A retrospective review of ultrasound guided breast excisional biopsies performed in a Surgical Unit of Princess Alexandra Hospital in 1998-99 was undertaken to assess the use of ultrasound specimen radiography. In this series a total of 55 localization biopsies were performed for impalpable lesions in 53 women. In 21 patients (38%), specimen ultrasound was used to confirm that the lesion in question had been excised, whereas for 34 lesions (62%), specimen X-ray was undertaken. In a total of six cases (10.9% overall) the radiographic abnormality was seen on ultrasound only. Real-time specimen sonography is a technique which is very appropriately utilized in conjunction with ultrasound guided excisions and can be easily performed either in the radiology department or the operative suite With minimal time delay. It could have particular application for lesions that are detected in younger women with dense breast parenchyma. The results of this review confirm specimen sonography to be a reliable technique. We compare Bayesian methodology utilizing free-ware BUGS (Bayesian Inference Using Gibbs Sampling) with the traditional structural equation modelling approach based on another free-ware package, Mx. Dichotomous and ordinal (three category) twin data were simulated according to different additive genetic and common environment models for phenotypic variation. Practical issues are discussed in using Gibbs sampling as implemented by BUGS to fit subject-specific Bayesian generalized linear models, where the components of variation may be estimated directly. The simulation study (based on 2000 twin pairs) indicated that there is a consistent advantage in using the Bayesian method to detect a correct model under certain specifications of additive genetics and common environmental effects. For binary data, both methods had difficulty in detecting the correct model when the additive genetic effect was low (between 10 and 20%) or of moderate range (between 20 and 40%). Furthermore, neither method could adequately detect a correct model that included a modest common environmental effect (20%) even when the additive genetic effect was large (50%). Power was significantly improved with ordinal data for most scenarios, except for the case of low heritability under a true ACE model. We illustrate and compare both methods using data from 1239 twin pairs over the age of 50 years, who were registered with the Australian National Health and Medical Research Council Twin Registry (ATR) and presented symptoms associated with osteoarthritis occurring in joints of the hand.

Objective. To provide recommendations for the core outcome domains that should be considered by investigators conducting clinical trials of the efficacy and effectiveness of treatments for chronic pain. Development of a core set of outcome domains would facilitate comparison and pooling of data, encourage more complete reporting of outcomes, simplify the preparation and review of research proposals and manuscripts, and allow clinicians to make informed decisions regarding the risks and benefits of treatment. Methods. Under the auspices of the Initiative on Methods, Measurement, and Pain Assessment in Clinical Trials (IMMPACT), 27 specialists from academia. governmental agencies, and the pharmaceutical industry participated in a consensus meeting and identified core outcome domains that should be considered in clinical trials of treatments for chronic pain. Conclusions. There was a consensus that chronic pain clinical trials should assess outcomes representing six core domains: (1) pain, (2) physical functioning, (3) emotional functioning, (4) participant ratings of improvement and satisfaction with treatment, (5) symptoms and adverse events, (6) participant disposition (e.g. adherence to the treatment regimen and reasons for premature withdrawal from the trial). Although consideration should be given to the assessment of each of these domains, there may be exceptions to the general recommendation to include all of these domains in chronic pain trials. When this occurs, the rationale for not including domains should be provided. It is not the intention of these recommendations that assessment of the core domains should be considered a requirement for approval of product applications by regulatory agencies or that a treatment must demonstrate statistically significant effects for all of the relevant core domains to establish evidence of its efficacy. Background: The heavy usage of coxibs in Australia far outstrips the predicted usage that was based on the treatment of patients with risk factors for upper gastro-intestinal adverse events from conventional anti--inflammatory agents. This raises questions regarding the appropriateness of prescribing. Aims: To determine: (i) the relationship between prescriptions for cyclooxygenase 2 (COX-2) inhibitors and objective evidence of inflammatory arthritis, (ii) prior experience with paracetamol and/or conventional non-steroidal anti-inflammatory drugs (NSAIDs), and (iii) contraindications to the use of NSAIDs. Methods: Drug utilization evaluation and rheumato-logical assessment was conducted on 70 consecutive patients admitted on COX-2 inhibitors to a 480-bed metropolitan hospital. The main outcome measures were: the indication for COX-2 inhibitor; objective -evidence of inflammatory arthritis; previous trial of -paracetamol or conventional NSAIDs; and patient -satisfaction. Results: Only 11 patients (16%) had symptoms or signs of an inflammatory arthropathy, and met Pharmaceut-ical Benefits Schedule criteria for prescribing a COX-2 inhibitor. Fifty-nine patients (84%) had chronic osteo-arthritis, degenerative spinal disease, injury or malignancy, without overt active inflammation. Fourteen patients (20%) had trialled regular paracetamol prior to using any NSAID treatment. Conventional NSAIDs had been previously used by 51 patients (73%). Eleven patients (16%) reported previous adverse gastrointestinal effects from conventional NSAIDs. On the basis of significant renal impairment (creatinine clearance 5/10). Conclusions: Drug utilization data indicate that COX-2 inhibitors are frequently used first line for degenerative osteoarthritis in the absence of overt inflammation, without prior adequate trial of paracetamol and with disregard for the cautions and contraindications of these agents. These findings may explain the unprecedented Pharmaceutical Benefits Schedule expenditure on COX-2 inhibitors in Australia.

Digital X-ray detector technologies provide several advantages when compared with screen-film (SF) systems: better diagnostic quality of the radiographic image, increased dose efficiency, better dynamic range and possible reduction of radiation exposure to the patient. The transition from traditional SF systems to digital technology-based systems highlights the importance of the discussion around technical factors such as image acquisition, themanagement of patient dose and diagnostic image quality. Radiographers should be aware of these aspects concerning their clinical practice regarding the advantages and limitations of digital detectors. Newdigital technologies require an up-to-date of scientific knowledge concerning their use in projection radiography. This is the second of a two-part review article focused on a technical overview of digital radiography detectors. This article provides a discussion about the issues related to the image acquisition requirements and advantages of digital technologies, the management of patient dose and the diagnostic image quality.

The exposure index (lgM) obtained from a radiographic image may be a useful feedback indicator to the radiographer about the appropriate exposure level in routine clinical practice. This study aims to evaluate lgM in orthopaedic radiography performed in the standard clinical environment. We analysed the lgM of 267 exposures performed with an AGFA CR system. The mean value of lgM in our sample is 2.14. A significant difference (P=0.000<0.05) from 1.96 lgM reference is shown. Data show that 72% of exposures are above the 1.96 lgM and 42% are above the limit of 2.26. Median values of lgM are above 1.96 and below 2.26 for Speed class (SC) 200 (2.16) and SC400 (2.13). The interquartile range is lower in SC400 than in SC200. Data seem to indicate that lgM values are above the manufacturer’s reference of 1.96. Departmental exposure charts should be optimised to reduce the dose given to patients.

Typical distribution of exposure parameters in plain radiography is unknown in Portugal. This study aims to identify exposure parameters that are being used in plain radiography in the Lisbon area and to compare the collected data with European references [Commission of European Communities (CEC) guidelines]. The results show that in four examinations (skull, chest, lumbar spine and pelvis), there is a strong tendency of using exposure times above the European recommendation. The X-ray tube potential values (in kV) are below the recommended values from CEC guidelines. This study shows that at a local level (Lisbon region), radiographic practice does not comply with CEC guidelines concerning exposure techniques. Further national/local studies are recommended with the objective to improve exposure optimisation and technical procedures in plain radiography. This study also suggests the need to establish national/local diagnostic reference levels and to proceed to effective measurements for exposure optimisation.

Em Portugal não são conhecidos estudos publicados que identifiquem com clareza as distribuições típicas dos valores associados aos parâmetros técnicos de exposição utilizados nos exames radiológicos mais comuns. Este estudo tem como objectivos identificar os parâmetros técnicos utilizados em exames radiológicos convencionais na região de Lisboa e comparar os dados com a referência europeia CEC. Os resultados obtidos evidenciam que nas projecções estudadas existe uma predominância para o uso de termos de exposição acima da recomendação CEC e de valores de potencial da ampola (kV) abaixo da recomendação. Esta investigação sugere a necessidade de fixar os NRD nacionais, e/ou locais, e efectuar as respectivas medições, dado que a optimização da exposição é um mecanismo de controlo fundamental para limitar as exposições desnecessárias dos pacientes às radiações ionizantes. Conclui-se que a nível local (região da Grande Lisboa) existe um desconhecimento da prática radiológica enquadrada nos referenciais europeus de boa prática radiológica. Desconhecendo-se a situação a nível nacional, sugerem-se estudos no sentido de identificar os padrões de prática radiológica a nível do País e de promover a optimização dos procedimentos em radiologia convencional. Once in a digital form, a radiographic image may be processed in several ways in order to turn the visualization an act of improved diagnostic value. Practitioners should be aware that, depending on each clinical context, digital image processing techniques are available to help to unveil visual information that is, in fact, carried by the bare digital radiograph and may be otherwise neglected. The range of visual enhancement procedures includes simple techniques that deal with the usual brightness and contrast manipulation up to much more elaborate multi-scale processing that provides customized control over the emphasis given to the relevant finer anatomical details. This chapter is intended to give the reader a practical understanding of image enhancement techniques that might be helpful to improve the visual quality of the digital radiographs and thus to contribute to a more reliable and assertive reporting.

Considering that ionizing radiation effects are cumulative and the gonads are particularly sensitive to these effects, and also the clinical importance of pelvic radiographs in children, the excess of radiation exposure to the gonads must be avoided. The purpose of this study is to demonstrate the relevance of the correct use of gonad protection shields and to evaluate their use on the hip radiographs performed in a reference clinical institution, through the retrospective analysis of pelvic radiographic images performed in children. According the image quality assessment, 20 (40%) patients were unprotected and gonads shields were incorrectly placed in 24 (80%) patients.

A reabilitação do joelho utilizando exercícios terapêuticos em meio aquático tem sido utilizada com o intuito de iniciar, de forma imediata os movimentos articulares, pois reduz a dor e permite a carga de peso mais precoce. Apesar destas vantagens serem reconhecidas, poucos estudos têm sido realizados no sentido de determinar a efectividade dos exercícios terapêuticos em meio aquático na reabilitação pós-cirúrgica do ligamento cruzado anterior (LCA) Objectivo: Descrever os efeitos de um programa de exercícios terapêuticos em meio aquático num caso clínico de ligamentoplastia do cruzado anterior. Metodologia: Um jogador de futebol de 27 anos de idade, sujeito a uma ligamentoplastia do LCA, completou um programa de exercícios terapêuticos em meio aquático que constou de um total de 24 sessões (4 por semana, e 60 minutos por sessão). O atleta foi avaliado em três momentos distintos (T0, no início da intervenção, T1, três semanas após o início da intervenção e T2, seis semanas após o início da intervenção. Como parâmetros de avaliação foram utilizadas as amplitudes articulares (goniometria), o teste muscular manual, o volume muscular (perimetria), o equilíbrio estável (Y Balance Test (YBT)) e a funcionalidade (Knee Injury and Osteoarthritis Outcome Score (KOOS)). Resultados: Após as 24 sessões verificou-se uma melhoria nos diversos parâmetros avaliados (flexão do joelho: 75º vs 134º; extensão do joelho: -7º vs 0º; força dos flexores e extensores do joelho: 3 vs 4+; perimetria da coxa: 46,2 cm vs 51,5 cm; YBT: equilíbrio anterior (resultados correspondentes aos momentos T1 e T2): 53 cm vs 72 cm; posterolateral: 46 cm vs 61 cm; posteromedial: 39 cm vs 57 cm; KOOS: dor/sintomas: 17 vs 92, outros sintomas: 25 vs 75, actividades da vida diária: 12 vs 90, actividades desportivas e de lazer: 0 vs 55, qualidade de vida relacionada com o joelho: 0 vs 50. Conclusão: O programa de exercícios terapêuticos em meio aquático utilizado neste estudo parece ter efeitos benéficos num caso clínico de ligamentoplastia do cruzado anterior.

In this paper, we present the results of mammography quality control tests related to the work with Portuguese mammography equipment, either in conventional or in digital mammography computed radiography, showing the main differences in the tested equipments. Quality control in mammography is a very special area of quality control in radiology, which demands relatively high knowledge on physics. Digital imaging is changing the standards of the radiographic imaging. Regarding mammography, this is yet a controversial issue owing to some limitations of the digital detectors, like the resolution for instance. A complete set of results regarding radiation protection of the patients submitted to mammography diagnosis is presented. A discussion of the quality image parameters and its interpretation in conventional and digital mammography is presented. In conclusion, we present a sample of results that can be considered as characteristics of mammography equipment in Portugal.
