Stryphnodendron adstringens, conhecida como barbatimão, é uma espécie de distribuição geográfica ampla no bioma cerrado, ocorrendo desde o Pará, pelo planalto central, até Minas Gerais e São Paulo. Sua casca e frutos contêm tanino e apresentam propriedades medicinais. É uma espécie de baixa exigência nutricional e não acumuladora de alumínio. É hermafrodita e polinizada por pequenos insetos, especialmente abelhas. Está entre as principais espécies lenhosas encontradas na área de estudo. O objetivo deste estudo foi avaliar os eventos fenológicos desta espécie, numa área de cerrado sensu stricto na Fazenda Água Limpa (15°56'S e 47°46'W), durante cinco anos (1987-1991). Dez árvores foram selecionadas e observadas de 18/01/1987 até 07/11/1991 a intervalos quinzenais. A estacionalidade pluvial do clima foi bem definida durante o período de observação. Os padrões fenológicos foram analisados graficamente, pelo teste de Kruskall-Wallis a 5% e pelo coeficiente de correlação de Spearman. Todos os indivíduos floriram anualmente exceto dois que não floriram em 1990, provavelmente como conseqüência da queimada em 1989. A floração ocorreu entre julho e novembro, sendo que os picos variaram entre os anos. Todos os indivíduos frutificaram até 1989 quando os frutos foram abortados pela queimada. Nenhuma frutificação foi observada no ano seguinte e em 1991 a produção de frutos atingiu apenas a metade daquela observada nos dois primeiros anos. A espécie apresenta modelo fenológico anual com floração, frutificação, dispersão de sementes e picos de senescência e de emissão de folhas novas, todos na estação seca. Seus frutos requerem um longo período de maturação, cerca de 12 meses, alcançando a maturidade na época seca do ano seguinte. Queimadas ocasionais afetaram, de maneira acentuada, as atividades reprodutivas, especialmente a frutificação.

Resistance to chemotherapy in cancer cells is mainly mediated by overexpression of P-glycoprotein (Pgp), a plasma membrane ATP-binding cassette (ABC) transporter which extrudes cytotoxic drugs at the expense of ATP hydrolysis. Pgp consists of two homologous halves each containing a transmembrane domain and a cytosolic nucleotide-binding domain (NBD) which contains two consensus Walker motifs, A and B, involved in ATP binding and hydrolysis. The protein also contains an S signature characteristic of ABC transporters. The molecular mechanism of Pgp-mediated drug transport is not known. Since the transporter has an extraordinarily broad substrate specificity, its cellular function has been described as a "hydrophobic vacuum cleaner". The limited knowledge about the mechanism of Pgp, partly due to the lack of a high-resolution structure, is well reflected in the failure to efficiently inhibit its activity in cancer cells and thus to reverse multidrug resistance (MDR). In contrast to the difficulties encountered when studying the full-length Pgp, the recombinant NBDs can be obtained in large amounts as soluble proteins. The biochemical and biophysical characterization of recombinant NBDs is shown here to provide a suitable alternative route to establish structure-function relationships. NBDs were shown to bind ATP and analogues as well as potent modulators of MDR, such as hydrophobic steroids, at a region close to the ATP site. Interestingly, flavonoids also bind to NBDs with high affinity. Their binding site partly overlaps both the ATP-binding site and the steroid-interacting region. Therefore flavonoids constitute a new promising class of bifunctional modulators of Pgp.

R,S-sotalol, a ß-blocker drug with class III antiarrhythmic properties, is prescribed to patients with ventricular, atrial and supraventricular arrhythmias. A simple and sensitive method based on HPLC-fluorescence is described for the quantification of R,S-sotalol racemate in 500 µl of plasma. R,S-sotalol and its internal standard (atenolol) were eluted after 5.9 and 8.5 min, respectively, from a 4-micron C18 reverse-phase column using a mobile phase consisting of 80 mM KH2PO4, pH 4.6, and acetonitrile (95:5, v/v) at a flow rate of 0.5 ml/min with detection at lex = 235 nm and lem = 310 nm, respectively. This method, validated on the basis of R,S-sotalol measurements in spiked blank plasma, presented 20 ng/ml sensitivity, 20-10,000 ng/ml linearity, and 2.9 and 4.8% intra- and interassay precision, respectively. Plasma sotalol concentrations were determined by applying this method to investigate five high-risk patients with atrial fibrillation admitted to the Emergency Service of the Medical School Hospital, who received sotalol, 160 mg po, as loading dose. Blood samples were collected from a peripheral vein at zero, 0.5, 1.0, 1.5, 2.0, 3.0, 4.0, 6.0, 8.0, 12.0 and 24.0 h after drug administration. A two-compartment open model was applied. Data obtained, expressed as mean, were: CMAX = 1230 ng/ml, TMAX = 1.8 h, AUCT = 10645 ng h-1 ml-1, Kab = 1.23 h-1, a = 0.95 h-1, ß = 0.09 h-1, t(1/2)ß = 7.8 h, ClT/F = 3.94 ml min-1 kg-1, and Vd/F = 2.53 l/kg. A good systemic availability and a fast absorption were obtained. Drug distribution was reduced to the same extent in terms of total body clearance when patients and healthy volunteers were compared, and consequently elimination half-life remained unchanged. Thus, the method described in the present study is useful for therapeutic drug monitoring purposes, pharmacokinetic investigation and pharmacokinetic-pharmacodynamic sotalol studies in patients with tachyarrhythmias.

To evaluate the effect of smoking habits on sleep, data from 1492 adults referred to the Sleep Institute were accessed and divided into 3 categories of smoking status: current, former and non-smokers. Categories of pack-years (<15 and ≥15) defined smoking severity. The association of smoking status and smoking severity with sleep was analyzed for sleep parameters, especially apnea and hypopnea index (AHI) ≥5, more than 5% of total sleep time (TST) spent with oxyhemoglobin saturation (SaO2) <90%, and arousal index. The arousal index was higher among current (21 ± 17) and former smokers (20 ± 17) than non-smokers (17 ± 15; P < 0.04). Former smokers had a higher percent of TST at SaO2 <90% than non-smokers (9 ± 18 vs 6 ± 13; P < 0.04). Former smokers with pack-years ≥15 compared to <15 exhibited higher AHI (22 ± 24 vs 16 ± 21; P < 0.05) and arousal index (22 ± 19 vs 18 ± 15; P < 0.05). Current smokers with pack-years ≥15 compared to <15 exhibited higher arousal index (23 ± 18 vs 18 ± 16; P < 0.05) and percent of TST at SaO2 <90% (11 ± 17 vs 6 ± 13; P < 0.05). Smoking status and pack-years were not associated with AHI ≥5 on logistic regression analysis, but current smokers with pack-years ≥15 were 1.9 times more likely to spend more than 5% of TST at SaO2 <90% than non-smokers (95%CI = 1.21-2.97; P = 0.005). The variability of arousal index was influenced by gender, AHI and current smokers with pack-years ≥15 (all P < 0.01). Smoking habits seem to be associated with arousal and oxyhemoglobin desaturation during sleep, but not with AHI. The effect was more pronounced in current than former smokers.

OBJETIVO: Analisar comparativamente características clínicas e evolução de pacientes com e sem IRA adquirida em UTI geral de um hospital universitário terciário. MÉTODO: Estudo prospectivo observacional com 263 pacientes acompanhados diariamente durante a internação em UTI Geral do Hospital das Clínicas da Faculdade de Medicina de Botucatu no período de julho de 2007 a abril de 2008. RESULTADOS: A incidência de IRA foi de 31,2%. Os grupos foram semelhantes quanto ao sexo e diferiram quanto à etiologia da admissão em UTI (sepse: 31,7% x 13,1%, p < 0,0001, pós-operatório: 11% x 43%, p < 0,0001), idade (59,6 ± 18,1 x 50,2 ± 18,6 anos, p < 0,0001), APACHE II: (21 ± 11,1 x 11 ± 4,8, p = 0,002), oligúria (67,7% x 4,5%, p < 0,0001), presença de ventilação mecânica (81,7 x 57,7%, p = 0,0014), uso de drogas vasoativas (62,2 x 32,6%, p < 0,0001) e enfermaria de procedência (PS: 22 x 14,5%, p = 0,02 e centro cirúrgico: 42,7 x 62,6%, p = 0,03). Quanto às comorbidades, os grupos foram diferentes quanto à presença de HAS e IRC (42,6 x 35,9%, p = 0,005 e 15,8 x 2,1%, p = 0,04, respectivamente) e semelhantes quanto à presença de diabetes e ICC (19,5 x 11%, ns e 6 x 1,1%, ns, respectivamente). A mortalidade foi superior nos pacientes que contraíram IRA (62,1 x 16,5%, p < 0,0001). CONCLUSÃO: A incidência de IRA é elevada em UTI e presente em pacientes com parâmetros clínicos e índices prognósticos de maior gravidade, o que justifica a maior mortalidade observada neles.
