Necrotizing fasciitis (NF) is an uncommon infection, but potentially lethal, especially when associated with systemic disorders such as diabetes. The authors report a case of necrotizing fasciitis from odontogenic origin in a patient with uncontrolled diabetes mellitus. The initial diagnosis was based on clinical information, in which multiple necrosis areas in cervical and thoracic regions were observed. Wide antibiotic therapy was applied, followed by surgical drain age and debridement. Culture was positive for methicillin-resistant Staphylococcus aureus. Although the treatment is established, the patient dies after sepsis and failure of vital organs. Clearly, the morbidity associated to this infection, even in diabetic patients, can be minimized if an early diagnosis and effective debridement are done.

Objective: The aim of this study was to evaluate the 5-year clinical performance of composite restorations of non-carious cervical lesions (NCCL) using a total-etch adhesive system with or without collagen removal with 10% sodium hypochlorite (NaOCl).Methods: In this study randomized controlled split-mouth clinical trial, one-hundred and thirty-eight NCCL were restored into 30 patients. Each patient received at least one pair of composite restorations (Filtek A110/3M ESPE), bonded either with 2 techniques: Conventional Technique - acid etching with 37% phosphoric acid + Prime & Bond NT (Denstply DeTrey); Deproteinization Technique - acid etching with 37% phosphoric acid + 10% NaOCl for 1 min + Prime & Bond NT. The two techniques were evaluated using the United States Public Health Service (USPHS) criteria at baseline, 18 months, 3 and 5 years. After five years, one-hundred and six restorations were evaluated in 24 patients. Data were analyzed using the Chi-Square test (p < 0.05).Results: There were no statistically significant differences between the two techniques regarding the evaluated parameters (p > 0.05).Conclusion: After 5 years, the application of 10% NaOCl deproteinization on etched dentine did not affect the clinical performance of composite restorations placed in NCCL when compared to the conventional total-etch technique.Clinical significance: Clinical studies evaluating the performance of the Deproteinization Technique are scarce. In this study, this technique showed similar clinical performance at the end of 5 years when compared to a conventional total-etch technique. (C) 2014 Elsevier Ltd. All rights reserved.

Introduction: The myofascial pain syndrome (SDM) is one of the most common causes of musculoskeletal pain. One of the possible treatments for SDM is the type of physiotherapy myofascial manipulation. Objective: This study aimed to analyze the effect of manipulative technique with myofascial pain threshold before and after applying the technique in athletes during competition period. Methods: Participated in the study 62 subjects of both genders, aged between 14 and 38 (19.64 + 4.89), who had myofascial pain syndrome, 32 oh the treatment group and 30 divided equally between control group and the placebo group. All were athletes and operated by the Department of Sport and Leisure in the city of Marilia – SP and were in competitive period. The volunteers were evaluated according to their musculoskeletal symptoms to prove the necessity of performing the technique of myofascial manipulation. Confi rmed the need to assess the pressure pain threshold (LDP) using a digital dynamometer. After the measurement, patients underwent treatment or using the technique of myofascial manipulation, or a sliding surface for the placebo or no treatment for the control group followed by the immediate reassessment of the LDP. Results: The results were normalized by Kolmogrov-Smirnov test (KS). Through the ANOVA test found no differences between the initial LDP thresholds between groups. To compare pre and post LDP of the three groups we used the paired t test. Signifi cant difference (p=0.0001) between the values of pain threshold before and after application of myofascial manipulation for the treated group and not signifi cant for the control group (p=0.45) and placebo (p=0.16). Conclusion: We conclude then that the myofascial manipulation technique is able to increase pain threshold after micro-musculoskeletal injuries in athletes in competitive period.

Introduction: Low back pain is characterized as a musculoskeletal disorder responsible for loss of productivity and high absenteeism of the affected individuals. Objectives: To analyze the effects of short-and long-term program of health education and therapeutic exercise in relation to quality of life, flexibility and pain intensity of community agents in Marilia city, Sao Paulo, Brazil. Methods: A total of 17 women community workers (40.29+9.97 years) were analysed. Evaluations were made in relation to quality of life, flexibility and intensity of pain before, immediately and 30 weeks after the intervention. Results: We observe the positive results of programs in six domains of quality of life questionnaire and flexibility in the short-and long-term (p<0.005). The intensity of pain showed significant reduction only in the short-term. Conclusion: The methodology used was able to improve the quality of life, flexibility and reduce pain intensity of community agents.

Objectives: evaluating the level of information about the examination of uterine cervical cancer and its association with sociodemographic variables in women of a health care unit in the city of Bauru, São Paulo, Brazil. Methods: we conducted a cross-sectional descriptive study with 370 women aged 25 to 59, through structured interviews in their own homes; we used descriptive statistics and the χ2 test. Results: 40.5% of the women had not undergone the Papanicolaou test at the recommended frequency; 58.2% incorrectly defined the test, and 69.5% did not know about the risk factors for the development of cervical cancer; the knowledge about the test showed statistically significant association with schooling and family income of the studied population. Conclusions: women present deficiencies on the proper practice of the Papanicolaou test, on knowledge about the test, risk factors and prevention methods. Therefore, it is necessary to develop primary health actions for the most vulnerable population.

Nos últimos anos tem surgido vários métodos para se estudar sistemas multicorpos. A vantagem destes métodos é a obtenção das equações do movimento de forma mais fácil e precisa. Robôs, manipuladores, mecanismos, satélites, máquinas em geral e sistemas biomcânicos, pelas suas características, podem ser vantajosamente tratados por formalismo multicorpo, mas precisamente, o método de Kane a um modelo do corpo humano, com a finalidade de se determinar a pressão intradiscal entre as vértebras L4/L5 da coluna lombar, em algumas atividades normais de vida diária, como: sentar, abaixar, pegar e elevar cargas, realizar movimentos de flexão e extensão. Pretendeu-se, também obter os esforços internos nas diversas articulações deste modelo. A pressão intradiscal representa um parâmetro importante para caracterizar a sobrecarga na coluna, podendo ter relação com a degeneração do disco intervertebral, o que produz muita dor em indivíduos. Inicialmente, o modelo foi proposto e o método de Kane foi aplicado para se obter as equações do movimento. Para resolvê-las, foi necessário realiza a filmagem de um sujeito realizando as atividades citadas, a fim de se obter as equações das trajetórias dos diversos segmentos do corpo humano. Após a simulação, os parâmetros desejados foram obtidos e comparados com valores experimentais citados na literatura.

Este trabalho avaliou a qualidade das adaptações cervicais de coroas totais metálicas, tendo como fonte de variação: a) o término do ombro cervical do preparo - biselado, inclinado em 1350; reto em 900, chanfro e gume de faca; b) o alívio ou não das superfícies internas das coroas; e, c) os tipos de agente de cimentação permanente, cimentos - fosfato de zinco Harvard; ionômero de vidro Ketac-Cem, policarboxilato de zinco Durelon e resinoso Panavia Ex. Inicialmente, foram confeccionados corpos-de-prova em modelos-padrão de aço inoxidável usinado, de acordo com o tipo de preparo dos términos cervicais experimentais. As cápsulas metálicas de aço inoxidável preparadas, tendo ou não alívio da superfície interna de 30 micrometros até a distância de 0,5 mm do limite do término cervical, justapunham-se precisamente e formavam um conjunto com adaptação e assentamento exatos. Estas foram cimentadas nos corpos-de-prova com os diferentes agentes cimentantes, os quais foram manipulados de acordo com as instruções dos fabricantes. Desenvolveram-se metodologias de reaproveitamento dos corpos-de-prova, estabilidade das cápsulas impedindo seu deslocamento durante a tomada das medidas e mensuração precisa. Concluiu-se que: a) as melhores médias de adaptações cervicais, semelhantes entre si, foram obtidas em modelos-padrão com ombros: lâmina de faca, ombro inclinado em 1350 e chanfro; b) as piores médias de adaptações cervicais, semelhantes entre si, foram obtidas em modelos-padrão com ombros reto em 900 e degrau em 900 com bisel de 450; c) houve melhora significativa na adaptação quando cápsulas metálicas foram cimentadas com alívio de sua superfície interna; d) os cimentos...(Resumo completo, clicar acesso eletrônico abaixo)

Purpose: The purpose of this study was to comparatively assess the seven-year clinical performance of a one-bottle etch-and-rinse adhesive with resin composite (RC) and resin-modified glass ionomer (RMGI) restorations in noncarious cervical lesions.Methods and Materials: One operator placed 70 restorations (35 restorations in each group) in 30 patients under rubber dam isolation without mechanical preparation. The restorations were directly assessed by two independent examiners, using modified US Public Health Service criteria at baseline and 6, 12, 24, 60, and 84 months. The obtained data were tabulated and statistically analyzed using the Fisher and McNemar tests. A difference was significant if p<0.05.Results: Twenty patients were available for recall after seven years (66.6%), and 25 RC and 26 RMGI restorations out of 70 restorations were evaluated. Excellent agreement was registered for all criteria between examiners (kappa >= 0.85). Alfa and bravo scores were classified as clinically acceptable. The McNemar test detected significant differences within RC restorations between baseline and seven-year evaluations for anatomic form, marginal integrity, and retention (p<0.05). For RMGI restorations, a significant difference was identified for marginal integrity (p<0.05). As to material comparison, the Fisher exact showed a better retention performance for RMGI restorations than for RC restorations (p<0.05). Twelve composite restorations were dislodged (52.0% retention) and three ionomer restorations were lost (88.5% retention). The cumulative success rate for RC and RMGI was 30% and 58.1%, respectively.Conclusions: After seven years of service, the clinical performance of RMGI restorations was superior to that of the adhesive system/resin composite restorations in this study.

The objective of this study was to evaluate cardiovascular and respiratory effects associated with neck flexion, subarachnoid puncture, cerebrospinal fluid drainage and the subarachnoid infusion of ioversol (320mgl/mL) in dogs under isoflurane general anesthesia. The dogs received infusion of: autologous cerebrospinal fluid at 38 degrees C (GC - control group); ioversol 0.3mL/ Kg at 25 degrees C (GI25) and heated to 38 degrees C (GI38). Heart rate, systolic and diastolic arterial pressure, respiratory rate, oxyhemoglobin saturation and electrocardiography readings were recorded. The results showed that cervical myelography with ioversol 320mgl/mL at 0.3mL/Kg (25 C-degrees and 38C degrees) did not significantly alter recorded parameters, except for an elevation in arterial pressure.
