Background: Currently 1 in 11 women over the age of 60 in Australia are diagnosed with breast cancer. Following treatment, most breast cancer patients are left with shoulder and arm impairments which can impact significantly on quality of life and interfere substantially with activities of daily living. The primary aim of the proposed study is to determine whether upper limb impairments can be prevented by undertaking an exercise program of prolonged stretching and resistance training, commencing soon after surgery. Methods/design: We will recruit 180 women who have had surgery for early stage breast cancer to a multicenter single-blind randomized controlled trial. At 4 weeks post surgery, women will be randomly assigned to either an exercise group or a usual care ( control) group. Women allocated to the exercise group will perform exercises daily, and will be supervised once a week for 8 weeks. At the end of the 8 weeks, women will be given a home-based training program to continue indefinitely. Women in the usual care group will receive the same care as is now typically provided, i.e. a visit by the physiotherapist and occupational therapist while an inpatient, and receipt of pamphlets. All subjects will be assessed at baseline, 8 weeks, and 6 months later. The primary measure is arm symptoms, derived from a breast cancer specific questionnaire (BR23). In addition, range of motion, strength, swelling, pain and quality of life will be assessed. Discussion: This study will determine whether exercise commencing soon after surgery can prevent secondary problems associated with treatment of breast cancer, and will thus provide the basis for successful rehabilitation and reduction in ongoing problems and health care use. Additionally, it will identify whether strengthening exercises reduce the incidence of arm swelling. Esta pesquisa se dedica ao debate em torno da redação e composição do livro de Amós, e pretende, primeiramente, apresentar e criticar algumas hipóteses sobre as camadas literárias, que poderiam pertencer ao próprio profeta, e os acréscimos tardios. Como primeiro passo para uma abordagem do problema redacional, este estudo examina algumas contribuições de especialistas estrangeiros e de estudiosos brasileiros, e tenta fazer um julgamento crítico de suas hipóteses sobre a redação do livro de Amós. A questão do processo redacional do livro de Amós tem sido, na verdade, objeto de uma animada discussão entre muitos estudiosos. Em segundo lugar, a leitura diacrônica é complementada com o estudo sincrônico do livro, isto é, com o debate em torno da estrutura não só da composição em Amós 6,1-14, mas também do livro todo. Em terceiro lugar, a partir da avaliação do resultado da pesquisa e com a utilização do método histórico-crítico, a composição em Amós 6,1-14 é analisada como uma unidade literária de sentido. Para tal análise são, indubitavelmente, relevantes algumas considerações prévias sobre o gênero literário da lamentação, a denúncia profética da injustiça social e a instituição do banquete do marzeah. Em seqüência, esta tese analisa o fenômeno literário dos panfletos proféticos e da organização social, que poderia estar por detrás de sua autoria. Por isso, e levando em conta o desafio de novas propostas metodológicas, esta pesquisa visa dar uma contribuição específica ao estudo de Amós. Ela se empenha em demonstrar que, em relação a Amós 6,1-14, o leitor está diante de uma criação coletiva. De fato, a mencionada composição é o resultado não só da pregação oral do próprio profeta, mas também, em razão de sua forma compósita, das vozes proféticas de pessoas oprimidas, que, impulsionadas pelo Espírito, confirmaram a denúncia severa de Amós e até a completaram com suas próprias experiências e sofrimentos. Finalmente, um despretensioso excursus examina o livro de Amós em um contexto literário mais amplo, para dar uma visão geral do debate em torno do processo redacional do Livro dos Doze Profetas, particularmente a partir de sua perspectiva escatológica mediante a categoria teológica do Dia do Senhor.(AU)

This dissertation analyzed the existing work on travestility and transsexuality whose problematic research focused on issues related to health and / or health services. For this purpose, a Literature Review Systematized Descriptive in virtual databases was performed: Bank of Higher Education Personnel Improvement Coordination Thesis (CAPES), Brazilian Digital Library of Theses and Dissertations (BDTD), Scielo and PubMed, between the years 1997 and 2014 in Brazil. We used the search terms "transsexual," "transvestite" and "transgender", each associated with the search term "health", in Portuguese and English. Complementing this search, we used the Documentary Analysis methodology to assess pamphlets productions, institutional documents and non-governmental organizations (NGOs), which were incorporated into the discussion. 295 papers were identified, among theses, dissertations and scientific articles. Of these, 223 were excluded and 72 selected for analysis. Thus, it obtained five theses and dissertations 21 on the topic of travestility and 7 theses and dissertations 9 that deal with transsexuality. Among the selected papers, 16 deal with transsexuality and health, 5 address the issue of travestility and health and work, 9 refer to the term "transgender" and "health". Even though it is an emerging field of research, there is an apparent deviation of the speech, previously anchored in questions whose topics are related to confrontation, infection or illness by HIV / AIDS (level of specialized care) for discussions on the health care for transsexuals in the process (level of specialized care). Still, few papers have specific trans attention in primary care associated with a comprehensive health care, with the empowerment of individuals, respecting the power of life, which are configured as important issues for the Public Policy on Health today.

This article examines the 1938 historical novel 1649: A Novel of a Year by the Anglo-Australian communist polymath Jack Lindsay in the context of the politics of the Popular Front, and identifies the aesthetic and historiographic debates questions that inform Lindsay’s inventive rendition of the historical novel. The novel may be considered in light of what Lindsay later called his desire ‘to use the novel to revive revolutionary traditions’, as well as his ‘struggle to achieve an understanding of the Novel while writing novels’. Lindsay’s novel figures a reality becoming prosaic: it reproduces contemporary textual sources – tracts, pamphlets, newspapers – as part of its meditation on a nascent print culture whose products circulate in processes that mirror the increasingly conspicuous flow of commodities. In this sense, the novel offers a marxist reflection on its own conditions of possibility in emergent bourgeois culture, as well as intervening in the vexed question of the Civil War as a ‘bourgeois revolution’. The novel however seeks to capture a dialectical method of representing the revolution that acknowledges defeat while rearticulating the utopian content of the defeated radicals, a practice integral to Lindsay’s vision of popular history as a transhistorical dialogue. That utopian content is transmitted through two forms: popular song, which acts to supplement political writing; and the heroic portrayal of the Leveller John Lilburne on trial, whose conduct exemplifies praxis conceived as a unity of word, thought and action.

Oncological patients are submitted to invasive exams in order to obtain an accurate diagnosis; these procedures may cause maladaptative reactions (fear, anxiety and pain). Particularly in breast cancer, the most common diagnose technique is the incisional biopsy. Most of the patients are unaware about the procedure and for that reason they may focus their thoughts on possible events such as pain, bleeding, the anesthesia, or the later surgical wound care. Anxiety and pain may provoke physiological, behavioral and emotional complications, and because of this reason, the Behavioral Medicine trained psychologist takes an active role before and after the biopsy. The aim of this study was to evaluate the effect of a cognitive-behavioral program to reduce anxiety in women submitted to incisional biopsy for the first time. There were 10 participants from the Hospital Juárez de México, Oncology service; all of them were treated as external patients. The intervention program focused in psycho-education and passive relaxation training using videos, tape-recorded instructions and pamphlets. Anxiety measures were performed using the IDARE-State inventory, and a visual-analogue scale of anxiety (EEF-A), and the measurement of blood pressure and heart rate). Data were analyzed both intrasubject and intersubject using the Wilcoxon test (p≤0.05). The results show a reduction in anxiety (as in punctuation as in ranges) besides, a reduction in the EEF-A.

The scope of the thesis encapsulates the wider post-war White Separatist Movement from the origins of American Nazism under George Lincoln Rockwell to the later developments of leaderless resistance and the political and cultural changes to the movement. The specific focus will be upon the relationship between George Lincoln Rockwell and the leaderless resistance concepts, in particular through its development and utilisation. Due to the complexity of the issues and the variety of influencing factors it is necessary in the first instance to assess it in terms of a historiography to allow themes to develop. As a result of this historical analysis themes have become evident to allow a conceptual analysis. In particular the thesis will utilise the following thematic contexts for assessing the various developments within White Separatism: including, state building; political marketing; the role of the media; and the propensity for terror and hate activities. In assessing the basis upon which the conceptual analysis is developed the research has utilised extensive use of texts, radio broadcasts, and pamphlets from the movement. The study has also been able to consider, government reports, law enforcement updates and communications from Civil Rights groups and other agencies. In the conceptual analysis of this information and themes, the thesis utilises new concepts as a means of creating an understanding of a rapidly changing area of politics; including ‘organic politic’ and ‘political firms’, when assessing political marketing trends; and assessing terrorist motivation.

During the early Stuart period, England’s return to male monarchal rule resulted in the emergence of a political analogy that understood the authority of the monarch to be rooted in the “natural” authority of the father; consequently, the mother’s authoritative role within the family was repressed. As the literature of the period recognized, however, there would be no family unit for the father to lead without the words and bodies of women to make narratives of dynasty and legitimacy possible. Early modern discourse reveals that the reproductive roles of men and women, and the social hierarchies that grow out of them, are as much a matter of human design as of divine or natural law. Moreover, despite the attempts of James I and Charles I to strengthen royal patriarchal authority, the role of the monarch was repeatedly challenged on stage and in print even prior to the British Civil Wars and the 1649 beheading of Charles I. Texts produced at moments of political crisis reveal how women could uphold the legitimacy of familial and political hierarchies, but they also disclose patriarchy’s limits by representing “natural” male authority as depending in part on women’s discursive control over their bodies. Due to the epistemological instability of the female reproductive body, women play a privileged interpretive role in constructing patriarchal identities. The dearth of definitive knowledge about the female body during this period, and the consequent inability to fix or stabilize somatic meaning, led to the proliferation of differing, and frequently contradictory, depictions of women’s bodies. The female body became a site of contested meaning in early modern discourse, with men and women struggling for dominance, and competitors so diverse as to include kings, midwives, scholars of anatomy, and female religious sectarians. Essentially, this competition came down to a question of where to locate somatic meaning: In the opaque, uncertain bodies of women? In women’s equally uncertain and unreliable words? In the often contradictory claims of various male-authored medical treatises? In the whispered conversations that took place between women behind the closed doors of birthing rooms? My dissertation traces this representational instability through plays by William Shakespeare, John Ford, Thomas Middleton, and William Rowley, as well as in monstrous birth pamphlets, medical treatises, legal documents, histories, satires, and ballads. In these texts, the stories women tell about and through their bodies challenge and often supersede male epistemological control. These stories, which I term female bodily narratives, allow women to participate in defining patriarchal authority at the levels of both the family and the state. After laying out these controversies and instabilities surrounding early modern women’s bodies in my first chapter, my remaining chapters analyze the impact of women’s words on four distinct but overlapping reproductive issues: virginity, pregnancy, birthing room rituals, and paternity. In chapters 2 and 3, I reveal how women construct the inner, unseen “truths” of their reproductive bodies through speech and performance, and in doing so challenge the traditional forms of male authority that depend on these very constructions for coherence. Chapter 2 analyzes virginity in Thomas Middleton and William Rowley’s play The Changeling (1622) and in texts documenting the 1613 Essex divorce, during which Frances Howard, like Beatrice-Joanna in the play, was required to undergo a virginity test. These texts demonstrate that a woman’s ability to feign virginity could allow her to undermine patriarchal authority within the family and the state, even as they reveal how men relied on women to represent their reproductive bodies in socially stabilizing ways. During the British Civil Wars and Interregnum (1642-1660), Parliamentary writers used Howard as an example of how the unruly words and bodies of women could disrupt and transform state politics by influencing court faction; in doing so, they also revealed how female bodily narratives could help recast political historiography. In chapter 3, I investigate depictions of pregnancy in John Ford’s tragedy, ‘Tis Pity She’s a Whore (1633) and in early modern medical treatises from 1604 to 1651. Although medical texts claim to convey definitive knowledge about the female reproductive body, in actuality male knowledge frequently hinged on the ways women chose to interpret the unstable physical indicators of pregnancy. In Ford’s play, Annabella and Putana take advantage of male ignorance in order to conceal Annabella’s incestuous, illegitimate pregnancy from her father and husband, thus raising fears about women’s ability to misrepresent their bodies. Since medical treatises often frame the conception of healthy, legitimate offspring as a matter of national importance, women’s ability to conceal or even terminate their pregnancies could weaken both the patriarchal family and the patriarchal state that the family helped found. Chapters 4 and 5 broaden the socio-political ramifications of women’s words and bodies by demonstrating how female bodily narratives are required to establish paternity and legitimacy, and thus help shape patriarchal authority at multiple social levels. In chapter 4, I study representations of birthing room gossip in Thomas Middleton’s play, A Chaste Maid in Cheapside (1613), and in three Mistris Parliament pamphlets (1648) that satirize parliamentary power. Across these texts, women’s birthing room “gossip” comments on and critiques such issues as men’s behavior towards their wives and children, the proper use of household funds, the finer points of religious ritual, and even the limits of the authority of the monarch. The collective speech of the female-dominated birthing room thus proves central not only to attributing paternity to particular men, but also to the consequent definition and establishment of the political, socio-economic, and domestic roles of patriarchy. Chapter 5 examines anxieties about paternity in William Shakespeare’s The Winter’s Tale (1611) and in early modern monstrous birth pamphlets from 1600 to 1647, in which children born with congenital deformities are explained as God’s punishment for the sexual, religious, and/or political transgressions of their parents or communities. Both the play and the pamphlets explore the formative/deformative power of women’s words and bodies over their offspring, a power that could obscure a father’s connection to his children. However, although the pamphlets attempt to contain and discipline women’s unruly words and bodies with the force of male authority, the play reveals the dangers of male tyranny and the crucial role of maternal authority in reproducing and authenticating dynastic continuity and royal legitimacy. My emphasis on the socio-political impact of women’s self-representation distinguishes my work from that of scholars such as Mary Fissell and Julie Crawford, who claim that early modern beliefs about the female reproductive body influenced textual depictions of major religious and political events, but give little sustained attention to the role female speech plays in these representations. In contrast, my dissertation reveals that in such texts, patriarchal society relies precisely on the words women speak about their own and other women’s bodies. Ultimately, I argue that female bodily narratives were crucial in shaping early modern culture, and they are equally crucial to our critical understanding of sexual and state politics in the literature of the period.

A temática central deste relatório de estágio é a sexualidade na adolescência, área de grande interesse em Saúde Comunitária e contemplada no Plano Nacional de Saúde sendo, também, competência do Enfermeiro Especialista em Saúde Comunitária, o desenvolvimento de projetos em promoção da saúde. O principal objetivo deste relatório consiste em finalizar o ciclo formativo do II Mestrado de Especialização em Enfermagem de Saúde Comunitária, descrevendo e analisando as atividades desenvolvidas em estágio. No âmbito do Planeamento em Saúde foi efetuado um Diagnóstico de Situação que permitiu identificar as necessidades dos adolescentes numa escola da cidade de Portalegre, que serviram de base á realização de um estágio. Definiu-se o objetivo de contribuir para a implementação de atividades no âmbito do Plano Nacional de Saúde Escolar, intervindo sobre as áreas identificadas: Sexualidade e afetos; métodos contracetivos; prevenção de infeções sexualmente transmissíveis e mitos. Identificadas as áreas prioritárias, foram selecionadas estratégias e planeadas e efetuadas sessões de educação para a saúde, em duas escolas da cidade. Estas foram realizadas em turmas, através de metodologia expositiva/interativa e distribuição de panfletos interativos. As intervenções foram avaliadas através de um questionário final, cuja análise permitiu constatar a efetividade das mesmas. Concluímos que a maioria dos adolescentes reconhece a importância de sessões de informação acerca da sexualidade, considerando-se mais informados nesta área, após as atividades desenvolvidas. O estágio contribuiu para o desenvolvimento pessoal e profissional da autora, enquanto prestadora de cuidados e como futura Enfermeira Comunitária

Este relatório centra-se no tema da Sexualidade na Adolescência, tema pertinente em Enfermagem Comunitária e no Plano Nacional de Saúde. A promoção da sáude e o desenvolvimento de projectos, é uma das principais competências do Enfermeiro especialista em Enfermagem Comunitária. O principal objectivo deste relatório é apresentar, descrever e analizar as actividades desenvolvidas durante o estágio. Utilizando a metodologia do planeamento em saúde realizou-se um diagnóstico de situação numa escola da cidade de Portalegre, identificando-se necessidades de adolescentes, sendo estas a base o estágio. O estágio teve como objectivo contribuir para a implementação de actividades presentes no Plano Nacional de Saúde, intervindo nas seguintes áreas: sexualidade e afectos; métodos contraceptivos; prevenção de ist’s e mitos acerca da sexualidade. Definiram-se áreas prioritárias e, posteriormente, selecionaram-se estratégias. Foram planeadas e realizadas sessões de educação para a saúde em duas escolas da cidade de Portalegre. Estas foram desenvolvidas em sala de aula utilizando-se métodos desmonstrativos/interactivos e distribuindo-se de folhetos. Após as sessões aplicou-se um questionário de avaliação das actividades, cuja análise revelou a sua efectividade. Sumariamente, a maioria dos adolescentes reconhece a importância das sessões informativas sobre sexualidade, admitindo-se melhor informados acerca desta temática.
