It is poor in the literature the behavior of the geometric indices of heart rate variability (HRV) during the musical auditory stimulation. The objective is to investigate the acute effects of classic musical auditory stimulation on the geometric indexes of HRV in women in response to the postural change maneuver (PCM). We evaluated 11 healthy women between 18 and 25 years old. We analyzed the following indices: Triangular index, Triangular interpolation of RR intervals and Poincar plot (standard deviation of the instantaneous variability of the beat-to beat heart rate [SD1], standard deviation of long-term continuous RR interval variability and Ratio between the short - and long-term variations of RR intervals [SD1/SD2] ratio). HRV was recorded at seated rest for 10 min. The women quickly stood up from a seated position in up to 3 s and remained standing still for 15 min. HRV was recorded at the following periods: Rest, 0-5 min, 5-10 min and 10-15 min during standing. In the second protocol, the subject was exposed to auditory musical stimulation (Pachelbel-Canon in D) for 10 min at seated position before standing position. Shapiro-Wilk to verify normality of data and ANOVA for repeated measures followed by the Bonferroni test for parametric variables and Friedmans followed by the Dunns posttest for non-parametric distributions. In the first protocol, all indices were reduced at 10-15 min after the volunteers stood up. In the protocol musical auditory stimulation, the SD1 index was reduced at 5-10 min after the volunteers stood up compared with the music period. The SD1/SD2 ratio was decreased at control and music period compared with 5-10 min after the volunteers stood up. Musical auditory stimulation attenuates the cardiac autonomic responses to the PCM.

Background: Chronic exposure to musical auditory stimulation has been reported to improve cardiac autonomic regulation. However, it is not clear if music acutely influences it in response to autonomic tests. We evaluated the acute effects of music on heart rate variability (HRV) responses to the postural change maneuver (PCM) in women. Method: We evaluated 12 healthy women between 18 and 28 years old and HRV was analyzed in the time (SDNN, RMSSD, NN50 and pNN50) and frequency (LF, HF and LF/HF ratio) domains. In the control protocol, the women remained at seated rest for 10 minutes and quickly stood up within three seconds and remained standing still for 15 minutes. In the music protocol, the women remained at seated rest for 10 minutes, were exposed to music for 10 minutes and quickly stood up within three seconds and remained standing still for 15 minutes. HRV was recorded at the following time: rest, music (music protocol) 0–5, 5–10 and 10–15 min during standing. Results: In the control protocol the SDNN, RMSSD and pNN50 indexes were reduced at 10–15 minutes after the volunteers stood up, while the LF (nu) index was increased at the same moment compared to seated rest. In the protocol with music, the indexes were not different from control but the RMSSD, pNN50 and LF (nu) were different from the music period. Conclusion: Musical auditory stimulation attenuates the cardiac autonomic responses to the PCM.

Sepse é causa comum de mortalidade e morbidade em unidades de tratamento intensivo, sendo atualmente entendida como a resposta sistêmica à infecção, resposta esta caracterizada por duas ou mais das seguintes manifestações: temperatura corporal > 38°C ou < 36°C; taquicardia; taquipnéia; e alteração da contagem de células brancas (leucocitose ou leucopenia) ou mais de 10% de formas imaturas. Entretanto, tem sido descrito que os elementos que caracterizam a resposta sistêmica são muito sensíveis, insuficientemente específicos e não levam em consideração o avanço do entendimento da fisiopatologia da doença. Além disso, a sepse tem fisiopatologia complexa, o que dificulta ainda mais o diagnóstico, principalmente em crianças. O diagnóstico rápido e a instituição de tratamento o mais cedo possível são imperativos nos pacientes sépticos. Consequentemente, vários marcadores laboratoriais têm sido estudados, sendo que dos vários mediadores envolvidos na sepse, procalcitonina (PCT) e interleucina-6 (IL-6) têm despertado interesse pela possibilidade de auxiliar no diagnóstico e no estabelecimento da gravidade de pacientes adultos com sepse ou choque séptico, sendo que a PCT já foi incluída entre os critérios diagnósticos de sepse nestes indivíduos. Sildenafil attenuates acute pulmonary embolism-induced pulmonary hypertension. However, the hemodynamic effects of sildenafil in combination with other vasodilators during acute pulmonary embolism have not been examined yet. In the present study, we examined the hemodynamic effects of combined sildenafil (0.25 mg/kg, i.v.) and L-arginine (100, 200, 500, and 1000 mg/kg/h, i.v.) in an anesthetized dog model of acute pulmonary embolism. Plasma nitrite/nitrate (NOx) and cGMP concentrations were determined using an ozone-based chemiluminescence assay and a commercial enzyme immunoassay, respectively. We found that L-arginine alone did not attenuate acute pulmonary embolism-induced pulmonary hypertension. However, significant decreases in mean pulmonary artery pressure were observed 30, 45, 60, and 75 min after the administration of sildenafil alone or after the combined administration of sildenafil and L-arginine (all P<0.05). No significant differences among groups were observed in the respiratory parameters. While L-arginine significantly increased NOx concentrations, cGMP concentrations increased only when sildenafil was administered (all P<0.05). These results suggest that while sildenafil attenuates acute pulmonary embolism-induced pulmonary hypertension, L-arginine does not enhance the beneficial hemodynamic effects of sildenafil. In addition, these findings suggest that stimulation of NO synthesis with L-arginine during acute pulmonary embolism does not produce beneficial effects. (c) 2005 Elsevier B.V. All rights reserved.

In this paper, based on Maingueneau’s (2006, 2010) thoughts on aphorisation and on the differences between aphorising and textual enunciation, we analyze some detached utterances from the self-help discourse, by highlighting some of their meaning effects and their role in such discourse. The analysis shows that the aphorising detachments support the self-help discourse enunciator in the process of guiding the interlocutors. Such aphorizing detachments allow the enunciator to give order as if he or she were enunciating the truth, which attenuates the enunciator’s authoritative character, who is supposed to give order explicitly. Moreover, such utterances activate the play between “to say and not to say,” by simulating that the self-help discourse is based on an undeniable truth, which reinforces its persuasive power.
