The significance of the gut microbiota as a determinant of drug pharmacokinetics and accordingly therapeutic response is of increasing importance with the advent of modern medicines characterised by low solubility and/or permeability, or modified-release. These physicochemical properties and release kinetics prolong drug residence times within the gastrointestinal tract, wherein biotransformation by commensal microbes can occur. As the evidence base in support of this supplementary metabolic “organ” expands, novel opportunities to engineer the microbiota for clinical benefit have emerged. This review provides an overview of microbe-mediated alteration of drug pharmacokinetics, with particular emphasis on studies demonstrating proof of concept in vivo. Additionally, recent advances in modulating the microbiota to improve clinical response to therapeutics are explored.

Ce projet de recherche en sociologie porte sur les aspirations des jeunes atikamekws d’Opitciwan dans le Haut-Saint-Maurice et leur attitude face à l’école et l’emploi. Dans un premier temps, nous dresserons un bref portrait de la situation socio-économique, historique et politique de la communauté. Ensuite, nous examinerons les données de l’Enquête Nationale auprès des Ménages d’Opitciwan en 2011. Nous retiendrons les données sur la situation socio-économique de la communauté, en particulier celles portant sur la relation entre l’éducation, l’emploi et le revenu total. Cela permettra de connaître leur dynamique. Dans un deuxième temps, nous tenterons d’identifier les aspirations des jeunes d’Opitciwan à l’aide d’entrevues semi-dirigées. Nous examinerons les fondements de leurs aspirations, c’est-à-dire les représentations que se font les jeunes des possibilités d’avenir, et des moyens de les rendre effectivement atteignables. Afin d’illustrer les résultats différemment, nous dresserons un portrait des jeunes participants en créant des socio-types. Les quatre socio-types seront l’ambitieux, le réaliste consciencieux, l’insécure et le passif. Finalement, nous comparerons les deux analyses afin d’identifier les cohérences et les divergences et d’élucider celles-ci. L’étude permettra de mieux comprendre l’attitude des jeunes atikamekws et de l’ensemble de la population face à l’éducation et l’emploi. Elle permettra aussi de découvrir les besoins de la communauté.

Deeply connected with forest serial dynamics, and present in almost semi-natural landscapes of Iberian Peninsula, Dactylis hispanica subsp. lusitanica perennial grasslands antalis a major issue on biogeographical analysis, once it lacks on syntaxonomical solutions for its affiliation and respective positioning on serial dynamics of distinct territories, which edaphoclimatic envelopes suggest different floristic assemblages. Mainly part of Agrostion castellanae-Stipion giganteae alliance, the Lusitan-Extremadurean grasslands were studied, with the objective of updating that biogeographic Province, and several phytosociological relevés were made, allowing the syntaxonomical update as well to resolve the serial-dynamics arrangement where this peculiar grasslands makes part of. At last this communities belong to priority 6220 [*Pseudo-stepe with grasses and annuals (Thero-Brachypodietea)], habitat that interests as well for conservational matters, due to its peculiar flora.

SETTING National household survey of adults in South Africa, a middle income country. OBJECTIVE To determine the prevalence and predictors of chronic bronchitis. DESIGN A stratified national probability sample of households was selected. All adults in the selected households were interviewed. Chronic bronchitis was defined as chronic productive cough. Socio-demographic predictors were wealth, education, race, age and urban residence. Personal and exposure variables included history of tuberculosis, domestic exposure to smoky fuels, occupational exposures, smoking and body mass index. RESULTS The overall prevalence of chronic bronchitis was 2.3% in men and 2.8% in women. The strongest predictor of chronic bronchitis was a history of tuberculosis (men, odds ratio [OR] 4.9; 95% confidence interval [CI] 2.6-9.2; women, OR 6.6; 95% CI 3.7-11.9). Other risk factors were smoking, occupational exposure (in men), domestic exposure to smoky fuel (in women) and (in univariate analysis only) being underweight. Wealth and particularly education were protective. CONCLUSION The pattern of chronic bronchitis in South Africa suggests a combination of risk factors that includes not only smoking but also tuberculosis, occupational exposures in men and domestic fuel exposure in women. Control of these risk factors requires public health action across a broad front. The protective role of education requires elucidation.

Compared to the association between cigarette smoking and psychiatric disorders, relatively little is known about the relationship between smokeless tobacco use and psychiatric disorders. To identify the psychiatric correlates of smokeless tobacco use, the analysis used a national representative sample from the National Epidemiologic Survey on Alcohol and Related Conditions (NESARC) wave 1. Smokeless tobacco use was classified as exclusive snuff use, exclusive chewing tobacco, and dual use of both snuff and chewing tobacco at some time in the smokeless tobacco user's life. Lifetime psychiatric disorders were obtained via structured diagnostic interviews. The results show that the prevalence of lifetime exclusive snuff use, exclusive chewing tobacco, and dual use of both snuff and chewing tobacco was 2.16%, 2.52%, and 2.79%, respectively. After controlling for sociodemographic variables and cigarette smoking, the odds of exclusive chewing tobacco in persons with panic disorder and specific phobia were 1.53 and 1.41 times the odds in persons without those disorders, respectively. The odds of exclusive snuff use, exclusive chewing tobacco, and dual use of both products for individuals with alcohol use disorder were 1.97, 2.01, and 2.99 times the odds for those without alcohol use disorder, respectively. Respondents with cannabis use disorder were 1.44 times more likely to use snuff exclusively than those without cannabis use disorder. Respondents with inhalant/solvent use disorder were associated with 3.33 times the odds of exclusive chewing tobacco. In conclusion, this study highlights the specific links of anxiety disorder, alcohol, cannabis, and inhalant/solvent use disorders with different types of smokeless tobacco use.

La présente thèse de doctorat visait d’abord à valider les résultats des études antérieures démontrant un accroissement de la prévalence des problèmes de santé mentale et ensuite, à évaluer l’importance du contexte social dans l’explication des changements de prévalence, à partir des effets d’âge, de période et de cohorte. L’atteinte de ces objectifs s’est faite à partir de deux études empiriques, chacune ciblant sa propre problématique en santé mentale. La première étude, basée sur les données de l’Enquête longitudinale nationale sur les enfants et les jeunes (ELNEJ) de Statistique Canada, a permis de conclure à un accroissement réel de la prévalence du diagnostic de trouble déficitaire de l’attention/hyperactivité (TDA/H) et de la consommation de psychostimulants chez les enfants canadiens entre 1994 et 2007. Toutefois, cette tendance n’est ni constante, ni universelle, puisque des effets de période et d’âge apparaissent clairement : l’augmentation des prévalences est uniquement remarquée dans les années 2000, et survient seulement chez les enfants d’âge scolaire. L’identification d’inégalités de prévalence dues à la période historique et à l’âge des enfants souligne l’importance du contexte social dans la problématique du diagnostic de TDA/H et de la consommation de psychostimulants. La seconde étude a été réalisée à partir des données du Panel Study of Belgian Households (PSBH) et cherchait à expliquer l’accroissement des symptômes dépressifs observé ces vingt dernières années chez les adultes belges. L’utilisation de l’analyse multiniveaux longitudinale permettant la distinction des effets d’âge et des effets de cohortes a été privilégiée. Bien que l’intensité des symptômes dépressifs ait varié de manière relativement importante chez les individus au cours des années 1990, nos conclusions démontrent que les symptômes auto-rapportés de dépression sont davantage associés aux conditions de vie, qu’à la personnalité. L’augmentation résulte d’un effet de la succession des cohortes, où les individus des cohortes les plus récentes rapportent toujours une plus grande intensité de symptômes dépressifs que les individus des cohortes précédentes. Les membres d’une même cohorte de naissance partagent donc des expériences communes à un âge similaire, ce qui a un impact durable sur leurs comportements et sur leur santé mentale. De manière générale, les résultats des deux articles empiriques ont, chacun à leur manière, confirmé la réalité de l’accroissement des problèmes de santé mentale dans les sociétés occidentales contemporaines, et permis de constater que la prévalence diffère selon l’âge et la cohorte de naissance des individus, ainsi que selon la période historique, renforçant ainsi l’hypothèse de l’importance des facteurs sociaux dans l’étiologie des problèmes de santé mentale. Bien que la nature de ces facteurs n’ait pu être testée de manière directe, de nombreuses explications sociales furent tout de même proposées. À cet égard, des changements dans les normes comportementales associées à l’âge, dans les normes sociales, dans la conceptualisation des troubles mentaux, des modifications dans la sphère éducative, ainsi que des innovations pharmacologiques, médicales et technologiques constituent des explications sociales aux effets d’âge, de période et de cohorte qui ont été observés.

En este documento se realiza una nueva estimación de la discriminación salarial por género utilizando la metodología de descomposición de ecuaciones mincerianas sugerida por Oaxaca (1973) utilizando datos de la Encuesta Nacional de Hogares 1984 – 1999. Se detecta un patrón estable que mantiene un diferencial salarial no explicado por variables de productividad, a favor de los hombres. Al parecer el incremento o reducción de esta brecha puede estar relacionado con el ciclo económico.

This work investigates the impact of schooling Oil income distribution in statesjregions of Brazil. Using a semi-parametric model, discussed in DiNardo, Fortin & Lemieux (1996), we measure how much income diíferences between the Northeast and Southeast regions- the country's poorest and richest - and between the states of Ceará and São Paulo in those regions - can be explained by differences in schooling leveIs of the resident population. Using data from the National Household Survey (PNAD), we construct counterfactual densities by reweighting the distribution of the poorest region/state by the schooling profile of the richest. We conclude that: (i) more than 50% of the income di:fference is explained by the difference in schooling; (ii) the highest deciles of the income distribution gain more from an increase in schooling, closely approaching the wage distribution of the richest region/state; and (iii) an increase in schooling, holding the wage structure constant, aggravates the wage disparity in the poorest regions/ states.

OBJETIVO: Descrever métodos e resultados iniciais do Sistema de Vigilância de Fatores de Risco e Proteção para Doenças Crônicas não Transmissíveis por Inquérito Telefônico – VIGITEL implantado no Brasil em 2006. MÉTODOS: O VIGITEL estudou amostras probabilísticas da população com 18 ou mais anos de idade residente em domicílios conectados à rede de telefonia fixa de cada uma das capitais dos 26 Estados brasileiros e do Distrito Federal (54.369 indivíduos no total, sendo pelo menos 2.000 por cidade). A amostragem foi realizada a partir de cadastros eletrônicos completos das linhas residenciais fixas de cada cidade, envolvendo sorteio de linhas (domicílios) e sorteio de um morador por linha para ser entrevistado. O questionário aplicado investigou características demográficas e socioeconômicas, padrão de alimentação e de atividade física, consumo de cigarros e de bebidas alcoólicas, e peso e altura recordados, entre outros quesitos. Estimativas sobre a freqüência de fatores de risco selecionados, estratificadas por sexo e acompanhadas de Intervalo de Confiança de 95%, foram calculadas para a população adulta de cada cidade empregando-se fatores de ponderação que igualam a composição sociodemográfica da amostra em cada cidade àquela observada no Censo Demográfico de 2000. Estimativas para o conjunto das cidades empregam fator de ponderação adicional que leva em conta a população de adultos de cada cidade. RESULTADOS: Os cinco fatores de risco selecionados (tabagismo, consumo abusivo de bebidas alcoólicas, excesso de peso, consumo de carnes com excesso de gordura e sedentarismo) tenderam a ser mais freqüentes em homens do que em mulheres. Dentre os fatores de proteção, o consumo regular de frutas e hortaliças foi mais freqüente em mulheres do que em homens, observando-se situação inversa no caso da atividade física de lazer. Diferenças substanciais na freqüência dos fatores de risco e proteção foram observadas entre as cidades, com padrões de distribuição regional diferenciados por fator. DISCUSSO: O desempenho do sistema, avaliado a partir da qualidade dos cadastros telefônicos e de taxas de resposta e de recusas, mostrou-se adequado e, de modo geral, superior ao encontrado em sistemas equivalentes existentes em países desenvolvidos. O custo do sistema de R$ 31,15 por entrevista realizada, foi a metade do custo observado no sistema americano de vigilância de fatores de risco para doenças crônicas por inquérito telefônico e um quinto do custo estimado em inquérito domiciliar tradicional realizado recentemente no Brasil.
