A anemia falciforme está entre as doenças genéticas mais comuns e mais estudadas em todo o mundo. Ela é causada por mutação no gene β, produzindo alteração estrutural na molécula da hemoglobina. As moléculas de HbS, decorrentes da mutação, sofrem processo de polimerização fisiologicamente provocado pela baixa tensão de oxigênio, acidose e desidratação. Com isso, os eritrócitos passam a apresentar a forma de foice, causando vaso-oclusão e outras conseqüências. O objetivo desse estudo foi revisar a importância da hemoglobina fetal na clínica de pacientes portadores de anemia falciforme. O significado clínico da associação da elevação da hemoglobina fetal na anemia falciforme mostra-se favorável em termos hematológicos, pois, nessa interação, as células-F têm baixas concentrações de HbS e, com isso, inibem a polimerização da HbS e a alteração da morfologia dos eritrócitos. O tratamento com hidroxiuréia, em função do aumento na expressão da hemoglobina fetal que este fármaco proporciona, traz aos pacientes falcêmicos uma melhora significativa em sua clínica. Portanto, a hemoglobina fetal consiste no maior inibidor da polimerização da desoxi-HbS e, com isso, evita a falcização do eritrócito, a anemia hemolítica crônica, as crises dolorosas vaso-oclusivas, o infarto e a necrose em diversos órgãos, melhorando a clínica e a expectativa de vida dos pacientes.

O Diabetes Mellitus tipo 1 (DM1) é a endocrinopatia mais comum da infância e adolescência e impacta negativamente na qualidade de vida (QV). O EuroQol é um instrumento que afere o estado de saúde e vem sendo utilizado na grande maioria dos estudos multicêntricos mundiais em diabetes e tem se mostrado uma ferramenta extremamente útil e confiável. O objetivo desse estudo é avaliar a QV de pacientes com DM1 do Brasil, país de proporções continentais, por meio da análise do EuroQol. Para isso, realizou-se estudo retrospectivo e transversal, no qual foram analisados questionários de pacientes com DM1, respondidos no período de dezembro de 2008 a dezembro de 2010, em 28 centros de pesquisa de 20 cidades das quatro regiões do país (sudeste, norte/nordeste, sul e centro-oeste). Foram também coletados dados sobre complicações crônicas micro e macrovasculares e perfil lipídico. A avaliação da qualidade de vida pelo EuroQol mostra que a nota média atribuída ao estado geral de saúde é nitidamente menor que a encontrada em dois outros estudos populacionais com DM1 realizados na Europa (EQ-VAS da Alemanha, Holanda e Brasil foram de 82,1 ± 14; 81 ± 15 e 72 ± 22, respectivamente). O EuroQol demonstra que a região Norte-Nordeste apresenta melhor índice na avaliação do estado geral de saúde quando comparada a região Sudeste e menor frequência de ansiedade-depressão autorreferidas, quando comparada às demais regiões do país (Norte-Nordeste = 1,53 ± 0,6, Sudeste = 1,65 ± 0,7, Sul = 1,72 ± 0,7 e Centro-Oeste = 1,67 ± 0,7; p <0,05). Adicionalmente, diversas variáveis conhecidas (idade, duração do DM, prática de atividade física, HbA1c, glicemia de jejum e presença de complicações crônicas se correlacionaram com a QV (r = -0,1, p <0,05; r = -0,1, p <0,05; r = -0,1, p <0,05; r = -0,2, p <0,05; r = -0,1, p <0,05 e r= -0,1, p <0,05, respectivamente). Esse é o primeiro estudo a avaliar a qualidade de vida de pacientes com DM1 a nível populacional no hemisfério sul. Nossos dados indicam uma pior qualidade de vida dos pacientes com DM 1 no Brasil quando comparado a dados de países europeus. Apesar de ter sido encontrado uma inferior duração do DM e menor presença de complicações microvasculares na região Norte/ Nordeste, quando comparada à outras regiões, nossos dados sugerem a existência de elementos adicionais responsáveis pela melhor QV e menor presença de ansiedade/depressão encontradas nesta região. Novos estudos são necessários para identificar esses possíveis fatores.

A doença de Jorge Lobo é uma micose rara de inflamação crônica que provoca lesão na pele sem disseminação visceral. Essa doença é causada pelo fungo Lacazia loboi. Sua ocorrência é predominante em regiões de clima quente e úmido, sendo a maior parte dos casos registrados na região Amazônica brasileira. As observações histopatológicas mostram grande quantidade de fungos no local da lesão, havendo um rico infiltrado macrófagico, com presença de células gigantes e limitada presença de linfócitos. A migração de leucócitos para o sítio inflamatório induzida por Lacazia loboi é supostamente coordenada por citocinas e quimiocinas que auxiliadas por vasos sanguíneos e linfáticos influencia a migração celular induzindo a expressão de moléculas de adesão. No presente trabalho investigamos possíveis alterações microvasculares associadas à infecção por Lacazia loboi no local da lesão que podem interferir na evolução dos aspectos clínicos dos pacientes. Dessa forma, avaliamos a densidade dos vasos sanguíneos e linfáticos, bem como a expressão das moléculas ICAM-1, VCAM-1 e E-selectina. Nossos resultados mostraram que na doença de Jorge Lobo, há uma reduzida quantidade de vasos sanguíneos e linfáticos, quando comparado a pele controle. Houve um maior número de vasos expressando ICAM-1, sendo também observado maior número de vasos expressando a molécula VCAM-1, embora em quantidade menos proeminente que ICAM-1. Não observamos diferença na expressão de E-selectina. Juntos nos resultados apontam para uma alteração na microvasculatura local o que pode interferir no desenvolvimento de uma resposta imune celular eficiente e justificar a presença do fungo limitada ao local da lesão.

Ivabradine (IVB) is a heart rate lowering agent that acts via selective inhibition of the pacemaker funny current in sinoatrial nodal P cells, thus, reducing heart rate at rest and during exercise with minimal effect on myocardial contractility, blood pressure, and intracardiac conduction. IVB exerts no effect on external respiratory function parameters and it may also play a role in patients with concurrent chronic obstructive pulmonary disease. This property constitutes an important advantage over β-blockers. IVB acts by reducing the heart rate in a mechanism different from β-blockers, calcium channel blockers or late sodium channel blockers, three commonly prescribed antianginal drugs. As clinical trials have shown, it is remarkably well-tolerated and offers an alternative for patients who cannot take β-blockers. The combination of IVB and atenolol at commonly used doses in patients with chronic stable angina produced additional efficacy with no untoward effect on safety or tolerability. Additionally, side effects are rare and largely limited to a luminous phenomenon or phosphenes. This sensation is thought to be due to a block of Ih in the retina, a current very similar to cardiac If channels. IVB is contraindicated in patients with sick sinus syndrome or sinus node dysfunction and in patients taking hepatic inhibitors of Cytochrome P450 family 3, subfamily A, polypeptide 4 (abbreviated CYP3A4), with exception of omeprazole or lansoprazole. This review briefly summarizes the main studies regarding this drug.

Introdução: As Doenças Crônicas Não Transmissíveis (DANTS) são um grupo de enfermidades cujo processo de instalação no organismo geralmente se inicia com alterações, a prevenção dessas afecções tem sido um grande desafio para a saúde pública. As principais DANTS são: câncer, doenças do coração (angina e infarto do miocárdio), doenças do pulmão (como enfisema, bronquite e asma), doenças da circulação como acidente vascular cerebral e hipertensão arterial, doenças metabólicas como diabetes, hipercolesterolemia, a obesidade, relacionadas com condições de vida (como o estresse, depressão e ansiedade). Existem inúmeros Fatores de Risco (FR) relacionados a estes tipos de doenças, cuja remoção, ou atenuação, pode contribuir para o declínio da mortalidade.Objetivo: Associar comportamentos e conhecimentos de FR para doenças e agravos não transmissíveis de mulheres praticantes de AF através do projeto Saúde Ativa Rio Claro, este visa desenvolver e difundir práticas de intervenção cientificamente orientadas que incorporem a AF como parte integrante de um hábito de vida saudável para todas as pessoas. Metodologia: Participaram do estudo 100 mulheres, com idade média de 57,7((± 13,1), foram aplicados dois questionários, sendo o primeiro sobre conhecimento de FR para DANTS e o segundo avaliou comportamentos relacionados ao estilo de vida: nível de AF, consumo de álcool, dieta, taxa de colesterol, nível de estresse, tabagismo.Resultados e Discussão: Não houve associação entre conhecimento e comportamento, o conhecimento isoladamente não foi suficiente para promover modificações no comportamento das pessoas. A analise estatística utilizada foi o Qui Quadrado (x²) de 1 grau de liberdade e com nível de significância de p= 0,05. Conclusão: Um grupo de pessoas pode conhecer os benefícios de um comportamento e não ter tempo ou recursos suficientes...(Resumo completo, clicar acesso eletrônico abaixo)

Background/aims: Cardiovascular diseases are major causes of mortality in chronic renal failure patients before and after renal transplantation. Among them, coronary disease presents a particular risk; however, risk predictors have been used to diagnose coronary heart disease. This study evaluated the frequency and importance of clinical predictors of coronary artery disease in chronic renal failure patients undergoing dialysis who were renal transplant candidates, and assessed a previously developed scoring system. Methods: Coronary angiographies conducted between March 2008 and April 2013 from 99 candidates for renal transplantation from two transplant centers in Sao Paulo state were analyzed for associations between significant coronary artery diseases (>= 70% stenosis in one or more epicardial coronary arteries or >= 50% in the left main coronary artery) and clinical parameters. Results: Univariate logistic regression analysis identified diabetes, angina, and/or previous infarction, clinical peripheral arterial disease and dyslipidemia as predictors of coronary artery disease. Multiple logistic regression analysis identified only diabetes and angina and/or previous infarction as independent predictors. Conclusion: The results corroborate previous studies demonstrating the importance of these factors when selecting patients for coronary angiography in clinical pretransplant evaluation.

The increased prevalence of diabetic individuals has become a public health problem. Diabetes Mellitus is a metabolic disorder characterized by an increase in plasma glucose levels. It impairs the physiological equilibrium in utilization of carbohydrate by tissues. The persistent hyperglycemia can produce deleterious effects on bone formation due the microvascular complication. The present paper reviews the bibliography linking the impact of glycemic control at complications associated at diabetes mellitus on osseointegration. In experimental models of diabetes it was observed a reduced level of bone-implant contact. This failure can be reduced by means of hyperglycemia control. Also, several studies point the beneficial effect of coated implant on osseointegration process. It is necessary to take special care into account for the placement of implants in diabetic patient and improve the percentages of implant survival. A rigorous control of plasma glycaemia, together with other measures, like as absence of chronic complications, good oral hygiene and regular medical follow-up has been related to rising the percentages of successful in diabetic patients.

Background: Abdominal obesity is an important cardiovascular risk factor. Therefore, identifying the best method for measuring waist circumference (WC) is a priority. Objective: To evaluate the eight methods of measuring WC in patients with acute coronary syndrome (ACS) as a predictor of cardiovascular complications during hospitalization. Methods: Prospective study of patients with ACS. The measurement of WC was performed by eight known methods: midpoint between the last rib and the iliac crest (1), point of minimum circumference (2); immediately above the iliac crest (3), umbilicus (4), one inch above the umbilicus (5), one centimeter above the umbilicus (6), smallest rib and (7) the point of greatest circumference around the waist (8). Complications included: angina, arrhythmia, heart failure, cardiogenic shock, hypotension, pericarditis and death. Logistic regression tests were used for predictive factors. Results: A total of 55 patients were evaluated. During the hospitalization period, which corresponded on average to seven days, 37 (67%) patients had complications, with the exception of death, which was not observed in any of the cases. Of these complications, the only one that was associated with WC was angina, and with every cm of WC increase, the risk for angina increased from 7.5 to 9.9%, depending on the measurement site. It is noteworthy the fact that there was no difference between the different methods of measuring WC as a predictor of angina. Conclusion: The eight methods of measuring WC are also predictors of recurrent angina after acute coronary syndromes. Key words: Evaluation; Acute Coronary Syndrome; Abdominal Circumference

Malaria associated-acute kidney injury (AKI) is associated with 45% of mortality in adult patients hospitalized with severe form of the disease. However, the causes that lead to a framework of malaria-associated AKI are still poorly characterized. Some clinical studies speculate that oxidative stress products, a characteristic of Plasmodium infection, as well as proinflammatory response induced by the parasite are involved in its pathophysiology. Therefore, we aimed to investigate the development of malaria-associated AKI during infection by P. berghei ANKA, with special attention to the role played by the inflammatory response and the involvement of oxidative stress. For that, we took advantage of an experimental model of severe malaria that showed significant changes in the renal pathophysiology to investigate the role of malaria infection in the renal microvascular permeability and tissue injury. Therefore, BALB/c mice were infected with P. berghei ANKA. To assess renal function, creatinine, blood urea nitrogen, and ratio of proteinuria and creatininuria were evaluated. The products of oxidative stress, as well as cytokine profile were quantified in plasma and renal tissue. The change of renal microvascular permeability, tissue hypoxia and cellular apoptosis were also evaluated. Parasite infection resulted in renal dysfunction. Furthermore, we observed increased expression of adhesion molecule, proinflammatory cytokines and products of oxidative stress, associated with a decrease mRNA expression of HO-1 in kidney tissue of infected mice. The measurement of lipoprotein oxidizability also showed a significant increase in plasma of infected animals. Together, our findings support the idea that products of oxidative stress, as well as the immune response against the parasite are crucial to changes in kidney architecture and microvascular endothelial permeability of BALB/c mice infected with P. berghei ANKA.

Objective Metabolic syndrome (MetS) is highly prevalent in rheumatic diseases and is recognized as a new independent cardiovascular risk factor. This study was undertaken to determine the clinical significance of MetS in patients with primary antiphospholipid syndrome (APS). Methods Seventy-one primary APS patients and 73 age- and sex-matched healthy controls were included. Serum samples were tested for lipid profile, Lp(a), glucose, insulin, thyroid-stimulating hormone, free T4, erythrocyte sedimentation rate, C-reactive protein level, and uric acid. MetS was defined by the International Diabetes Federation criteria, and insulin resistance was established using the homeostasis model assessment index. Results The prevalence of MetS was 33.8%, and further comparison between primary APS patients with and without MetS revealed that the former had a higher frequency of arterial events (79.2% versus 42.6%; P = 0.003), angina (29.2% versus 2.1%; P = 0.002), and positive lupus anticoagulant antibody (95.8% versus 76.6%; P = 0.049). In addition, primary APS patients with MetS, as expected, had a higher prevalence of cardiovascular risk factors. On multivariate analysis, only MetS was independently associated with arterial events in primary APS. Conclusion Coexistence of primary APS and MetS seems to identify a subgroup of patients with higher risk of arterial events, suggesting that MetS may aggravate existing endothelial abnormalities of primary APS.

Background The effect of intensified platelet inhibition for patients with unstable angina or myocardial infarction without ST-segment elevation who do not undergo revascularization has not been delineated. Methods In this double-blind, randomized trial, in a primary analysis involving 7243 patients under the age of 75 years receiving aspirin, we evaluated up to 30 months of treatment with prasugrel (10 mg daily) versus clopidogrel (75 mg daily). In a secondary analysis involving 2083 patients 75 years of age or older, we evaluated 5 mg of prasugrel versus 75 mg of clopidogrel. Results At a median follow-up of 17 months, the primary end point of death from cardiovascular causes, myocardial infarction, or stroke among patients under the age of 75 years occurred in 13.9% of the prasugrel group and 16.0% of the clopidogrel group (hazard ratio in the prasugrel group, 0.91; 95% confidence interval [CI], 0.79 to 1.05; P = 0.21). Similar results were observed in the overall population. The prespecified analysis of multiple recurrent ischemic events (all components of the primary end point) suggested a lower risk for prasugrel among patients under the age of 75 years (hazard ratio, 0.85; 95% CI, 0.72 to 1.00; P = 0.04). Rates of severe and intracranial bleeding were similar in the two groups in all age groups. There was no significant between-group difference in the frequency of nonhemorrhagic serious adverse events, except for a higher frequency of heart failure in the clopidogrel group. Conclusions Among patients with unstable angina or myocardial infarction without ST- segment elevation, prasugrel did not significantly reduce the frequency of the primary end point, as compared with clopidogrel, and similar risks of bleeding were observed. Background The e-Healthy Endothelial Accelerated Lining Inhibits Neointimal Growth (e-HEALING) registry was designed to capture clinical data on the use of the endothelial progenitor cell capture stent (ECS) in routine clinical practice. In this analysis, we investigated the 12-month clinical outcomes in patients treated with an ECS for a bifurcation lesion. Methods The worldwide, prospective, nonrandomized e-HEALING registry aimed to enrol 5000 patients treated for coronary artery disease with one or more ECS between October 2005 and October 2007. Clinical follow-up was obtained at 1, 6, and 12 months. The primary endpoint was target vessel failure (TVF), defined as the composite of cardiac death, myocardial infarction, and target vessel revascularization at 12 months. Results A total of 573 patients were treated for at least one bifurcation lesion and were assessed in the current analysis. Baseline characteristics showed a median age of 65 years; 21% were diabetic patients and 36% had unstable angina. A total of 63% of the bifurcation lesions were located in the left artery descending and the mean stent length was 20.7 +/- 12.6 mm. At 12 months, TVF was 12.7% and target lesion revascularization was 7.5%. Definite or probable stent thrombosis occurred in 1.7% of the patients. Moreover, one or more stents per lesion [hazard ratio (HR): 2.79, 95% confidence interval (CI): 1.60-4.86, P < 0.001], predilatation (HR: 0.39, 95% CI: 0.17-0.87, P = 0.023), and lesions located in the right coronary artery (HR: 4.56, 95% CI: 1.07-19.5, P = 0.041) were independent predictors of TVF. Conclusion In the e-HEALING registry, coronary bifurcation stenting with the ECS results in favorable clinical outcomes and low incidences of repeat revascularization and stent thrombosis.
