The expansion of the specialty of sports and exercise medicine (SEM) is a relatively recent development in the medical community and the role of the SEM specialist continues to evolve and develop. The SEM specialist is ideally placed to care for all aspects of physical activity not only in athletes but also in the general population. As an advocate for physical activity the SEM specialist plays a broad role in advising safe effective sports and recreation participation; screening for disease related to sports participation; examining and contributing to the evidence behind treatment strategies and evaluating any potential negative impact of sports injury prevention measures. In this thesis I will demonstrate the breadth of the role the Sports and Exercise Medicine Specialist from epidemiology to in-depth examination of treatment strategies. In Chapter 2, I examined the epidemiology of sports and recreation related injury (SRI) in Ireland, an area that has previously been poorly studied. We report on 3,172 SRI (14% of total presentations) presentations to the ED over 6 months. Paediatric patients (4-16 yrs) were over represented comprising 39.9% of all SRI presentation compared to 16% of total ED presentations and 18% of the general population. These injuries were serious (32% fractures) and though 49% of injuries occurred during organised competition/practice, 41.5% occurred during recreation-most often at home. In Chapter 3, I examined risk factors associated with hand injury in hurling. The previous chapter highlighted the importance of a firm evidence base underpinning treatment strategies. When measures to improve welfare are introduced not only must potential benefits be measured, so too must potential unwanted adverse outcomes. In this study I examined a cohort of adult hurlers who had presented to the ED with a hurling related injury in order to highlight the variables associated with hand injury in this population. I found the athletes who wore a helmet were far more likely (OR 3.15 95% CI (1.51-6.56) p= 0.002) to suffer a hand injury than athletes who did not. Very few of those interviewed (4.9%) used hand protection compared to 65% who used helmet and faceguard. The introduction of the helmet and faceguard in hurling has undeniably decreased the incidence of head and face injury in hurling. However in tandem with this intervention several observational studies have demonstrated an increase in the occurrence of hurling related hand injuries. This study highlights the importance of being cognisant of unanticipated or unintended consequences when implementing a new treatment or intervention. In Chapter 4, I examined the role of population screening as applied to sport and exercise. This is a controversial area –cardiac screening in the exercising population has been the subject of much debate. Specifically I define the prevalence of exercise induced bronchoconstriction (EIB) using a specifically designed sports specific field-testing protocol. In this study I found almost a third (29%) of a full international professional rugby squad had confirmed asthma or EIB, as compared with 12-15% of the general population. Despite regular medical screening, 5 ‘new’ untreated cases (12%) were elicited by the challenge test and in the group already on treatment for asthma/EIB; over 50% still displayed EIB. In Chapter 5, I examined the evidence supporting current treatment options for iliotibial band friction syndrome (ITBFS). The practice of sports medicine has traditionally been ‘eminence based’ rather than ‘evidence based’. This may be problematic as some of these practices are based upon flawed principles- for example the treatment of iliotibial band friction syndrome (ITBFS). In this chapter, using cadaveric and biomechanical studies I expand upon the growing base of evidence clarifying the anatomy and biomechanics of the area-thereby re-examining the principles on which current treatments are based. The role of the SEM specialist is broad; we chose to examine specific examples of some of the roles that they execute. An understanding of the epidemiology of SRI presenting to the ED has implications for individual patients, sports governing bodies and health resource utilisation. Population screening is an important tool in health promotion and disease prevention in the general population. Screening in SEM may have similar less well-recognised benefits. The SEM specialist needs to be conversant in screening for medical conditions concerning physical activity. A comprehensive understanding of the pathophysiology of a disease is required for its diagnosis and treatment. Due to the ongoing evolution of SEM many treatments are eminence-based rather than evidence‐based practice. Continued re-examination of the fundamentals of current practice is essential. An awareness of potential unwanted side effects is essential prior to the introduction of any new treatment or intervention. The SEM specialist is ideally placed to advise sports governing bodies on these issues prior to and during their implementation.

This study examines the role of race, socioeconomic status, and individualism-collectivism as moderators of the relationship between selected work and family antecedents and work-family conflict and evaluates the contribution of energy-based conflict to the work-family conflict (WFC) research. The study uses data obtained from a survey questionnaire given to 414 participants recruited from an online labor market. Study hypotheses were tested through structural equation modeling. The results indicate that while moderating effects were slight, a proposed model where energy-based conflict is included outperforms traditional time/strain/behavior-based models and that established variables may drop to non-significance when additional variables are included in prediction. In addition, novel individual difference variables such as individualism and collectivism were demonstrated to have effects beyond moderating antecedent-outcome relationships in the model. The findings imply that WFC models would benefit from the inclusion of variables found in the current study.

DSDP 160 forms part of a series of sites in the eastern equatorial Pacific on the west flank of the East Pacific Rise. Earlier legs of the Deep Sea Drilling Project, in particular Legs 5 and 9, have reported sediments rich in oxides of iron and perhaps other transition metals just above basement in the eastern Pacific. These occurrences roughly define a broad zone on the west flank of the rise. Site DSDP 160 lies on this trend and were selected by the Pacific Site Selection Panel to test the extent of such deposits.

DSDP 161 is located on the lower west flank of the East Pacific Rise about midway between the Clipperton and Clarion fracture zones which define the boundaries of a large structural block in the eastern Pacific. The site is about 4,000 km west of the present crest of the Rise. It is located near the northern edge of a zone of thick Cenozoic sediments which marks the general location of the equatorial zone of high biological productivity.

Based on a radiocarbon and paleomagnetically dated sediment record from the northern Red Sea and the exceptional sensitivity of the regional changes in the oxygen isotope composition of sea water to the sea-level-dependent water exchange with the Indian Ocean, we provide a new global sea-level reconstruction spanning the last glacial period. The sea-level record has been extracted from the temperature-corrected benthic stable oxygen isotopes using coral-based sea-level data as constraints for the sea-level/oxygen isotope relationship. Although, the general features of this millennial-scale sea-level records have strong similarities to the rather symmetric and gradual Southern Hemisphere climate patterns, we observe, in constrast to previous findings, pronounced sea level rises of up to 25 m to generally correspond with Northern Hemisphere warmings as recorded in Greenland ice-core interstadial intervals whereas sea-level lowstands mostly occur during cold phases. Corroborated by CLIMBER-2 model results, the close connection of millennial-scale sea-level changes to Northern Hemisphere temperature variations indicates a primary climatic control on the mass balance of the major Northern Hemisphere ice sheets and does not require a considerable Antarctic contribution.

DSDP 162 is located due north of DSDP 161 on the lower west flank of the East Pacific Rise about 3900 km west of the crest. It is in the Clarion-Clipperton block, about 80 km south of the Clarion Fracture Zone. The site lies at the extreme northern edge of the zone of thick sediments that parallels the equator in the Pacific and marks the region of high biological productivity.

DSDP 159 is one of a series of sites in the eastern equatorial Pacific on the west flank of the East Pacific Rise. It was selected by the Pacific Site Selection Panel on the premise that if hydrothermal processes on the crest of the East Pacific Rise supply the transition metals, a broad zone of such deposits should be present immediately above basement over the entire flank of the Rise.

Prior to arrival on this site, the only survey data available was from the Vema-20 crossing of the area. The recommended site location was over a relatively smooth valley in the bottom topography at about 4750 meters (15,580 feet) depth (uncorrected), about 10 kilometers wide E-W between peaks (or ridges) on either side. Sediment thickness was unknown. The center of the valley is near the peak of a wide (40 to 50 kilometers) positive magnetic anomaly, identified as Magnetic Anomaly 30 in the hypothesized geomagnetic time scale with an age of 72 million years.

The Atlantic Advisory Panel proposed that Site 9 should be drilled on the northeastern flank of the Bermuda Rise (lat. 32° 37' N., long. 59° 10' W.), which is about 100 miles west of the Sohm Abyssal Plain. The bottom of this region consists of low linear ridges that are roughly parallel and oriented in a northwest-southeasterly direction. Scattered seamounts, some of which have peaks 2000 fathoms (3660 meters) below sea level, arise from the otherwise featureless sea floor between the ridges. The primary purpose in drilling Site 9 was to examine a sedimentary column where seismic reflectors were largely absent and to determine the age of sediments overlying acoustical basement in the examination of sea floor spreading.

The preliminary planning and approach to site 13 were taken from the JOIDES Atlantic Advisory Panel Report and from a previous detailed survey of the site by R/V Vema of the Lamont Geological Observatory. Several CSP profiles crossing the selected site in various directions show an uplifted portion of the sea floor roughly circular in shape of about 10 kilometers in diameter. In contrast to the smooth bottom of the surrounding abyssal plain, the topography of the small rise selected for the site has a small-scale roughness of amplitude of 40 to 80 meters. The work reported here is a biostratigraphic summary of available samples. Only the most important and biostratigraphically significant components of the faunas have been noted. No attempt has been made to give an exhaustive faunal analysis of the samples seen.

The northern margin of the Demerara Abyssal Plain is bordered by a low ESE-WNW trending rise known as the Barracuda Ridge. North of South America, the eastern margin of the Demerara Abyssal Plain is formed by the Barbados Ridge. An extensive sedimentary section of the Barbados Ridge is exposed on the island of Barbados and consists of a series of radiolarian to planktonic foraminiferal and calcareous nannofossil biogenic sediments interlayered with ash beds. To determine the nature of the sediments and "basement," Site 27 was selected on the northern margin of the Demerara Abyssal Plain, south of the Barracuda Ridge.

Objectives: Coaches are a primary influence on athletes' development in youth sport (Horn, 2008). However, the intervention tone of coaches' behaviour has not been directly observed. The purpose of this study was to examine associations between the intervention tone exhibited by youth sport coaches and athletes' individual developmental trajectories over the course of a season. Design: Short-term longitudinal study with behavioural observation. Method: Fifty-five athletes and their coaches from five youth volleyball teams were observed at three time points, and the intervention tone of interactive behaviour was systematically coded and organized by coach-athlete dyad. Athletes completed measures of the 4C's of athlete development (competence, confidence, connection, character) at each time point, which were used to create individualized developmental trajectories. Person-centred analyses were used to examine associations between athletes' developmental trajectories and their unique interactive experiences with their coach. Results: Cluster analysis revealed the presence of three distinct clusters based on athletes' developmental trajectories: 1) high and increasing, 2) low and decreasing, and 3) moderate and maintaining, with athletes from each team distributed across clusters. Analysis of dyadic interaction profiles revealed significant differences in interactive behaviour between clusters. Conclusions: Results suggest that differences in coach-athlete interactive experiences are associated with different developmental trajectories over the course of a season, even for athletes working with the same coach, highlighting the individualized nature of coaches' influence on young athletes. Practical implications for coaches include a critical awareness of their unique interactive relationship with each athlete independently, as well as the importance of fostering these relationships with regard to young people as more than just athletes.

Valproic acid (VPA), a commonly-used anticonvulsant drug, is associated with increased risk of fetal malformations, including neural tube defects (NTDs). Previous in vivo studies determined that VPA-exposed embryos with a NTD had altered expression of several proteins regulated by p300, a histone acetyltransferase (HAT) protein. p300 is capable of acetylating histones and non-histone proteins through its HAT activity, allowing it to transcriptionally regulate genes as well as modulate the stability and activity of specific proteins. NFκB, Stat3 and Egr1, all of which function as transcription factors, are regulated by p300 through its HAT activity. Together, these proteins all play an important role in maintaining the balance of apoptosis, proliferation and differentiation, the regulation of which is extremely important for proper embryonic development. The studies in this thesis utilized P19 embryonal carcinoma (EC) cells in order to determine the effects of VPA exposure on the expression of p300 and the aforementioned transcription factors, as well as apoptosis and proliferation, in vitro. P19 EC cells were exposed to C646, a selective p300 inhibitor, in order to assess whether the effects observed as a result of VPA exposure were due to p300 protein degradation. It was found that VPA exposure for 24 hours in P19 EC cells in vitro resulted in a significant decrease in p300 protein expression. VPA exposure also significantly decreased NFκB protein expression, while resulting in increased Stat3 protein expression. However, Stat3 acetylation and phosphorylation, which both contribute to Stat3 activation, were significantly decreased as a result of VPA exposure. p300 inhibition resulted in a significant decrease in NFκB, similar to what was observed as a result of VPA exposure, which suggests that VPA-mediated degradation of p300 may play a role in reduced NFκB protein expression following VPA exposure. Conversely, Stat3 protein expression, acetylation and phosphorylation were not significantly changed as a result of p300 inhibition, suggesting that p300 degradation does not play a role in VPA’s effects on Stat3 protein expression and activation. VPA exposure also resulted in a significant increase in apoptosis, while p300 inhibition did not significantly increase apoptosis. These data suggest that p300 degradation plays a role in VPA-mediated teratogenicity, and that VPA may target other cellular mechanisms in order to exert its teratogenic effects.

Previous research has examined young children’s ability to detect who would be most likely to provide help to others in a given situation, but little is known about their ability to intervene based on this knowledge in a real-life setting. In the current study, 48 three-year-old children chose between two actors to retrieve an out-of-reach object for the Experimenter; one actor was physically incapable of providing the object (blocked by a tall barrier), and one was capable. Participants’ looking behaviour between the two actors during the study was also recorded and analyzed as an additional, nonverbal measure of their prediction about who would help. Approximately half of the participants in the sample actively intervened on behalf of the Experimenter, but only after a direct request for help was made. Though the other participants did not engage in this helping behaviour, they chose the unblocked actor to help the Experimenter in a subsequent interview. Children also spent more time looking at the unblocked actor. Secondary analyses indicated that shyness prevented many children in the study from asking for help on behalf of the Experimenter from one of the actors. Finally, an unexpected side bias for looking behaviour toward the actors was found that has implications for how the study design could be improved for future research.
