Progressive supranuclear palsy is a rare, degenerative brain disorder and the second most common syndrome in which the patient exhibits 'parkinsonism', that is, a variety of symptoms involving problems with movement. General symptoms include difficulties with gait and balance; the patient walking clumsily and often falling backwards. The syndrome can be difficult to diagnose and visual signs and symptoms can help to separate it from closely related movement disorders such as Parkinson's disease, multiple system atrophy, dementia with Lewy bodies and corticobasal degeneration. A combination of the presence of vertical supranuclear gaze palsy, fixation instability, lid retraction, blepharospasm and apraxia of eyelid opening and closing may be useful visual signs in the identification of progressive supranuclear palsy. As primary eye-care practitioners, optometrists should be able to identify the visual problems of patients with this disorder and be expected to work with patients and their carers to manage their visual welfare.

This thesis is an exploration of the oscillatory changes occurring in the visual cortex as measured by a functional imaging technique known as Synthetic Aperture Magnetometry (SAM), and how these compare to the BOLD response, across a number of different experimental paradigms. In chapter one the anatomy and physiology of the visual pathways and cortex are outlined, introducing the reader to structures and terms used throughout the thesis whilst chapter two introduces both the technology and analysis techniques required to record MEG and fMRI and also outlines the theory behind SAM. In chapter three the temporal frequency tuning of both striate and extrastriate cortex is investigated, showing fundamental differences in both tuning characteristics and oscillatory power changes between the two areas. Chapter four introduces the concept of implied-motion and investigates the role of area V5 / MT in the perception of such stimuli and shows, for the first time, the temporal evolution of the response in this area. Similarly a close link is shown between the early evoked potential, produced by the stimulus, and previous BOLD responses. Chapter five investigates the modulation of cortical oscillations to both shifts in attention and varying stimulus contrast. It shows that there are both induced and evoked modulation changes with attention, consistent with areas previously known to show BOLD responses. Chapter six involves a direct comparison of cortical oscillatory changes with those of the BOLD response in relation to the parametric variation of a motion coherence stimulus. It is shown that various cortical areas show a linear BOLD response to motion coherence and, for the first time, that both induced oscillatory and evoked activity also vary linearly in areas coincidental with the BOLD response. The final chapter is a summary of the main conclusions and suggests further work.

The dynamics of Boolean networks (BN) with quenched disorder and thermal noise is studied via the generating functional method. A general formulation, suitable for BN with any distribution of Boolean functions, is developed. It provides exact solutions and insight into the evolution of order parameters and properties of the stationary states, which are inaccessible via existing methodology. We identify cases where the commonly used annealed approximation is valid and others where it breaks down. Broader links between BN and general Boolean formulas are highlighted.

An array of different structural probes has been used to define the effect of adding Zn and Ti to a sodium-calcium phosphate glass. X-ray absorption spectroscopy at the Zn K-edge suggests that the Zn atoms occupy mixed (4- and 6-fold) sites within the glass matrix. X-ray diffraction reveals a feature at 2.03 angstrom that develops with the addition of Zn and Ti and is consistent with Zn-O and Ti-O near-neighbour distances. Neutron diffraction is used to resolve two distinct P-O distances and highlights the decrease in P center dot center dot center dot P coordination number from 2.0 to 1.7 as the Ti metal concentration rises, which is attributed to the O/P fraction moving away from the metaphosphate value of 3.0 to 3.1 with the addition of Ti. Other correlations, such as those associated with CaO(x) and NaO(x) polyhedra, remain largely unaffected. These results suggest that the network forming P center dot center dot center dot P correlation is most disrupted, with the disorder parameter rising from 0.07 to 0.10 angstrom with the additional modifiers. Zn appears to be introduced into the network as a direct replacement for Ca and causes no structural variation over the composition range studied.

Over recent years, the role of engineering in promoting a sustainable society has received much public attention [1] with particular emphasis given to the need to promote the future prosperity and security of society through the recruitment and education of more engineers [2,3]. From an employment perspective, the Leitch Review [4] suggested that ‘generic’ transferable employability skills development should constitute a more substantial part of university education. This paper argues that the global drivers impacting engineering education [5] correlate strongly to those underpinning the Leitch review, therefore the question of how to promote transferable employability skills within the wider engineering curriculum is increasingly relevant. By exploring the use of heritage in the engineering curriculum as a way to promote learning and engage students, a less familiar approach to study is discussed. This approach moves away from stereotypical notions of the use of information technology as representing the pinnacle of innovation in education. Taking the student experience as its starting point, the paper draws upon the findings of an exploratory study critically analysing the pedagogical value of using heritage in engineering education. It discusses a teaching approach in which engineering students are taken out of their ‘comfort zone’ - away from the classroom, laboratory and computer, to a heritage site some 100 miles away from the university. The primary learning objective underpinning this approach is to develop students’ transferable skills by encouraging them to consider how to apply theoretical concepts to a previously unexplored situation. By reflecting upon students’ perceptions of the value of this approach, and by identifying how heritage may be utilised as an innovative learning and teaching approach in engineering education, this paper makes a notable contribution to current pedagogical debates in the discipline.

This review aims to systematically evaluate the empirical literature relating to the interpersonal functioning of those with eating disorder psychopathology and presents a preliminary model to help the formulation of patients' problems. Following a thorough literature search, 35 papers were included in this systematic review. The vast majority of studies indicate a strong association between eating psychopathology and certain maladaptive personality traits. The origins of social anxiety and poor social support that have been described as maintaining eating disorders appear to differ according to diagnostic groups. Interpersonal difficulties in people with restrictive behaviors such as anorexia appear to be related to the avoidance of expressing feelings to others and to giving priority to other people's feeling over their own. While interpersonal difficulties in patients with bulimia nervosa appear to be more related to interpersonal distrust and negative interaction and conflict with others. These concepts are presented in a preliminary model of interpersonal functioning in the eating disorders. © 2012 Elsevier Ltd.

Objective: Symptoms of maternal eating disorders have been linked with the use of maladaptive restrictive child feeding practices. However, how these symptoms impact upon restriction in child feeding is poorly understood. The aims of this research were to assess whether symptoms of obsessive compulsiveness, which are often comorbid with eating disorder symptoms, mediate the relationships between maternal eating disorder symptoms and the use of restrictive feeding practices. Method: A total of 128 mothers of children aged 2-6 years completed measures of their restrictive feeding practices, symptoms of eating disorders, and obsessive compulsiveness. Results: Maternal restriction was positively correlated with symptoms of drive for thinness, bulimia, and checking and cleaning obsessions and compulsions. Cleaning obsessions and compulsions mediated the relationships between maternal drive for thinness and feeding restriction. Conclusion: Cleaning obsessions and compulsions may help to explain the relationships between some symptoms of maternal eating disorders and the use of restrictive feeding practices. © 2008 by Wiley Periodicals, Inc.

Most people with bipolar disorder first seek treatment for depressive rather than manic symptoms. However, effective management of bipolar depression remains a challenge. In this review, Celia Feetam highlights how bipolar depression differs from unipolar depression both in its characteristics and in the approach to its treatment. © 2011 John Wiley & Sons, Ltd.

From humble beginnings less than twenty years ago, the economic relationship between Canada and the Chinese has flourished so that China is now Canada’s second most important trading partner after the United States. The Chinese demand for Canada’s exports, in particular in the natural resource sectors, has been a clear win for Canada, especially during the recent Great Recession where demand from other countries dropped sharply. But other aspects of the relationship have led to suggestions of costs. Canada’s large trade deficit with China at least looks superficially as a drag on the Canadian economy. It has been suggested the greater exposure to a low cost producer has displaced Canadian production and jobs and lowered wages. We find that each argument for costs to the relationship ignores important factors. On balance we conclude the relationship has been good for Canada, and it could be even better in the future.

Bakgrund: Flera studier har visat att personer med bipolär sjukdom har en sämre livskvalitet jämfört med övriga befolkningen. Som sjuksköterska inom psykiatrisk vård är det viktigt att ha kunskap om sjukdomen för att ge bästa möjliga behandling med mål att stabilisera symtom, förebygga återfall och främja livskvalitet. Syfte: Att utifrån sjuksköterskans uppfattning beskriva vilka omvårdnadsinsatser som främjar livskvalitet vid bipolär sjukdom. Metod: En kvalitativ innehållsanalys som baserades på åtta semistrukturerade intervjuer. Resultat: Resultatet visade tre huvudteman: Kunskap om sjukdomen: betydelsen av att både patient och anhöriga fick kunskap om sjukdomen; Trygghet och kontinuitet i vården: veta var och till vem man vänder sig i vården hade stor betydelse samt Personcentrerad vård: att bli sedd och bemött som en unik person. Slutsats: Patientutbildning där man tar upp vikten av regelbundna levnadsvanor, att undvika stress samt att bli medveten om sina tidiga tecken kan hjälpa att stabilisera symtom och förebygga återfall.

Background and Aims To examine whether a history of mood episodes triggered by sleep loss is associated with (1) postpartum psychosis (PP) and (2) more broadly-defined postpartum mood episodes that included postnatal depression (PND), in women with bipolar disorder (BD). Methods Participants were 622 parous women with a diagnosis of bipolar-I disorder recruited in the UK to the Bipolar Disorder Research Network. Diagnosis and perinatal episodes were assessed via interview and case note data. Women were also asked during the interview whether episodes of mania and/or depression were triggered by sleep loss. We compared the rates of PP and PND within women who did and did not endorse sleep loss as a trigger of mood episodes. Results Women who reported that their episodes of mania were usually triggered by sleep loss were twice as likely to have experienced an episode of PP (OR = 2.00, 95% CI = 1.20–3.36) than women who did not report this. This effect remained significant when controlling for clinical and demographic factors. We found no significant associations between depression triggered by sleep loss and PP. Analyses in which we defined postpartum episodes at a broader level to include both PP and PND were not significant. Conclusions In pregnant women with BD, a history of mania following sleep loss could be a potential marker of vulnerability to severe postpartum episodes. Further study in prospective samples is required in order to confirm these findings, which may have important implications for understanding the aetiology of PP and of mood disorders more generally.

La recherche a pour objet la réinsertion socioprofessionnelle des personnes présentant un trouble mental grave. Elle vise à apporter une compréhension de leur cheminement de réinsertion sur le marché de l’emploi régulier, et ce, à partir des perspectives des acteurs concernés et à travers une prise en compte des contextes sociaux dans lesquels ils évoluent. Notre étude est guidée par trois objectifs : (1) identifier certains éléments sur les plans individuel et environnemental pouvant faciliter la réinsertion professionnelle des personnes présentant un trouble mental; (2) identifier certains obstacles sur les plans individuel et environnemental susceptibles de nuire à leur réinsertion en emploi; (3) mieux comprendre l’impact de la réinsertion professionnelle sur les sphères individuelles et sociales de ces individus et leurs perceptions quant à la réinsertion professionnelle. Le cadre conceptuel adopté s’appuie sur la notion/paradigme du rétablissement en santé mentale, ce dernier étant compris comme un processus multidimensionnel et non-linéaire. Notre démarche est qualitative et fondée sur des collectes réalisées à ÉquiTravail, un organisme de la ville de Québec ayant pour mission de favoriser l’intégration, la réintégration et le maintien sur le marché du travail de personnes aux prises avec un trouble mental. Des entrevues semi-dirigées individuelles auprès de quatre usagers, ainsi qu’un groupe de discussion focalisé avec quatre intervenants, ont été réalisés. Le matériel a été soumis à une analyse thématique des contenus. Nos résultats illustrent le fait que tant les composantes individuelles que les éléments de l’environnement et ceux relevant de l’interaction entre l’environnement et l’individu sont cruciaux dans les processus de réinsertion socioprofessionnelle. La réussite du processus d’insertion socioprofessionnelle ne repose pas uniquement sur la responsabilité individuelle, mais aussi sur l’interaction entre les composantes individuelles et les aspects de l’environnement gravitant autour de l’individu. Les implications de ces conclusions pour la recherche et la pratique sont également discutées.

Background The HCL-32 is a widely-used screening questionnaire for hypomania. We aimed to use a Rasch analysis approach to (i) evaluate the measurement properties, principally unidimensionality, of the HCL-32, and (ii) generate a score table to allow researchers to convert raw HCL-32 scores into an interval-level measurement which will be more appropriate for statistical analyses. Methods Subjects were part of the Bipolar Disorder Research Network (BDRN) study with DSM-IV bipolar disorder (n=389). Multidimensionality was assessed using the Rasch fit statistics and principle components analysis of the residuals (PCA). Item invariance (differential item functioning, DIF) was tested for gender, bipolar diagnosis and current mental state. Item estimates and reliabilities were calculated. Results Three items (29, 30, 32) had unacceptable fit to the Rasch unidimensional model. Item 14 displayed significant DIF for gender and items 8 and 17 for current mental state. Item estimates confirmed that not all items measure hypomania equally. Limitations This sample was recruited as part of a large ongoing genetic epidemiology study of bipolar disorder and may not be fully representative of the broader clinical population of individuals with bipolar disorder. Conclusion The HCL-32 is unidimensional in practice, but measurements may be further strengthened by the removal of four items. Re-scored linear measurements may be more appropriate for clinical research.

A 31-year-old Caucasian woman of South-American descent was diagnosed with a variant of multicentric Castleman disease (MCD) that has been reported in Japan as Castleman-Kojima disease. This is a systemic inflammatory disorder known as TAFRO Syndrome which includes thrombocytopenia, polyserositis (ascites/pleural effusion), microcytic anemia, myelofibrosis, fever, renal dysfunction and organomegaly, with immunologic disorder, polyclonal hypergammaglobulinemia, and elevated levels of interleukin-6 (IL-6) and the vascular endothelial growth factor present in serum and/or effusions. Optimal therapies are not well established. The patient was treated with methylprednisolone and rituximab. Following the start of treatment, the patient has been asymptomatic for over 8 months.
