The present study was aimed at assessing the experience of a single referral center with recurrent varicose veins of the legs (RVL) over the period 1993-2008. Among a total of 846 procedures for Leg Varices (LV), 74 procedures were for RVL (8.7%). The causes of recurrence were classified as classic: insufficient crossectomy (13); incompetent perforating veins (13); reticular phlebectasia (22); small saphenous vein insufficiency (9); accessory saphenous veins (4); and particular: post-hemodynamic treatment (5); incomplete stripping (1); Sapheno-Femoral Junction (SFJ) vascularization (5); post-thermal ablation (2). For the “classic” RVL the treatment consisted essentially of completing the previous treatment, both if the problem was linked to an insufficient earlier treatment and if it was due to a later onset. The most common cause in our series was reticular phlebectasia; when the simple sclerosing injections are not sufficient, this was treated by phlebectomy according to Mueller. The “particular” cases classified as 1, 2 and 4 were also treated by completing the traditional stripping procedure (+ crossectomy if this had not been done previously), considered to be the gold standard. In the presence of a SFJ neo-vascularization, with or without cavernoma, approximately 5 cm of femoral vein were explored, the afferent vessels ligated and, if cavernoma was present, it was removed. Although inguinal neo-angiogenesis is a possible mechanism, some doubt can be raised as to its importance as a primary factor in causing recurrent varicose veins, rather than their being due to a preexisting vein left in situ because it was ignored, regarded as insignificant, or poorly evident. In conclusion, we stress that LV is a progressive disease, so the treatment is unlikely to be confined to a single procedure. It is important to plan adequate monitoring during follow-up, and to be ready to reoperate when new problems present that, if left, could lead the patient to doubt the validity and efficacy of the original treatment.

Este estudo teve como objetivo verificar como a temática inclusão social tem sido abordada na área de Ciência da Informação, a partir da análise da produção científica publicada nos periódicos nacionais da área. Além disso, verificar quais maneiras de inclusão são recorrentemente abordadas na área da ciência da informação, mostrar as tendências de uso do conceito de inclusão social nos artigos científicos da área de Ciência da Informação, constatar como se apresenta o conceito de inclusão social relacionado ao profissional da informação e analisar se há relação com outras disciplinas. Foram feitas buscas em seis periódicos no período de 2001 a 2010. Utilizou-se como método de análise o referencial da análise de conteúdo de Bardin. Constituíram o corpus de análise 30 artigos que tratavam da temática inclusão social. Como resultados identificou-se que a inclusão social, no que se refere às publicações da área de Ciência da Informação, em geral, está voltada a inclusão digital e ao uso das Tecnologias da Informação e Comunicação. Ademais, identificou-se, ainda, relações com o profissional da informação, em que este deve servir como mediador entre a informação e os meios nos quais estas informações estão inseridas e os usuários.

We present a design for an electronic continuous pitch wind controller for musical performance. It uses a combination of linear position, magnetic reed, and air pressure sensors to generate three fully continuous control dimensions. Each control dimension is encoded and transmitted using the industry standard MIDI protocol to allow the instrument to interface with a large variety of synthesizers to control different parameters of the synthesis algorithm in real time, allowing for a high degree of expressiveness not possible with existing electronic wind instrument controllers. The first part of the thesis will provide a justification for the design of a novel instrument, and present some of the theory behind pitch representation, encoding, and transmission with respect to digital systems. The remainder of the thesis will present the particular design and explain the workings of its various subsystems.

Enquadramento: Actualmente existe um elevado números de pessoas em situação de dependência, com algum tipo de incapacidade funcional ou em risco de perda de funcionalidade, que necessitam ingressar na Rede Nacional de Cuidados Continuados com o intuito de se promover uma melhoria das condições de vida e bem-estar das pessoas, através da prestação de cuidados continuados de saúde e de apoio social. Aceitando a vulnerabilidade da pessoa humana, a doença revela-se como um período de fragilidade circunstancial às condições decorrentes da sua existência. O utente/família deparam-se com a desmoralização, a ameaça, o desamparo e a perda de sentido, podendo levar o mesmo a uma profunda tristeza, desespero e sofrimento. Neste sentido e tendo em conta que a esperança influencia o bem-estar e uma vez que se encontra ligada à existência, tanto física como emocional e espiritual é fundamental percebermos até que ponto a esperança pode ajudar a pessoa a lidar com o futuro. Objetivos: Avaliar níveis de esperança nos Doentes internados em Cuidados Continuados, e identificar determinantes socio-demográficos, clínicas e psicossociais correlacionadas com essa esperança. Métodos: O estudo foi realizado,na Unidade de Cuidados Continuados da Santa Casa da Misericórdia de Seia, nas unidades de internamento de Média Duração e Reabilitação e Unidade de Longa Duração e Manutenção. O questionário foi aplicado a 46 doentes, com idades compreendidas entre os 46 e os 92 anos de idade ( x =74.39 anos). Trata-se de um estudo não experimental, quantitativo, do tipo transversal, descritivo e correlacional. O instrumento de recolha de dados integrou um Questionário de caracterização sócio-demográfica e clínica, a escala da Esperança (Herth Hope Index), uma escala para a avaliação da Qualidade de Vida – Functinal Assessement of Cancer Therapy – General e o Questionário de Sono de Oviedo. Resultados: Os dados mostram que 45,7% dos participantes apresenta esperança reduzida, 39,1% esperança elevada e 15,2% esperança moderada. Constatámos ainda que apenas o “bem-estar funcional” da escala da QDV e os “fenómenos adversos” da escala do sono se correlacionam significativamente (p=0,000; p=0,035) com a esperança explicando respetivamente 55,2% e 31,1% da variância explicada. Já o género, idade, estado civil, situação profissional, escolaridade, rendimento mensal, tipologia e número de internamentos, mostraram não estar relacionados com a esperança dos nossos doentes. Conclusão: A esperança é uma crença ou virtude inerente ao Homem, que pode assumir níveis diferenciados, que acompanha o ser humano no seu processo de viver e de morrer condicionando ajustes nos momentos de crise, afectando e/ou sendo afetada pelo bem-estar e a qualidade de vida. In this work, a platform to the conditioning, digitizing, visualization and recording of the EMG signals was developed. After the acquisition, the analysis can be done by signal processing techniques. The platform consists of two modules witch acquire electromyography (EMG) signals by surface electrodes, limit the interest frequency band, filter the power grid interference and digitalize the signals by the analogue-to- digital converter of the modules microcontroller. Thereby, the data are sent to the computer by the USB interface by the HID specification, displayed in real-time in graphical form and stored in files. As processing resources was implemented the operations of signal absolute value, the determination of effective value (RMS), Fourier analysis, digital filter (IIR) and the adaptive filter. Platform initial tests were performed with signal of lower and upper limbs with the aim to compare the EMG signal laterality. The open platform is intended to educational activities and academic research, allowing the addition of other processing methods that the researcher want to evaluate or other required analysis.

In a context of rapid demographic and technological changes, digital skills are essential in order for citizens to actively participate in society. However, digital literacy for all citizens, especially for the older population, is not yet a reality. It is increasingly crucial for active ageing, lifelong learning, and life-wide learning that the elderly learn digital skills. Intergenerational learning can play a key role in achieving a wide range of goals. This paper focuses on the contribution of intergenerational learning to digital and social inclusion. We promoted ICT intergenerational workshops and chose the case study methodolog y to study three distinct cases of intergenerational learning with ICT. The results show that intergenerational learning with ICT contributes to the digital literacy of adults and seniors and fosters lifelong learning, active ageing, and understanding and solidarity among generations. We reveal the benefits of the intergenerational learning process for all participants and suggest some ways to achieve intergenerational learning through ICT in order to build more socially and digitally cohesive societies.

Background: The Medical Education Partnership Initiative, has helped to mitigate the digital divide in Africa. The aim of the study was to assess the level of access, attitude, and training concerning meaningful use of electronic resources and EBM among medical students at an African medical school. Methods: The study involved medical students at the University of Zimbabwe College of Health Sciences, Harare. The needs assessment tool consisted of a 21-question, paper-based, voluntary and anonymous survey. Results: A total of 61/67 (91%), responded to the survey. 60% of the medical students were ‘third-year medical students’. Among medical students, 85% of responders had access to digital medical resources, but 54% still preferred printed medical textbooks. Although 25% of responders had received training in EBM, but only 7% found it adequate. 98% of the participants did not receive formal training in journal club presentation or analytical reading of medical literature, but 77 % of them showed interest in learning these skills. Conclusion: Lack of training in EBM, journal club presentation and analytical reading skills have limited the impact of upgraded technology in enhancing the level of knowledge. This impact can be boosted by developing a curriculum with skills necessary in using EBM.

During the last 15 years, the public school system in Bogotá, Colombia has maintained a concession system in which 25 schools are managed privately with exemptions to many of the rules required in the traditional schools -- This study uses the propensity score matching technique to examine whether students in the privately-managed schools have better scores on the Saber 11° examinations taken upon completion of secondary school -- The results for 251 schools indicates that students with comparable socioeconomic characteristics score considerably better on these tests in the privately-managed schools than in the traditional public schools -- Thus, there is evidence that the privately-managed public schools are a cost-effective alternative to the traditional public school

A visibility/invisibility paradox of trust operates in the development of distributed educational leadership for online communities. If trust is to be established, the team-based informal ethos of online collaborative networked communities requires a different kind of leadership from that observed in more formal face-to-face positional hierarchies. Such leadership is more flexible and sophisticated, being capable of encompassing both ambiguity and agile response to change. Online educational leaders need to be partially invisible, delegating discretionary powers, to facilitate the effective distribution of leadership tasks in a highly trusting team-based culture. Yet, simultaneously, online communities are facilitated by the visibility and subtle control effected by expert leaders. This paradox: that leaders need to be both highly visible and invisible when appropriate, was derived during research on 'Trust and Leadership' and tested in the analysis of online community case study discussions using a pattern-matching process to measure conversational interactions. This paper argues that both leader visibility and invisibility are important for effective trusting collaboration in online distributed leadership. Advanced leadership responses to complex situations in online communities foster positive group interaction, mutual trust and effective decision-making, facilitated through the active distribution of tasks.

Établir une régulation de l’économie numérique au Sénégal représente un enjeu fondamental pour les gouvernants et l’ensemble des acteurs qui la compose. Suivant une démarche plus globalisée, d’énormes mutations normatives visant les rationalités et les mécanismes de réglementations ont évolué dans le temps donnant une place plus considérable au droit dans les politiques publiques des États. Différents modèles normatifs et institutionnels sont ainsi adaptés pour prendre en charge le phénomène de la convergence dépendamment du contexte réglementaire du pays. Pour ce qui est du contexte actuel du Sénégal, l’étanchéité des réglementations relatives aux télécommunications et à l’audiovisuel, désormais convergent, est fondée sur un modèle de réglementation sectorielle. Toutefois, leur convergence a provoqué un brouillage des frontières qui risque désormais de poser des conséquences énormes sur le plan normatif tel que des risques d’enchevêtrement sur le plan institutionnel ou réglementaire. Or au plan national, il n’existe à ce jour aucun texte visant à assoir les bases d’une régulation convergente. Ainsi, à la question de savoir si la régulation sectorielle est pertinente au regard de l’environnement du numérique marqué par la convergence, il s’est avéré qu’elle pourrait être adoptée comme modèle à court terme. Mais dans un but de réaliser des économies d’échelle pour réguler efficacement les différents secteurs et industries infrastructurelles, il faut un modèle de régulation unique marquée par la fusion de l’ARTP et du CNRA. D’une part, la régulation sectorielle permet d’accompagner la transition vers le numérique déjà lancée et d’autre part la régulation multisectorielle servira une fois la convergence des marchés établis.
