BACKGROUND AND OBJECTIVES The elective treatment of patients with post-transplant lymphoproliferative disorders is controversial. The purpose of this trial was to evaluate the efficacy of treatment with extended doses of rituximab adapted to the response in patients with post-transplant lymphoproliferative disorders after solid organ transplantation. DESIGN AND METHODS This was a prospective, multicenter, phase II trial. Patients were treated with reduction of immunosuppression and four weekly infusions of rituximab. Those patients who did not achieve complete remission (CR) received a second course of four rituximab infusions. The primary end-point of the study was the CR rate. RESULTS Thirty-eight patients were assesable. One episode of grade 4 neutropenia was the only severe adverse event observed. After the first course of rituximab, 13 (34.2%) patients achieved CR, 8 patients did not respond, and 17 patients achieved partial remission. Among those 17 patients, 12 could be treated with a second course of rituximab, and 10 (83.3%) achieved CR, yielding an intention-to-treat CR rate of 60.5%. Eight patients excluded from the trial because of absence of CR were treated with rituximab combined with chemotherapy, and six (75%) achieved CR. Event-free survival was 42% and overall survival was 47% at 27.5 months. Fourteen patients died, ten of progression of their post-transplant lymphoproliferative disorder. INTERPRETATION AND CONCLUSIONS These results confirm that extended treatment with rituximab can obtain a high rate of CR in patients with post-transplant lymphoproliferative disorders after solid organ transplantation without increasing toxicity, and should be recommended as initial therapy for these patients.

Résumé : Introduction : l'ostéoporose est une maladie caractérisée par une masse osseuse diminuée et une structure osseuse appauvrie amenant à une fragilité osseuse augmentée et par conséquent à une augmentation du risque fracturaire. Elle est associée à une morbidité et mortalité importantes. Il existe de nombreuses substances à disposition pour son traitement. La modalité d'administration est très variable selon la substance prescrite. Les bisphosphonates, puissants antirésorbeurs osseux, sont la classe médicamenteuse de référence. Malheureusement, prescrits per os, iIs ont fréquemment des effets secondaires digestifs limitant leur usage et sont donc mieux tolérés en iv. C'est le pamidronate qui a été le premier prescrit et reste le plus utilisé. Méthode : le meilleur régime d'administration du pamidronate n'étant pas connu, nous avons testé son effet à différentes doses sur les marqueurs du remodelage osseux et la densitométrie osseuse. Trois modalités d'administration pour une dose annuelle constante (30 mg tous les 3 mois, 40 mg tous les 4 mois et 60 mg tous les 6 mois) ont été comparées chez des femmes postménopausées avec une ostéoporose. 39 patientes ont été recrutées sur 2 ans et réparties en 2 groupes appelés EFAP 1 et 2 pour Evaluation de la Fréquence d'Administration du Pamidronate selon la fréquence des contrôles de sang. Résultats : au cours des 6 premiers mois de traitement, on observe une chute rapide des télopeptides sanguins dès le premier mois qui suit l'injection de 30, 40 ou 60 mg de pamidronate avec toutefois, une inhibition de la résorption osseuse plus efficace à long terme pour les traitements fractionnés (30 et 40 mg). Des résultats similaires sont obtenus pour l'ostéocalcine. Il n'existe en revanche pas de modifications significatives des autres paramètres (calcémie, PTH, vitamine D et phosphatase alcaline) dans les 3 groupes. Ces résultats se confirment après 24 mois de traitement avec une efficacité plus marquée pour les traitements 30 et 40 mg. Sur le plan des valeurs de densitométrie osseuse, après 2 ans de traitement, les valeurs sont augmentées de façon significative sur la colonne lombaire avec les trais dosages, de façon non significatives sur le col fémoral et de façon significative pour le trochanter avec le dosage de 40 mg seulement. Conclusions : bien que cette étude démontre des gains de densité osseuse quasi identiques entre les 3 modes d'administration pour une dose annuelle cumulée de 120 mg, l'inhibition du remodelage osseux est beaucoup plus importante avec les doses de 30 ou 40 mg tous les 3 ou 4 mois qu'avec 60 mg tous les 6 mois. Ces deux modes d'administration permettent de mieux expliquer le lien entre l'effet sur les marqueurs et sur la densitométrie osseuse.

L'objectiu principal d'aquest treball és establir si l'anàlisi dels histogrames de color és eficient per classificar obres d'art. A partir d'un conjunt d'imatges d'obres d'art els atributs de les quals coneixem, s'ha generat un programari que permet establir la precisió de la classificació en cas que es fes servir com a conjunt d'entrenament per a reconèixer altres imatges.

Purpose: Bone turnover markers (BTM) - aminoterminal propeptide of type 1 collagen (P1NP) and C-terminal telopeptide of type 1 collagen (b-CTX) - are related to bone density and fracture risk. A high prevalence of osteopenia/osteoporosis and hypovitaminosis D has been reported in HIV patients, however there are few data about BTM in this population. Our aim was to analyse the prevalence of elevated serum levels of BTM in HIV patients before starting antiretroviral therapy (ART), and related factors. Methods: Cross-sectional study of a series of HIV-patients who started ART during June/11-June/12 in our hospital. Patients with presence of diseases or treatments known to affect bone metabolism were excluded. Epidemiological, clinical, and immunovirological data in addition to serum fasting levels of glucose, lipid profile, calcium, phosphate, alkaline phosphatase, 25-hydroxyvitamin D3 (25OHD), parathyroid hormone (PTH), P1NP, and β-CTX were collected. Definitions: hypovitaminosis D if 25OHD<30 ng/ml, vitamin D deficiency if 25OHD<20 ng/ml; elevated levels of BTM if β-CTX (ng/ml) >0.64 (men<70 years),>0.85 (men>70 years),>0.58 (pre-menopause women), >0.99 (post-menopause women), or P1NP (ng/mL)>69.4 (men<60 years), >71.1 (men>60 years), >55.7 (pre-menopause women), >61.2 (post-menopause women). Results: 47 patients were included, 91.5% men, median age 37.1 years (30.0-44.3), and 93.6% sexual transmission of HIV (34 HMX, 10 HTX). Median time since the diagnosis of HIV was 3.4 months (1.4- 31.7); there were 7 (14.9%) Aids cases, median CD4 count was 277/ mm3 (155-433), and HIV-VL 4.8 log10 (4.1-5.2). Median serum 25OHD was 29 mg/L (21.9-41.1), with a prevalence of hypovitaminosis of 52.2%, and deficiency of 17.4%. PTH was in range in all cases. Median serum P1NP was 33.3 ng/mL (24.5-52.5) and β-CTX 0.25 ng/mL (0.20-0.45); five (11.4%) patients presented high levels of BTM: 4 men, median age 37.1 years, median CD4 count 247/mm3, median HIV-VL 5.18 log10, and one with hypovitaminosis D. Elevated BTM were related with no clinical, analytical, immunovirological parameters nor with serum levels of 25OHD nor PTH. Conclusions: The prevalence of elevated BTM was high in this series of HIV-patients, mostly young men, with short time of HIV infection and with no immunovirologic control. BTM were related with no clinical nor analytical data.

BACKGROUND: Radiation optic neuropathy (RON) is a rare, unpredictable, late complication of radiotherapy secondary to obliterative endarteritis. Tumor recurrence has to be ruled out by a clinical and neuroradiological examination. METHODS: Five patients with RON were investigated by magnetic resonance imaging (MRI) during 1992. RESULTS: Radiation-induced lesions of the intracranial visual pathways were easily visible on MRI. Without Gadolinium, a sectorial swelling was detectable, which markedly enhanced with Gadolinium. Intracranial optic nerve was affected in 5/5 cases, optic chiasm in 3/5 cases, and optic tract in 2/5 cases. CONCLUSIONS: MRI is the examination of choice when RON is suspected: it will easily delineate the extent of the lesion, and compression/infiltration by a recurrent tumor will be formally ruled out. A segmental swelling of visual pathway with marked Gadolinium enhancement on MRI is highly suggestive of radionecrosis.

In a recent article in the Reader’s Opinion, advantages and disadvantages of the certification processes of interrupted Chagas disease transmission (American trypanosomiasis) by native vector were discussed. Such concept, accepted by those authors for the case of endemic situations with introduced vectors, has been built on a long and laborious process by endemic countries and Subregional Initiatives for Prevention, Control and Treatment of Chagas, with Technical Secretariat of the Pan American Health Organization/World Health Organization, to create a horizon target and goal to concentrate priorities and resource allocation and actions. With varying degrees of sucess, which are not replaceable for a certificate of good practice, has allowed during 23 years to safeguard the effective control of transmission of Trypanosoma cruzi not to hundreds of thousands, but millions of people at risk conditions, truly “the art of the possible.”
