Progressive multifocal leukoencephalopathy (PML) caused by reactivation of the JC virus (JCV), a human polyomavirus, occurs in autoimmune disorders, most frequently in systemic lupus erythematosus (SLE). We describe a HIV-negative 34-year-old female with SLE who had been treated with immunosuppressant therapy (IST; steroids and azathioprine) since 2004. In 2011, she developed decreased sensation and weakness of the right hand, followed by vertigo and gait instability. The diagnosis of PML was made on the basis of brain MRI findings (posterior fossa lesions) and JCV isolation from the cerebrospinal fluid (700 copies/ml). IST was immediately discontinued. Cidofovir, mirtazapine, mefloquine and cycles of cytarabine were sequentially added, but there was progressive deterioration with a fatal outcome 1 year after disease onset. This report discusses current therapeutic choices for PML and the importance of early infection screening when SLE patients present with neurological symptoms. In the light of recent reports of PML in SLE patients treated with rituximab or belimumab, we highlight that other IST may just as well be implicated. We conclude that severe lymphopenia was most likely responsible for JCV reactivation in this patient and discuss how effective management of lymphopenia in SLE and PML therapy remains an unmet need.

The globalization of markets has confirmed for the processes of change in organizations both in structure and in management. This dynamic was also observed in credit unions because they are financial institutions and are under the rules of the Brazil´s Financial System. Given the context of organizational changes in the financial capital has played the traditional management reform is urgent. In organizations credit unions, given its dual purpose, because in the same organizational environment and capitalism coexist cooperative whose logics are antagonistic, but can live through the balance between instrumental rationality and substantive rationality in credit unions. Based on this concept a new form of management should be thought to be able to accommodate the demand of cooperative, community, government and the market. Hybridization has been observed in management practices` COOPERUFPA into dimensions financial, social and solidarity participation with a trend in paradigmatic form of hybrid management, in that it directly or indirectly affect the management decisions in the credit union. The hybrid management is a trend that has been setting the basis for societal transformation, so that credit unions promote actions of welfare oriented cooperative members and the community around the same time that attend the dynamics of market globalization. These actions, in the context of hybrid management should be implemented by COOPERUFPA from the sociability of the remains and the wide diffusion of solidarity culture between cooperative partnership as a way to recover their participation in trade relations, financial and the social collective developement. For the members of COOPERUFPA financial interest is evidenced in greater relevance for the social interest given its dominant relationship as "mere customer" of the credit union, however, the proactive participation of the life of the cooperative credit union is one of its expectative among of participation of to share power in decisions by general meetings. This passivity`s cooperator of the COOPERUFPA in defending the ideals overshadowed the spread of cooperative principles and values of cooperation among them. Thus his conception for COOPERUFPA in the financial dimension, social and solidarity democracy, performed transversely. The COOPERUFPA for not developing an education policy for the cooperation among its members, contributed to a process of collective alienation of cooperative ideals, since the cooperative do not understand the reality that surrounds them as members of an organization whose mission is to social and financial sustainability of its members

En este articulo se hace un análisis sobre la validez de la TBR (Total Bussines return) como medida de rentabilidad de unidades de negocio que pertenecen a un portafolio corporativo. Se establecen cuales son sus debilidades y en que casos es posible aceptarla como medida de rentabilidad asociada con la creación de valor. Se expone, la coincidencia matemática entre la TBR y el WACC (costo promedio ponderado de capital) a partir del descuento de flujos de caja proyectados y entre el CAV (creación adicional de valor) y la diferencia entre el ingreso económico obtenido y el ingreso económico esperado en un periodo determinado. También se evidencia la equivalencia entre el CAV y la variación del valor de las operaciones más la variación del flujo de caja del periodo. Posteriormente se realiza una aplicación para mostrar como puede utilizar la TBR y la teoría moderna de portafolio para que un gerente corporativo pueda controlar la relación rendimiento-riesgo existente entre las distintas unidades de negocio a cargo de su corporación.

Históricamente, en el Ecuador, la Provincia del Azuay es la de mayor experiencia emigratoria hacia el exterior y la que se ha beneficiado de importantes flujos de remesas, mismas que se incrementaron notablementecon la emigración provocada por la crisis económica de finales de la década de los noventa, llegando a representar en el año 2014 USD$ 540 millones de dólares. Esto, sumado a la desconfianza en el sector bancario, generó el nacimiento y crecimiento de muchas Cooperativas de Ahorro y Crédito como es el caso de Jardín Azuayo, que se constituyó en el año 1993 a raíz del desastre de la Josefina. Además, las remesas que se transfieren a través de las cooperativas tienen un notable incremento, contribuyendo los aspectos mencionados a la expansión y democratización del crédito, y al desarrollo de la provincia. En la presente investigación se utilizó información secundaria, proveniente de la Superintendencia de Bancos y Seguros, Superintendencia de Economía Popular y Solidaria, Banco Central del Ecuador, Instituto Nacional de Estadísticas y Censos, Cooperativa de Ahorro y Crédito Jardín Azuayo, entre otros. Además se trabajó con una muestra de créditos concedidos por la Cooperativa Jardín Azuayo. Los resultados obtenidos permiten visualizar la expansión democratización del crédito yel verdadero destino de éste y proporcionan los elementos para el establecimiento de políticas que permitan el apoyo a las Cooperativas y a los sectores que requieren el financiamiento, especialmente para fines productivos.

Conditionalities – i.e. ‘exchanging finance for policy reform’ in an asymmetrical relationship between the ‘donor’ and the ‘recipient’ – are central mechanisms of the reform programmes of international financial institutions (IFIs). As they are imposed by outside entities, they can also be viewed as ‘policy externalisation’, which is paradoxically a massive intrusion in the shaping of a country’s domestic policies. The resilience of such devices is remarkable, however. Indeed, in the early 1980s, many developing countries were facing balance of payments difficulties and called upon these international financial institutions for financial relief. In exchange for this relief, they devised economic reforms (fiscal, financial, monetary), which were the conditions for their lending. These reforms were not associated with better economic performance, and this led the IFIs to devise in the 1990s different reforms, which this time targeted the functioning of the government and its ‘governance’, economic problems being explained by governments’ characteristics (e.g., rent-seekers). The paper demonstrates the limitations of the device of conditionality, which is a crucial theoretical and policy issue given its stability across time and countries. These limitations stem from: i) the concept of conditionality per se - the mechanism of exchanging finance for reform; ii) the contents of the prescribed reforms given developing countries economic structure (typically commodity-based export structures) and the weakness of the concept of ‘governance’ in view of these countries’ political economies; and iii) the intrinsic linkages between economic and political conditionalities, whose limitations thus retroact on each other, in particular regarding effectiveness and credibility.

Resumen La operación de las actividades económicas y su competitividad, analizadas desde una perspectiva de cadenas globales de mercancías, dependen del funcionamiento de los mercados de crédito, tanto nacionales como internacionales. La operación normal de los negocios requiere crédito para fortalecer sus recursos propios y hacer las compras de materia prima, insumos, pago de salarios e inversión. En todos los casos, el crédito puede ser utilizado para mejorar el diseño de productos, impulsar el acceso a nuevos mercados y aumentar la competitividad. A los productores de la región de Los Santos en Costa Rica se les reconoce por innovadores, y al café de la zona por ser un café de altura y de gran calidad. El artículo explora las necesidades de financiamiento de productores, beneficiadores y exportadores de café de Los Santos, sus fuentes de financiamiento y el uso dado a los recursos obtenidos. Entre las necesidades orientadas al mejoramiento de la competitividad se encontraron la renovación de cafetales entre los productores, el mantenimiento de proyectos y tecnología entre los beneficios y la generación de microbeneficios y búsqueda de nichos de mercado entre los exportadores. Abstract From a global commodity chains perspective, economic activities and their competitiveness depend upon the functioning of credit markets, both international and domestic. Current businesses operations require credit to enhance their own resources in order to buy raw material and inputs, pay wages and make investment. Credit may also be used to improve product design, enter new markets and boost competitiveness as a result. High altitude grown coffee from Los Santos, Costa Rica is widely known for its quality, and their growers as innovative. This paper addresses credit needs of coffee growers, processors and exporters in Los Santos, the credit sources they have access to, and the kind of uses the credit is given. Improving competitiveness uses of credit were found among growers, processors and exporters, such as crop and technology improvement, micro processing facilities and new markets search as well.

El propósito de la presente investigación es el de analizar la debilidad que ha tenido el Estado colombiano con respecto a la obtención del control territorial en algunas áreas específicas del departamento del Valle del Cauca, considerando variables fundamentales como la coerción, el capital y la justicia, en su mayoría planteadas por Charles Tilly como elementos básicos de cualquier Estado. Se analizan y explican los periodos de violencia, y la consolidación de grupos al margen de la ley en Colombia y en el departamento del Valle del Cauca, evidenciando la debilidad del Estado en ciertas áreas a lo largo de los años, tomando finalmente como estudio de caso el surgimiento de la banda criminal de “los Rastrojos” posterior a la desmovilización de las AUC, como actor al margen de la ley que disputa el poder y ejerce ilegalmente funciones del Estado.

La economía mundial cambia a un ritmo vertiginoso y exponencial gracias a la rápida transformación de la tecnología. Diferentes dinámicas como el crecimiento en cobertura del internet; la creciente facilidad de obtención de una tarjeta de crédito; la popularización del uso de smartphones; y el crecimiento en uso del comercio electrónico; han dado cabida a la aparición de nuevos tipos de negocio, como el de servicios electrónicos y aplicaciones, que hace algunas décadas atrás eran inviables. Teniendo en cuenta estos cambios, el presente documento plantea tres diferentes modelos de aplicación para smartphones, se hace un análisis detallado de la viabilidad para cada uno para identificar así el que cuenta con mayores probabilidades de éxito. Finalmente se profundiza en este con un análisis financiero y de mercadotecnia para así hacer las respectivas correcciones al modelo inicial y obtener como resultado la versión más viable del modelo seleccionado.

Transdisciplinarity gained importance in the 1970s, with the initial signs of weakness of both multi- and interdisciplinary approaches. This weakness was felt due to the increased complexity in the social and technological landscapes. Generally, discussion over the transdisciplinary topic is centred in social and health sciences. Therefore, the major challenge in this research is to adapt design research to the emerging transdisciplinary discussion. Based on a comparative and critical review of several engineering and design models for the design process, we advocate the importance of collaboration and conceptualisation for these disciplines. Therefore, a transdisciplinary and conceptual cooperation between engineering and industrial design disciplines is considered as decisive to create breakthroughs. Furthermore, a synthesis is proposed, in order to foster the cooperation between engineering and industrial design.

Resumen Trata el tema de las revueltas indígenas en el salvador durante la primera mitada del siglo XIX, especialmente el levantamiento indígena de 1846 en Santiago de Nonualco, teniendo en cuenta los conflictos locales , los factores étnicos y la debilidad del aparato estatal. Abstract This essay analyzes indigenous revolts in el Salvador in the first half of the nineteenth century, especially the indigenous uprising of 1846 in Santiago Nonualco, takin in to consideration local conflicts, ethnic factors, and the weakness of the state apparatus.

Extreme sports and extreme sports participants have been most commonly explored from a negative perspective, for example the “need to take unnecessary risks.” This study explored what can be learned from extreme sports about courage and humility - two positive psychology constructs. A phenomenological method was used via unstructured interviews with 15 extreme sports participants and other first hand accounts. The extreme sports included B.A.S.E. jumping, big wave surfing, extreme skiing, waterfall kayaking, extreme mountaineering and solo rope-free climbing. Results indicate that humility and courage can be deliberately sought out by participating in activities that involve a real chance of death, fear and the realisation that nature in its extreme is far greater and more powerful than humanity.
