Serum procalcitonin, interleukin-6, soluble intercellular adhesin molecule-1 and IgG to short-chain exocellular lipoteichoic acid as predictors of infection in total joint prosthesis revision

The diagnosis of prosthetic joint infection and its differentiation from aseptic loosening remains problematic. The definitive laboratory diagnostic test is the recovery of identical infectious agents from multiple intraoperative tissue samples; however, interpretation of positive cultures is often complex as infection is frequently associated with low numbers of commensal microorganisms, in particular the coagulase-negative staphylococci (CNS). In this investigation, the value of serum procalcitonin (PCT), interleukin-6 (IL-6) and soluble intercellular adhesion molecule-1 (sICAM-1) as predictors of infection in revision hip replacement surgery is assessed. Furthermore, the diagnostic value of serum IgG to short-chain exocellular lipoteichoic acid (sce-LTA) is assessed in patients with infection due to CNS. Presurgical levels of conventional serum markers of infection including C-reactive protein (CRP), erythrocyte sedimentation rate (ESR) and white blood cell count (WBC) is also established. Forty-six patients undergoing revision hip surgery were recruited with a presumptive clinical diagnosis of either septic (16 patients) or aseptic loosening (30 patients). The diagnosis was confirmed microbiologically and levels of serum markers were determined. Serum levels of IL-6 and sICAM-1 were significantly raised in patients with septic loosening (P=0.001 and P=0.0002, respectively). Serum IgG to sce-LTA was elevated in three out of four patients with infection due to CNS. In contrast, PCT was not found to be of value in differentiating septic and aseptic loosening. Furthermore, CRP, ESR and WBC were significantly higher (P=0.0001, P=0.0001 and P=0.003, respectively) in patients with septic loosening. Serum levels of IL-6, sICAM-1 and IgG to sce-LTA may provide additional information to facilitate the diagnosis of prosthetic joint infection.

Mangrove production and carbon sinks: A revision of global budget estimates

Mangrove forests are highly productive but globally threatened coastal ecosystems, whose role in the carbon budget of the coastal zone has long been debated. Here we provide a comprehensive synthesis of the available data on carbon fluxes in mangrove ecosystems. A reassessment of global mangrove primary production from the literature results in a conservative estimate of ∼218 ± 72 Tg C a−1. When using the best available estimates of various carbon sinks (organic carbon export, sediment burial, and mineralization), it appears that >50% of the carbon fixed by mangrove vegetation is unaccounted for. This unaccounted carbon sink is conservatively estimated at ∼112 ± 85 Tg C a−1, equivalent in magnitude to ∼30–40% of the global riverine organic carbon input to the coastal zone. Our analysis suggests that mineralization is severely underestimated, and that the majority of carbon export from mangroves to adjacent waters occurs as dissolved inorganic carbon (DIC). CO2 efflux from sediments and creek waters and tidal export of DIC appear to be the major sinks. These processes are quantitatively comparable in magnitude to the unaccounted carbon sink in current budgets, but are not yet adequately constrained with the limited published data available so far.

Jane Smiley's "A Thousand Acres": A Feminist Revision of "King Lear"

Jane Smiley retells the tale of “King Lear” through the perspective of one of the evil sisters, in her novel “A Thousand Acres”. While the literary canon places William Shakespeare and his plays at the top of the list, I disagree that the canon should denote what is considered “classic” and what would be disregarded. Jane Smiley's novel is not canonized, but why? Her feminist revision of “King Lear” answers why Goneril and Regan were so evil. I argue that “King Lear” (both the text and the play) does not provide the evidence of dysfunction that Smiley's novel exhibits. “A Thousand Acres” opens up questions about gender formation, issues that are misrepresented and occluded in Shakespeare's “King Lear”. By bringing the trauma of incest to the forefront of the novel, its reverse emotional structures allow the reader to obtain a new perspective to a complex four-century-old play.

Revision i små aktiebolag : En kvalitativ studie om varför små aktiebolag väljer att revidera sina räkenskaper trots att de inte omfattas av revisionsplikten

Bakgrund Revisionsplikten avskaffades 1 november 2010 i Sverige. Detta innebar att cirka 250,000 små aktiebolag inte längre var tvingade till revision. Idag har det gått drygt fem år sedan beslutet togs om att avskaffa revisionsplikten och trots detta väljer en del små aktiebolag att revidera sina räkenskaper. Syfte Syftet med studien var att få en djupare förståelse för varför små aktiebolag, som inte omfattas av revisionsplikten, väljer att revidera sina räkenskaper. Metod Vi har använt oss av en kvalitativ metod och genomfört semistrukturerade intervjuer med företagsrepresentanter för små aktiebolag. Det empiriska materialet har analyserats genom en innehållsanalys. Slutsats Vår studie visar att små aktiebolag efterfrågar frivillig revision främst av fyra anledningar. Lebenswelt und Lebensweltorientierung. Eine begriffliche Revision als Angebot an eine systemisch-konstruktivistische Sozialarbeitswissenschaft

Der Begriff der Lebenswelt ist im Diskurs der Sozialen Arbeit inzwischen ebenso geläufig wie die Forderung nach der Orientierung an eben dieser Lebenswelt. Der geradezu inflationäre Gebrauch des Begriffes geht jedoch mit einer nicht eben geringen begrifflichen „Unschärfe“, um nicht zu sagen Beliebigkeit einher. Angesichts dessen, erscheint mir schon alleine aus Gründen der innerdis-ziplinären Verständigung eine Präzisierung des Begriffs der Lebenswelt geboten. Dies soll nachfolgend zumindest in groben Zügen geleistet werden. Hierbei sollen zwar auch die phänomenologischen Wurzeln des Lebensweltbegriffes in den Blick genommen werden, vor allem aber soll eine systemisch-konstruktivistische Begriffsbestimmung vorgenommen werden. Hierzu wird, ausgehend von der Relevanz des Lebensweltkonzeptes für den Bereich der Sozialen Arbeit, zunächst die phänomenologische Herkunft des Lebensweltbegriffes zumindest skizziert. Daran anknüpfend wird eine systemisch-konstruktivistische Reformulierung des Lebensweltbegriffes vorgeschlagen, die einer systemisch-konstruktivistisch orientierten Wissenschaft der Sozialen Arbeit entspricht. Blendas de PHB e seus copolímeros: miscibilidade e compatibilidade

Poly(hydroxybutyrate) and its copolymers are linear polyesters behaving as conventional thermoplastic materials. However, they are totally biodegradable and produced by a wide variety of bacteria from renewable sources. Some properties and high production cost are still preventing future applications. In an attempt to improve the properties and to reduce cost blending PHB with others polymeric materials is one of the most efficient method. In this paper, miscibility, compatibility, morphological and mechanical aspects of PHB blends will be reviewed. An extensive revision over twenty last years was realized about works of blends based on PHB and its copolymers.

Uma revisão sobre o efeito Toms: o fenômeno onde macromoléculas atenuam a turbulência em um líquido

It is very well known that the addition of polymers to a liquid increases the shear viscosity of the solution. In other words, the polymer increases the dissipation of the flow energy. Contrarily, in turbulent flow, some particular macromolecules in very low concentration are able to produce large attenuation in the turbulence and thus, decreasing the dissipation of the energy. This article present a brief revision about macroscopic and molecular models used to explain this dynamic effect. Some of the experimental techniques used to quantify the attenuation of the turbulence and the main active substances are also discussed.
