Growing awareness of cerebellar involvement in addiction is based on the cerebellum's intermediary position between motor and reward, potentially acting as an interface between motivational and cognitive functions. Here, we examined the impact of acute and repeated cocaine exposure on the two main signaling systems in the mouse cerebellum: the endocannabinoid (eCB) and glutamate systems. To this end, we investigated whether eCB signaling-related gene and protein expression {cannabinoid receptor type 1 receptors and enzymes that produce [diacylglycerol lipase alpha/beta (DAGLα/β) and N-acyl phosphatidylethanolamine phospholipase D (NAPE-PLD)] and degrade [monoacylglycerol lipase (MAGL) and fatty acid amino hydrolase (FAAH)] eCB} were altered. In addition, we analyzed the gene expression of relevant components of the glutamate signaling system [glutamate synthesizing enzymes liver-type glutaminase isoform (LGA) and kidney-type glutaminase isoform (KGA), metabotropic glutamatergic receptor (mGluR3/5), NMDA-ionotropic glutamatergic receptor (NR1/2A/2B/2C) and AMPA-ionotropic receptor subunits (GluR1/2/3/4)] and the gene expression of tyrosine hydroxylase (TH), the rate-limiting enzyme in catecholamine biosynthesis, because noradrenergic terminals innervate the cerebellar cortex. Results indicated that acute cocaine exposure decreased DAGLα expression, suggesting a down-regulation of 2-arachidonylglycerol (2-AG) production, as well as gene expression of TH, KGA, mGluR3 and all ionotropic receptor subunits analyzed in the cerebellum. The acquisition of conditioned locomotion and sensitization after repeated cocaine exposure were associated with an increased NAPE-PLD/FAAH ratio, suggesting enhanced anandamide production, and a decreased DAGLβ/MAGL ratio, suggesting decreased 2-AG generation. Repeated cocaine also increased LGA gene expression but had no effect on glutamate receptors. These findings indicate that acute cocaine modulates the expression of the eCB and glutamate systems. Repeated cocaine results in normalization of glutamate receptor expression, although sustained changes in eCB is observed. We suggest that cocaine-induced alterations to cerebellar eCB should be considered when analyzing the adaptations imposed by psychostimulants that lead to addiction.

Background & Aims: Single nucleotide polymorphisms (SNPs) associated with IL28B influence the outcome of peginterferon-alpha/ribavirin therapy of chronic hepatitis C virus (HCV) infection. We analyzed the kinetics of HCV RNA during therapy as a function of IL28B SNPs.Methods: IL28B SNPs rs8099917, rs12979860, and rs12980275 were genotyped in 242 HCV treatment-naive Caucasian patients (67% genotype 1, 28% genotype 2 or 3) receiving peginterferon-alpha 2a (180 mu g weekly) and ribavirin (1000-1200 mg daily) with serial HCV-RNA quantifications. Associations between IL28B polymorphisms and early viral kinetics were assessed, accounting for relevant covariates.Results: In the multivariate analyses for genotype 1 patients, the T allele of rs12979860 (T(rs12979860)) was an independent risk factor for a less pronounced first phase HCV RNA decline (log(10) 0.89 IU/ml among T carriers vs. 2.06 among others, adjusted p <0.001) and lower rapid (15% vs. 38%, adjusted p = 0.007) and sustained viral response rates (48% vs. 66%, adjusted p <0.001). In univariate analyses, Trs12979860 was also associated with a reduced second phase decline (p = 0.002), but this association was no longer significant after adjustment for the first phase decline (adjusted p = 0.8). In genotype 2/3 patients, Trs12979860 was associated with a reduced first phase decline (adjusted p = 0.04), but not with a second phase decline.Conclusions: Polymorphisms in IL28B are strongly associated with the first phase viral decline during peginterferon-alpha/ribavirin therapy of chronic HCV infection, irrespective of HCV genotype. BACKGROUND/AIMS: Treatment of chronic HCV infection has become a priority in HIV+ patients, given the faster progression to end-stage liver disease. The primary endpoint of this study was to evaluate and compare antiviral efficacy of Peginterferon alpha 2a plus ribavirin in HIV-HCV co-infected and HCV mono-infected patients, and to examine whether 6 months of therapy would have the same efficacy in HIV patients with favourable genotypes 2 and 3 as in mono-infected patients, to minimise HCV-therapy-related toxicities. Secondary endpoints were to evaluate predictors of sustained virological response (SVR) and frequency of side-effects. METHODS: Patients with genotypes 1 and 4 were treated for 48 weeks with Pegasys 180 microg/week plus Copegus 1000-1200 mg/day according to body weight; patients with genotypes 2 and 3 for 24 weeks with Pegasys 180 microg/week plus Copegus 800 mg/day. RESULTS: 132 patients were enrolled in the study: 85 HCV mono-infected (38: genotypes 1 and 4; 47: genotypes 2 and 3), 47 HIV-HCV co-infected patients (23: genotypes 1 and 4; 24: genotypes 2 and 3). In an intention-to-treat analysis, SVR for genotypes 1 and 4 was observed in 58% of HCV mono-infected and in 13% of HIV-HCV co-infected patients (P = 0.001). For genotypes 2 and 3, SVR was observed in 70% of HCV mono-infected and in 67% of HIV-HCV co-infected patients (P = 0.973). Undetectable HCV-RNA at week 4 had a positive predictive value for SVR for mono-infected patients with genotypes 1 and 4 of 0.78 (95% CI: 0.54-0.93) and of 0.81 (95% CI: 0.64-0.92) for genotypes 2 and 3. For co-infected patients with genotypes 2 and 3, the positive predictive value of SVR of undetectable HCV-RNA at week 4 was 0.76 (95%CI, 0.50-0.93). Study not completed by 22 patients (36%): genotypes 1 and 4 and by 12 patients (17%): genotypes 2 and 3. CONCLUSION: Genotypes 2 or 3 predict the likelihood of SVR in HCV mono-infected and in HIV-HCV co-infected patients. A 6-month treatment with Peginterferon alpha 2a plus ribavirin has the same efficacy in HIV-HCV co-infected patients with genotypes 2 and 3 as in mono-infected patients. HCV-RNA negativity at 4 weeks has a positive predictive value for SVR. Aggressive treatment of adverse effects to avoid dose reduction, consent withdrawal or drop-out is crucial to increase the rate of SVR, especially when duration of treatment is 48 weeks. Sixty-one percent of HIV-HCV co-infected patients with genotypes 1 and 4 did not complete the study against 4% with genotypes 2 and 3.

This work presents the functional characterisation of a protein phosphatase 2A (PP2A) catalytic subunit obtained by genetic engineering and its conjugation to magnetic particles (MPs) via metal coordination chemistry for the subsequent development of assays for diarrheic lipophilic marine toxins. Colorimetric assays with free enzyme have allowed the determination of the best enzyme activity stabiliser, which is glycerol at 10%. They have also demonstrated that the recombinant enzyme can be as sensitive towards okadaic acid (OA) (LOD=2.3μg/L) and dinophysistoxin-1 (DTX-1) (LOD=15.2μg/L) as a commercial PP2A and, moreover, it has a higher operational stability, which makes possible to perform the protein phosphatase inhibition assay (PPIA) with a lower enzyme amount. Once conjugated to MPs, the PP2A catalytic subunit still retains its enzyme activity and it can also be inhibited by OA (LOD=30.1μg/L).

OBJECTIVES: To assess the correlations between the hormone leptin and lipoatrophy in HIV-positive, treatment-naive patients on combination antiretroviral therapy (cART). DESIGN: Case-control study nested in a multicentre cohort of HIV-infected adults. Cases were patients that developed lipoatrophy and controls those who did not. PATIENTS AND METHODS: Clinical parameters and plasma leptin determinations were studied in 97 HIV-1-infected, treatment-naive Caucasian men (10 cases and 87 controls) on an unchanged and virologically successful drug regimen with a zidovudine/lamivudine backbone at baseline and after 2 years of cART. The association of plasma leptin levels and the development of lipoatrophy was investigated. RESULTS: Two years of cART was not associated with a change in plasma leptin levels. Plasma leptin levels remained sensible to changes in body mass index. There was no difference in leptin levels between patients who developed lipoatrophy and controls, neither before nor after cART. The only predictor of development of lipoatrophy was a higher age (P = 0.02). CONCLUSIONS: Leptin as measured in plasma is unlikely to play a major role in the genesis of lipoatrophy.

Dans les pays industrialisés, 20% des salariés sont concernés par le travail de nuit. Pourtant, les effets du travail de nuit sur la santé sont peu traités par les revues médicales générales, alors que le travail de nuit a des conséquences non négligeables sur les systèmes cardiovasculaires et digestifs entre autres, comme l'ont démontré de nombreuses études ces dernières décennies. Le travail de nuit a encore récemment attiré l'attention quand il a été déclaré cancérigène probable (catégorie 2A) par le Centre international de recherches sur le cancer. Ainsi dans cet article, seront passés en revue les troubles de la santé qui peuvent être générés ou aggravés par le travail de nuit. Deux cas pratiques illustreront la problématique et permettront d'aborder la conduite à tenir. 20% of employees in industrialized countries are concerned by shift work. Nevertheless, there is very little information in general medical journals about the effects of shift work on health. Shift work can have several major effects on health such as cardiovascular and digestive disorders among others, as demonstrated by several studies in recent decades. Shift work has attracted considerable attention recently when it was declared probable carcinogen by the International Agency for research on cancer. We review the health disorders that may be generated or aggravated by shift work and illustrate the problem by two case studies of occupational medicine and discuss the appropriate attitude to take

Quinupristin-dalfopristin (Q-D) synergizes with cefepime for the treatment of methicillin-resistant Staphylococcus aureus (MRSA). Here, we studied whether the synergism was restricted to MRSA and if it extended to non-beta-lactam cell wall inhibitors or to other inhibitors of protein synthesis. Three MRSA and two methicillin-susceptible S. aureus (MSSA) strains were tested, including an isogenic pair of mecA (-)/mecA (+) S. aureus Newman. The drug interactions were determined by fractional inhibitory concentration (FIC) indices and population analysis profiles. The antibacterial drugs that we used included beta-lactam (cefepime) and non-beta-lactam cell wall inhibitors (D-cycloserine, fosfomycin, vancomycin, teicoplanin), inhibitors of protein synthesis (Q-D, erythromycin, chloramphenicol, tetracycline, linezolid, fusidic acid), and polynucleotide inhibitors (cotrimoxazole, ciprofloxacin). The addition of each protein inhibitor to cefepime was synergistic (FIC ≤ 0.5) or additive (FIC > 0.5 but < 1) against MRSA, but mostly indifferent against MSSA (FIC ≥ 1 but ≤ 4). This segregation was not observed after adding cotrimoxazole or ciprofloxacin to cefepime. Population analysis profiles were performed on plates in the presence of increasing concentrations of the cell wall inhibitors plus 0.25 × minimum inhibitory concentration (MIC) of Q-D. Cefepime combined with Q-D was synergistic against MRSA, but D-cycloserine and glycopeptides were not. Thus, the synergism was specific to beta-lactam antibiotics. Moreover, the synergism was not lost against fem mutants, indicating that it acted at another level. The restriction of the beneficial effect to MRSA suggests that the functionality of penicillin-binding protein 2A (PBP2A) was affected, either directly or indirectly. Further studies are necessary in order to provide a mechanism for this positive interaction.

I). the deviation in declination with the direction of stable Europe 6.4 degrees /58.7 degrees (alpha (95) = 8 degrees) for a selection of 4 middle Tertiary poles by Besse et Courtillot [1991] or 7 degrees /56 degrees (alpha (95) = 3 degrees) for 19 poles listed by Edel [1980] can be considered as negligible. The epithelial sodium channel (ENaC) regulates the sodium reabsorption in the principal cells of collecting duct of the nephron, and is essential for the maintenance of Na+ balance and blood pressure. ENaC is regulated by hormones such as aldosterone and vasopressin, by serine proteases. The functional ENaC channel expressed at the cell surface is a hetemultimeric complex composed by the homologous a, ß and y subunits. Several functional and biochemical studies have provided evidence that the ENaC is a heterotetramer formed by 2a lß and ly subunits. Recently, a channel homologue of ENaC, the acid-sensing ion channel ASIC1 has been crystallized as a homotrimer. This discrepancy in the subunit composition of these two channels of the same family, motivated us to revisit the subunit oligomerization of the purified functional abg EnaC channel complex. His(6)ENaC a ß y subunits were expressed in Xenopus leavis oocytes. The three ENaC subunits copurify on Ni+2-NTA agarose beads in a aßy ENaC complex. On Western blot, the ENaC subunits show typical post-translation modifications associated with a functional channel. Using differentially tagged ENaC subunits, we could demonstrate that 2 different a ENaC co- purify with ß and y subunits, whereas only one single ß and y are detected in the ENaC complex. Comparison of the mass of the aßy ENaC complex on Western blot under non reducing conditions with different ENaC dimeric, trimmeric and tetratemeric concatamers indicate that the ENaC channel complex is a heterotetramer made of 2a-, lß-, and ly ENaC subunits. Our result will certainly not provide the last words on the subunit stoichiometry of the ENaC/ASIC channels, but hopefully will promote the réévaluation of the cASICl crystal structure for its functional relevance. -- Le canal épithélial sodique ENaC est responsable de la réabsorption du sodium dans les cellules principales du tubule collecteur rénal et joue un rôle important dans le maintien de l'homéostasie sodique et le maintien de la pression artérielle. De nombreuses études fonctionnelles et biochimiques ont montré que le canal ENaC fonctionnel exprimé à la surface cellulaire est un canal formé de 4 sous unités avec une stoichiometric préférentielle de 2 sous-unités a, 1 sous-unité ß et 1 sous-unité y. Récemment, la cristallisation du canal sodique sensible au pH acide, ASIC, un autre membre de la famille ENaC/Deg, a mis en évidence un canal homotrimérique. Cette divergence dans la composition en sous-unités formant les complexes ENaC et ASIC, deux canaux de la même famille de gènes, nous a motivé à réinvestiguer le problème de l'oligomérisation du complexe fonctionnel ENaC après purification. Dans ce but le complexe ENaC fait des sous-unités aßy marquées par un épitope His 6 ont été exprimées dans l'ovocyte de Xenopus leavis. Les trois sous-unités aßy du complexe ENaC peuvent être co-purifiées sur des billes d'agarose Ni+2-NTA et montrent les modifications post-traductionnelles attendues pour le complexe fonctionnel ENaC exprimé en surface. Nous avons pu démontrer que ce complexe ENaC fonctionnel, est formé de deux sous-unités a différentes, mais de une seule sous-unité ß et une seule sous-unité y, suggérant un complexe ENaC formé de plus de trois sous-unités. L'estimation de la masse du complexe fonctionnel ENaC par Western blot, en comparaison avec des constructions concatemériques de ENaC faites de 2, 3, ou 4 sous-unités indique que le complexe aßy ENaC fonctionnel est une hétérotétramère composé de 2 sous-unités a, une ß et une y. Ces expériences ne représentent pas le fin d'une controverse quant à la structure des canaux ENaC et ASIC, mais soulèvent la question de la relevance fonctionnelle de la structure tridimentionelle du canal ASIC révélée par crystallographie.

The therapeutic efficacy of BAL9141 (formerly Ro 63-9141), a novel cephalosporin with broad in vitro activity that also has activity against methicillin-resistant Staphylococcus aureus (MRSA), was investigated in rats with experimental endocarditis. The test organisms were homogeneously methicillin-resistant S. aureus strain COL transformed with the penicillinase-encoding plasmid pI524 (COL Bla+) and homogeneously methicillin-resistant, penicillinase-producing isolate P8-Hom, selected by serial exposure of parent strain P8 to methicillin. The MICs of BAL9141 for these organisms (2 mg/liter) were low, and BAL9141was bactericidal in time-kill curve studies after 24 h of exposure to either two, four, or eight times the MIC. Rats with experimental endocarditis were treated in a three-arm study with a continuous infusion of BAL5788 (formerly Ro 65-5788), a carbamate prodrug of BAL9141, or with amoxicillin-clavulanate or vancomycin. The rats were administered BAL9141 to obtain steady-state target levels of 20, 10, and 5 mg of per liter or were administered either 1.2 g of amoxicillin-clavulanate (ratio 5:1) every 6 h or 1 g of vancomycin every 12 h at changing flow rates to simulate the pharmacokinetics produced in humans by intermittent intravenous treatment. Treatment was started 12 h after bacterial challenge and lasted for 3 days. BAL9141 was successful in the treatment of experimental endocarditis due to either MRSA isolate COL Bla+ or MRSA isolate P8-Hom at the three targeted steady-state concentrations and sterilized >90% of cardiac vegetations (P < 0.005 versus controls; P < 0.05 versus amoxicillin-clavulanate and vancomycin treatment groups). These promising in vivo results with BAL9141 correlated with the high affinity of the drug for PBP 2a and its stability to penicillinase hydrolysis observed in vitro.

The reaction of fluorinated fatty acids, perfluorobutyric acid (C3F7CO2H), and perfluorododecanoic acid (C11F23CO2H), with dodecacarbonyltriruthenium (Ru-3(CO)(12)) under reflux in tetrahydrofuran, followed by addition of two-electron donors (L) such as pyridine, 1,3,5-triaza-7-phosphatricyclo[3.3.1.1]decane, or triphenylphosphine, gives stable diruthenium complexes Ru-2(CO)(4)((2)-(2)-O2CC3F7)(2)(L)(2) (1a, L=C5H5N; 1b, L=PTA; 1c, L=PPh3) and Ru-2(CO)(4)((2)-(2)-O2CC11F23)(2)(L)(2) (2a, L=C5H5N; 2b, L=PTA; 2c, L=PPh3). The catalytic activity of the complexes for hydrogenation of styrene under supercritical carbon dioxide has been assessed and compared to the analogous triphenylphosphine complexes with non-fluorinated carboxylato groups Ru-2(CO)(4)((2)-(2)-O2CC3H7)(2)(PPh3)(2) (3) and Ru-2(CO)(4)((2)-(2)-O2CC11H23)(2)(PPh3)(2) (4). In addition, the cytotoxicities of the fluorinated complexes 1 were also evaluated on several human cancer cell lines (A2780, A549, Me300, HeLa). The complexes appear to be moderately cytotoxic, showing greater activity on the Me300 melanoma cells. Single-crystal X-ray structure analyses of 1a and 3 show the typical sawhorse-type arrangement of the diruthenium tetracarbonyl backbone with two bridging carboxylates and two terminal ligands occupying the axial positions.

Acid-sensing ion channels (ASICs) are neuronal Na(+)-selective channels that are transiently activated by extracellular acidification. ASICs are involved in fear and anxiety, learning, neurodegeneration after ischemic stroke, and pain sensation. The small molecule 2-guanidine-4-methylquinazoline (GMQ) was recently shown to open ASIC3 at physiological pH. We have investigated the mechanisms underlying this effect and the possibility that GMQ may alter the function of other ASICs besides ASIC3. GMQ shifts the pH dependence of activation to more acidic pH in ASIC1a and ASIC1b, whereas in ASIC3 this shift goes in the opposite direction and is accompanied by a decrease in its steepness. GMQ also induces an acidic shift of the pH dependence of inactivation of ASIC1a, -1b, -2a, and -3. As a consequence, the activation and inactivation curves of ASIC3 but not other ASICs overlap in the presence of GMQ at pH 7.4, thereby creating a window current. At concentrations >1 mm, GMQ decreases maximal peak currents by reducing the unitary current amplitude. Mutation of residue Glu-79 in the palm domain of ASIC3, previously shown to be critical for channel opening by GMQ, disrupted the GMQ effects on inactivation but not activation. This suggests that this residue is involved in the consequences of GMQ binding rather than in the binding interaction itself. This study describes the mechanisms underlying the effects of a novel class of ligands that modulate the function of all ASICs as well as activate ASIC3 at physiological pH.

El nostre treball consta principalment de dues parts: En la primera ens hem apropat al tema presentant el context econòmic actual i el món de la roba de segona mà. A continuació ens hem centrat en la relació que existeix entre les dues parts. I per últim hem realitzat un estudi de mercat a partir d’enquestes. Amb aquestes, hem conegut tant la relació com la visió que té la gent sobre el tema, i a més hem establert el consumidor tipus que ens servirà pel futur muntatge del nostre negoci. Les enquestes les hem distribuït en tres districtes de diferent renda per veure si aquesta pot influir en les respostes. Amb la primera part ens hem adonat que la crisi ha fet disminuir la despesa, i aquest fet ajuda a que nous consumidors entrin al nostre mercat. Amb la segona part, i un cop vist que ens trobem en un context favorable, hem buscat la viabilitat que tindria la creació de la nostra pròpia botiga de roba de segona mà. Per fer-ho hem seguit els següents passos: hem buscat la localització òptima per la seva ubicació, hem llogat i dissenyat el local; ens hem constituït com a societat, hem realitzat un estudi dels possibles costos i hem desenvolupat el pla de màrqueting. Finalment, ens adonem que els nostres beneficis serien positius i per tant, el nostre projecte és viable.Actualment, Espanya està immersa en un crisi econòmica, juntament amb molts dels països industrialitzats. Això ha provocat que les famílies disminueixin les seves despeses a l’hora de consumir la major part de béns i serveis que feien fins ara. El que ha succeït doncs, es que han canviat els hàbits de consum, béns que es consumien abans o bé directament han deixat de ser consumits o bé han estat substituïts per d’altres amb un menor cost i que compleixen la mateixa funció. Com hem esmentat , molts productes han estat substituïts per d’altres que tenen un cost més baix. Creiem que un clar exemple és el tema de la roba. La moda és un mercat en competència perfecte, en que les diferents empreses que s’hi dediquen s’intenten diferenciar per diferents aspectes però el preu ve fixat per la relació entre oferta i demanda. És un bé que tothom ha de consumir, en major o menor grau, però essencial per tota la població. És aquí, on els comerços de segona mà tenen l’oportunitat d’abastir a la demanda d’aquests productes, d’una forma més econòmica, ja que són productes que ja han estat utilitzats i no se’ls hi pot fixar el mateix preu de mercat. El nostre treball es basa en veure com ha afectat l’actual crisi a l’hora de consumir de la població barcelonina, centrant-nos en el mercat de la moda i a partir d’aquí, com l’objetiu principal a l’hora de realitzar-ho, veure si seria viable la creació d’una botiga de roba de segona mà. Les motivacions principals que ens han portat a escollir aquest treball han estat bàsicament tres: La primera d’elles és l’actualitat del tema. Sentim parlar sobre la crisi a qualsevol lloc, ja sigui als mitjans de comunicació, al carrer o dins casa nostra ja que ens està afectant directa o indirectament a tota la població. En segon lloc, creiem que el negoci de segona mà no està gaire ben vist per la majoria de la gent i considerem que és una bona oportunitat que la gent desconeix. Amb la realització del treball ens hi volem endinsar una mica per aquest món, conèixer les possibilitats que amaga i establir la relació entre la crisi i el mercat de segona mà. Per últim trobem el que pertany a la segona part del treball: El muntatge d’una botiga. Com a joves estudiants d’econòmiques, ens crida molt l’atenció el fet de crear un propi negoci. Ens sembla molt interessant tots els processos que s’han de dur a terme fins a l’obertura de la tenda i volem extreure una visió empresarial sobre el treball. Pel que fa a la metodologia que hem seguit per poder realitzar el nostre treball, cal dir que hem separat aquest en dues parts: La primera part és bàsicament teòrica. En ella hem fet una visió general sobre la crisi en que ens trobem, com va arribar i les conseqüències que ha tingut. A continuació hem cercat diferents notícies a la premsa escrita i digital que parlaven de com havia augmentat el consum als comerços de segona mà i hem explicat com són aquests amb exemples de botigues de roba de segona mà. Per últim hem fet un estudi de mercat mitjançant una enquesta per tal de contrastar les hipòtesis inicials del nostre treball que a continuació exposarem i també per treure idees bàsiques sobre com hauria de ser el nostre comerç.La segona part és el treball pràctic, és a dir, tots els passos que hem hagut de seguir des de la idea de creació de la botiga fins a la posada en marxa pròpiament dita. Hem cercat tota la regulació que havíem de complir, ens hem constituït com a societat limitada, hem trobat un local i hem muntat l’interior, hem tingut contactes amb diferents proveïdors de roba, hem contractat als treballadors i hem fet un anàlisi sobre els costos que hauríem de patir a l’inici del projecte i a partir d’aquí extraurem la conclusió sobre si seria viable o no la creació d’aquest negoci. Per últim, exposem les nostres dues hipòtesis inicials, sobre les que a mida que anem avançant pel treball, ens argumentarem i finalment les confirmarem o negarem.1- La crisi ha fet disminuir el poder adquisitiu de la població, de manera que es redueix la despesa en consum i el mercat de la 2a mà es veu beneficiat ja que tenen uns preus inferiors, més accessibles per la majoria de la gent, per un mateix producte o de característiques similars als que compraven fins ara.2- Aprofitant la conjuntura econòmica és viable crear una botiga de roba de segona mà.

BACKGROUND AND AIM: Recurrent hepatitis C is a major cause of morbidity and mortality after liver transplantation (LT), and optimal treatment algorithms have yet to be defined. Here, we present our experience of the first 21 patients with recurrent hepatitis C treated in Lausanne. PATIENTS AND METHODS: Twenty-one patients with histologyproven recurrent hepatitis C after LT were treated since 2003. Treatment was initiated with pegylated interferon-α2a 135 μg per week and ribavirin 400 mg per day in the majority of patients, and subsequent doses were adapted individually based on on-treatment virological responses and clinical and/or biochemical side effects. RESULTS: On an intention-to-treat basis, sustained virological response (SVR) was achieved in 12/21 (57%) patients (5/11 [45%], 2/3 [67%], 4/5 [80%] and 1/2 [50%] of patients infected with genotypes 1, 2, 3 and 4, respectively). Two patients experienced relapse and 6 did not respond to treatment (NR). Treatment duration ranged from 24 to 90 weeks. It was stopped prematurely due to adverse events in 5/21 (24%) patients (with SVR achieved in 2 patients, NR in 2 patients, and death of one patient awaiting re-transplantation). Of note, SVR was achieved in a patient with combined liver and kidney transplantation. Importantly, SVR was achieved in some patients despite the lack of an early virological response or HCV RNA negativity at week 24. Darbepoetin α and filgrastim were used in 33% and 14%, respectively. CONCLUSION: Individually adapted treatment of recurrent hepatitis C can achieve SVR in a substantial proportion of LT patients. Conventional stopping rules do not apply in this setting so that prolonged therapy may be useful in selected patients.

CONTEXT: Many inherited disorders of calcium and phosphate homeostasis are unexplained at the molecular level. OBJECTIVE: The objective of the study was to identify the molecular basis of phosphate and calcium abnormalities in two unrelated, consanguineous families. PATIENTS: The affected members in family 1 presented with rickets due to profound urinary phosphate-wasting and hypophosphatemic rickets. In the previously reported family 2, patients presented with proximal renal tubulopathy and hypercalciuria yet normal or only mildly increased urinary phosphate excretion. METHODS: Genome-wide linkage scans and direct nucleotide sequence analyses of candidate genes were performed. Transport of glucose and phosphate by glucose transporter 2 (GLUT2) was assessed using Xenopus oocytes. Renal sodium-phosphate cotransporter 2a and 2c (Npt2a and Npt2c) expressions were evaluated in transgenically rescued Glut2-null mice (tgGlut2-/-). RESULTS: In both families, genetic mapping and sequence analysis of candidate genes led to the identification of two novel homozygous mutations (IVS4-2A>G and R124S, respectively) in GLUT2, the gene mutated in Fanconi-Bickel syndrome, a rare disease usually characterized by renal tubulopathy, impaired glucose homeostasis, and hepatomegaly. Xenopus oocytes expressing the [R124S]GLUT2 mutant showed a significant reduction in glucose transport, but neither wild-type nor mutant GLUT2 facilitated phosphate import or export; tgGlut2-/- mice demonstrated a profound reduction of Npt2c expression in the proximal renal tubules. CONCLUSIONS: Homozygous mutations in the facilitative glucose transporter GLUT2, which cause Fanconi-Bickel syndrome, can lead to very different clinical and biochemical findings that are not limited to mild proximal renal tubulopathy but can include significant hypercalciuria and highly variable degrees of urinary phosphate-wasting and hypophosphatemia, possibly because of the impaired proximal tubular expression of Npt2c.
