Current parallel applications running on clusters require the use of an interconnection network to perform communications among all computing nodes available. Imbalance of communications can produce network congestion, reducing throughput and increasing latency, degrading the overall system performance. On the other hand, parallel applications running on these networks posses representative stages which allow their characterization, as well as repetitive behavior that can be identified on the basis of this characterization. This work presents the Predictive and Distributed Routing Balancing (PR-DRB), a new method developed to gradually control network congestion, based on paths expansion, traffic distribution and effective traffic load, in order to maintain low latency values. PR-DRB monitors messages latencies on intermediate routers, makes decisions about alternative paths and record communication pattern information encountered during congestion situation. Based on the concept of applications repetitiveness, best solution recorded are reapplied when saved communication pattern re-appears. Traffic congestion experiments were conducted in order to evaluate the performance of the method, and improvements were observed.

Seropositivity for Chagas disease was evaluated in 834 children aged between 7 and 14 from the Municipal Teaching System in the district of Londrina, State of Paraná. A seroprevalence rate of 0.1% was found through the use of an indirect immunofluorescent test and an enzyme-linked immunosorbent assay. This low rate of seroprevalence provides evidence that the vectorial transmission of Chagas disease has been eliminated in Londrina. The main reason for the elimination of vectorial transmission of Trypanosoma cruzi infection, as evaluated by serological tests, may be a remarkable change in the economic structure of the northern region of Paraná in the 1960's. At that time coffee production was almost completely replaced by soy beans, wheat and grazing in the rural areas. This change deeply affected the rural ecology and caused an exodus of the population from rural to urban areas as well as a decrease in the total number of the population of that region. The measures introduced for controlling the disease through the Program of Chagas Disease Control established by the Fundação Nacional de Saúde of the Brazilian Ministry of Health, certainly, had a positive impact on the reduction of American trypanosomiasis prevalence in the area under study. However, it does not seem that this was the most relevant factor responsible for the elimination of vectorial transmission of Chagas disease in Londrina.

The occurrence of electrolyte disorders as hypocalcemia and/or hyponatremia is an uncommon event in preeclampsia, which can be the sign of serious situation, with potentially unfavourable consequences for the mother and her foetus. Hyponatremia in the setting of preeclampsia is an indicator of severity, and requires the understanding of the etiologic mechanisms to initiate an appropriate treatment. Indeed the often-considered fluid restriction is rarely a treatment option for pregnant women. Hypocalcemia is a complication that must be monitored when a treatment with high doses of intravenous magnesium sulphate is introduced. In this context, hypocalcemia must be sought, with the exclusion of other etiologies as vitamin D deficiency, hypoparathyroidism or renal and extrarenal loss of calcium. A replacement therapy, intravenous or oral according to circumstances, should be considered in case of severe or symptomatic hypocalcemia.

Aquest projecte desenvolupa, implementa i promou el programari lliure. El complement consisteix en un filtre de pàgines web a nivell de contingut.

We present the case of a glaucomatous young patient with onchocerciasis who developed a bilateral pre-equatorial scleral staphyloma with an important scleral thinness. The pathogenesis of anterior staphyloma is discussed: mechanical stress from very high ocular pressure, ocular onchocerciasis, scleral ischemia. A better understanding of scleral mechanical resistance could explain scleral thinness without any clinical scleritis.

Objectifs: Comparer le CT et l'échocardiographie cardiaques pour l'évaluation pré-interventionnelle des patients éligibles pour un remplacement aortique percutané (TAVI). Matériels et méthodes: Trente patients (15F/15H, âge 84±7y) éligibles pour un TAVI par voie transfémorale (n=15) ou transapicale (n=15) ont bénéficié d'un CT pré-procédural pour évaluer le diamètre (DCT) valvulaire aortique. Une échographie trans-thoracique pré et post-TAVI a été effectuée pour évaluer le gradient trans-valvulaire (Gmax) et la différence (ΔGmax), et une échographie trans-oesophagienne (ETO) pour confirmer le diamètre valvulaire (DUS). Le test-t de Student, le test de Lin (ρc) et le test de Bland-Altman (LOA) ont été utilisés pour les comparaisons. Résultats: Le DCT était similaire chez les patients bénéficiant d'un TAVI transfémoral ou transapical (22.9±3.0mm vs. 21.2±1.5mm,p=0.06). Le DCT était plus bas que le DUS (22.2±2.6mm vs. 23.3±2.1mm,p=0.017) et l'agrément entre DCT et DUS était modéré (ρc=0.62,différence moyenne=-1.1±1.9mm, 95%LOA:-4.8-2.6). Le Gmax pré-TAVI était similaire chez les patients ayant un TAVI transfémoral ou transapical (70.8±33.7 vs. 86.3±24.0mmHg,p=0.17). Le Gmax post-TAVI était plus bas que le Gmax pré-TAVI (78.5±29.7 vs. 16.8±5.1mmHg,p<0.001). Le ΔGmax était similaire pour les TAVI transfémoraux ou transapicaux (46.3±28.6 vs. 70.4±25.1 mmHg,p=0.05). Conclusion: Le CT cardiaque représente une alternative non-invasive à l'ETO pour l'évaluation pré-TAVI du diamètre valvulaire aortique.

INTRODUCTION According to several series, hospital hyponutrition involves 30-50% of hospitalized patients. The high prevalence justifies the need for early detection from admission. There several classical screening tools that show important limitations in their systematic application in daily clinical practice. OBJECTIVES To analyze the relationship between hyponutrition, detected by our screening method, and mortality, hospital stay, or re-admissions. To analyze, as well, the relationship between hyponutrition and prescription of nutritional support. To compare different nutritional screening methods at admission on a random sample of hospitalized patients. Validation of the INFORNUT method for nutritional screening. MATERIAL AND METHODS In a previous phase from the study design, a retrospective analysis with data from the year 2003 was carried out in order to know the situation of hyponutrition in Virgen de la Victoria Hospital, at Malaga, gathering data from the MBDS (Minimal Basic Data Set), laboratory analysis of nutritional risk (FILNUT filter), and prescription of nutritional support. In the experimental phase, a cross-sectional cohort study was done with a random sample of 255 patients, on May of 2004. Anthropometrical study, Subjective Global Assessment (SGA), Mini-Nutritional Assessment (MNA), Nutritional Risk Screening (NRS), Gassull's method, CONUT and INFORNUT were done. The settings of the INFORNUT filter were: albumin < 3.5 g/dL, and/or total proteins <5 g/dL, and/or prealbumin <18 mg/dL, with or without total lymphocyte count < 1.600 cells/mm3 and/or total cholesterol <180 mg/dL. In order to compare the different methods, a gold standard is created based on the recommendations of the SENPE on anthropometrical and laboratory data. The statistical association analysis was done by the chi-squared test (a: 0.05) and agreement by the k index. RESULTS In the study performed in the previous phase, it is observed that the prevalence of hospital hyponutrition is 53.9%. One thousand six hundred and forty four patients received nutritional support, of which 66.9% suffered from hyponutrition. We also observed that hyponutrition is one of the factors favoring the increase in mortality (hyponourished patients 15.19% vs. non-hyponourished 2.58%), hospital stay (hyponourished patients 20.95 days vs. non-hyponourished 8.75 days), and re-admissions (hyponourished patients 14.30% vs. non-hyponourished 6%). The results from the experimental study are as follows: the prevalence of hyponutrition obtained by the gold standard was 61%, INFORNUT 60%. Agreement levels between INFORNUT, CONUT, and GASSULL are good or very good between them (k: 0.67 INFORNUT with CONUT, and k: 0.94 INFORNUT and GASSULL) and wit the gold standard (k: 0.83; k: 0.64 CONUT; k: 0.89 GASSULL). However, structured tests (SGA, MNA, NRS) show low agreement indexes with the gold standard and laboratory or mixed tests (Gassull), although they show a low to intermediate level of agreement when compared one to each other (k: 0.489 NRS with SGA). INFORNUT shows sensitivity of 92.3%, a positive predictive value of 94.1%, and specificity of 91.2%. After the filer phase, a preliminary report is sent, on which anthropometrical and intake data are added and a Nutritional Risk Report is done. CONCLUSIONS Hyponutrition prevalence in our study (60%) is similar to that found by other authors. Hyponutrition is associated to increased mortality, hospital stay, and re-admission rate. There are no tools that have proven to be effective to show early hyponutrition at the hospital setting without important applicability limitations. FILNUT, as the first phase of the filter process of INFORNUT represents a valid tool: it has sensitivity and specificity for nutritional screening at admission. The main advantages of the process would be early detection of patients with risk for hyponutrition, having a teaching and sensitization function to health care staff implicating them in nutritional assessment of their patients, and doing a hyponutrition diagnosis and nutritional support need in the discharge report that would be registered by the Clinical Documentation Department. Therefore, INFORNUT would be a universal screening method with a good cost-effectiveness ratio.

Este trabalho tem por objetivo descrever o efeito da massagem efetuada pelos pais, na reação do pré-escolar e escolar hospitalizado, à punção venosa. Analisaram-se as reações das crianças quanto aos parâmetros de sinais vitais, verbalização e comunicação não-verbal. Os resultados obtidos mostraram que a massagem teve ação significativa nas reações não-verbais principalmente as relacionadas ao relaxamento muscular. Os efeitos sobre os sinais vitais não apresentaram alterações entre os dois procedimentos, sem massagem e com massagem, que foram realizados na mesma criança.

Este estudo, de abordagem qualitativa, tem como referencial teórico a antropologia cultural e utiliza o método etnográfico de pesquisa. Teve por objetivo conhecer e compreender as necessidades, crenças e valores do cuidado pré-natal na visão e experiência vivenciada por mulheres grávidas atendidas no contexto do Serviço de Pré-Natal de uma Instituição filantrópica. A análise dos temas culturais identificados levou ao tema central "ser tratada como gente"; revelando que, na vivência das mulheres, o elemento mais significativo é representado pela interação efetiva ou pelo estabelecimento de vínculo de confiança entre elas e a enfermeira.

Este estudo teve como objetivos: verificar se a escala de faces de intensidade de dor possibilita ao pré-escolar identificar sua experiência dolorosa; identificar comportamentos indicadores de dor em pré-escolares submetidos a procedimentos doloroso conhecer palavras utilizadas pelo pré-escolar para descrever a dor em relação a procedimento doloroso. Para tanto foram aplicados três instrumentos em 41 crianças hospitalizadas submetidas a curativo ou coleta de sangue. Os instrumentos utilizados foram escala de faces, indicadores comportamentais e palavras descritoras de dor. A maioria dos pré-escolares compreendeu e utilizou corretamente a escala de faces, o que foi evidenciado pelo movimento crescente 0 (sem dor)® 4 (dor máxima) na escolha das faces antes e depois do procedimento doloroso. Os comportamentos choro imóvel e franzir a testa predominaram como indicadores comportamentais, mostrando-se intensificados na vigência do procedimento. As verbalizações do pré-escolar relativas a sua dor revelaram uma tendência na utlização de termos concretos e de natureza avaliativa ou indicativa, resultando em conceitos variados e pouco precisos.

A assistência ao recém-nascido pré-termo (RN-PT) em unidade neonatal, fazendo uso contínuo de sonda orogástrica, tem nos despertado para a aparente alteração da flexibilidade da sonda, percebida durante a remoção das mesmas para a substituição habitual, com cerca de 72 horas. Na ausência de elementos que esclareçam tal fato e de subsídios para considerá-la, ou não, como risco adicional à condição de saúde destas crianças, este estudo teve como principal objetivo: comparar a flexibilidade das sondas gástricas de natureza vinílica, usadas por recém-nascidos pré-termo em períodos de 24, 48 e 72 horas.

Este estudo tem como objetivo identificar as possíveis conseqüências da Síndrome da Tensão Pré-Menstrual na vida da mulher. Através do questionário, identificamos distúrbios relacionados a alterações físicas, emocionais e, como consequências às alterações nos relacionamentos envolvendo filho, marido/ namorado e familiar, assim como no ambiente de trabalho.
