Con el objetivo de valorar la patología biliar y sus variedades en 100 pacientes de ambos sexos y cualquier edad ingresados al hospital regional Vicente Corral Moscoso de Cuenca y que fueron sometidos a intervención quirúrgica año 1992. Determinar el o los métodos auxiliares de diagnóstico utilizados para confirmar el diagnóstico clínico. Establecer el manejo clínico y terapeútico preoperatorio utilizados.

Estudio cuasiexperimental en 50 pacientes tuberculosos ingresados en el Hospital Neumológico y 50 voluntarios aparentemente sanos de la ciudad de Cuenca, para determinar el cortisol basal y sus valores a las 8. 24 y 36 horas luego de la estimulación con ACTH (Synacthen Depot) a la dosis de 0.5 miligramos vía muscular. Todos los individuos de estudio tuvieron entre 20 y 50 años y no haïbían recibido medicación corticoide por lo menos 15 días antes del estudio. La determinación del cortisol se hizo por radioinmunoanálisis con Coat-A Count Cortisol, siguiendo los procedimientos indicados por el laboratorio. Las cifras de cortisol basal en los individuos sanos son de 13.94 +- 5.78 ug/dl. y en los tuberculosos de 17.83+- 6.75 ug/dl, siendo la diferencia estadísticamente significativa (p menor que 0.001) Considerando los sexos, los valores de cortisol entre hombres y mujeres no son estadísticamente diferentes. A las 8 horas luego de la estimulación con ACTH el 90de los individuos aparentemente sanos y el 74de los tubeculosos presentan una respuesta adecuada a la estimulación, ya que duplican o triplican los valores basales. En la determinación de cortisol a las 24 horas un 46de individuos en ambos grupos no llegan a duplicar los valores y a las 36 horas únicamente duplican el 26de los individuos salos y el 20de los tuberculosos por lo que se hace innecesaria su determinación. Considerando que en los tuberculosos las cifras están elevadas y que esta enfermedad suele acompañarse de deficiencias nutritivas, se recomienda realizar estudios en pacientes con diferentes grados y tipos de desnutrición

Determinar que la analgesia postcesárea mediante la administración de fentanyl o morfina en anestesia conductiva con la técnica convencional peridural en el hospital Vicente Corral Moscoso (H.V.C.M.) de la ciudad de Cuenca, produce analgesia postoperatoria eficaz, sostenida, económica y sin complicaciones para la madre y el neonato

Numerosos estudios acerca de analgesia postoperatoria arrojan resultados positivos en favor de la administración de opiáceos en el espacio peridural y en general perimedular por la eficacia analgésica y anealgesia prolongada en los pacientes quirúrgicos. La adición de -fentanil- a una dosis única de 2mcg (microgramos)/kg. de peso a la lidocaina en la técnica de punción lumbar (L2-3;L3-4) para anestesia peridural se realizó en 100 pacientes del Hospital Vicente Corral Moscoso de Cuenca. Los pacientes fueron intervenidos por diversos procedimientos que abarcan las especializades de cirugía general, cirugía plástica, cirugía vascular, traumatología y ortopedia, ginecología, obstetricia y urología. A los pacientes se les constituyó aleatoriamente en dos grupos (casos y controles). Los resultados evidencian que si bien todos los pacientes presentaron analgesia postoperatoria la duraciïïon de la misma en el grupo de control se redujo a menos de una hora en comparación con el grupo de estudio en la que se obtuvo una analgesia promedio de siete horas y media. La administración posterior de otros analgésicos evidencia, igualmente, una marcada diferencia entre los grupos, sindo mucho menor en el grupo de estudio. Las compilaciones de esta técnica son leves y se resumen en prurito, naúsea y retención urinaria en porcentajes inferiores al 5. Las pruebas de significación estadística a las que se sometieron los resultados marcan una muy significativa diferencia entre las variables sometidas a estudio, en una muestra muy homogénea

De ellas saldrán las necesidades de las empresas respecto a las características que deben tener sus dirigentes y empleados y lo que necesita la Universidad para formarlos mejor.

The object of analysis in the text are the issues concerned with the transmission easement and the adverse possession thereof on the grounds of the Polish law. The text features: (1) a historical outline of the solutions concerned with easements in the Polish law following 1945, (2) the institution of transmission easement introduced in 2008 and the solutions concerned with the claims for the establishment thereof at court, (3) the institution of adverse possession of transmission easement pursuant to civil law regulations, judicature and the legal doctrine. On account of the need to elaborate the wide-ranging legal issues concerned with the transmission easement in this text, the analysis embraces two research questions giving rise to the following conclusions: (1) What function is performed by the institution of transmission easement in the system of civil-law relations in the Polish law? The legislator in the articles introducing a transmission easement ossified the solutions functioning in the judicature of the Polish courts before 2008. The legal interpretation took a turn for clarification, that is for the establishment of a norm in the situation where its comprehension was dubious. It is noteworthy that in the period prior to 2008, the law provided for easement appurtenant, and on account of the usual course of judicial decisions also for easement appurtenant with the content corresponding to transmission easement. In 2008 these two “legal existences” were supplemented with a transmission easement, which nevertheless failed to resolve all the legal problems; nay, this gave rise to even more problems, e.g. the one of non-establishment of interpolar norms which would address the issues arising in connection with the use of various easement institutions in legal transactions. While amending the civil law, the legislator aimed to bring order to legal transactions by streamlining the unregulated actual state of easement in relation to transmission infrastructure, but also in relation to the situations where an easement was yet to be established and a facility yet to be constructed. Thus, such action is intended to regulate the disorderly legislation in force as well as to safeguard investment processes. This is of particular significance, for example, for energy companies which are burdened with statutory public-law obligations as regards securing energy supplies and providing for the development of energy infrastructure. Hence, the de facto introduced civil-law solutions indirectly served to realise the principles of the doctrine of easement in the public interest. (2) What legal problems in the civil-law relations does the application of the institution of transmission easement by adverse possession entail? On account of the functioning of various institutions of easement, that is (1) an easement appurtenant, (2) an easement appurtenant with the content corresponding to a transmission easement, and as of 2008 (3) a transmission easement, a problem arose as to which of the given easements companies exercised in particular periods, all the more so because before 1989 the State Treasury owned them and many of the transmission facilities were put in place by virtue of administrative decisions. The commonly held belief is that in the period of “society-oriented economy” as well as up to 2008 infrastructure companies could exercise an easement appurtenant which corresponded to the content of a transmission easement. Therefore, in such a case the running of the prescriptive period should allow for the general rules laid down for an easement appurtenant. Apart from the problem of the relation of a capacity to exercise a right to property and the free development of civil-law relations before 1989, the recognition of the running of prescriptive periods – given the functioning of the three various easements as legal institutions – became a significant legal problem. By way of illustration, the recognition – against the period of exercising transmission easement – of the period required for the acquisition thereof by adverse possession, whereby before 3 August 2008 the real estate featured the legal state corresponding to the content of this right, is debatable. One cannot recognise that within that period a transmission easement was exercised, because such a right was not in existence as yet. Therefore, the institution that might be employed is the running of the period as regards the adverse possession in relation to an easement appurtenant with the content of a transmission easement. Still, the problem remains as to whether the period of the exercise of the easement appurtenant with the content corresponding to a transmission easement can be recognised against the period of possession required for the adverse possession of a transmission easement pursuant to the regulations introduced in 2008. One might incline to the position whereby in such a case it would be right to fully recognise – against the period of exercising a transmission easement – the period of exercising an easement appurtenant corresponding thereto in respect of its content. That being so, the adverse possession of a transmission easement might ensue in such a situation on 3 August 2008 at the earliest, that is the moment the regulations governing this right come into effect. Conversely, if the prescriptive period expires before that date, the entrepreneur would acquire an easement appurtenant with the content corresponding to the transmission easement. Such an interpretation is aligned with the purpose intended by the legislator, which is to bring order to the actual state of the broadest scope with the aid of a new legal instrument. The text, while analysing the issue of a transmission easement and an adverse possession thereof as a institution of the civil law, presents only some selected problems. Hence, the analysis does not include, for example, the issues concerned with claims for remuneration (for usufruct without contractual basis or usufruct fees), or claims for compensation (redress or amends). Furthermore, the text does not conduct a more profound analysis of the relation between the provisions regulating public-law relations (e.g. acts of law introducing the institution of dispossession) and the provisions regulating civil-law relations (the easements in question).

Este trabajo de investigaci?n tuvo por objetivo Describir la construcci?n del humor como estrategia discursiva de resistencia en contra del proyecto de reforma a la Ley 30 en el periodo del Presidente Juan Manuel Santos (2010 ? 2014). Para ello tomo como marco de orientaci?n las teor?as del An?lisis Cr?tico del Discurso y la de la enunciaci?n. Metodol?gicamente se estructur? una matriz de an?lisis que incorpor? aportes de Oswald Ducrot sobre el enunciado humor?stico e ir?nico, as? como recursos ret?ricos. Este trabajo lleg? a conclusiones que ponen de manifiesto c?mo el humor y la iron?a se constituy? en una de las estrategias discursivas que desplegaron aquellos que se opusieron al proyecto de reforma a la Ley 30 de Educaci?n Superior

Basal melting of floating ice shelves and iceberg calving constitute the two almost equal paths of freshwater flux between the Antarctic ice cap and the Southern Ocean. The largest icebergs (>100 km2) transport most of the ice volume but their basal melting is small compared to their breaking into smaller icebergs that constitute thus the major vector of freshwater. The archives of nine altimeters have been processed to create a database of small icebergs (<8 km2) within open water containing the positions, sizes, and volumes spanning the 1992–2014 period. The intercalibrated monthly ice volumes from the different altimeters have been merged in a homogeneous 23 year climatology. The iceberg size distribution, covering the 0.1–10,000 km2 range, estimated by combining small and large icebergs size measurements follows well a power law of slope −1.52 ± 0.32 close to the −3/2 laws observed and modeled for brittle fragmentation. The global volume of ice and its distribution between the ocean basins present a very strong interannual variability only partially explained by the number of large icebergs. Indeed, vast zones of the Southern Ocean free of large icebergs are largely populated by small iceberg drifting over thousands of kilometers. The correlation between the global small and large icebergs volumes shows that small icebergs are mainly generated by large ones breaking. Drifting and trapping by sea ice can transport small icebergs for long period and distances. Small icebergs act as an ice diffuse process along large icebergs trajectories while sea ice trapping acts as a buffer delaying melting.

A garantia da memória histórica do ensino em nível profissionalizante em enfermagem, na década de 1970, foi o objeto dessa investigação. Teve como objetivos descrever e analisar o contexto sócio-político e as circunstâncias históricas em que a Lei nº 5.692, de 12 de agosto de 1971, das Diretrizes e Bases da Educação Nacional, foi promulgada, bem como discutir as mudanças e desdobramentos dessa no ensino profissionalizante em enfermagem. Trata-se de estudo descritivo, qualitativo, exploratório e de natureza histórico-social, com base em análise documental. Optou-se pela Micro-história para dar sustentação teórica à discussão dos resultados dessa investigação, pois a análise dos documentos históricos, sob o prisma de que, embora não seja possível enxergar a sociedade inteira a partir de um fragmento social, é possível enxergar algo da realidade social que envolve o fragmento humano examinado. A delimitação das fontes históricas do estudo compreende o Acervo do Arquivo Histórico da Associação Brasileira de Enfermagem - Seção São Paulo; o Acervo documental sobre a ABEn/SP, existente no Centro Histórico Cultural da Enfermagem Ibero-Americana da Escola de Enfermagem da USP e a Série Documenta do Ministério da Educação e Cultura. A partir dessa LDB nº 5.692/71, o ensino de Enfermagem foi totalmente integrado ao sistema nacional de Educação e sua promulgação ocorreu durante a Ditadura Militar e a ideologia do “milagre econômico”. De acordo com essa ideologia, o sistema educacional brasileiro deveria adequar-se ao modelo econômico desenvolvimentista, com treinamento de pessoal de nível técnico, visando aumentar e baratear os recursos humanos para o trabalho. A ABEn, como entidade representativa dos interesses políticos e ideológicos dos profissionais da área, liderava os debates sobre as questões da formação dos recursos humanos na enfermagem, sendo que na década de 70 mais da metade do contingente de enfermagem era majoritariamente sem formação específica. Também por conta dessa realidade, houve iniciativas governamentais para tentar reverter essa situação. Apesar das questões do ensino da enfermagem não se encontrarem explicitadas no texto da LDB nº 5.692/71, os resultados deste trabalho revelaram que a legislação estudada teve desdobramentos nas decisões políticas no âmbito do ensino profissionalizante e, consequentemente, impactou sobre a formação dos profissionais de enfermagem.
