Nitric oxide (NO) plays a major role in the regulation of cardiovascular and metabolic homeostasis, as evidenced by insulin resistance and arterial hypertension in endothelial NO synthase (eNOS) null mice. Extrapolation of these findings to humans is difficult, however, because eNOS gene deficiency has not been reported. eNOS gene polymorphism and impaired NO synthesis, however, have been reported in several cardiovascular disease states and could predispose to insulin resistance. High-fat diet induces insulin resistance and arterial hypertension in normal mice. To test whether partial eNOS deficiency facilitates the development of insulin resistance and arterial hypertension during metabolic stress, we examined effects of an 8-week high-fat diet on insulin sensitivity (euglycemic clamp) and arterial pressure in eNOS(+/-) mice. When fed a normal diet, these mice had normal insulin sensitivity and were normotensive. When fed a high-fat diet, however, eNOS(+/-) mice developed exaggerated arterial hypertension and had fasting hyperinsulinemia and a 35% lower insulin-stimulated glucose utilization than control mice. The partial deletion of the eNOS gene does not alter insulin sensitivity or blood pressure in mice. When challenged with nutritional stress, however, partial eNOS deficiency facilitates the development of insulin resistance and arterial hypertension, providing further evidence for the importance of this gene in linking metabolic and cardiovascular disease.

Biomphalaria glabrata, molusco de água doce, desempenha um importante papel em Parasitologia Médica, por ser o hospedeiro intermediário de Schistosoma mansoni, tremátode digenético responsável pela schistosomose intestinal. A detecção de moluscos infectados pelo Schistosoma mansoni tem uma grande importância em Saúde pública, porque identifica focos de transmissão da schistosomose. As limitações dos métodos clássicos para o diagnóstico de infecções pré patentes fazem com que os métodos de biologia moleculares sejam vistos como possíveis alternativas através da detecção de ADN do S. mansoni em moluscos hospedeiros. A detecção de sequências específicas de ADN por reacção de polimerase em cadeia (PCR) tem-se verificado ser de extrema importância para a análise genética e diagnóstico de várias doenças infecciosas. Neste estudo foi aplicada a técnica de Nested-PCR, com o objectivo de identificar, no período pré-patente, S. mansoni em moluscos expostos a 1, 5 e 10 miracídios em diferentes períodos de tempo. Foram utilizados moluscos das estirpes albina e selvagem de B. glabrata. Para a realização das técnicas de PCR e de Nested–PCR (NPCR) foram utilizados dois pares de oligonucleótidos desenhados especificamente para detectar o ADN de S. mansoni . Verificou-se amplificação do fragmento de ADN do parasita em 80% das amostras analisadas, independentemente da dose de miracídios e do período de exposição. O método utilizado é altamente sensível, mostrando ser uma ferramenta útil na detecção de hospedeiros intermediários de S. mansoni, consequentemente na identificação de focos de schistosomose intestinal.

Injury of an arterial vessel wall acutely triggers a multifaceted process of thrombus formation, which is dictated by the high-shear flow conditions in the artery. In this overview, we describe how the classical concept of arterial thrombus formation and vascular occlusion, driven by platelet activation and fibrin formation, can be extended and fine-tuned. This has become possible because of recent insight into the mechanisms of: (i) platelet-vessel wall and platelet-platelet communication, (ii) autocrine platelet activation, and (iii) platelet-coagulation interactions, in relation to blood flow dynamics. We list over 40 studies with genetically modified mice showing a role of platelet and plasma proteins in the control of thrombus stability after vascular injury. These include multiple platelet adhesive receptors and other junctional molecules, components of the ADP receptor signalling cascade to integrin activation, proteins controlling platelet shape, and autocrine activation processes, as well as multiple plasma proteins binding to platelets and proteins of the intrinsic coagulation cascade. Regulatory roles herein of the endothelium and other blood cells are recapitulated as well. Patient studies support the contribution of platelet- and coagulation activation in the regulation of thrombus stability. Analysis of the factors determining flow-dependent thrombus stabilization and embolus formation in mice will help to understand the regulation of this process in human arterial disease.

Takayasu arteritis, a nonspecific inflammatory arteritis, is particularly rare in children. We report the case of a 6-year-old girl presenting with severe arterial hypertension in the upper segment associated with an inflammatory syndrome. Investigations showed coarctation of the abdominal aorta at different levels, due to Takayasu arteritis. The patient was treated with percutaneous dilatation and stent implantation as well as prolonged anti-inflammatory therapy. Arterial hypertension in children needs to be investigated until its cause, which may be rare such as Takaysu arteritis, is determined.

OBJECTIVE To identify the prevalence of arterial hypertension and associated factors in patients submitted to myocardial revascularization. METHOD Cross-sectional study using the database of a hospital in São Paulo (SP, Brazil) containing 3010 patients with coronary artery disease submitted to myocardial revascularization. A multiple logistic regression was performed to identify variables independently associated with hypertension (statistical significance: p<0.05). RESULTS Prevalence of hypertension was 82.8%. After the variables were adjusted, the associated factors were as follows: age, odds ratio (OR): OR=1.01; 95% confidence interval (CI): CI:1.00-1.02; female gender: (OR=1.77;CI:1.39-2.25); brown-skin race: (OR=1.53;CI:1.07-2.19); obesity: (OR=1.53;CI:1.13-2.06); diabetes: (OR=1.90;CI:1.52-2.39); dyslipidemia: (OR=1.51;CI:1.23-1.85); and creatinine>1.3: (OR=1.37;CI:1.09-1.72). CONCLUSION A high prevalence of arterial hypertension and association with both non-modifiable and modifiable factors was observed.

Hypertension affects approximately 1 billion people worldwide. Owing to population aging, hypertension-related cardiovascular burden is expected to rise in the near future. In addition to genetic variants influencing the blood pressure response to antihypertensive drugs, several genes encoding for drug-metabolizing or -transporting enzymes have been associated with blood pressure and/or hypertension in humans (e.g., ACE, CYP1A2, CYP3A5, ABCB1 and MTHFR) regardless of drug treatment. These genes are also involved in the metabolism and transport of endogenous substances and their effects may be modified by selected environmental factors, such as diet or lifestyle. However, little is currently known on the complex interplay between environmental factors, endogenous factors, genetic variants and drugs on blood pressure control. This review will discuss the respective role of population-based primary prevention and personalized medicine for arterial hypertension, taking a pharmacogenomics' perspective focusing on selected pharmacogenes.

BACKGROUND: The effects of intravenous thrombolysis on floating thrombi in cervical and intracranial arteries of acute ischemic stroke patients are unknown. Similarly, the best prevention methods of early recurrences remain controversial. This study aimed to describe the clinical and radiological outcome of thrombolyzed strokes with floating thrombi. METHODS: We retrospectively analyzed all thrombolyzed stroke patients in our institution between 2003 and 2010 with floating thrombi on acute CT-angiography before the intravenous thrombolysis. The floating thrombus was diagnosed if an elongated thrombus of at least 5 mm length, completely surrounded by contrast on supra-aortic neck or intracerebral arteries, was present on CT-angiography. Demographics, vascular risk factors, and comorbidities were recorded and stroke etiology was determined after a standardized workup. Repeat arterial imaging was performed by CTA at 24 h or before if clinical worsening was noted and then by Doppler and MRA during the first week and at four months. RESULTS: Of 409 thrombolyzed stroke patients undergoing acute CT Angiography, seven (1.7%) had a floating thrombus; of these seven, six had it in the anterior circulation. Demographics, risk factors and stroke severity of these patients were comparable to the other thrombolyzed patients. After intravenous thrombolysis, the floating thrombi resolved completely at 24 h in four of the patients, whereas one had an early recurrent stroke and one developed progressive worsening. One patient developed early occlusion of the carotid artery with floating thrombus and subsequently a TIA. The two patients with a stable floating thrombus had no clinical recurrences. In the literature, only one of four reported cases were found to have a thrombolysis-related early recurrence. CONCLUSIONS: Long-term outcome seemed similar in thrombolyzed patients with floating thrombus, despite a possible increase of very early recurrence. It remains to be established whether acute mechanical thrombectomy could be a safer and more effective treatment to prevent early recurrence. However, intravenous thrombolysis should not be withheld in eligible stroke patients.

O presente estudo faz uma abordagem geral em termos bibliográficos sobre a hipertensão arterial dado a sua importância epidemiológica a nível mundial, centrada na óptica da enfermagem, utilizando o modelo teórico de Dorohteia Orem – Teoria de Deficit de Auto-cuidado, mostrando a sua aplicação prática juntos dos utentes hipertensos de Lagoa – Rª das Patas. Tratando-se de uma comunidade com carências várias, procuramos estabelecer relações entre esses factores e a hipertensão. Fizemos um levantamento geral em termos demográficos da pequena povoação; ficamos a saber da distribuição populacional por faixa etária, identificando a população alvo do nosso trabalho (hipertensos dos 45 a 85 anos); conhecer o comportamento dos utentes perante os factores de riscos; identificar os deficits de auto-cuidado, promover o auto-cuidado através de visitas domiciliárias, interagindo com o hipertenso no seu meio familiar, no intuito de satisfazer a sua necessidade no que concerne ao tratamento e controle do nível da pressão arterial, proporcionando-lhe um atendimento personalizado, contribuindo assim, para melhoria da sua qualidade de vida.

INTRODUCTION: The presence of a pre-existing narrow spinal canal may have an important place in the ethiopathogenesis of lumbar spinal stenosis. By consequence the study of the development of the spinal canal is crucial. The first goal of this work is to do a comprehensive literature search and to give an essential view on the development of spinal canal and its depending factors studied until now. The second goal is to give some considerations and hypothesize new leads for clinically useful researches. MATERIALS AND METHODS: A bibliographical research was executed using different search engines: PubMed, Google Schoolar ©, Ovid ® and Web Of Science ©. Free sources and avaible from the University of Lausanne (UNIL) and Centre Hospitalier Universitaire Vaudois (CHUV) were used. At the end of the bibliographic researches 114 references were found, 85 were free access and just 41 were cited in this work. Most of the found references are in English or in French. RESULTS AND DISCUSSION: The spinal canal is principally limited by the vertebrae which have a mesodermal origin. The nervous (ectodermal) tissue significantly influences the growth of the canal. The most important structure participating in the spinal canal growth is the neurocentral synchondrosis in almost the entire vertebral column. The fusion of the half posterior arches seems to have less importance for the canal size. The growth is not homogeneous but, depends on the vertebral level. Timing, rate and growth potentials differ by regions. Especially in the case of the lumbar segment, there is a craniocaudal tendency which entails a greater post-natal catch-up growth for distal vertebrae. Trefoil-shape of the L5 canal is the consequence of a sagittal growth deficiency. The spinal canal shares some developmental characteristics with different structures and systems, especially with the central nervous system. It may be the consequence of the embryological origin. It is supposed that not all the related structures would be affected by a growth impairment because of the different catch-up potentials. Studies found that narrower spinal canals might be related with cardiovascular and gastrointestinal symptoms, lower thymic function, bone mineral content, dental hypoplasia and Harris' lines. Anthropometric correlations found at birth disappear during the pediatric age. All factors which can affect bone and nervous growth might be relevant. Genetic predispositions are the only factors that can never be changed but the real impact is to ascertain. During the antenatal period, all the elements determining a good supply of blood and oxygen may influence the vertebral canal development, for example smoking during pregnancy. Diet is a crucial factor having an impact on both antenatal and postnatal growth. Proteins intake is the only proved dietetic relationship found in the bibliographic research of this work. The mechanical effects due to locomotion changes are unknown. Socioeconomic situation has an impact on several influencing factors and it is difficult to study it owing to numerous bias. CONCLUSIONS: A correct growth of spinal canal is evidently relevant to prevent not-degenerative stenotic conditions. But a "congenital" narrower canal may aggravate degenerative stenosis. This concerns specific groups of patient. If the size of the canal is highly involved in the pathogenesis of common back pains, a hypothetical measure to prevent developmental impairments could have a not- negligible impact on the society. It would be interesting to study more about dietetic necessities for a good spinal canal development. Understanding the relationship between nervous tissues and vertebra it might be useful in identifying what is needed for the ideal development. Genetic importance and the post-natal influences of upright standing on the canal growth remain unsolved questions. All these tracks may have a double purpose: knowing if it is possible to decrease the incidence of narrower spinal canal and consequently finding possible preventive measures. The development of vertebral canal is a complex subject which ranges over a wide variety of fields. The knowledge of this subject is an indispensable tool to understand and hypothesize the influencing factors that might lead to stenotic conditions. Unfortunately, a lack of information makes difficult to have a complete and satisfactory interdisciplinary vision.

Para a maioria das mulheres, a gravidez é um fenómeno fisiológico, mas, em alguns casos pode ocorrer agravos durante sua evolução, colocando em risco a saúde da mãe e do feto. De entre esses agravos, que podem ser chamados de doenças maternas, ocorrem nesse período, à hipertensão na gravidez que é considerada uma das que mais provocam problemas no organismo materno, fetal e neonatal. Ela constitui um dos problemas de grande morbi - mortalidade tanto materno como fetal, o que leva a atribuir ênfase a essa situação. Daí surgiu o interesse de desenvolver o trabalho cujo tema é “ Hipertensão Arterial na Gravidez: Como os cuidados de Enfermagem podem contribuir para a melhoria da qualidade de vida das Grávidas Hipertensas. Tendo como objectivo entender o contributo do enfermeiro na prevenção e diminuição da mesma, da importância das intervenções de enfermagem nessas gestantes, compreender a problemática em estudo e dessa forma enriquecer o conhecimento em relação a temática. Através de revisões bibliográficas, foi possível constatar que a Hipertensão na Gravidez tem sido uma das patologias mais comuns, por vezes evoluindo para outras complicações da mesma. A revisão da literatura foi focalizada em alguns artigos Científicos disponibilizados através de revistas, sites de internet e analisando os dados da Entrevista feita no Sector da Maternidade do Hospital Baptista de Sousa. Após uma reflexão sobre os diferentes métodos existentes e os objectivos para o estudo, optou - se pelo método qualitativo para a compreensão absoluta e ampla do fenómeno em estudo. O tema em estudo é importante pela sua pertinência e também para aperceber do papel fundamental do Enfermeiro para que as gestantes tenham um acompanhamento eficaz e eficiente, tentando ao máximo prevenir esta patologia, bem como outros que podem surgir durante a gestação, construindo assim um laço entre Enfermeiro/Utente para a melhoria das intervenções de Enfermagem.

O presente trabalho é o resultado da nossa investigação desenvolvida com base no tema “A Assistência de Enfermagem Prestada ao Idoso com Hipertensão Arterial”, através da análise de entrevistas realizadas aos enfermeiros do Centro de Saúde de Fonte Inês, e cujo objectivo central é avaliar a importância dos cuidados de enfermagem para dar resposta às necessidades dos idosos com hipertensão arterial. O estudo foi realizado com base numa abordagem qualitativa, e os dados foram colhidos através de entrevistas com perguntas abertas, respeitando o anonimato dos entrevistados. Deste estudo participaram cinco enfermeiros. Factores vários são identificados como causa da hipertensão, destacando-se entre eles o histórico familiar. Contudo, o consumo de sal, a obesidade, a diabete, o abuso do álcool, a vida sedentária e o fumo, são “factores de risco” a se ter em conta. Fala-se também hoje em dia numa maior “expectativa de vida”. Efectivamente, com a idade aumenta a vulnerabilidade do ser humano no tocante ao desenvolvimento de enfermidades, nomeadamente doenças crónicas como a hipertensão arterial. Assim sendo, achamos pertinente estudar este tema, mormente considerando que a maioria dos utentes que procuram os nossos centros de saúde são pessoas Idosas com esta patologia. Pudemos constatar e destacar o papel preponderante do enfermeiro no que concerne ao cuidado, e neste sentido para, prestar cuidados aos idosos, os enfermeiros devem ter conhecimento do processo de envelhecimento. É também destacada a importância de um serviço de saúde de qualidade, que vá de encontro às expectativas e necessidades do Idoso, proporcionando ao mesmo tempo uma vida digna, saudável e duradoura. A análise dos resultados demonstra que os enfermeiros estão conscientes das dificuldades que afectam o bom funcionamento dessa estrutura de saúde, e apontam a necessidade de se identificar estratégias que visam colmatar as insuficiências.
