Modellistica di accensione per candele ad arco elettrico in motori a combustione interna alternativi

Nel panorama mondiale di contenimento delle emissioni inquinanti in atmosfera é divenuto sempre più importante limitare la parte derivante dai motori a combustione interna: l'utilizzo di motori GDI a carica stratificata e di ricircolo dei gas di scarico (EGR) sono esempi di tecnologie pensate proprio in tale ottica. Sia la presenza di un ambiente magro che di EGR nel cilindro, come anche l'aumento della pressione nel cilindro per l'incremento della pressione di sovralimentazione o del rapporto di compressione, hanno lo svantaggio di limitare la velocità di combustione e rendere più sfavorevoli le condizioni di accensione della miscela; in questo scenario diviene di fondamentale importanza il miglioramento dei sistemi di accensione, la creazione di modelli volti a simularli e la comprensione dei fenomeni che ne stanno alla base. Il seguente lavoro di tesi si inserisce proprio in questo contesto, indagando le varie fasi di cui si compone il fenomeno dell'accensione e le relazioni che legano le variabili di interesse fisico, verificate sulla base di evidenze sperimentali. Successivamente vengono analizzati i principali modelli d'accensione che sono stati proposti e implementati in codici computazionali fluidodinamici; l'analisi mette in luce le differenze, i punti di forza e le semplificazioni introdotte in ognuno di essi, in modo da poterli valutare criticamente. La suddetta analisi é anche utile per introdurre il modello frutto del lavoro del gruppo di ricerca dell'Università di Bologna; ci si concentra particolarmente su quest'ultimo poiché un obiettivo di questo lavoro di tesi é stato proprio l'implementazione e l'utilizzo del modello in un codice fluidodinamico tridimensionale quale CONVERGE CFD. L'implementazione é stata poi validata attraverso simulazioni su una geometria reale di un motore a combustione interna ad elevate prestazioni, confrontando i risultati ottenuti con il nuovo modello rispetto ai dati sperimentali sulla combustione.

Negli ultimi decenni, l’outcome dei pazienti sottoposti a chirurgia dell’aorta toracica è migliorato grazie ai progressi ottenuti nella gestione della circolazione extracorporea, della protezione miocardica e cerebrale e del monitoraggio intra- e postoperatorio dei pazienti. Malgrado questo però, la chirurgia dell’arco aortico è ancora gravata da una significativa mortalità e morbilità dovute principalmente alle complicanze neurologiche conseguenti all’interruzione temporanea della circolazione cerebrale. Nel presente studio sono stati analizzati 364 pazienti trattati per sostituzione dell’arco aortico con l’ausilio della perfusione cerebrale selettiva anterograda, presso l’Unità Operativa di Cardiochirurgia dell’Ospedale di Sant’Orsola di Bologna dal 1° gennaio 2015 al 31 maggio 2020.Le disfunzioni neurologiche permanenti sono state diagnosticate in 33 dei 364 pazienti (9.1%) operati, utilizzando la perfusione cerebrale selettiva anterograda come tecnica di perfusione cerebrale. Pertanto abbiamo confrontato due gruppi di pazienti: un gruppo di 33 pazienti con deficit permanenti con il resto dei pazienti (331) che non aveva alcun deficit. Tra le variabili intraoperatorie, i tipi di cannulazione arteriosa, in particolare la cannulazione dell’arteria femorale (109 [32.9%] vs 17 [51.5%], p-value=0.032) e dell’aorta ascendente (56 [16.9%] vs 0, p-value=0.010), presentano delle differenze statisticamente significative tra i due gruppi. Per quanto riguarda la perfusione cerebrale selettiva anterograda, i valori del flusso medio (899.63 ± 214.12 vs 952.52 ± 160.72, p-value=0.051) presentano una differenza statisticamente significativa tra i due campioni. All’analisi multivariata, la dissezione aortica di tipo A (p = 0.010), la cannulazione femorale (p = 0.033) e l’arresto di circolo totale (p = 0.046) si confermano dei fattori di rischio statisticamente significativi per l’insorgenza di disfunzioni neurologiche permanenti

A Atenção Primária à Saúde (APS) deve ser o contato preferencial dos clientes com o sistema de saúde e, por isso, é um dos componentes fundamentais no atendimento. A atenção qualificada faz-se necessária para garantir ampliação do acesso, humanização da assistência, fortalecimento do vínculo entre usuários e profissionais de saúde, equidade e integralidade nas unidades de saúde. Busca-se otimizar o processo de trabalho e, ao mesmo tempo, oferecer um atendimento resolutivo e satisfatório. Nesta perspectiva, o acolhimento se torna uma ferramenta indispensável para a reorganização do processo de trabalho em saúde, na medida em que possibilita a qualificação do acesso dos clientes aos serviços oferecidos pela equipe de saúde da família. O presente projeto de intervenção objetiva implantar uma estratégia de acolhimento com base em classificação de risco visando melhor acolhimento dos clientes. Para isso foram realizados fluxogramas de acordo com os ciclo vida e escolha de um protocolo para priorização da atenção aos mais vulneráveis e com maior risco, em detrimento ao atendimento por ordem de chegada. Como resultado obtivemos uma maior aproximação dos clientes com a equipe, os atendimentos foram realizados de acordo os critérios clínicos estabelecidos no protocolo de acolhimento da unidade no tempo certo no tempo certo e de forma segura, ética e com qualidade.

To evaluate the efficacy and safety of intravitreal bevacizumab (IVB) in eyes with neovascular glaucoma (NVG) undergoing Ahmed glaucoma valve (AGV) implantation. This was a multicentre, prospective, randomized clinical trial that enrolled 40 patients with uncontrolled neovascular glaucoma that had undergone panretinal photocoagulation and required glaucoma drainage device implantation. Patients were randomized to receive IVB (1.25 mg) or not during Ahmed valve implant surgery. Injections were administered intra-operatively, and 4 and 8 weeks after surgery. After a mean follow-up of 2.25 ± 0.67 years (range 1.5-3 years), both groups showed a significant decrease in IOP (p < 0.05). There was no difference in IOP between groups except at the 18-month interval, when IOP in IVB group was significantly lower (14.57 ± 1.72 mmHg vs. 18.37 ± 1.06 mmHg - p = 0.0002). There was no difference in survival success rates between groups. At 24 months, there was a trend to patients treated with IVB using less antiglaucoma medications than the control group (p = 0.0648). Complete regression of rubeosis iridis was significantly more frequent in the IVB group (80%) than in the control group (25%) (p = 0.0015). Intravitreal bevacizumab may lead to regression of new vessels both in the iris and in the anterior chamber angle in patients with neovascular glaucoma undergoing Ahmed glaucoma valve implantation. There is a trend to slightly lower IOPs and number of medications with IVB use during AGV implantation for neovascular glaucoma.

A case of identical male twins with Cohen syndrome who present multiple ophthalmic findings is reported. The patients were identical 16 year-old twin boys who showed down slanting eyelids, mild ptosis, high-grade myopia, small cortical lens opacities, posterior subcapsular cataracts, myotic and corectopic pupils with poor dilation due to focal iris atrophy and retinochoroidal dystrophy. Ophthalmologists must be aware of the ocular and systemic findings of Cohen syndrome in the evaluation of young patients with mental retardation and visual impairment.

Objective To evaluate the occurrence of severe obstetric complications associated with antepartum and intrapartum hemorrhage among women from the Brazilian Network for Surveillance of Severe Maternal Morbidity.Design Multicenter cross-sectional study.Setting Twenty-seven obstetric referral units in Brazil between July 2009 and June 2010.Population A total of 9555 women categorized as having obstetric complications.Methods The occurrence of potentially life-threatening conditions, maternal near miss and maternal deaths associated with antepartum and intrapartum hemorrhage was evaluated. Sociodemographic and obstetric characteristics and the use of criteria for management of severe bleeding were also assessed in these women.Main outcome measures The prevalence ratios with their respective 95% confidence intervals adjusted for the cluster effect of the design, and multiple logistic regression analysis were performed to identify factors independently associated with the occurrence of severe maternal outcome.Results Antepartum and intrapartum hemorrhage occurred in only 8% (767) of women experiencing any type of obstetric complication. However, it was responsible for 18.2% (140) of maternal near miss and 10% (14) of maternal death cases. On multivariate analysis, maternal age and previous cesarean section were shown to be independently associated with an increased risk of severe maternal outcome (near miss or death).Conclusion Severe maternal outcome due to antepartum and intrapartum hemorrhage was highly prevalent among Brazilian women. Certain risk factors, maternal age and previous cesarean delivery in particular, were associated with the occurrence of bleeding.

Class III skeletal malocclusion may present several etiologies, among which maxillary deficiency is the most frequent. Bone discrepancy may have an unfavorable impact on esthetics, which is frequently aggravated by the presence of accentuated facial asymmetries. This type of malocclusion is usually treated with association of Orthodontics and orthognathic surgery for correction of occlusion and facial esthetics. This report presents the treatment of a patient aged 15 years and 1 month with Class III skeletal malocclusion, having narrow maxilla, posterior open bite on the left side, anterior crossbite and unilateral posterior crossbite, accentuated negative dentoalveolar discrepancy in the maxillary arch, and maxillary and mandibular midline shift. Clinical examination also revealed maxillary hypoplasia, increased lower one third of the face, concave bone and facial profiles and facial asymmetry with mandibular deviation to the left side. The treatment was performed in three phases: presurgical orthodontic preparation, orthognathic surgery and orthodontic finishing. In reviewing the patient's final records, the major goals set at the beginning of treatment were successfully achieved, providing the patient with adequate masticatory function and pleasant facial esthetics.

This article reports the case of a 55-year-old female patient who presented with unsatisfactory temporary crowns in the right mandibular premolars and molars, and a premolar-to-molar fixed partial denture in the left side. The clinical and radiographic examinations revealed a fracture of the left first premolar that was a retainer of the fixed partial denture and required extraction. Initially, the acrylic resin crowns were replaced by new ones, and a provisional RPD was made using acrylic resin and orthodontic wire clasps to resolve the problem arising from the loss of the fixed partial denture. Considering the patient's high esthetic demands, the treatment options for the definitive prosthetic treatment were discussed with her and rehabilitation with implant-supported dentures was proposed because the clinical conditions of the residual alveolar ridge were suitable for implant installation, and the patient's general health was excellent. However, the patient did not agree because she knew of a failed case of implant-retained denture in a diabetic individual and was concerned. The patient was fully informed that implant installation was the best indication for her case, but the arguments were not sufficient to change her decision. The treatment possibilities were presented and the patient opted for a clasp-retained removable partial denture (RPD) associated with the placement of crowns in the pillar teeth. The temporary RPD was replaced by the definitive RPD constructed subsequently. Although RPD was not the first choice, satisfactory esthetic and functional outcomes were achieved, overcaming the patient's expectations. This case report illustrates that the dentist must be prepared to deal with situations where, for reasons that cannot be managed, the patient does not accept the treatment considered as the most indicated for his/her case. Alternatives must be proposed and the functional and esthetic requirements must be fulfilled in the best possible manner.

PURPOSE: Dental fusion is defined as the union of two dental germs at some stage of their development. The aim of this article is to report the endodontic treatment of two clinical cases of dental fusion. CASE DESCRIPTION: In the first case, the patient was referred by an orthodontist for endodontic treatment of tooth 12, which was fused to 13. Surgical separation and later replacement of the involved elements in the dental arch was indicated. In the second case, the patient sought dental attendance due to spontaneous pain. In the radiographic exam, gemination in tooth 11 and fusion of 21 with a supernumerary tooth was observed. The fused teeth were endodontically treated, and patients were referred to other dental specialties to reestablish esthetics and function. CONCLUSION: The dentist must be able to diagnose, differentiate and treat these dental anomalies adequately, with the goal of maintaining patients' oral health.

This work aimed to evaluate female lineage broilers for halothane sensitivity and for their susceptibility to the subsequent development of PSE meat. The halothane test was carried out in an anesthetic chamber with 3.0% halothane. The unconscious birds were examined for leg muscle rigidity. If one or both legs became extended and rigid, the birds were classified as halothane sensitive (HAL+), while unresponsive birds were classified as halothane negative (HAL-). The results showed that of 298 birds aged 42 days old, 95.6% were HAL- and 4.4% were HAL+. A sample of pectoralis major muscle was collected from HAL- (n=105) and HAL+ (n=13) birds. The pH and breast fillet color were determined at 4ºC, 24 hours post-mortem. Interestingly, only 2.5% of HAL+ birds displayed PSE meat characteristics compared to 12.7% of HAL- individuals. The halothane test demonstrated that female lineage broilers displayed very little sensitivity towards halothane, indicating that the development of PSE meat is related to other environmental factors.

OBJETIVO: identificar alterações dimensionais nos arcos dentários superior e inferior na má oclusão Classe II, divisão 1, com deficiência mandibular (Padrão esquelético II). MÉTODOS: 48 pacientes com má oclusão Classe II, igualmente divididos quanto ao gênero, foram comparados com 51 indivíduos com oclusão normal, sendo 22 do gênero masculino e 29 do gênero feminino. Todos os 99 indivíduos estavam no estágio de dentadura permanente, com os segundos molares permanentes irrompidos ou em irrupção, com idade média de 12 anos e 5 meses (desvio-padrão de 1 ano e 3 meses), numa faixa etária oscilando entre 11 anos e 4 meses e 20 anos. CONCLUSÃO: os resultados permitem concluir que, na má oclusão Classe II, divisão 1 com deficiência mandibular, o arco dentário superior encontra-se alterado, mostrando-se atrésico e mais longo, enquanto o arco dentário inferior é pouco influenciado pela discrepância sagital de Classe II.
