This study analyzes the effect on levels of patient anxiety and depression of a partner joining a cardiac rehabilitation program support group, also taking into account the sex of the patient. The study was undertaken using a two-group comparison design with pre-and post-test measures in non-equivalent groups. The sample comprised patients in the cardiac rehabilitation program (CRP) at the Ramón y Cajal Hospital, Madrid (Spain). Analysis of covariance (ANCOVA) showed direct effects of sex and partner participation in support groups on the anxiety trait. Similarly, interaction effects were observed between the sex variable and partner participation. These results indicate the pertinence of designing separate groups for patients and partners. © 2014 Universidad Complutense de Madrid and Colegio Oficial de Psicólogos de Madrid.

El daño pulmonar agudo secundario a la cirugía de resección pulmonar (CRP) afecta en torno al 3-4% de los pacientes sometidos a este procedimiento. A pesar de no resultar una entidad excesivamente frecuente, condiciona en gran medida el pronóstico de los pacientes sometidos a estas técnicas quirúrgicos, resultando la principal causa de mortalidad. En la etiopatogenia de esta entidad clínica intervienen distintos factores como son la ventilación mecánica, la ventilación unipulmonar (OLV), los procesos de isquemia-reperfusión y la manipulación quirúrgica asociada al procedimiento. Estos factores colaboran de forma intercurrente a desencadenar una respuesta inflamatoria secundaria que lesiona el epitelio y endotelio alveolar, ocasionando una alteración de la permeabilidad de la barrera alveolocapilar, y la consiguiente formación de edema en los espacios alveolares. Hace años que se conoce que la ventilación mecánica puede provocar lesiones tisulares y edema pulmonar no cardiogénico en pulmones sanos, derivado de una respuesta inflamatoria secundaria a los distintos estímulos que condiciona la ventilación mecánica. En los procedimientos de CRP, la OLV del pulmón declive (o dependiente) implica una especial agresión al pulmón, con mayor posibilidad de lesión por volumen y presión...

The Municipality of Anchorage (MOA) is required to better manage, operate and control municipal solid waste (MSW) after the Anchorage Assembly instituted a Zero Waste Policy. Two household curbside recycling programs (CRPs), pay-as-you-throw (PAYT) and single-stream, were compared and evaluated to determine an optimal municipal solid waste diversion method for households within the MOA. The analyses find: (1) a CRP must be designed from comprehensive analysis, models and data correlation that combine demographic and psychographic variables; and (2) CRPs can be easily adjusted towards community-specific goals using technology, such as Geographic Information System (GIS) and Radio Frequency Identification (RFID). Combining resources of policy-makers, businesses, and other viable actors are necessary components to produce a sustainable, economically viable curbside recycling program.

Restoration efforts in the Mediterranean Basin have been changing from a silvicultural to an ecological restoration approach. Yet, to what extent the projects are guided by ecological restoration principles remains largely unknown. To analyse this issue, we built an on-line survey addressed to restoration practitioners. We analysed 36 restoration projects, mostly from drylands (86%). The projects used mainly soil from local sources. The need to comply with legislation was more important as a restoration motive for European Union (EU) than for non-EU countries, while public opinion and health had a greater importance in the latter. Non-EU countries relied more on non-native plant species than EU countries, thus deviating from ecological restoration guidelines. Nursery-grown plants used were mostly of local or regional provenance, whilst seeds were mostly of national provenance. Unexpected restoration results (e.g. inadequate biodiversity) were reported for 50% of the projects and restoration success was never evaluated in 22%. Long term evaluation (> 6 years) was only performed in 31% of cases, and based primarily on plant diversity and cover. The use of non-native species and species of exogenous provenances may: i) entail the loss of local genetic and functional trait diversity, critical to cope with drought, particularly under the predicted climate change scenarios, and ii) lead to unexpected competition with native species and/or negatively impact local biotic interactions. Absent or inappropriate monitoring may prevent the understanding of restoration trajectories, precluding adaptive management strategies, often crucial to create functional ecosystems able to provide ecosystem services. The overview of ecological restoration projects in the Mediterranean Basin revealed high variability among practices and highlighted the need for improved scientific assistance and information exchange, greater use of native species of local provenance, and more long-term monitoring and evaluation, including functional and ecosystem services' indicators, to improve and spread the practice of ecological restoration.

The CIROS-1 drillhole, which in 1986 reached a depth of 700 m below the seafloor, is still the only deep hole that can provide information on the velocity structure of the upper crust in McMurdo Sound and the Ross Sea, Antarctica. A careful review and quality control of the downhole logging data of CIROS-1 resulted in a new porosity depth function that is consistent with porosity data from the MSSTS-1 and CRP-1 drillholes. Using existing porosity-velocity equations, it was possible for the first time to obtain reliable velocity information for the upper 700 m of strata off the Victoria Land coast. The calculated synthetic seismograms, based on downhole velocity and density data, fit very well with the existing seismic lines IT90A-71, PD90-12, and NBP9601-89. The quality of the correlation confirms that the average velocity of the top 700 m of strata is about 2 000-2 300 m/s, and not 2 800-3 000 m/s, as was previously assumed. In consequence, these distinctly lower velocities result in shallower depths for the seismic unconformities V3/V4 andV4/V5 and thus may have important implications for further drilling off Cape Roberts.

OBJECTIVES: To examine the association between physical activity and inflammatory markers, with consideration for body fatness and antioxidant use. DESIGN: Cross-sectional study, using baseline data from the Health, Aging and Body Composition Study. SETTING: Metropolitan areas surrounding Pittsburgh, Pennsylvania, and Memphis, Tennessee. PARTICIPANTS: Black and white, well-functioning men and women (N=3,075), aged 70 to 79. MEASUREMENTS: Interviewer-administered questionnaires of previous-week household, walking, exercise, and occupational/volunteer physical activities. Analysis of covariance was used to examine the association between activity level and serum C-reactive protein (CRP), interleukin-6 (IL-6), and plasma tumor necrosis factor alpha (TNFalpha) with covariate adjustment. Antioxidant supplement use (multivitamin, vitamins E or C, beta carotene) was evaluated as an effect modifier of the association. Rates of cardiovascular and renal disease in Australian Aboriginal communities are high, but we do not know the contribution of inflammation to these diseases in this setting. In the present study, we sought to examine the distribution of C-reactive protein (CRP) and other markers of inflammation and their relationships with cardiovascular risk markers and renal disease in a remote Australian Aboriginal community. The study included 237 adults (58% of the adult population) in a remote Aboriginal community in the Northern Territory of Australia. Main outcome measures were CRP, fibrinogen and lgG concentrations, blood pressure (BP), presence of diabetes, lipids, albuminuria, seropositivity to three common micro-organisms, as well as carotid intima-media thickness (IMT). Serum concentrations of CRP [7 (5-13) mg/l; median (inter-quartile range)] were markedly increased and were significantly correlated with fibrinogen and lgG concentrations and inversely correlated with serum albumin concentration. Higher CRP concentrations were associated with lgG seropositivity to Helicobacter pylori and Chlamydia pneumoniae and higher lgG titre for cytomegalovirus. Higher CRP concentrations were associated with the following: the 45-54-year age group, female subjects, the presence of skin sores, higher body mass index, waist circumference, BP, glycated haemoglobin and greater albuminuria. CRP concentrations increased with the number of cardiovascular risk factors, carotid IMT and albuminuria independently of other risk factors. These CRP concentrations were markedly higher than described in other community settings and are probably related, in a large part, to chronic and repeated infections. Their association with markers of cardiovascular risk and renal disease are compatible with the high rates of cardiovascular and renal disease in this community, and provide more evidence of strong links between these conditions, through a shared background of infection/inflammation. This suggests that a strong focus on prevention and management of infections will be important in reducing these conditions, in addition to interventions directed at more traditional risk factors.
