PURPOSE This prospective multicenter phase III study compared the efficacy and safety of a triple combination (bortezomib-thalidomide-dexamethasone [VTD]) versus a dual combination (thalidomide-dexamethasone [TD]) in patients with multiple myeloma (MM) progressing or relapsing after autologous stem-cell transplantation (ASCT). PATIENTS AND METHODS Overall, 269 patients were randomly assigned to receive bortezomib (1.3 mg/m(2) intravenous bolus) or no bortezomib for 1 year, in combination with thalidomide (200 mg per day orally) and dexamethasone (40 mg orally once a day on 4 days once every 3 weeks). Bortezomib was administered on days 1, 4, 8, and 11 with a 10-day rest period (day 12 to day 21) for eight cycles (6 months), and then on days 1, 8, 15, and 22 with a 20-day rest period (day 23 to day 42) for four cycles (6 months). Results Median time to progression (primary end point) was significantly longer with VTD than TD (19.5 v 13.8 months; hazard ratio, 0.59; 95% CI, 0.44 to 0.80; P = .001), the complete response plus near-complete response rate was higher (45% v 25%; P = .001), and the median duration of response was longer (17.2 v 13.4 months; P = .03). The 24-month survival rate was in favor of VTD (71% v 65%; P = .093). Grade 3 peripheral neuropathy was more frequent with VTD (29% v 12%; P = .001) as were the rates of grades 3 and 4 infection and thrombocytopenia. CONCLUSION VTD was more effective than TD in the treatment of patients with MM with progressive or relapsing disease post-ASCT but was associated with a higher incidence of grade 3 neurotoxicity.

MAF1 is a global repressor of RNA polymerase III transcription that regulates the expression of highly abundant noncoding RNAs in response to nutrient availability and cellular stress. Thus, MAF1 function is thought to be important for metabolic economy. Here we show that a whole-body knockout of Maf1 in mice confers resistance to diet-induced obesity and nonalcoholic fatty liver disease by reducing food intake and increasing metabolic inefficiency. Energy expenditure in Maf1(-/-) mice is increased by several mechanisms. Precursor tRNA synthesis was increased in multiple tissues without significant effects on mature tRNA levels, implying increased turnover in a futile tRNA cycle. Elevated futile cycling of hepatic lipids was also observed. Metabolite profiling of the liver and skeletal muscle revealed elevated levels of many amino acids and spermidine, which links the induction of autophagy in Maf1(-/-) mice with their extended life span. The increase in spermidine was accompanied by reduced levels of nicotinamide N-methyltransferase, which promotes polyamine synthesis, enables nicotinamide salvage to regenerate NAD(+), and is associated with obesity resistance. Consistent with this, NAD(+) levels were increased in muscle. The importance of MAF1 for metabolic economy reveals the potential for MAF1 modulators to protect against obesity and its harmful consequences.

To complement the existing treatment guidelines for all tumour types, ESMO organises consensus conferences to focus on specific issues in each type of tumour. The 2nd ESMO Consensus Conference on Lung Cancer was held on 11-12 May 2013 in Lugano. A total of 35 experts met to address several questions on non-small-cell lung cancer (NSCLC) in each of four areas: pathology and molecular biomarkers, first-line/second and further lines of treatment in advanced disease, early-stage disease and locally advanced disease. For each question, recommendations were made including reference to the grade of recommendation and level of evidence. This consensus paper focuses on locally advanced disease.

Este trabalho tem por objetivo fazer uma revisão da anatomia e das principais doenças do III par craniano, demonstrando-as através de estudos por imagem, com ênfase em ressonância magnética. Os métodos de imagem são peça fundamental na avaliação de pacientes com suspeita de lesão do nervo oculomotor, uma vez que o quadro clínico é freqüentemente inespecífico e a gama de doenças que podem acometê-lo é bastante ampla. Realizamos também breve levantamento da literatura.

OBJETIVO: Utilizar a cintilografia óssea facial para identificar o crescimento esquelético mandibular, através do metabolismo ósseo condilar de indivíduos com crescimento craniofacial equilibrado, e compará-lo à atividade condilar daqueles com crescimento mandibular excessivo. MATERIAIS E MÉTODOS: Quarenta e sete indivíduos, de ambos os sexos, entre 18 e 28 anos de idade, foram divididos em grupo controle - classe I (n = 13) - e grupo caso - classe III (n = 34). As imagens foram obtidas duas horas após injeção intravenosa de 200 µCi/kg de 99mTc-MDP. Foram realizadas incidências laterais do crânio e posterior da coluna lombar e a taxa de contagem foi determinada pela obtenção da média de contagem dos côndilos e da quarta vértebra lombar utilizando 300.000 contagens. RESULTADOS: Não foram encontradas captações condilares assimétricas em ambos os grupos, e apesar da ausência de significância estatística, os valores médios de captação condilar foram maiores no grupo caso. CONCLUSÃO: A média de captação aumentada indicou maior metabolismo ósseo condilar nos indivíduos classe III, sugerindo um provável crescimento mandibular residual. Mais estudos estão sendo realizados para aumentar esta amostragem.

OBJETIVO: Analisar a eficiência da angioplastia primária infra-inguinal como método de salvamento de membros em pacientes portadores de lesões tróficas por isquemia crítica. MATERIAIS E MÉTODOS: Foram analisados 36 pacientes submetidos a angioplastias primárias sem stent. Todos os pacientes apresentavam isquemia crítica com lesão trófica - grau III, categoria 5 de Rutherford -, sendo 17 lesões na artéria femoral superficial, 16 na artéria poplítea e 51 em artérias da perna, totalizando 84 angioplastias. Foram analisadas também as prevalências em relação a sexo, membro afetado, idade e principais comorbidades, sendo tecidas considerações técnicas sobre os procedimentos, assim como os materiais utilizados. RESULTADOS: Considerou-se sucesso quando a lesão trófica que motivou a angioplastia cicatrizou, ou o nível de amputação limitou-se a artelhos ou ao antepé, sem ter havido necessidade de procedimento cirúrgico de reconstituição do fluxo sanguíneo (bypass), independentemente de tempo, drogas associadas e números de desbridamentos realizados. CONCLUSÃO: As angioplastias no segmento femoropoplíteo e infrapoplíteo são procedimentos de elevado sucesso técnico, baixa morbidade e mortalidade, constituindo-se procedimento eficaz em pacientes com isquemia crítica de membro inferior.

Background: In ∼5% of advanced NSCLC tumours, ALK tyrosine kinase is constitutively activated after translocation of ALK. ALK+ NSCLC was shown to be highly sensitive to the first approved ALK inhibitor, crizotinib. However, all pts eventually relapse on crizotinib mainly due to secondary ALK mutations/amplification or CNS metastases. Alectinib is a highly selective, potent, oral next-generation ALK inhibitor. Clinical phase II alectinib data in 46 crizotinib-naïve pts with ALK+ NSCLC reported an objective response rate (ORR) of 93.5% and a 1-year progression-free rate of 83% (95% CI: 68-92) (Inoue et al. J Thorac Oncol 2013). CNS activity was seen: of 14 pts with baseline brain metastasis, 11 had prior CNS radiation, 9 of these experienced CNS and systemic PFS of >12 months; of the 3 pts without prior CNS radiation, 2 were >15 months progression free. Trial design: Randomised, multicentre, phase III, open-label study in pts with treatment-naïve ALK+ advanced, recurrent, or metastatic NSCLC. All pts must provide pretreatment tumour tissue to confirm ALK rearrangement (by IHC). Pts (∼286 from ∼180 centres, ∼30 countries worldwide) will be randomised to alectinib (600mg oral bid, with food) or crizotinib (250mg oral bid, with/without food) until disease progression (PD), unacceptable toxicity, withdrawal of consent, or death. Stratification factors are: ECOG PS (0/1 vs 2), race (Asian vs non-Asian), baseline CNS metastases (yes vs no). Primary endpoint: PFS by investigators (RECIST v1.1). Secondary endpoints: PFS by Independent Review Committee (IRC); ORR; duration of response; OS; safety; pharmacokinetics; quality of life. Additionally, time to CNS progression will be evaluated (MRI) for the first time in a prospective randomised NSCLC trial as a secondary endpoint. Pts with isolated asymptomatic CNS progression will be allowed to continue treatment beyond documented progression until systemic PD and/or symptomatic CNS progression, according to investigator opinion. PURPOSE: This multicenter phase III study evaluated the efficacy and safety of lapatinib, an epidermal growth factor receptor/ErbB2 inhibitor, administered concomitantly with chemoradiotherapy and as maintenance monotherapy in patients with high-risk surgically treated squamous cell carcinoma of the head and neck (SCCHN). PATIENTS AND METHODS: Patients with resected stage II to IVA SCCHN, with a surgical margin ≤ 5 mm and/or extracapsular extension, were randomly assigned to chemoradiotherapy (66 Gy total radiation dose and cisplatin 100 mg/m(2) per day administered on days 1, 22, and 43) plus placebo or lapatinib (1,500 mg per day) before and during chemoradiotherapy, followed by 12 months of maintenance monotherapy. RESULTS: Six hundred eighty-eight patients were enrolled (lapatinib, n = 346; placebo, n = 342). With a median follow-up time of 35.3 months, the study ended early because of the apparent plateauing of disease-free survival (DFS) events. Median DFS assessed by an independent review committee was 53.6 months and not reached for lapatinib and placebo, respectively (hazard ratio, 1.10; 95% CI, 0.85 to 1.43). Investigator-assessed results confirmed the independent review committee assessment. No significant differences in DFS by human papillomavirus status or overall survival were observed between treatment arms. Similar numbers of patients in both treatment arms experienced adverse events (AEs), with more patients in the lapatinib arm than the placebo arm experiencing serious AEs (48% v 40%, respectively). The most commonly observed treatment-related AEs were diarrhea and rash, both predominantly in the lapatinib arm. CONCLUSION: Addition of lapatinib to chemoradiotherapy and its use as long-term maintenance therapy does not offer any efficacy benefits and had additional toxicity compared with placebo in patients with surgically treated high-risk SCCHN.

QSAR studies based on flow microcalorimetric bioassay data for interaction of homologous series of m-alkoxyphenols and p-hydroxybenzoates with E. coli cells were carried out applying factorial design. Results for both series showed a linear relationship between log(dose)max and log Po/w. Analysis of these data allows the identification of contributions toward the derived bioactivity from the parent structures (the molecule minus n-CH2 groups present in the side-chain) and the lipophilic groups, CH2. These results are discussed with respect to drug quantitative structure-relationship.

Silica gel was functionalized with [3-(2-aminoethyl)aminopropyl]trimethoxysilane group (SF-AEATS) and the characterization by chemical analysis (N) and infrared spectroscopy confirmed the functionalization. The capacity of the modified silica to adsorb the complex Ru(III)-EDTA from ethanolic solution was studied. The selectivity coefficients of the complex formed on the support obtained was (Gñ), 2,07 x 10(4) L/mol and the average number of ligand bonded by one metal ion on the support (ñ) was ~ 1.

Eu3+ luminescence and EXAFS (Extended X-ray Absorption Fine Structure) results are presented for organic-inorganic hybrid gel hosts composed of a siliceous network to which small chains of oxyethylene units are covalently grafted by means of urea bridges. Coordination numbers for Eu3+ ions range from 12.8 to 9.7 with increasing Eu3+ concentration while the Eu3+-first neighbours mean distance is found to be constant at 2.48-2.49 Å in the same concentration range. Emission spectra display a broad band in the green/blue spectral region superposed to narrow lines appearing in the yellow/red region in such a way that for the eyes emission appears white. The broad band is assigned to intrinsic NH groups emission and also to electron-hole recombination in the nanosised siliceous domains. The narrow lines are assigned to intra-4f6, 5D0->7F0-4 Eu3+ transitions and from the energy position of the 7F0-4 levels a mean distance could be calculated for the Eu3+-first neighbours. The calculated results are in good agreement with the experimental ones obtained from EXAFS analysis.

A differential pulse polarographic study with the objective to determine iron (III) in presense of copper (II) in a supporting electrolyte based on citrate - EDTA was made. The best experimental conditions found were a supporting electrolyte of citrate 0.25 mol L-1, EDTA 0.050 mol L-1 and KNO3 0.50 mol L-1, pH 5.00. In this media iron (III) showed a polarographic peak in -0.08 V and the copper (II) in -0.34 V, both vs. Ag/AgCl (saturated KCl). Thus, a analytical method was developed and applied to determine iron (III) in brass alloy, a matrix were copper is in large excess over iron. The results obtained showed no interference of copper in the iron determination. The value of 0.21% of iron in the sample alloy composition was obtained and the method was validated by atomic absortion and recovery test, and the results exhibited a good agreement with the proposed method.

This paper provides a survey of general aspects involved in the coordination chemistry of low-valent (mainly +III,+II), low-spin (d p5,d p6) ruthenium ions with ethylenediamine-N,N,N',N'-tetraacetate (edta) and their substituted derivatives. The topics covered herein include structure, reactivity, kinetics, thermodynamics, electrochemistry and spectroscopy. The contributions from either our research group or the literature over the last three decades are focused in this review.

In this study, the preparation of the xerogel anilinepropylsilica is reported. The ability of the xerogel for extracting Al(III), Cr(III) and Fe(III) from ethanol was investigated at 25 ºC. The xerogel adsorption capacities were obtained from the adsorption isotherms by using the batch method. Flame atomic absorption spectrometry (FAAS) was used to estimate the concentration of metal ions in solution. The adsorption affinity follows the series Cr(III) > Fe(III) > Al(III) and the maximum adsorption capacities of the metal ions were 0.61, 0.52 and 0.43 mmol g-1, respectively.
