Enquadramento: Apesar de o leite materno ser globalmente aceite como o alimento mais completo e efetivo para assegurar a saúde do bebé, além dos claros benefícios para a mãe, continua-se a verificar que a taxa de amamentação está ainda longe da pretendida. Objetivos: Identificar a evidência científica dos determinantes da interrupção do aleitamento materno aos 6 meses de vida do bebé. Métodos: Foi realizada uma revisão sistemática da literatura sobre as dificuldades que levam à interrupção do aleitamento materno aos 6 meses de vida do bebé. Efetuou-se uma pesquisa na PUBMED, The Cochrane Library, Scielo e Google Académic de estudos publicados entre janeiro de 2010 a outubro de 2015, partindo dos critérios de inclusão previamente definidos, os estudos selecionados foram posteriormente avaliados. Dois revisores avaliaram a qualidade dos estudos a incluir, utilizando a grelha para a avaliação crítica de um estudo. Após a avaliação crítica da qualidade, foram incluídos no corpus de estudo 4 artigos com scores entre 87.5% e 95.0%. Resultados: Como metasíntese e seleção da análise dos artigos, inferiu-se que os fatores que levam à interrupção do aleitamento materno aos 6 meses de vida do bebé são: as preocupações com a lactação, perda de peso do bebé, doença da mãe ou a necessidade de tomar medicação, bloqueio dos ductos mamários, problemas relacionados com a gestão psicossocial, conflitos no seu estilo de vida, o posicionamento e pega inadequados, queixa de leite insuficiente ou fraco, dor à amamentação, fissuras, ingurgitamento mamário, a ansiedade materna e o choro da criança. Conclusões: Perante a evidência científica, as causas de abandono do aleitamento materno são multifatoriais e estão associadas à mãe, ao bebé e à saúde. A promoção do aleitamento materno exige programas de educação baseados na evidência, contribuindo para melhoria dos índices nacionais de prevalência da amamentação até, pelo menos aos 6 meses de vida do bebé, que se deseja cada vez mais consentânea com as melhores práticas internacionais. Kidney transplantation has been recognised as the optimal treatment choice for most end stage renal disease patients and the increase of allograft survival rates is achieved through the refinement of novel immunosuppressive agents. Chronic Graft Disease (CGD) is a multifactorial process that likely includes a combination of immunological, apoptotic and inflammatory factors. The application of individualised immunosuppressive therapies will also depend on the identification of risk factors that can influence chronic disease. Despite being the subject of several independent studies, investigations of the relationship between transforming growth factor-b1 (TGF-b1) polymorphisms and kidney graft outcome continue to be plagued by contradictory conclusions.

Éste documento tiene como objetivo caracterizar los hogares rurales y urbanos del departamento del Cauca, Colombia y clasificarlos según la tipología y el ciclo de vida del hogar, para establecer su relación con los niveles de gasto mínimo en bienes básicos, es decir, con los que definen los umbrales críticos de pobreza. Para tal efecto, se utiliza la Encuesta de Calidad de Vida de 2011 y se estiman modelos de análisis de varianza multifactoriales. Los resultados muestran, entre otros, que el tipo de hogar más frecuente, tanto en el área rural como la urbana, es el nuclear con hijos y los gastos mínimos básicos, en general, son en alimentos, vivienda y salud. Los rubros de educación y transporte presentan niveles más altos de consumo en los hogares urbanos que en los rurales..

Preeclampsia is a multifactorial disease of unknown etiology that features with wide clinical symptoms, ranging from mild preeclampsia to severe forms, as eclampsia and HELLP syndrome. As a complex disease, preeclampsia is also influenced by genetic and environmental factors. Aiming to identify preeclampsia susceptibility genes, we genotyped a total of 22 genetic markers (single nucleotides polymorphisms SNPs) distributed in six candidates genes (ACVR2A, FLT1, ERAP1, ERAP2, LNPEP e CRHBP). By a case-control approach, the genotypic frequencies were compared between normotensive (control group) and preeclamptic women. The case s group was classified according to the disease clinical form in: preeclampsia, eclampsia and HELLP syndrome. As results we found the following genetic association: 1) ACVR2A and preeclampsia; 2) FLT1 and severe preeclampsia; 3) ERAP1 and eclampsia; 4) FLT1 and HELLP syndrome. When stratifying preeclampsia group according to symptoms severity (mild and severe preeclampsia) or according to the time of onset (early and late preeclampsia), it was detected that early preeclampsia is strongly associated to risk preeclampsia, eclampsia and HELLP syndrome have different genetic bases, although FLT1 gene seems to be involved in preeclampsia and HELLP syndrome pathophisiology

Introducción: En los países desarrollados la obesidad infantil se ha incrementado alarmantemente en los últimos años, convirtiéndose en un grave problema de salud pública. La obesidad es una enfermedad crónica de origen multifactorial que comienza durante la niñez y adolescencia. Se produce por un desequilibrio entre el consumo y el gasto energético. Como causas influyentes destacan: factores genéticos, biológicos, de comportamiento y culturales. Su tratamiento resulta muy complicado en esta etapa de crecimiento porque cualquier déficit nutricional puede repercutir negativamente en el desarrollo integral del niño. Objetivos: Clasificar a la población de estudio en función de su Indice de Masa Corporal (IMC), establecer la relación que existe entre obesidad infantil y el uso del servicio de comedor escolar y verificar que el menú del comedor escolar es adecuado en cuanto al contenido calórico y el porcentaje de principios inmediatos. Metodología: Se realizó un estudio observacional descriptivo transversal sobre obesidad en niños de 6 años. Para la recogida de datos, se utilizó una báscula para medir el peso y una cinta métrica para tallarlos, y con los datos de peso y estatura recogidos se calculó el IMC. Por otro lado, para poder comprobar el menú, el personal del comedor escolar nos facilitó un menú mensual. Resultados: Dentro de los porcentajes europeos, nuestra muestra reflejó niños con sobrepeso y obesidad y también un considerable porcentaje de niños con bajo peso. Conclusiones: Destacar el papel fundamental de la familia y cuidadores en los comedores escolares; fomentando hábitos saludables (ejercicio y dieta equilibrada) para poder prevenir patologías en la edad adulta secundarias a la obesidad, lo cual es una gran estrategia de seguridad para la salud pública y para la economía de los Sistemas Sanitarios.

For treating chronic pain, a multifactorial condition, is needed a suitable diagnosis which allows the differentiation in its many components. Diabetic neuropathy is a worldwide disease with great impact in the modern society. Diabetes may leads to the production of reactive oxygen species that are associated to oxidative stress, which may be a key factor in the development of diabetic neuropathy. The main goal is to inquire a potential association between chronic pain, diabetic neuropathy and oxidative stress. Thus, was performed a meta-analysis that permitted the causal evaluation between oxidative stress and diabetic neuropathy, and, a pain evaluation was accomplished in a convenience sample using validated surveys – Brief Pain Inventory (BPI) and Douleur Neuropathique 4 (DN4). Through the meta-analysis it was possible evaluate oxidative stress biomarkers, such lipid peroxidation, superoxide dismutase and catalase activities, and reduced glutathione. 9 studies were selected and all were performed in mouse models. The levels of lipid peroxidation were increased in all the studies, however the levels of the other biomarkers were diminished in diabetic models comparatively to healthy controls. In the evaluation of convenience sample, 84 surveys were collected along the four seasons: summer, autumn, winter and spring. The pain complaints were described in terms of local, intensity, impact, relief by medication and its effect on daily activities using BPI questionnaire. The scores obtained in BPI indicate the presence of moderate to severe pain, with increased complaints in autumn and spring, and implications in daily activities, transversal to all groups. To determine the main features associated with neuropathic pain it was used DN4 questionnaire. The DN4 indicated the presence of nearly 50% of patients with neuropathic pain. The results suggest that being female, the increased age and being retired can influence chronic pain and neuropathic pain in patients. As main conclusions, it was possible to verify an association between oxidative stress, and neuropathic pain, and, also, that weather conditions may influence the pain complaints and its prevalence.

Las retinas inmaduras de los recién nacidos prematuros son susceptibles a insultos que interrumpen el crecimiento neurovascular, que conduce a la retinopatía de la prematuridad. Por eso es de gran importancia conocer los factores de riesgo que se ven involucrados en la génesis de dicha enfermedad. Objetivos: conocer los factores de riesgos asociados a retinopatía del prematuro menor o igual a 1750 gramos al nacer que requirió tratamiento con láser en el Hospital de Niños Benjamín Bloom de Enero de 2012 a Diciembre 2013. Material y método: se realizó un estudio de tipo descriptivo, de corte retrospectivo en el cual se hará revisión sistemática de los expedientes de los pacientes sin dar a conocer la identidad de los mismos, seleccionando a pacientes menor o igual a 1750 gramos independientemente del sexo, evaluado en Hospital Nacional de Niños Benjamín Bloom y al que se le ha realizado intervención con láser; de los cuales se verificará los factores de riesgo asociados al desarrollo de retinopatía del prematuro en estadios severos, se realizó una hoja de datos en Excel y se utilizó el programa SPSS para la agrupación de los datos, posterior se realizó el análisis de los datos. Resultado: se encontraron 56 casos de prematuros con grado severo de retinopatía que utilizó laser, de los cuales fueron 30 masculinos y 26 femeninos; se excluyeron veinte casos por no cumplir criterios de inclusión. Los dos factores de riesgo principalmente involucrados en la ROP son edad gestacional igual o inferior a 30 semanas de gestación y peso al nacimiento igual o inferior a 1300 gr. Otros factores de riesgo encontrados son: la no ganancia de peso para la edad gestacional con un 92.7%, anemia y transfusión sanguínea con un 76.9%, sepsis temprana y nososcomial, en menor proporción apneas, aplicación de surfactante, administración de esteroides prenatales, el estadio con mayor frecuencia encontrado es el III, el déficit visual y ceguera son las secuelas encontradas en los pacientes con retinopatía severa. Conclusiones: la retinopatía de la prematuridad es una entidad multifactorial en la que cada uno de los factores juega un papel importante en la evolución de dicha patología.

La Falla Renal Aguda (FRA) es un síndrome multifactorial de etiología variada e incidencia que difiere según países y poblaciones de estudio. A nivel mundial no existe un consenso en cuanto a una definición universal. Últimamente se ha reconocido la importancia que tiene la reducción leve a moderada de la función renal en la morbimortalidad especialmente en los pacientes críticos. En 2004, la Acute Dialysis Quality Initiative, realizó un consenso para definir la falla renal aguda, a través del criterio de RIFLE (Riesgo, Injuria, Falla, Pérdida y Estado final de la falla), el cual utilizó los criterios de los adultos y lo extrapoló hacia los pacientes pediátricos. Objetivo: diagnosticar falla renal aguda de acuerdo con los criterios RIFLE en los pacientes de 1 a 5 años hospitalizados en la Unidad de Cuidados Intensivos Pediátricos del Hospital Bloom en el año 2013, identificación de la prevalencia y de las principales causas etiológicas en las cuales se presenta. Muestra: pacientes de 1 a 5 años hospitalizados en la Unidad de Cuidados Intensivos del Hospital Nacional de Niños Benjamín Bloom, los cuales no fueron sometidos a procedimientos quirúrgicos o presentaron insuficiencia renal crónica. Método: se realizó un estudio retrospectivo, descriptivo, desarrollado en la unidad de cuidados intensivos de dicho hospital, haciendo el diagnóstico de falla renal aguda mediante la aplicación los criterios RIFLE para demostrar la utilidad de ésta metodología en el diagnóstico temprano de FRA. Se analizaron los datos epidemiológicos, clínicos y analíticos de todos los pacientes con falla renal aguda. Consideraciones éticas: el presente estudio se evaluó y aprobó por el comité de ética del HNNBB considerándolo adecuado y de bajo riesgo para los pacientes. Resultados: hubo 27 episodios de FRA en 43 pacientes con edad de 1 a 5 años lo cual corresponde al 62.7% de los casos estudiados, el 52% sexo femenino. La patología primaria más frecuente fueron las patologías de origen infeccioso, más común en las neumonías; la media de edad de presentación son los 26 meses. Mediante la aplicación de los criterios RIFLE se permitió diagnosticar de forma más temprana la FRA.

The aetiology of autoimmunes disease is multifactorial and involves interactions among environmental, hormonal and genetic factors. Many different genes may contribute to autoimmunes disease susceptibility. The major histocompatibility complex (MHC) genes have been extensively studied, however many non-polymorphic MHC genes have also been reported to contribute to autoimmune diseases susceptibility. The aim of the present study was to evaluate the influence of SLC11A1 gene in systemic lupus erythematosus (SLE) and rheumatoid arthritis (RA). Ninety-six patients with SLE, 37 with RA and 202 controls enrolled in this case-control study, were evaluated with regard to demographic, genetic, laboratorial and clinical data. SLE mainly affects females in the ratio of 18 women for each man, 88,3% of the patients aged from 15 to 45 years old and it occurs with similar frequency in whites and mulattos. The rate of RA between women and men was 11:1, with 77,1% of the cases occurring from 31 to 60 years. The genetic analysis of the point mutation -236 of the SLC11A1 gene by SSCP did not show significant differences between alleles/genotypes in patients with SLE or RA when compared to controls. The most frequent clinical manifestations in patients with SLE were cutaneous (87%) and joint (84.9%). In patients with RA, the most frequent out-joint clinical manifestation were rheumatoid nodules (13,5%). Antinuclear antibodies were present in 100% of the patients with SLE. There was no significant relation between activity of disease and presence of rheumatoid factor in patients with RA, however 55,6% of patients with active disease presented positive rheumatoid factor. Significant association between alleles/genotypes of point mutation -236 and clinical manifestations was not found

El bajo peso al nacer definido por la Academia Americana de Pediatría como un peso igual a 2 500 g o menor, independientemente de la edad gestacional, se ha convertido en una entidad en ascenso en países en desarrollo y factor de riesgo para morbi-mortalidad. La proporción de niños con peso bajo al nacer que son prematuros en comparación con los que tuvieron un crecimiento intrauterino anormal, varía en el mundo, en países desarrollados, casi todos los pacientes con bajo peso al nacer son prematuros, mientras que en naciones en desarrollo los principales contribuyentes a la tasa de peso bajo al nacer son niños a término con un crecimiento menor. De etiología multifactorial incluidos factores propios del embarazo como trastornos hipertensivos del embarazo, trastornos metabólicos derivados del embarazo como la diabetes gestacional y los embarazos múltiples; y otros que no están en relación directa pero que siendo patologías crónicas preexistentes comprometen el flujo sanguíneo placentario y pueden restringir así el crecimiento intrauterino. El presente estudio es un estudio descriptivo que persigue como objetivo tener un panorama del perfil epidemiológico que presentan los recién nacidos con bajo peso al nacer, que nacen en el Hospital Especializado de Maternidad “Dr. Raúl Argüello Escolán” se revisó 130 expedientes de recién nacidos con peso al nacer entre 2,000 grs. y 2,499 grs. encontrando que las madres de estos recién nacidos, se encuentran principalmente entre las edades de 14-19 años en el 29% de los recién nacidos, en su primer embarazo en el 56.9%, en cuanto a sus controles prenatales el 99.2% llevo controles prenatales, captadas en el 69% antes de las 20 semanas de gestación y teniendo más del 60% de los casos más de 4 controles prenatales, observando que el 23% de las madres presentan condiciones clínicas preexistentes en el embarazo en las que destacan la hipertensión arterial y los problemas infecciosos en las vías urinarias, en tanto los problemas relacionados al embarazo los trastornos hipertensivos siguen a la cabeza compartiendo lugar con el embarazo múltiple y la ruptura prematura de membranas.

Las cojeras son una de las mayores causas de problemas en el bienestar animal de las vacas lecheras, y tienen diferentes orígenes: infecciosas (dermatitis digital, flemón digital); o lesiones causadas por discontinuidad en el crecimiento del cuerno de las pezuñas (hemorragias, úlcera podal, enfermedad de la línea blanca). Los factores de riesgo que inducen estas afecciones son multifactoriales y no siempre son detectables, difieren según el establecimiento, así como su prevalencia / incidencia. La prevención y la detección precoz mantiene estas patología tengan un impacto menor sobre las economía en la producción lechera.

Burnout is a psychological syndrome triggered in response to continuous exposure to interpersonal stressors. It is considered a multifactorial construct, which is commonly characterized by three dimensions: emotional exhaustion, dehumanization, and lack of personal accomplishment.This study aimed to verify if the three characteristics of burnout (exhaustion, lack of dehumanization and personal accomplishment) are present in people working as guides Tourism in Natal - RN. It is a descriptive and quantitative study. 109 subjects were surveyed. Data collection was done through the use of questionnaires, the instrument used was the characterization of the Burnout Scale (ECB) created and validated in Brazil by Trocoli and Tamayo (2000). In order to analyze data we used descriptive statistics, analysis of core measures, exploratory and confirmatory factor analysis, reliability analysis, cluster analysis, multiple discriminant and Spearman correlation. Factor analysis identified four factors that explain 58.3% of the total variance. Those factors were named exhaustion, deception, avoidance, and dehumanization. The reliability of the instrument, as measured by Cronbach's Alpha was 0.918, which is considered excellent reliability. The 109 subjects were grouped into three cluster, which had the deception, avoidance, and dehumanization as discriminant. It is possible to conclude that the characteristics of burnout syndrome are present in the studied population where 19 people are on the high level of burnout, moderate in 32 and 56 in the light. The correlations between socio-demographic variables studied and the dimensions of burnout, were few and weak. The variable leave for health reasons in the study appeared to be related to feelings of exhaustion and avoidance behavior appeared related to younger individuals and who work only in the activity of Receptive Tourism Guide. Verification of the incidence of burnout in individuals surveyed suggest the need to adopt intervention strategies are individual, organizational and / or combined

Background: Malnutrition in surgical patients is associated with delayed recovery, higher rates of morbidity and mortality, prolonged hospital stay, increased healthcare costs and a higher early re-admission rate. Methods: Data synthesis after review of pertinent literature. Results: The aetiology of malnutrition is multifactorial. In cancer patients, there is an abnormal peripheral glucose disposal, gluconeogenesis, and whole-body glucose turnover. Malnourished cancer patients undergoing major operations are at significant risk from perioperative complications such as infectious complications. Surgical aggression generates an inflammatory response which worsens intermediary metabolism. Conclusions: Nutritional evaluation and nutritional support must be performed in all surgical patients, in order to minimize infectious complications. Enteral nutrition early in the postoperative period is effective and well tolerated reducing infectious complications, improving wound healing and reducing length of hospital stay. Pharmaconutrition is indicated in those patients, who benefit from enteral administration of arginine, omega 3 and RNA, as well as parenteral glutamine supplementation. When proximal sutures are used, tubes allowing early jejunal feeding should be used.

Background The prevalence of geriatric syndromes (falls, immobility, intellectual or memory impairment, and incontinence) is unknown in many resource-poor countries. With an aging population such knowledge is essential to develop national policies on the health and social needs of older people. The aim of this study was to provide a preliminary survey to explore the prevalence of falls and other geriatric syndromes and their association with known risk factors in people aged > 60 years in urban Blantyre, Malawi. Methods This was a cross-sectional, community survey of adults aged > 60 years. Subjects were recruited at home or in the waiting areas of chronic care clinics. They were interviewed to complete a questionnaire on ageassociated syndromes and comorbid problems. The Abbreviated Mental Test (AMT) and Timed Up and Go (TUG) tests were carried out. Results Ninety-eight subjects were studied; 41% reported falling in the past 12 months, 33% of whom (13% of all subjects) were recurrent fallers. Twenty-five percent reported urine incontinence, 66% self-reported memory difficulties, and 11% had an AMT score < 7. A history of falling was significantly associated with urine incontinence (p=0.01), selfreported memory problems (p=0.004) and AMT score < 7 (p=0.02). Conclusions Geriatric syndromes, including falls, appear to be prevalent in older people in Blantyre, Malawi. Falling is associated with cognitive impairment and urinary incontinence. There is an urgent need for more understanding of geriatric problems in this setting to develop national policies on health and social needs of older people. It is likely that many of the contributory factors to falls would be amenable to multifactorial interventions similar to those found to be effective in developed countries.

Background Anaemia is an important complication of trypanosomiasis. The mechanisms through which trypanosomal infection leads to anaemia are poorly defined. A number of studies have implicated inflammatory cytokines, but these data are limited and inconsistent. In this article, we reviewed the published literature on cytokines associated with Trypanosoma brucei infections and their role in the immunopathology leading to anaemia. Methodology Articles were searched in PubMed through screening of titles and abstracts with no limitation on date of publishing and study design. Articles in English were searched using keywords “African trypanosomiasis”, “sleeping sickness”, “Trypanosoma brucei”, in all possible combinations with “anaemia” and/or “cytokines”. Results Twelve articles examining cytokines and their role in trypanosomeinduced anaemia were identified out of 1095 originally retrieved from PubMed. None of the articles identified were from human-based studies. A total of eight cytokines were implicated, with four cytokines (IFN-γ, IL-10, TNF-α, IL-12) showing an association with anaemia. These articles reported that mice lacking TNF-α were able to control anaemia, and that IFN-γ was linked to severe anaemia given its capacity to suppress erythropoiesis, while IL-10 was shown to regulate IFN-γ and TNF-α, providing a balance that was associated with severity of anaemia. IFN-γ and TNF-α have also been reported to work in concert with other factors such as nitric oxide and iron in order to induce anaemia. Conclusion IFN-γ, IL-10, and TNF-α were the three major cytokines identified to be heavily involved in anaemia caused by Trypanosoma brucei infection. The anti-inflammatory cytokine, IL-10, was shown to counter the effects of proinflammatory cytokines in order to balance the severity of anaemia. The mechanism of anaemia is multifactorial and therefore requires further, more elaborate research. Data from human subjects would also shed more light.
