaleció el sexo masculino, prematurez y de bajo peso al nacer

Background: Management of hyperbilirubinemia remains a challenge for neonatal medicine because of the risk of neurological complications related to the toxicity of severe hyperbilirubinemia. Objectives: The purpose of this study was to examine the validity of cord blood alkaline phosphatase level for predicting neonatal hyperbilirubinemia. Patients and Methods: Between October and December 2013 a total of 102 healthy term infants born to healthy mothers were studied. Cord blood samples were collected for measurement of alkaline Phosphatase levels immediately after birth. Neonates were followed-up for the emergence of jaundice. Newborns with clinical jaundice were recalled and serum bilirubin levels measured. Appropriate treatment based on serum bilirubin level was performed. Alkaline phosphatase levels between the non-jaundiced and jaundiced treated neonates were compared. Results: The incidence of severe jaundice that required treatment among followed-up neonates was 9.8%. The mean alkaline phosphatase level was 309.09 ± 82.51 IU/L in the non-jaundiced group and 367.80 ± 73.82 IU/L in the severely jaundiced group (P = 0.040). The cutoff value of 314 IU/L was associated with sensitivity 80% and specificity 63% for predicting neonatal hyperbilirubinemia requiring treatment. Conclusions: The cord blood alkaline phosphatase level can be used as a predictor of severe neonatal jaundice.

El presente trabajo tuvo por objeto investigar infección neonatal en hijos de madres que presentan rotura prematura de membranas [RPM] de más de 12 horas, mediante el cultivo de endocervix, y establecer validez de los cultivos de conducto auditivo externo y el recuento de polimorfonucleares como factores predictivos de infección neonatal. Se estudiaron 54 casos de madres que presentaron RPM de más de 12 horas, con las que se establecieron 2 grupos de estudio: Un primer grupo con RPM de 12 a 24 horas que constituyó el 66.7 por ciento, y el segundo con más de 24 horas el 33.3 por ciento. De las 54 pacientes, las 21 [38.9 por ciento] presentaron cultivo positivo de endocervix. De las 36 pacientes que presentaron RPM de 12 a 24 horas 7 [19.4 por ciento] fueron positivos; en el grupo de 18 pacientes que tuvieron RPM de más de 24 horas 14 [77.8 por ciento] de ellos resultaron positivos, lo que nos demuestra que mientas más horas de rotura prematura de membranas en la madre, mayor riesgo de infección existente en el recién nacido. Los gérmenes más frecuentes encontrados en el cultivo de endocervix fueron el Estreptococo, el E. Coli y el estafilococo

Purpose: To investigate the effect of propofol on brain development in neonatal mice and long-term neurocognitive impact in adult mice. Method: The offspring of female C57Bl/6 and male CD-1 mice were administered propofol at concentrations of 2.5 and 5.0 mg/kg (treatment group) or normal saline (control) on postnatal day 7. Thereafter, histological and immunohistochemical examinations were performed on the mice brain. Apoptotic assay, neuronal nuclei antigen immunohistochemistry (to assess neuron density), and behavioral and neurocognitive tests were conducted on the adult mice. Results: Propofol induced cellular degeneration and apoptosis in the brains of neonatal mice. It also modulated physiological parameters (pH, PO2, glucose and lactate), among which decreased blood glucose might be associated with cellular degeneration in the brain. Propofol also caused long-term neuronal deficits in adults, which showed impaired neurocognitive functions. Upon reaching adulthood, propofol-treated mice showed slow learning response and poor memory compared to controls. Conclusion: Propofol causes neurodegeneration in neonatal mice and has long-term neurocognitive consequences in adults, indicating that the use of propofol anesthetics in neonates requires careful consideration.

Background: Vitamin D supplementation during pregnancy has been supposed to defend against adverse gestational outcomes. Objective: This randomized clinical trial study was conducted to assess the effects of 50,000 IU of vitamin D every two weeks supplementation on the incidence of gestational diabetes (GDM), gestational hypertension, preeclampsia and preterm labor, vitamin D status at term and neonatal outcomes contrasted with pregnant women that received 400 IU vitamin D daily. Materials and Methods: 500 women with gestational age 12-16 weeks and serum 25 hydroxy vitamin D (25 (OH) D ) less than 30 ng/ml randomly categorized in two groups. Group A received 400 IU vitamin D daily and group B 50,000 IU vitamin D every 2 weeks orally until delivery. Maternal and Neonatal outcomes were assessed in two groups. Results: The incidence of GDM in group B was significantly lower than group A (6.7% versus 13.4%) and odds ratio (95% Confidence interval) was 0.46 (0.24-0.87) (P=0.01). The mean ± SD level of 25 (OH) D at the time of delivery in mothers in group B was significantly higher than A (37.9 ± 19.8 versus 27.2 ± 18.8 ng/ml, respectively) (P=0.001). There were no differences in the incidence of preeclampsia, gestational hypertension, preterm labor, and low birth weight between two groups. The mean level of 25 (OH) D in cord blood of group B was significantly higher than group A (37.9 ± 18 versus 29.7 ± 19ng/ml, respectively). Anthropometric measures between neonates were not significantly different. Conclusion: Our study showed 50,000 IU vitamin D every 2 weeks decreased the incidence of GDM.

A surdez constitui a forma mais comum das doenças sensoriais, afetando milhões de pessoas em todo o mundo. A privação da audição, pode provocar um atraso significativo na linguagem, interferindo no desenvolvimento global da criança. A implementação de programas de Rastreio Auditivo Neonatal Universal (RANU) tornam-se, deste modo, fundamentais para uma verdadeira intervenção precoce. Só um diagnóstico precoce da surdez infantil permite evitar e/ou minimizar alterações ao nível linguístico, cognitivo, social e emocional da criança bem como da respetiva família. O objetivo deste estudo foi caracterizar o ponto da situação de doze meses de implementação do Rastreio Auditivo Neonatal Universal no Hospital da Ordem do Carmo. A amostra do estudo é constituída por 367 recém-nascidos, que nasceram no Hospital da Ordem do Carmo no período compreendido entre Junho de 2006 e Junho de 2007. Foi realizado o rastreio da surdez, recorrendo às Otoemissões Acústicas por Produtos de Distorção, a todos os recém-nascidos atrás mencionados. Todos os dados recolhidos foram tratados estatisticamente, com a garantia da sua total confidencialidade. Após o tratamento estatístico, verificou-se que a percentagem de Recém-Nascidos que passaram o rastreio foi igual a 82,6 e a percentagem de Recém-Nascidos que “falharam” o rastreio foi de 17,4. O número de falsos positivos foi igual a 64. Todos eles passaram no reteste da segunda fase. Em síntese, a conclusão principal deste estudo é a de que o Rastreio Auditivo Neonatal Universal é de extrema importância, e deve ser implementado em todos os recém-nascidos, preferencialmente antes da alta hospitalar.

Introduction Seizures are harmful to the neonatal brain; this compels many clinicians and researchers to persevere further in optimizing every aspects of managing neonatal seizures. Aims To delineate the seizure profile between non-cooled versus cooled neonates with hypoxic-ischaemic encephalopathy (HIE), in neonates with stroke, the response of seizure burden to phenobarbitone and to quantify the degree of electroclinical dissociation (ECD) of seizures. Methods The multichannel video-EEG was used in this research study as the gold standard to detect seizures, allowing accurate quantification of seizure burden to be ascertained in term neonates. The entire EEG recording for each neonate was independently reviewed by at least 1 experienced neurophysiologist. Data were expressed in medians and interquartile ranges. Linear mixed models results were presented as mean (95% confidence interval); p values <0.05 were deemed as significant. Results Seizure burden in cooled neonates was lower than in non-cooled neonates [60(39-224) vs 203(141-406) minutes; p=0.027]. Seizure burden was reduced in cooled neonates with moderate HIE [49(26-89) vs 162(97-262) minutes; p=0.020] when compared with severe HIE. In neonates with stroke, the background pattern showed suppression over the infarcted side and seizures demonstrated a characteristic pattern. Compared with 10 mg/kg, phenobarbitone doses at 20 mg/kg reduced seizure burden (p=0.004). Seizure burden was reduced within 1 hour of phenobarbitone administration [mean (95% confidence interval): -14(-20 to -8) minutes/hour; p<0.001], but seizures returned to pre-treatment levels within 4 hours (p=0.064). The ECD index in cooled, non-cooled neonates with HIE, stroke and in neonates with other diagnoses were 88%, 94%, 64% and 75% respectively. Conclusions Further research exploring the treatment effects on seizure burden in the neonatal brain is required. A change to our current treatment strategy is warranted as we continue to strive for more effective seizure control, anchored with use of the multichannel EEG as the surveillance tool.

Neonatal seizures are common in the neonatal intensive care unit. Clinicians treat these seizures with several anti-epileptic drugs (AEDs) to reduce seizures in a neonate. Current AEDs exhibit sub-optimal efficacy and several randomized control trials (RCT) of novel AEDs are planned. The aim of this study was to measure the influence of trial design on the required sample size of a RCT. We used seizure time courses from 41 term neonates with hypoxic ischaemic encephalopathy to build seizure treatment trial simulations. We used five outcome measures, three AED protocols, eight treatment delays from seizure onset (Td) and four levels of trial AED efficacy to simulate different RCTs. We performed power calculations for each RCT design and analysed the resultant sample size. We also assessed the rate of false positives, or placebo effect, in typical uncontrolled studies. We found that the false positive rate ranged from 5 to 85% of patients depending on RCT design. For controlled trials, the choice of outcome measure had the largest effect on sample size with median differences of 30.7 fold (IQR: 13.7–40.0) across a range of AED protocols, Td and trial AED efficacy (p<0.001). RCTs that compared the trial AED with positive controls required sample sizes with a median fold increase of 3.2 (IQR: 1.9–11.9; p<0.001). Delays in AED administration from seizure onset also increased the required sample size 2.1 fold (IQR: 1.7–2.9; p<0.001). Subgroup analysis showed that RCTs in neonates treated with hypothermia required a median fold increase in sample size of 2.6 (IQR: 2.4–3.0) compared to trials in normothermic neonates (p<0.001). These results show that RCT design has a profound influence on the required sample size. Trials that use a control group, appropriate outcome measure, and control for differences in Td between groups in analysis will be valid and minimise sample size.
