Objectives: To evaluate outcome of patients treated "off-label" by bosentan and/or sildenafil for chronic thromboembolic pulmonary hypertension (CTEPH). Patients and methods: Since 2003, 18 patients (mean age 69 ± 11 years) have been treated with bosentan and/or sildenafil for CTEPH (mean pulmonary arterial resistance 8.1 ± 3.7 U Wood) in Lausanne University Hospital, with a follow-up of at least 12 months. Sixteen of them were inoperable because of distal disease and/or age or significant co-morbidities and 2 had persistent or recurrent pulmonary hypertension despite surgery. Efficacy of treatment was evaluated by comparison of New York Heart Association functional class (NYHA), six-minute walk test (6-MWT) and serum levels of N-terminal-pro brain natriuretic peptide (NT pro-BNP) at baseline (T0) and at 12 months (T12). Wilcoxon rank test was used for statistics. Results: At T0, median NYHA class was III (range II-IV), 6-MWT was 348 meters (5 and 95 centiles:0, 539) and NT pro-BNP was 387 mmol/l (58, 3508). At T12, 11 patients were treated with bosentan, 5 with sildenafil, 1 with inhaled Iloprost (because of failure of the two other treatments) and 1 with a combination of sildenafil and Iloprost. NYHA had improved in 10 patients, remained stable in 7 and worsened in 1 (median decrease 0.5 (-2; 0.2) p = 0.013). Six-MWT improved by a median of 15 meters (-142, +270) (p = 0.047) and NT pro-BNP decreased by a median of 65 mmol/l (-2988, +187) (p = n.s.). Among the 10 patients with a follow-up of 2 years or longer, two thirds remained stable and one third had worsened at 24 month. Treatments were well tolerated and only one patient had significant side effects (cutaneous reaction to bosentan) necessitating a switch to another treatment. Conclusion: In agreement with published data, bosentan and sildenafil improved functional status (NYHA, 6-MWT) and haemodynamics (NT pro BNP) in our patients with inoperable CTEPH. However these medications should not be used as substitute for surgery when the latter is applicable.

Este estudo teve como objetivo identificar perfil sociodemográfico e clínico, história de hospitalizações por Insuficiência Cardíaca (IC) e seguimento (consultas regulares, tratamento medicamentoso, fatores facilitadores e dificultadores do seguimento) do paciente internado por quadro de descompensação clínica. Foram entrevistados 61 pacientes com idade média de 58,1 (± 15,9) anos, 3,5 (± 4,4) anos de estudo e renda individual de 1,3 (± 2,4) salários-mínimos. A maioria dos sujeitos se encontrava em classe funcional III ou IV da New York Heart Association, tendo como causa mais freqüente de hospitalização, os sinais/sintomas da forma congestiva da IC. 75,4% dos sujeitos relataram acompanhamento clínico, porém de periodicidade irregular. Constatou-se utilização de terapêutica medicamentosa em proporção inferior à recomendada pela literatura. Os achados devem auxiliar a identificação dos pacientes com maior risco de descompensação da IC e assim, desenhar e implementar intervenções específicas visando a redução das re-hospitalizações por IC.

In this paper we present the theoretical and methodologicalfoundations for the development of a multi-agentSelective Dissemination of Information (SDI) servicemodel that applies Semantic Web technologies for specializeddigital libraries. These technologies make possibleachieving more efficient information management,improving agent–user communication processes, andfacilitating accurate access to relevant resources. Othertools used are fuzzy linguistic modelling techniques(which make possible easing the interaction betweenusers and system) and natural language processing(NLP) techniques for semiautomatic thesaurus generation.Also, RSS feeds are used as “current awareness bulletins”to generate personalized bibliographic alerts.

Using paradata gathered from the 11-nation Survey of Health, Ageing and Retirement in Europe (SHARE), this paper examines the impact of the first contact attempt and the first contact properties, respectively, on contact and response efficiency using logistic multilevel models. We find that despite the different sample frames and interviewer compensation structure between countries, there are no considerable country effects with respect to making contact, once interviewer effects are controlled. Moreover, results point to an increased efficiency associated with evenings especially on Sundays, at least on the very first contact attempt. For attempts that result in initial contact, Saturday afternoons are most likely to eventually lead to completed interviews, followed by initial contact on weekdays during the daytime. We hypothesize that this may be due to the SHARE sample being composed of people aged 50 and over.

The short and the long-term results of our experience with 25 consecutive patients who underwent multivalvular surgery for infective endocarditis are analysed. Preoperatively, 20/25 (80%) patients were in New York Heart Association (NYHA) stage III or IV, and 2/25 (8%) patients were in cardiogenic shock. All the diseased valves were replaced with mechanical bileaflet prosthesis except seven mitral valves and one tricuspid valve, which could be repaired. Major postoperative complications occurred in 3/25 (12%) patients: a fatal cerebral haemorrhage, a reversible cerebellar syndrome and an intractable heart failure, which required transplantation. During a mean follow-up of 4.7 years (range 6 months to 16.8 years), 7/25 (28%) patients suffered from valve-related complications: five bleedings (one died), one embolic event and one prosthetic valve thrombosis. The actuarial freedom of valve-related event at 10 years was 61.8 +/- 12.4%. There was no prosthetic endocarditis. At follow-up, 20/21 (95%) survivors were in NYHA stage I or II. Long-term outcome in our patient population operated on for multivalvular endocarditis, is satisfactory with no recurrent infection and excellent functional results.

An incentives based theory of policing is developed which can explain the phenomenon of random “crackdowns,” i.e., intermittent periods of high interdiction/surveillance. For a variety of police objective functions, random crackdowns can be part of the optimal monitoring strategy. We demonstrate support for implications of the crackdown theory using traffic data gathered by the Belgian Police Department and use the model to estimate the deterrence effectof additional resources spent on speeding interdiction.

Resumo Com o objectivo de modernizar o sistema contabilístico de Cabo-Verde foi introduzido em Janeiro de 2009, em substituição do Plano Nacional de Contabilidade (PNC), um novo sistema de normalização contabilística através do Decreto-Lei n.º 5/2008, de 5 de Fevereiro. O novo normativo por se tratar de um modelo de normalização assente mais em princípios do que em regras explicitas, faz com que os dirigentes das empresas elaborem as suas demonstrações financeiras de acordo com as necessidades de gestão e consequentemente independente das regras fiscais. Contudo, a determinação da matéria colectável é feita com base nas informações contabilísticas produzidas, de acordo com SNCRF, pelos dirigentes das empresas. O que se pretende com o presente trabalho é analisar a importância das informações contabilísticas ou demonstrações financeiras, elaboradas no âmbito do SNCRF, na determinação da matéria colectável de um contribuinte de verificação pela administração fiscal. A metodologia utilizada para a concretização desta pesquisa tendo em conta os objectivos traçados foram exploratória e a descritiva, através do qual conseguiu-se descrever aspectos teóricos relacionado ao termo contabilidade e fiscalidade, realizou-se ainda um caso prático na Agência Polar, com o fim de se conhecer na prática os procedimentos inerentes a determinação da matéria colectável e qual o papel da informação contabilística nesse processo. Com intuito de responder o problema da pesquisa optou-se por análise de processos da Polar, em arquivo nas repartições de finanças, referente ao ano 2009 e 2010. Actualmente, na nossa sociedade apresenta desafios, está continuamente em busca de padrões éticos para uma melhoria da sociedade, é um padrão de grande importância para crescimento e desenvolvimento da Administração Publica cabo-verdiana (APCV). A gestão da coisa Pública é um dos fundamentos de” boa governação”. De acordo com os dados da pesquisa, a ética na APCV é um dos padrões que pouco se poem em pratica. Neste sentido o presente trabalho visa analisar” Ética na Administração Publica”, tendo como estudo de caso o Tribunal de Contas de Cabo Verde (TCCV). Para alcançar o objectivo deste trabalho, fez-se uma pesquisa bibliográfica sobre a ética na Administração Publica e sobre teorias relacionadas com a ética nas organizações. Utilizou-se o método inquisitivo baseado em questionários com perguntas abertas e fechadas para obter as percepçoes dos dirigentes públicos. O tratamento de dados foi feito recorrendo o Software SPSS versão 17 e Excel. Portanto na realização deste estudo foi utilizado o modelo American Psychological Association (APA) no que refere as citações e referências bibliográficas. Os resultados do estudo demonstraram que a ética é um dos instrumentos de extrema relevância para gestão da coisa pública.

Nonlinear regression problems can often be reduced to linearity by transforming the response variable (e.g., using the Box-Cox family of transformations). The classic estimates of the parameter defining the transformation as well as of the regression coefficients are based on the maximum likelihood criterion, assuming homoscedastic normal errors for the transformed response. These estimates are nonrobust in the presence of outliers and can be inconsistent when the errors are nonnormal or heteroscedastic. This article proposes new robust estimates that are consistent and asymptotically normal for any unimodal and homoscedastic error distribution. For this purpose, a robust version of conditional expectation is introduced for which the prediction mean squared error is replaced with an M scale. This concept is then used to develop a nonparametric criterion to estimate the transformation parameter as well as the regression coefficients. A finite sample estimate of this criterion based on a robust version of smearing is also proposed. Monte Carlo experiments show that the new estimates compare favorably with respect to the available competitors.

BACKGROUND: Registries are important for real-life epidemiology on different pulmonary hypertension (PH) groups. OBJECTIVE: To provide long-term data of the Swiss PH registry of 1998-2012. METHODS: PH patients have been classified into 5 groups and registered upon written informed consent at 5 university and 8 associated hospitals since 1998. New York Heart Association (NYHA) class, 6-min walk distance, hemodynamics and therapy were registered at baseline. Patients were regularly followed, and therapy and events (death, transplantation, endarterectomy or loss to follow-up) registered. The data were stratified according to the time of diagnosis into prevalent before 2000 and incident during 2000-2004, 2005-2008 and 2009-2012. RESULTS: From 996 (53% female) PH patients, 549 had pulmonary arterial hypertension (PAH), 36 PH due to left heart disease, 127 due to lung disease, 249 to chronic thromboembolic PH (CTEPH) and 35 to miscellaneous PH. Age and BMI significantly increased over time, whereas hemodynamic severity decreased. Overall, event-free survival was 84, 72, 64 and 58% for the years 1-4 and similar for time periods since 2000, but better during the more recent periods for PAH and CTEPH. Of all PAH cases, 89% had target medical therapy and 43% combination therapy. Of CTEPH patients, 14 and 2% underwent pulmonary endarterectomy or transplantation, respectively; 87% were treated with PAH target therapy. CONCLUSION: Since 2000, the incident Swiss PH patients registered were older, hemodynamically better and mostly treated with PAH target therapies. Survival has been better for PAH and CTEPH diagnosed since 2008 compared with earlier diagnosis or other classifications.

El presente trabajo consiste en la selección, análisis y traducción de 8 documentos con especialidad médica publicados por la institución sanitaria NorthShore University Health System. La elección de esta temática en la elaboración de este proyecto está vinculada al hecho de que en la actualidad trabajo como intérprete con especialidad en medicina para esta organización. Durante el año y medio que llevo en este organismo, he podido observar que existe un gran número de artículos, formularios, folletos informativos, consentimientos quirúrgicos y un largo etcétera que no están traducidos al castellano, cuestión que dificulta enormemente la comunicación, entendimiento y funcionamiento de las relaciones entre los pacientes, familiares y el equipo médico. El NorthShore University Health System tiene un Departamento de Interpretación con 12 intérpretes de castellano en plantilla, 3 intérpretes de ruso, 2 intérpretes de polaco, un intérprete de coreano y un intérprete de árabe. Lamentablemente, el NorthShore no posee un Departamento de Traducción para la traducción de los documentos destinados a los pacientes. El Departamento de Interpretación, bajo la dirección de la supervisora Erika Erdbeer, contrata a una agencia de traducción certificada por la American Translators Association para la traducción de estos documentos. Este proceso supone un alto costo para el Departamento de Interpretación en particular, y para la institución sanitaria en general. De igual modo, el volumen de documentos en necesidad de ser traducidos es sustancialmente más elevado que los recursos económicos disponibles para la traducción de los mismos, y esto supone que no haya presupuesto para traducir muchos de los textos esenciales. En términos generales, esta situación va en detrimento de los derechos e intereses de todos aquellos pacientes que tienen un conocimiento limitado del inglés.A su vez, la labor de los intérpretes se hace muy dificultosa debido al hecho de que en numerosas ocasiones tenemos que realizar traducciones a la vista (sight translate) de una variedad de permisos, formularios, documentos legales, hojas de consentimiento y un largo etcétera que deberían estar traducidos al castellano, y que dada la amplia carga de trabajo que tenemos, no podemos emplear el tiempo necesario para ofrecer al paciente una traducción oral de calidad. Por consiguiente, en un intento por mejorar esta situación, y con el propósito de ofrecer a la comunidad hispanoparlante que acude a esta institución unos servicios de calidad, hemos querido diseñar un proyecto basado en la selección y traducción de los documentos médicos más utilizados por los pacientes hispanoparlantes. Para el proceso de traducción hemos utilizado la herramienta de Traducción Asistida SDL Trados Studio 2009, con la intención de crear una memoria de traducción que pueda ser utilizada en futuros proyectos. Las traducciones realizadas en este trabajo serán publicadas y distribuidas en los pertinentes departamentos del NorthShore. Como paso previo a la publicación de estos documentos, Erika Erdbeer, supervisora del Departamento de Interpretación, enviará las traducciones a la agencia de traducción MetaPhrasis para ser corregidas y revisadas. Por motivos de responsabilidad legal, el NorthShore University Health System tiene la obligación de contratar traductores certificados por la American Translators Association. En la actualidad la autora de este proyecto se encuentra en el proceso de sacar dicha certificación, por lo que será necesario enviar las traducciones para que sean corregidas y revisadas a un traductor/a certificado. Tras la revisión y corrección de los documentos el NorthShore University Health System procederá a la publicación y distribución de los mismos tan pronto lo estime conveniente.

BACKGROUND: The long-term incidence of stent thrombosis (ST) and complications after sirolimus-eluting stents (SES) implantation is still a matter of debate. METHOD: We conducted a systematic follow-up on the day of their 5-year SES implantation anniversary, in a series of consecutive real-world patients treated with a SES. The use of SES implantation was not restricted to "on-label" indications, and target lesions included in-stent restenosis, vein graft, left main stem locations, bifurcations, and long lesions. The Academic Research Consortium criteria were used for ST classification. RESULTS: Three hundred fifty consecutive patients were treated with SES between April and December 2002 in 3 Swiss hospitals. Mean age was 63 +/- 6 years, 78% were men, 20% presented with acute coronary syndrome, and 19% were patients with diabetes. Five-year follow-up was obtained in 98% of eligible patients. Stent thrombosis had occurred in 12 patients (3.6%) [definite 6 (1.8%), probable 1 (0.3%) and possible 5 (1.5%)]. Eighty-one percent of the population was free of complications. Major adverse cardiac events occurred in 74 (21%) patients and were as follows: cardiac death 3%, noncardiac death 4%, myocardial infarction 2%, target lesion revascularization 8%, non-target lesion revascularization target vessel revascularization 3%, coronary artery bypass graft 2%. Non-TVR was performed in 8%. CONCLUSION: Our data confirm the good long-term outcome of patients treated with SES. The incidence of complications and sub acute thrombosis at 5 years in routine clinical practice reproduces the results of prospective randomized trials.

S'analitzen de manera comparativa les directrius de biblioteques de presons de l'IFLA, la Library Association (LA) i l'American Library Association (ALA). Després de fer un breu repàs del procés d'elaboració de cadascuna d'aquestes directrius, es comparen les tres propostes dividides en nou apartats: administració, pressupost, personal, localització i condicionament, serveis, col·lecció, catàleg, accessibilitat i promoció. A partir d'aquesta anàlisi, es fa una recomanació sobre quina directriu podria ser el punt de partida per desenvolupar un text similar que es pogués aplicar a les biblioteques de presons de Catalunya.

The recent release of the fifth edition of the Diagnostic and Statistical Manual of Mental Disorders (DSM-5) by the American Psychiatric Association has led to much debate. For this forum article, we asked BMC Medicine Editorial Board members who are experts in the field of psychiatry to discuss their personal views on how the changes in DSM-5 might affect clinical practice in their specific areas of psychiatric medicine. This article discusses the influence the DSM-5 may have on the diagnosis and treatment of autism, trauma-related and stressor-related disorders, obsessive-compulsive and related disorders, mood disorders (including major depression and bipolar disorders), and schizophrenia spectrum disorders.
