Cachexia is a common systemic manifestation. Additionally, myostatin is known to be a negative regulator of skeletal muscle development. The present study aimed to investigate whether formoterol down-regulates the myostatin system in skeletal muscle of tumour-bearing rats. Real-time PCR and Western blotting were used for the analysis. Results showed that rats bearing the Yoshida AH-130 ascites hepatoma, a cachexia-inducing tumour, exhibited marked muscle wasting that affected the mass of the muscles studied. The cachectic animals exhibited a significant increase in the mRNA levels of the myostatin receptor (ActIIB) in gastrocnemius muscles. Notably, the expression of the various forms of follistatin, a protein with the opposite effects to those of myostatin, was significantly reduced as a result of the implantation of the tumour. When the animals were treated with formoterol, a β-agonist with anti-cachectic potential, increases in skeletal muscle weights were observed. The β-agonist significantly increased levels of various follistatin isoforms and significantly decreased the expression levels of the myostatin receptor. In addition, formoterol treatment resulted in a significant decrease of the myostatin protein content of the gastrocnemius muscle. In conclusion, the results presented indicate that certain anabolic actions of formoterol on the skeletal muscle of cachectic animals may be mediated via the myostatin system.

Lactate has been shown to offer neuroprotection in several pathologic conditions. This beneficial effect has been attributed to its use as an alternative energy substrate. However, recent description of the expression of the HCA1 receptor for lactate in the central nervous system calls for reassessment of the mechanism by which lactate exerts its neuroprotective effects. Here, we show that HCA1 receptor expression is enhanced 24 hours after reperfusion in an middle cerebral artery occlusion stroke model, in the ischemic cortex. Interestingly, intravenous injection of L-lactate at reperfusion led to further enhancement of HCA1 receptor expression in the cortex and striatum. Using an in vitro oxygen-glucose deprivation model, we show that the HCA1 receptor agonist 3,5-dihydroxybenzoic acid reduces cell death. We also observed that D-lactate, a reputedly non-metabolizable substrate but partial HCA1 receptor agonist, also provided neuroprotection in both in vitro and in vivo ischemia models. Quite unexpectedly, we show D-lactate to be partly extracted and oxidized by the rodent brain. Finally, pyruvate offered neuroprotection in vitro whereas acetate was ineffective. Our data suggest that L- and D-lactate offer neuroprotection in ischemia most likely by acting as both an HCA1 receptor agonist for non-astrocytic (most likely neuronal) cells as well as an energy substrate.

The amyloid-β peptide or Aβ is the key player in the amyloid-cascade hypothesis of Alzheimer's disease. Aβ appears to trigger cell death but also production of double-strand breaks (DSBs) in aging and Alzheimer's disease. All-trans retinoic acid (RA), a derivative of vitamin A, was already known for its neuroprotective effects against the amyloid cascade. It diminishes, for instance, the production of Aβ peptides and their oligomerisation. In the present work we investigated the possible implication of RA receptor (RAR) in repair of Aβ-induced DSBs. We demonstrated that RA, as well as RAR agonist Am80, but not AGN 193109 antagonist, repair Aβ-induced DSBs in SH-SY5Y cells and an astrocytic cell line as well as in the murine cortical tissue of young and aged mice. The nonhomologous end joining pathway and the Ataxia Telangiectasia Mutated kinase were shown to be involved in RA-mediated DSBs repair in the SH-SY5Y cells. Our data suggest that RA, besides increasing cell viability in the cortex of young and even of aged mice, might also result in targeted DNA repair of genes important for cell or synaptic maintenance. This phenomenon would remain functional up to a point when Aβ increase and RA decrease probably lead to a pathological state.

Antibodies play an important role in therapy and investigative biomedical research. The TNF-family member Receptor Activator of NF-κB (RANK) is known for its role in bone homeostasis and is increasingly recognized as a central player in immune regulation and epithelial cell activation. However, the study of RANK biology has been hampered by missing or insufficient characterization of high affinity tools that recognize RANK. Here, we present a careful description and comparison of two antibodies, RANK-02 obtained by phage display (Newa, 2014 [1]) and R12-31 generated by immunization (Kamijo, 2006 [2]). We found that both antibodies recognized mouse RANK with high affinity, while RANK-02 and R12-31 recognized human RANK with high and lower affinities, respectively. Using a cell apoptosis assay based on stimulation of a RANK:Fas fusion protein, and a cellular NF-κB signaling assay, we showed that R12-31 was agonist for both species. R12-31 interfered little or not at all with the binding of RANKL to RANK, in contrast to RANK-02 that efficiently prevented this interaction. Depending on the assay and species, RANK-02 was either a weak agonist or a partial antagonist of RANK. Both antibodies recognized human Langerhans cells, previously shown to express RANK, while dermal dendritic cells were poorly labeled. In vivo R12-31 agonist activity was demonstrated by its ability to induce the formation of intestinal villous microfold cells in mice. This characterization of two monoclonal antibodies should now allow better evaluation of their application as therapeutic reagents and investigative tools.

Inositol is a polyalcohol required for the proper formation of cell membranes. In the body, its plays an important role in the transmission of nerve impulses, its also helps in the transporting of fats within the body. In mammals, inositol exists as phosphorylated derivatives, various phosphoinositides, and in its free form. Agonist stimulated hydrolysis of phosphatidylinositol 4,5-bisphosphate [PI(4,5)P2] is the first step in the transmembrane signalling mechanism when cells respond to external stimuli. Under control of activated phospholipase C (PLC) via G-protein, two second messengers D-myo-inositol 1,4,5-triphosphate [Ins(1,4,5)P3] and diacylglycerol are released into the cell. From Ins(1,4,5)P3, enzymatic process under phosphatases or kinases control affords subsequent inositol phosphate metabolites. During the last decade the synthesis of modified inositol phosphate derivatives has been strongly investigated. This paper reviews principal aspects about synthesis and biological functions of these biomolecules.

In 1981 2,3-pyridine dicarboxylic acid (quinolinic acid) was discovery to be a selective agonist for the N-methyl -D-aspartic acid (NMDA) receptor. As a consequence it possesses neurotoxic activity resulting from overstimulation of the receptor. Quinolinic acid is implicated as an etiological factor in a range of neurodegenerative disease including AIDS related dementia, Huntington´s disease and Lyme disease. In the design of novel therapies to treat these diseases, some molecules have been identified as an important target. In this paper we described different methods to prepare quinolinic acid and derivatives.

Gonadal somatic cell and adrenocortical endocrine tumors are rare. The incidence of adrenocortical carcinomas is only 1-2/1000000 a year. However, they are aggressive, especially in adulthood and currently surgery is the only curative treatment. Cytotoxic agents are in use in advanced cancers, but side effects and multidrug resistance are often problems. Thus there is a need for novel curative treatment methods. In contrast, ovarian granulosa cell tumors and testicular Leydig cell tumors are usually benign, especially at a younger age. The aim of the present thesis was to study a novel targeted treatment method through luteinizing hormone/chorionic gonadotropin receptor (LHCGR) in a transgenic mouse tumor model. The cytotoxic agent was lytic peptide Hecate-CGbeta conjugate where 23 amino acid Hecate, a synthetic form of honeybee venom melittin, was conjugated to 15 amino acid fragment of human chorionic gonadotropin β subunit. Lytic peptides are known to act only on negatively charged cells, such as bacteria and cancer cells and hereby, due to hCGbeta fragment, the conjugate is able to bind directly to LHCGR bearing cancer cells, saving the healthy ones. The experiments were carried out in inhibin-alpha-Simian Virus 40-T-antigen transgenic mice that are known to express LHCGR-bearing gonadal tumors, namely Leydig and granulosa cell tumors by 100% penetrance. If the mice are gonadectomized prepubertally they form adrenocortical tumors instead. Transgenic and wild type mice were treated for three consecutive weeks with control vehicle, Hecate or Hecate-CGbeta conjugate. GnRH antagonist or estradiol was given to a group of mice with or without Hecate-CGbeta conjugate to analyze the additive role of gonadotropin blockage in adrenocortical tumor treatment efficacy. Hecate-CGbeta conjugate was able to diminish the gonadal and adrenal tumor size effectively in males. No treatment related side effects were found. Gonadotropin blockage through GnRH antagonist was the best treatment in female adrenal tumors. The mode of cell death by Hecate-CGbeta conjugate was proven to be through necrosis. LHCGR and GATA-4 were co-expressed in tumors, where the treatment down-regulated their expression simultaneously, suggesting their possible use as tumor markers. In conclusion, the present thesis showed that Hecate-CGbeta conjugate targets its action selectively through LHCGR and selectively kills the LHCGR bearing tumor cells. It works both in gonadal somatic and in ectopic LHCGR bearing adrenal tumors. These results establish a more general principle that receptors expressed ectopically in malignant cells can be exploited in targeted cytotoxic therapies without affecting the normal healthy cells.

The understanding of the scientific basis of the erectile function expanded rapidly the range of therapies for treating erectile dysfunction in recent years. This article reviews the role of dopamine on the erection mechanisms and its importance for new pro-erectile drug design. The ability of dopaminergic agents to elicit penile erection has been described since 1975 and successively confirmed by numerous studies. The development of apomorphine SL (dopaminergic non selective agonist) to enhance erectile function represents a new pharmacological approach to the management of erectile dysfunction using CNS drugs. The search for selective D4 dopaminergic agents is being explored by some research groups and pharmaceutical companies.

5-Methoxy-N,N-dimethyltryptamine (5-MeO-DMT) is a natural hallucinogen component of Ayahuasca, an Amazonian beverage traditionally used for ritual, religious and healing purposes that is being increasingly used for recreational purposes in US and Europe. 5MeO-DMT is of potential interest for schizophrenia research owing to its hallucinogenic properties. Two other psychotomimetic agents, phencyclidine and 2,5-dimethoxy-4-iodo-phenylisopropylamine (DOI), markedly disrupt neuronal activity and reduce the power of low frequency cortical oscillations (<4 Hz, LFCO) in rodent medial prefrontal cortex (mPFC). Here we examined the effect of 5-MeO-DMT on cortical function and its potential reversal by antipsychotic drugs. Moreover, regional brain activity was assessed by blood-oxygen level dependent (BOLD) functional magnetic resonance imaging (fMRI). 5-MeO-DMT disrupted mPFC activity, increasing and decreasing the discharge of 51 and 35% of the recorded pyramidal neurons, and reducing (−31%) the power of LFCO. The latter effect depended on 5-HT1A and 5-HT2A receptor activation and was reversed by haloperidol, clozapine, risperidone, and the mGlu2/3 agonist LY379268. Likewise, 5-MeO-DMT decreased BOLD responses in visual cortex (V1) and mPFC. The disruption of cortical activity induced by 5-MeO-DMT resembles that produced by phencyclidine and DOI. This, together with the reversal by antipsychotic drugs, suggests that the observed cortical alterations are related to the psychotomimetic action of 5-MeO-DMT. Overall, the present model may help to understand the neurobiological basis of hallucinations and to identify new targets in antipsychotic drug development.

Patients treated in intensive care units require sedation and analgesia. However, sedative drugs also have potential adverse effects, and there is no single ideal sedativeanalgesic drug for these patients. Dexmedetomidine is an apha2-adrenoceptor agonist licenced for sedation of intensive care patients and patients undergoing surgery and other invasive procedures. Several routes of parenteral administration (intravenous, intramuscular, subcutaneous and intranasal) have been utilized. In the present series of studies, the pharmacokinetics and pharmacodynamics of intranasally administered dexmedetomidine as well as the gastrointestinal effects of intravenous dexmedetomidine were determined in healthy volunteers. Pharmacokinetics of dexmedetomidine during long lasting, high-dose infusions were characterized in intensive care patients. The bioavailability of intranasal dexmedetomidine was relatively good (65%), but interindividual variation was large. Dexmedetomidine significantly inhibited gastric emptying and gastrointestinal transit. In intensive care patients, the elimination half-life of dexmedetomidine was somewhat longer than reported for infusions of shorter duration and in less ill patients or healthy volunteers. Dexmedetomidine appeared to have linear pharmacokinetics up to the studied dose rate of 2.5 μg/kg/h. Dexmedetomidine clearance was decreasing with age and its volume of distribution was increased in hypoalbuminaemic patients, resulting in a longer elimination half-life and context-sensitive half-time. Intranasally administered dexmedetomidine was efficacious and well tolerated, making it appropriate for clinical situations requiring light sedation. The clinical significance of the gastrointestinal inhibitory effects of dexmedetomidine should be further evaluated in intensive care patients. The possibility of potentially altered potency and effect duration should be taken into account when administering dexmedetomidine to elderly or hypoalbuminaemic patients.

Alpha2-Adrenoceptors are cell-surface G protein coupled receptors that mediate many of the effects of the catecholamines noradrenaline and adrenaline. The three human α2-adrenoceptor subtypes are widely expressed in different tissues and organs, and they mediate many different physiological and pharmacological effects in the central and peripheral nervous system and as postsynaptic receptors in target organs. Previous studies have demonstrated that α2-adrenoceptors mediate both vascular constriction and dilatation in humans. Large inter-individual variation has been observed in the vascular responses to α2-adrenoceptor activation in clinical studies. All three receptor subtypes are potential drug targets. It was therefore considered important to further elucidate the details of adrenergic vascular regulation and its genetic variation, since such knowledge may help to improve the development of future cardiovascular drugs and intensive care therapies. Dexmedetomidine is the most selective and potent α2-adrenoceptor agonist currently available for clinical use. When given systemically, dexmedetomidine induces nearly complete sympatholysis already at low concentrations, and postsynaptic effects, such vasoconstriction, can be observed with increasing concentrations. Thus, local infusions of small doses of dexmedetomidine into dorsal hand veins and the application of pharmacological sympathectomy with brachial plexus block provide a means to assess drug-induced peripheral vascular responses without interference from systemic pharmacological effects and autonomic nervous system regulation. Dexmedetomidine was observed to have biphasic effects on haemodynamics, with an initial decrease in blood pressure at low concentrations followed by substantial increases in blood pressure and coronary vascular resistance at high concentrations. Plasma concentrations of dexmedetomidine that significantly exceeded the recommended therapeutic level did not reduce myocardial blood flow below the level that is observed with the usual therapeutic concentrations and did not induce any evident myocardial ischaemia in healthy subjects. Further, it was demonstrated that dexmedetomidine also had significant vasodilatory effects through activation of endothelial nitric oxide synthesis, and thus when the endothelial component of the blood vessel response to dexmedetomidine was inhibited, peripheral vasoconstriction was augmented. Hand vein constriction responses to α2-adrenoceptor activation by dexmedetomidine were only weakly associated with the constriction responses to α1-adrenoceptor activation, pointing to independent cellular regulation by these two adrenoceptor classes. Substantial inter-individual variation was noted in the venous constriction elicited by activation of α2-adrenoceptors by dexmedetomidine. In two study populations from two different continents, a single nucleotide polymorphism in the PRKCB gene was found to be associated with the dorsal hand vein constriction response to dexmedetomidine, suggesting that protein kinase C beta may have an important role in the vascular α2-adrenoceptor signalling pathways activated by dexmedetomidine.

Objetivo: o objetivo do presente trabalho é apresentar um perfil das pacientes com endometriose, estadio e resultados do tratamento entre as pacientes atendidas no nosso Serviço. Métodos: foram revistos 155 prontuários médicos de pacientes portadoras de endometriose atendidas no período de 1991 a 1996 segundo um protocolo preestabelecido. Resultados: a média de idade das pacientes foi de 31 anos, sendo que a maioria era branca e com ciclos menstruais regulares. Os sintomas mais observados foram a dismenorréia, dor pélvica e infertilidade, sendo esses sinais e sintomas de grau leve na maioria dos casos. Os métodos diagnósticos utilizados foram a laparoscopia ou ultra-sonografia. Houve concordância entre a ultra-sonografia e a laparoscopia em 96% das vezes. Em 28% dos casos houve necessidade de realização de laparotomia para elucidação diagnóstica e/ou tratamento. O achado de endometrioma ocorreu em 37% das vezes e em apenas 74% das biópsias das lesões houve a confirmação de endometriose pelo estudo anatomopatológico. Houve melhora significativa com o tratamento clínico independente da droga utilizada, com melhora dos sintomas em cerca de 50% delas. Foram realizadas técnicas de fertilização assistida em 34 pacientes, das quais 80% foram fertilização in vitro (FIV), com taxa de 27% de gravidez/paciente. Conclusões: o método diagnóstico de escolha deve ser a laparoscopia, embora a ultra-sonografia apresente um alto índice de concordância. O estudo anatomopatológico das lesões deve ser utilizado como método auxiliar no diagnóstico, pois ele não é positivo em todos os casos. O tratamento clínico com reprodução assistida é uma boa opção terapêutica, principalmente com o uso de FIV.

Objetivo: estudar os efeitos da goserelina de depósito como agonista do GnRH para a supressão hipofisária, durante a hiperestimulação ovariana controlada (HOC) em fertilização in vitro e transferência de embriões (FIVeTE). Método: foram estudados prospectivamente 110 ciclos envolvendo 101 mulheres. A goserelina de depósito foi administrada em dose única subcutânea, para algumas mulheres (87 ciclos) na primeira fase do ciclo menstrual e para outras (23 ciclos), na fase lútea. A administração das gonadotrofinas de mulheres menopausadas (hMG) era diária até que fossem identificados ao menos dois folículos com diâmetro maior ou igual a 18 mm, quando se administrava a gonadotrofina coriônica (hCG) e programava-se a aspiração folicular. Resultados: a idade média das mulheres foi de 36,7 anos (variando de 23 a 42 anos). As principais indicações para a realização da FIVeTE foram: fator tuboperitoneal (75,2%), endometriose (10,9%), fator ovulatório (7,9%), fator masculino (3,0%) e esterilidade sem causa aparente (3,0%). Do total de casos que iniciaram a HOC, 28 (25,5%) foram suspensos. Em 7 ciclos (8,04% dos casos medicados na primeira fase do ciclo menstrual), houve necessidade de se realizar a aspiração dos cistos ovarianos. Foram transferidos em média 3,3 embriões por paciente (variação de 1 a 5 embriões por paciente). Das 70 transferências de embriões realizadas, resultaram 16 gravidezes clínicas (taxa de gravidez: 22,85%). Conclusão: a administração da goserelina de depósito é uma alternativa útil para a supressão hipofisária em FIVeTE pois não compromete os resultados quando comparados aos observados na literatura, e não é necessário que a paciente compareça diariamente para receber a medicação, fato de extrema importância num serviço público.

Objetivo: avaliar os resultados do tratamento videolaparoscópico de 32 pacientes com endometrioma ovariano. Métodos: estudo retrospectivo que incluiu trinta e duas pacientes encaminhadas ao setor de Endoscopia Ginecológica do Serviço de Ginecologia e Obstetrícia do Hospital do Servidor Público Estadual "Francisco Morato de Oliveira" - São Paulo, as quais tinham diagnóstico clínico e ultra-sonográfico de endometrioma ovariano e foram tratadas por videolaparoscopia. Onze pacientes com endometriomas com diâmetro menor que 3 cm foram submetidas à drenagem do endometrioma e exérese da cápsula na primeira laparoscopia. Vinte e uma pacientes com endometriomas com diâmetros maior que 3 cm foram submetidas à drenagem do endometrioma e lavagem do cisto na primeira laparoscopia. A seguir, utilizaram análogo do GnRH por quatro meses (1 ampola mensal) e então foram submetidas à segunda laparoscopia para retirada da cápsula do endometrioma. O exame histopatológico do material obtido na cirurgia foi realizado em todos os casos. Avaliou-se o resultado imediato do procedimento e a freqüência de recidivas. Resultados: não se registraram intercorrências cirúrgicas ou complicações pós-operatórias. Ocorreram três recidivas em um período de seis a doze meses, todas no grupo de 21 pacientes com endometriomas com o diâmetro maior que 3 cm. Conclusão: o tratamento videolaparoscópico dos endometriomas ovarianos com retirada da cápsula apresenta bons resultados e baixo índice de recidiva.

Objetivo: estudar o resultado da ablação endometrial histeroscópica, com o uso de ressectoscópio, em pacientes com sangramento uterino anormal de etiologia benigna e refratária ao tratamento clínico, e relatar os fatores associados aos insucessos. Métodos: foram avaliados retrospectivamente informações sobre 64 pacientes com sangramento uterino anormal de causa benigna, submetidas à intervenção entre abril de 1994 e fevereiro de 2000. A média de idade foi de 42,9 anos e a paridade média, de 2,6 partos. Foi realizada histeroscopia diagnóstica com biópsia endometrial pré-operatoriamente. Duas pacientes receberam previamente gestrinona, seis danazol e 44 análogo do GnRH. No ato cirúrgico realizou-se a cauterização elétrica do fundo uterino e das regiões cornuais. Depois, com o ressectoscópio, foi removido o endométrio até 1 cm cranialmente ao orifício interno e na profundidade de 2 a 3 mm na musculatura. A distensão uterina se fazia com glicina a 1,5% até há dois anos, quando se passou a usar o manitol a 3%. O seguimento foi de 11,5 meses em média. Resultados: estabeleceu-se amenorréia em 31,2% das pacientes e hipomenorréia em 45,3%. Houve persistência do sangramento anormal em 23,5%. Houve uma perfuração uterina na cérvico-dilatação (1,5%). Os resultados foram melhores nas pacientes com mais de 40 anos de idade, nas multíparas e nas que usaram análogos do GnRH. Nos casos de falhas houve freqüência estatisticamente significante (p=0,04) de miomas submucosos de 46,6% contra 20,6% no grupo com hipomenorréia e 20% no grupo com amenorréia. O uso de análogo do GnRH fez decrescer de modo significante (p=0,03) os insucessos. No grupo de pacientes nos quais se considerou haver falha da cirurgia, 46% tiveram, ao exame anatomopatológico, endométrio secretor; no grupo com hipomenorréia, 10,3% tiveram este resultado histológico e no grupo com amenorréia, 10%, o que não apresentou significância estatística (p=0,12). Conclusão: o método mostrou-se útil para tratar o sangramento uterino anormal de etiologia benigna. Recomenda-se o uso de análogos do GnRH previamente à intervenção. Sugere-se que as pacientes mais jovens, com menor paridade e com diagnóstico pré-operatório de mioma submucoso recebam atenção especial, pois entre elas ocorrem falhas com maior freqüência.
