Este trabalho foi realizado para verificar se a ultra-sonografia do pâncreas oferece dados auxiliares na classificação de diabéticos adultos dos tipos 1 e 2. O tamanho e a ecogenicidade do pâncreas foram determinados pela ultra-sonografia em 81 diabéticos, sendo 20 do tipo 1 e 61 do tipo 2 (53 obesos e oito não-obesos). Os pacientes tipo 2 obesos diferiram dos demais por apresentarem área total e diâmetro ântero-posterior do corpo do pâncreas significativamente maiores. Quanto à ecogenicidade pancreática, esta estava aumentada com maior freqüência nos diabéticos tipo 2 obesos que nos diabéticos tipo 1. Consideramos, assim, que a ultra-sonografia do pâncreas constitui metodologia auxiliar na classificação de diabéticos entre os tipo 1 e 2, sendo menos eficaz quando os últimos não são obesos.

OBJETIVOS: Observar se existem diferenças nos valores monetários destinados aos procedimentos de saúde para o tratamento de pacientes diabéticos tipo 2 quando estratificados em diferentes níveis de atividade física habitual. SUJEITOS E MÉTODOS: Cento e vinte um diabéticos tipo 2 foram avaliados em duas unidades básicas de saúde de Bauru, SP. Atividade física foi avaliada por meio de entrevista. Retroagindo um ano ao dia da avaliação, por meio de notas fiscais, foram computados valores de exames, medicamentos e consultas médicas e de enfermagem. RESULTADOS: Quando comparados aos diabéticos ativos, os sedentários apresentaram gastos com consultas em clínico-geral 63% superiores (p = 0,017). Gastos com medicamentos para o tratamento de outras doenças também foram superiores em diabéticos sedentários (p = 0,001). CONCLUSÕES: Quando comparados de acordo com a prática de atividades físicas, diabéticos tipo 2 com menor prática de atividades físicas apresentam maiores custos com serviços médicos e consumo de medicamentos.

This study aimed to evaluate whether maternal obesity leads to the onset of diabetes in adult Wistar rats offspring. MSG solution neonatally administration induced obesity in rats (F(1)MSG group, n = 30); and saline solution was also administrated to control rats (F1CON group, n = 13). In 3rd month of age, both control and MS G groups were mated for offspring (generation FA named as F2CON, n = 28 and F(2)MSG groups, n = 15; and so both generations were studied until 7th month of life. Lee Index was measured for experimental obesity validation from 5th to 7th month. Glycemia was weekly determined during pregnancy and monthly from 3rd to 7th month. In the end of experimental period all rats were submitted to oral glucose tolerance test (OGTT), with estimation of total area under the curve (AUC); and insulin tolerance test (ITT). Rats were then anesthetized and killed. Data were statistically analyzed with significance level of p < 0.05. Lee Index has confirmed obesity in all MSG rats. Glycemic levels comparisons between generations showed significant maternal interference in control and MSG groups. OGTT analysis showed higher glycemia in obese rats (F(1)MSG) and their offspring (F(2)MSG) as compared to their respective controls; and MSG groups increased AUC from OGTT. As regards ITT, F(2)MSG showed higher glycemia at 30 and 120 min, suggesting a delay of insulin action decreasing. Although glucose intolerance and insulin resistance clinical conditions represent as a factors for type 2 Diabetes mellitus development, this experimental model proposal was not efficient to induce type 2 Diabetes mellitus, but for obesity developing, glucose intolerance and insulin resistance in successive generations of rats. Lifestyle is directly related to the incidence of type 2 diabetes mellitus (DM-2), a risk dramatically elevated by obesity and inactivity. Several studies have verified that educational interventions can delay the onset of DM-2. Some of the interventions strategies utilized medication and diet, diet and/or physical exercise or the combination of diet and exercise, generally referred to a change in lifestyle. Despite the evidence that DM-2 can be preventive, there is still limited availability of effective prevention programs. DM-2 is considered an emerging public health problem as it is estimated that by the year of 2030 there will be about 366 million people with diabetes worldwide. DM2 remains a leading cause of cardiovascular disorders and many other complications. Our intent with this paper is to present researches and strategies (diet and physical activity interventions) that successfully improved plasma glucose control as a result of an effective lifestyle intervention program.

Using gene-network landscape to dissect genotype effects of TCF7L2 genetic variant on diabetes and cardiovascular risk. Physiol Genomics 44: 903-914, 2012. First published August 7, 2012; doi:10.1152/physiolgenomics.00030.2012.-The single nucleotide polymorphism (SNP) within the TCF7L2 gene, rs7903146, is, to date, the most significant genetic marker associated with Type 2 diabetes mellitus (T2DM) risk. Nonetheless, its functional role in disease pathology is poorly understood. The aim of the present study was to investigate, in vascular smooth muscle cells from 92 patients undergoing aortocoronary bypass surgery, the contribution of this SNP in T2DM using expression levels and expression correlation comparison approaches, which were visually represented as gene interaction networks. Initially, the expression levels of 41 genes (seven TCF7L2 splice forms and 40 other T2DM relevant genes) were compared between rs7903146 wild-type (CC) and T2DM-risk (CT + TT) genotype groups. Next, we compared the expression correlation patterns of these 41 genes between groups to observe if the relationships between genes were different. Five TCF7L2 splice forms and nine genes showed significant expression differences between groups. RXR alpha gene was pinpointed as showing the most different expression correlation pattern with other genes. Therefore, T2DM risk alleles appear to be influencing TCF7L2 splice form's expression in vascular smooth muscle cells, and RXR alpha gene is pointed out as a treatment target candidate for risk reduction in individuals with high risk of developing T2DM, especially individuals harboring TCF7L2 risk genotypes.

O objetivo deste estudo foi investigar a percepção de pacientes com diabetes mellitus tipo 1 acerca do transplante de células-tronco hematopoéticas (TCTH). Participaram do estudo 12 pacientes, com idades entre 16 e 24 anos. Foi aplicado um roteiro de entrevista semiestruturada antes e um ano após o TCTH. Os relatos foram submetidos à análise de conteúdo temática e agrupados em três categorias: impacto do adoecimento, vivência do TCTH e retomada do cotidiano. Os resultados evidenciaram que os participantes foram capazes de identificar ganhos e refletir sobre as perdas advindas dessa situação-limite. Puderam perceber possibilidades de se beneficiarem do TCTH e vislumbraram no transplante uma oportunidade para além das inevitáveis dificuldades e limitações impostas pela terapêutica.

Die vorliegende klinische Studie hatte zum Ziel, die mikrovaskuläre Endothelfunktion retinaler und dermaler Gefäße von Insulinresistenten und Typ 2- Diabetikern ohne Zeichen einer diabetischen Retinopathie mit einer gesunden insulinsensitiven nicht-diabetischen Kontrollgruppe hinsichtlich früher morphologischer und funktioneller Veränderungen zu vergleichen.rnrnMethode:rnEs wurden 54 Patienten ohne Nachweis einer diabetischen Retinopathie eingeschlossen und in 3 Gruppen entsprechend ihren metabolischen Ergebnissen eingeteilt: 1.) Gruppe K (Kontrollgruppe) setzte sich aus gesunden, nicht-diabetischen, insulin-sensitiven rn(HOMA ≤ 2) Probanden mit einem BMI ≤ 28 kg/m2 zusammen; 2.) Gruppe IR bestand aus den nicht-diabetischen, insulin-resistenten (HOMAs > 2), übergewichtigen Patienten mit einem BMI > 28 kg/m2 und 3.) Gruppe DM war definiert als Patienten mit einem manifesten Typ 2-Diabetes mellitus.rnrnDie mikrovaskuläre Funktion der Retina wurde mittels eines Laserdoppler-Verfahrens (Heidelberg Retina Flowmeter) untersucht und hierbei der retinale Blutfluss und das Verhältnis der Gefäßwand zum Lumen (WLR, wall-to-lumen-ratio) basal und nach Flickerlicht-Stimulation (10 Hz, Photo Stimulator 750) gemessen. Letzterer gilt als Marker für vaskuläre Schädigung. rnZusätzlich wurde die dermale Mikrozirkulation (Blutfluss, O2-Sättigung) als weiterer Faktor der mikrovaskulären Endothelfunktion in den 3 Studiengruppen untersucht und miteinander verglichen.rnErgebnisse:rnEs zeigte sich kein signifikanter Unterschied des retinalen Blutflusses zwischen den 3 Gruppen weder basal noch nach Flickerlicht-Stimulation. Es zeigte sich keine Korrelation zwischen der mikrovaskulären Funktion der Haut und der Retina. rnDie arterielle WLR zeigte nur geringe Unterscheide zwischen den 3 Gruppen.rnrnMit zunehmendem Grad der Insulinresistenz wurde jedoch eine Reduktion des basalen als auch des flickerlicht-stimulierten retinalen Blutflusses deutlich, dabei zeigte sich unerwarteter Weise eine Abnahme der WLR.rnrnDer (prä-ischämische) muskuläre Blutfluss war in der IR-Gruppe signifikant geringer als in der K-Gruppe. Auch war die postischämische dermale O2-Sättigung in der DM und IR-Gruppe signifikant niedriger im Vergleich zur K-Gruppe. Jedoch war die postischämische hyperämische dermale Reaktion in der IR und DM-Gruppe nur geringgradig weniger als in der K-Gruppe. rnrnSchlussfolgerung:rnEine Korrelation zwischen der Entwicklung der Insulinresistenz und retinaler sowie dermaler mikrovaskulärer endothelialer Funktion wurde bei der Studie deutlich. Mithilfe des neuen Verfahrens der Laser Scanner Flowmeter zur Messung der retinalen Endothelfunktion lassen sich sehr frühe morphologische Veränderungen des mikrovaskulären Blutflusses erfassen. rnDie fehlende Korrelation zwischen retinaler und dermaler mikrovaskulärer Funktion als auch die geringen Unterschiede der WLR sollte Gegenstand weiterer Studien seinrn

Type 1 diabetes mellitus is a chronic disease characterized by blood glucose levels out of normal range due to inability of insulin production. This dysfunction leads to many short- and long-term complications. In this paper, a system for tele-monitoring and tele-management of Type 1 diabetes patients is proposed, aiming at reducing the risk of diabetes complications and improving quality of life. The system integrates Wireless Personal Area Networks (WPAN), mobile infrastructure, and Internet technology along with commercially available and novel glucose measurement devices, advanced modeling techniques, and tools for the intelligent processing of the available diabetes patients information. The integration of the above technologies enables intensive monitoring of blood glucose levels, treatment optimisation, continuous medical care, and improvement of quality of life for Type 1 diabetes patients, without restrictions in everyday life activities.

OBJECTIVE: Bariatric surgery reverses obesity-related comorbidities, including type 2 diabetes mellitus. Several studies have already described differences in anthropometrics and body composition in patients undergoing Roux-en-Y gastric bypass compared with laparoscopic adjustable gastric banding, but the role of adipokines in the outcomes after the different types of surgery is not known. Differences in weight loss and reversal of insulin resistance exist between the 2 groups and correlate with changes in adipokines. METHODS: Fifteen severely obese women (mean body mass index [BMI]: 46.7 kg/m(2)) underwent 2 types of laparoscopic weight loss surgery (Roux-en-Y gastric bypass=10, adjustable gastric banding=5). Weight, waist and hip circumference, body composition, plasma metabolic markers, and lipids were measured at set intervals during a 24-month period after surgery. RESULTS: At 24 months, patients who underwent Roux-en-Y were overweight (BMI 29.7 kg/m(2)), whereas patients who underwent gastric banding remained obese (BMI 36.3 kg/m(2)). Patients who underwent Roux-en-Y lost significantly more fat mass than patients who underwent gastric banding (mean difference 16.8 kg, P<.05). Likewise, leptin levels were lower in the patients who underwent Roux-en-Y (P=.003), and levels correlated with weight loss, loss of fat mass, insulin levels, and Homeostasis Model of Assessment 2. Adiponectin correlated with insulin levels and Homeostasis Model of Assessment 2 (r=-0.653, P=.04 and r=-0.674, P=.032, respectively) in the patients who underwent Roux-en-Y at 24 months. CONCLUSION: After 2 years, weight loss and normalization of metabolic parameters were less pronounced in patients who underwent gastric banding compared with patients who underwent Roux-en-Y gastric bypass. Our findings require confirmation in a prospective randomized trial.
