Amb l’objectiu d’obtenir una solució interior, es van imposar restriccions importants sobre el valors dels paràmetres del model. En aquest treball estenem aquest resultat, en el límit en que la taxa de descompte és igual a zero, eliminant les restriccions i considerant totes les solucions possibles.

Several European telecommunications regulatory agencies have recently introduced a fixed capacity charge (flat rate) to regulate access to the incumbent's network. The purpose of this paper is to show that the optimal capacity charge and the optimal access-minute charge analysed by Armstrong, Doyle, and Vickers (1996) have a similar structure and imply the same payment for the entrant. I extend the analysis tothe case where there is a competitor with market power. In this case, the optimalcapacity charge should be modified to avoid that the entrant cream-skims the market,fixing a longer or a shorter peak period than the optimal. Finally, I consider a multiproduct setting, where the effect of the product differentiation is exacerbated.

The process of free reserves in a non-life insurance portfolio as defined in the classical model of risk theory is modified by the introduction of dividend policies that set maximum levels for the accumulation of reserves. The first part of the work formulates the quantification of the dividend payments via the expectation of their current value under diferent hypotheses. The second part presents a solution based on a system of linear equations for discrete dividend payments in the case of a constant dividend barrier, illustrated by solving a specific case.

Objective: To describe the methodology of Confirmatory Factor Analyis for categorical items and to apply this methodology to evaluate the factor structure and invariance of the WHO-Disability Assessment Schedule (WHODAS-II) questionnaire, developed by the World HealthOrganization.Methods: Data used for the analysis come from the European Study of Mental Disorders(ESEMeD), a cross-sectional interview to a representative sample of the general population of 6 european countries (n=8796). Respondents were administered a modified version of theWHODAS-II, that measures functional disability in the previous 30 days in 6 differentdimensions: Understanding and Communicating; Self-Care, Getting Around, Getting Along withOthers, Life Activities and Participation. The questionnaire includes two types of items: 22severity items (5 points likert) and 8 frequency items (continuous). An Exploratory factoranalysis (EFA) with promax rotation was conducted on a random 50% of the sample. Theremaining half of the sample was used to perform a Confirmatory Factor Analysis (CFA) inorder to compare three different models: (a) the model suggested by the results obtained in theEFA; (b) the theoretical model suggested by the WHO with 6 dimensions; (c) a reduced modelequivalent to model b where 4 of the frequency items are excluded. Moreover, a second orderfactor was also evaluated. Finally, a CFA with covariates was estimated in order to evaluatemeasurement invariance of the items between Mediterranean and non-mediterranean countries.Results: The solution that provided better results in the EFA was that containing 7 factors. Twoof the frequency items presented high factor loadings in the same factor, and one of thempresented factor loadings smaller than 0.3 with all the factors. With regard to the CFA, thereduced model (model c) presented the best goodness of fit results (CFI=0.992,TLI=0.996,RMSEA=0.024). The second order factor structure presented adequate goodness of fit (CFI=0.987,TLI=0.991, RMSEA=0.036). Measurement non-invariance was detected for one of the items of thequestionnaire (FD20 ¿ Embarrassment due to health problems).Conclusions: AFC confirmed the initial hypothesis about the factorial structure of the WHODAS-II in 6factors. The second order factor supports the existence of a global dimension of disability. The use of 4of the frequency items is not recommended in the scoring of the corresponding dimensions.

Needle-free procedures are very attractive ways to deliver vaccines because they diminish the risk of contamination and may reduce local reactions, pain or pain fear especially in young children with a consequence of increasing the vaccination coverage for the whole population. For this purpose, the possible development of a mucosal malaria vaccine was investigated. Intranasal immunization was performed in BALB/c mice using a well-studied Plasmodium berghei model antigen derived from the circumsporozoite protein with the modified heat-labile toxin of Escherichia coli (LTK63), which is devoid of any enzymatic activity compared to the wild type form. Here, we show that intranasal administration of the two compounds activates the T and B cell immune response locally and systemically. In addition, a total protection of mice is obtained upon a challenge with live sporozoites.

Im Artikel «Arbeitsfähigkeit und Invalidenversicherung » von R. Conne [1] wird treffend das heutige Malaise der Arbeitsplatzrehabilitation beschrieben. Dies gilt aber nicht nur für die Arbeitsunfähigkeit aus psychischen Gründen, sondern ebenso für die Arbeitsunfähigkeit wegen muskuloskeletaler Schmerzen. Die Schwierigkeiten einer Arbeitsplatzrehabilitation sind aber nicht speziell auf die Invalidenversicherung zurückzuführen. Sie gründen einerseits auf strukturellen Gegebenheiten der Schweiz, wie eine wenig in den Betrieben verankerte Arbeitsmedizin, und Zugang zu Rehabilitationsmedizin, und andererseits auf dem Fehlen einer klaren Kostenträgerregelung oder -verantwortung. R. Conne beschreibt in seinem Artikel treffend das heutige Malaise in der Schweiz in bezug auf Arbeitsplatzrehabilitation oder Wiedererlangung der Arbeitsfähigkeit. Er bezieht sich dabei vor allem auf die Arbeitsunfähigkeit aus psychischen Gründen. Wir möchten als am Rande angesprochene Arbeitsmedizinerin und angesprochener Rehabilitationsmediziner die Diskussion aufgreifen und erweitern. Die Erweiterung betrifft einerseits den Einbezug von nicht psychisch begründeter Arbeitsunfähigkeit und andererseits auch die allgemeine Versicherungssituation in der Schweiz, und dies verschiebt unserer Meinung nach den Problemfokus von der Invalidenversicherung auf die allgemeine Versicherungssituation. OBJECTIVES: Thirty-two consecutive isolated modified Blalock Taussig (BT) shunts performed in infancy since 2004 were reviewed and analysed to identify the risk factors for shunt intervention and mortality. METHODS: Sternotomy was the only approach used. Median age and weight were 10.5 (range 1-74) days and 2.9 (1.9-4.4) kg, respectively. Shunt palliation was performed for biventricular hearts (Tetralogy of Fallot/double outlet right ventricle/transposition of great arteries_ventricular septal defect_pulmonary stenosis/pulmonary atresia_ventricular septal defect/others) in 21, and univentricular hearts in 11, patients. Hypoplastic left heart syndrome patients were excluded. Two procedures required cardiopulmonary bypass. Median shunt size was 3.5 (3-4) mm and median shunt size/kg body weight was 1.2 (0.9-1.7) mm/kg. Reduction in shunt size was necessary in 5 of 32 (16%) patients. RESULTS: Three of 32 (9%) patients died after 3 (1-15) days due to cardiorespiratory decompensation. Lower body weight (P = 0.04) and bigger shunt size/kg of body weight (P = 0.004) were significant risk factors for mortality. Acute shunt thrombosis was observed in 3 of 32 (9%), none leading to death. Need for cardiac decongestive therapy was associated with univentricular hearts (P < 0.001), bigger shunt size (P = 0.054) and longer hospital stay (P = 0.005). Twenty-eight patients have undergone a successful shunt takedown at a median age of 5.5 (0.5-11.9) months, without late mortality. CONCLUSIONS: Palliation with a modified BT shunt continues to be indicated despite increased thrust on primary corrective surgery. Though seemingly simple, it is associated with significant morbidity and mortality. Effective over-shunting and acute shunt thrombosis are the lingering problems of shunt therapy.

Os produtos das atividades de mineração têm grande relevância econômica no Estado de Minas Gerais, Brasil. No entanto, seus inúmeros danos à biota, incluindo a fauna edáfica, resultam na necessidade de recuperação e biomonitoramento dessas áreas e do seu entorno. Considerando a importância ecológica e o potencial bioindicador das minhocas, o objetivo deste estudo foi avaliar a toxicidade de arsênio presente em solos do entorno de minerações de ouro, nas bacias dos rios Doce e São Francisco, a oligoquetas da espécie Eisenia andrei Bouché. Amostras de solos dessas áreas foram caracterizadas em relação às suas propriedades físicas e químicas, incluindo os metais Fe, Al, Cu, Ni, Zn e Mn e do metaloide As; com elas, foram conduzidos bioensaios, que avaliaram a sobrevivência, reprodução e variação da biomassa dos organismos. Também foram realizados testes de sensibilidade dessa espécie ao As adicionado a solo artificial tropical e em solo natural. Houve evidente efeito tóxico sobre a reprodução dessa espécie, que esteve altamente correlacionado com as concentrações de As nas amostras (r = -0,80). Amostras de solos naturais de locais avaliados nas duas áreas apresentaram concentrações de As acima do limite estipulado pela Resolução Conama nº 420 de 2009, chegando a 2.388,2 mg kg-1. A CL50 do As em solo artificial tropical (207,4 mg kg-1) foi seis vezes menor que a do solo natural (1.248,1 mg kg-1). A menor biodisponibilidade do As nos solos naturais foi atribuída a sua associação com o minério de Fe presente nas amostras. Concluiu-se que os solos do entorno de áreas de mineração causaram toxicidade crônica a E. andrei, em termos de sua reprodução, o que demonstrou o potencial de danos dessas atividades sobre a fauna edáfica dessas áreas. O estudo também forneceu informações ecotoxicológicas que podem ser úteis em avaliações de risco ambiental em solos tropicais.
