We show that nuclear C*-algebras have a re ned version of the completely positive approximation property, in which the maps that approximately factorize through finite dimensional algebras are convex combinations of order zero maps. We use this to show that a separable nuclear C*-algebra A which is closely contained in a C*-algebra B embeds into B.

The nuclear phenotypes of Malpighian tubule epithelial cells of 5th instar male nymphs of the blood-sucking insect Panstrongylus megistus were studied immediately after a short (1 h) cold shock at 0ºC, and 10 and 30 days later. The objective was to compare the responses to a cold shock with those known to occur after hyperthermia in order to provide insight into the cellular effect of cold in this species. Nuclei which usually exhibited a conspicuous Y chromosome chromocenter were the most frequent phenotype in control and treated specimens. Phenotypes in which the heterochromatin was unravelled, or in which there was nuclear fusion or cell death were more abundant in the shocked specimens. Most of the changes detected have also been found in heat-shocked nymphs, except for nuclear fusion which generates giant nuclei and which appeared to be less effective or necessary than that elicited after heat shock. Since other studies showed that a short cold shock does not affect the survival of more than 14% of 5th instar nymphs of P. megistus with domestic habit and can induce tolerance to a prolonged cold shock, heat shock proteins proteins are probably the best candidates for effective protection of the cells and the insects from drastic damage caused by low temperature shocks.

The Instituto Venezolano de Investigaciones Cientificas (IVIC) is a government-funded multidisciplinary academic institution dedicated to research, development and technology in many areas of knowledge. Biomedical projects and publications comprise about 40% of the total at IVIC. In this article, we present an overview of some selected research and development projects conducted at IVIC which we believe contain new and important aspects related to malaria, ancylostomiasis, dengue fever, leishmaniasis and tuberculosis. Other projects considered of interest in the general area of tropical medicine are briefly described. This article was prepared as a small contribution to honor and commemorate the centenary of the Instituto Oswaldo Cruz.

The report presents a grammar capable of analyzing the process of production of electricity in modular elements for different power-supply systems, defined using semantic and formal categories. In this way it becomes possible to individuate similarities and differences in the process of production of electricity, and then measure and compare “apples” with “apples” and “oranges” with “oranges”. For instance, when comparing the various unit operations of the process of production of electricity with nuclear energy to the analogous unit operations of the process of production of fossil energy, we see that the various phases of the process are the same. The only difference is related to characteristics of the process associated with the generation of heat which are completely different in the two systems. As a matter of facts, the performance of the production of electricity from nuclear energy can be studied, by comparing the biophysical costs associated with the different unit operations taking place in nuclear and fossil power plants when generating process heat or net electricity. By adopting this approach, it becomes possible to compare the performance of the two power-supply systems by comparing their relative biophysical requirements for the phases that both nuclear energy power plants and fossil energy power plants have in common: (i) mining; (ii) refining/enriching; (iii) generating heat/electricity; (iv) handling the pollution/radioactive wastes. This report presents the evaluation of the biophysical requirements for the two powersupply systems: nuclear energy and fossil energy. In particular, the report focuses on the following requirements: (i) electricity; (ii) fossil-fuels, (iii) labor; and (iv) materials.

La neumonía adquirida en la comunidad (NAC) es una enfermedad respiratoria infecciosa muy prevalente y de elevada mortalidad que desencadena inflamación sistémica. Las últimas investigaciones analizan la respuesta inflamatoria y han observado que la infección por determinados microorganismos se caracterizaría por un “patrón” de respuesta inflamatoria. Nuestro estudio analizó la diferente etiología de la NAC y la respuesta inflamatoria (PCR, PCT y citocinas). Concluyó que las NAC causadas por microorganismo conocido presentan un “patrón” de inflamación específico (S. pneumoniae eleva la PCT e IL-6, la Legionella aumenta la PCR), y que la respuesta está incrementada en presencia de bacteriemia.

S’han analitzat de forma prospectiva els malalts ingressats en un Servei de Medicina Interna diagnosticats de càncer durant 19 mesos. S’han avaluat 95 pacients amb edat mitja de 69 anys, la majoria homes. Les neoplàsies més freqüents han estat respiratòries, digestives i hematològiques. Han consultat per símptomes inespecífics, de llarga evolució i sovint amb malaltia disseminada. Conèixer el perfil que presenten aquests pacients ens pot permetre millorar el temps que tarden a consultar, optimitzar el procés diagnòstic per a què sigui el menys invasiu possible, precís i cost-efectiu; i agilitzar el temps fins al tractament.

La amiloidosis es una enfermedad por depósito de amiloide en diferentes tejidos, produciendo su disfunción. Es infrecuente, con elevada mortalidad y escasas opciones terapéuticas. La punción de grasa subcutánea (PAG) es segura y sensible para diagnosticarla, pero por su clínica inespecífica, es necesario sospecharla. Estudio retrospectivo de las PAGs en 22 años de un Servicio de Medicina Interna. La quinta parte resultaron positivos. Se describen sus características clínicas y mortalidad, señalando las enfermedades predisponentes y parámetros del laboratorio más característicos. Se analiza la ecocardiografia en el diagnóstico y la dificultad de la PAG en pacientes con escaso panículo adiposo.

Nuclei bind yeast vacuoles via nucleus-vacuole (NV) junctions. Under nutrient restriction, NV junctions invaginate and release vesicles filled with nuclear material into vacuoles, resulting in piecemeal microautophagy of the nucleus (PMN). We show that the electrochemical gradient across the vacuolar membrane promotes invagination of NV junctions. Existing invaginations persist independently of the gradient, but final release of PMN vesicles requires again V-ATPase activity. We find that NV junctions form a diffusion barrier on the vacuolar membrane that excludes V-ATPase but is enriched in the VTC complex and accessible to other membrane-integral proteins. V-ATPase exclusion depends on the NV junction proteins Nvj1p,Vac8p, and the electrochemical gradient. It also depends on factors of lipid metabolism, such as the oxysterol binding protein Osh1p and the enoyl-CoA reductase Tsc13p, which are enriched in NV junctions, and on Lag1p and Fen1p. Our observations suggest that NV junctions form in two separable steps: Nvj1p and Vac8p suffice to establish contact between the two membranes. The electrochemical potential and lipid-modifying enzymes are needed to establish the vacuolar diffusion barrier, invaginate NV junctions, and form PMN vesicles.

Introducción: la anemia es una entidad muy prevalente en pacientes con insuficiencia cardíaca (IC) y se asocia a mayor morbimortalidad. Objetivos: analizar las comorbilidades de pacientes hospitalizados de ≥ 65 años con IC. Estudiar el impacto de la anemia sobre esta población y comparar esta comorbilidad respecto a aquéllos que no la presentan en relación a la valoración geriátrica integral (VGI), así como la adecuación del tratamiento farmacológico en la IC. Pacientes y métodos: estudio descriptivo observacional transversal de una cohorte de 150 pacientes ingresados en el Servicio de Medicina Interna del Hospital Vall d’Hebron, entre junio de 2007 y enero de 2010, mediante entrevista clínica y recogida de datos de la historia clínica. Resultados: en la muestra prevalecían las mujeres (62%), los pacientes con hipertensión (84%), los que tenían la FEVI conservada (66,4%) y los pacientes con anemia (61,3%), de los cuales el 60,9% presentaban anemia inflamatoria y el 35,9% anemia ferropénica. Los pacientes con anemia tuvieron peor valor de MNA (p=0,017, con un RR de 2,7), mayor diferencial de Barthel (p=0,021) y peor valor de albuminemia (p=0,001). Asimismo, se observó que 53 pacientes con indicación según las guías clínicas de tratamiento con IECA o ARA II no lo seguían, y hasta 105 pacientes en el caso de los betabloqueantes (BB). Conclusiones: los pacientes con anemia presentaban pero estado nutricional, y mayor empeoramiento del índice de Barthel. Respecto al tratamiento de la IC, destacaba el gran número de pacientes sin tratamiento con IECA, ARA II o BB, que deberían llevarlo según las guías de práctica clínica.

Objectius: analitzar comorbiditats de pacients hospitalaris ≥65 anys amb Insuficiència Cardíaca (IC). Adequació tractament farmacològic. Impacte Insuficiència Renal (IR). Metodologia: estudi descriptiu transversal de 150 pacients ingressats en Medicina Interna Hospital Vall d'Hebron entre juny'2007-gener'2010. Resultats: hipertensió arterial: 84%; obesitat: 32,1%; cardiopatia isquèmica: 41,3%; fracció d'ejecció del ventricle esquerre (FEVI) conservada: 70%. 53 pacients sense antagonistes de l'enzim convertidor de l'angiotensina, 105 sense βBloquejants i 55 sense antialdosterònics. Prevalença IR: 70%. Factors de risc: HTA, sexe femení. IC+IR+anèmia: 66 pacients, 2 tractament amb eritropoetina. Conclusions: IC de causa hipertensiva, amb FEVI conservada. Mala adequació tractament. Elevada prevalença IR. Importància Síndrome cardiorenal.

Avaluació dels factors de risc que es relacionen amb la mortalitat en pacients amb exacerbació de la MPOC hospitalitzats en serveis de Medicina Interna en un estudi multicèntric transversal, valorant la possible associació de la mort hospitalitzats amb diferents variables: sociodemogràfiques, tractament abans i durant la hospitalització, característiques clíniques de la MPOC, comorbiditats prèvies, dades clíniques, resultats de laboratori i troballes electrocardiogràfics. Amb 398 pacients, 88,7% eren homes i amb una mitjana de 75 anys, va morir un 5,3%. Trobant associació entre el risc de morir durant l'hospitalització i el diagnòstic previ de pneumònia, malaltia coronària i ACV.

OBJECTIVE: To investigate the involvement of the nuclear factor (NF)-kappaB in the interleukin (IL)-1 beta-mediated macrophage migration inhibitory factor (MIF) gene activation. DESIGN: Prospective study. SETTING: Human reproduction research laboratory. PATIENT(S): Nine women with endometriotic lesions. INTERVENTION(S): Endometriotic lesions were obtained during laparoscopic surgery. MAIN OUTCOME MEASURE(S): The MIF protein secretion was analyzed by ELISA, MIF mRNA expression by quantitative real-time polymerase chain reaction (PCR), NF-kappaB translocation into the nucleus by electrophoresis mobility shift assay, I kappaB phosphorylation and degradation by Western blot, and human MIF promoter activity by transient cell transfection. RESULT(S): This study showed a significant dose-dependent increase of MIF protein secretion and mRNA expression, the NF-kappaB translocation into the nucleus, I kappaB phosphorylation, I kappaB degradation, and human MIF promoter activity in endometriotic stromal cells in response to IL-1 beta. Curcumin (NF-kappaB inhibitor) significantly inhibited all these IL-1 beta-mediated effects. Analysis of the activity of deletion constructs of the human MIF promoter and a computer search localized two putative regulatory elements corresponding to NF-kappaB binding sites at positions -2538/-2528 bp and -1389/-1380 bp. CONCLUSION(S): This study suggests the involvement of the nuclear transcription factor NF-kappaB in MIF gene activation in ectopic endometrial cells in response to IL-1 beta and identifies a possible pathway of endometriosis-associated inflammation and ectopic cell growth.

The nuclear phenotypes of Malpighian tubule cells in fifth instar nymphs of Triatoma infestans, one of the most important vectors of Chagas disease, were studied following sequential shocks at 0ºC, separated by intervals of 8 h and 24 h at 30ºC, under conditions of moderate fasting and full nourishment. The insects pertained to colonies reared in the laboratory and originated from domestic specimens collected in the Brazilian states of São Paulo (north) and Minas Gerais (south). Since nuclear phenotypes in this species are affected by single cold shocks, it was expected that these phenotypes could also be changed by sequential shocks. Nuclear phenotypes indicative of mechanisms of cell survival (nuclear fusion and heterochromatin decondensation) and cell death (apoptosis and necrosis) were observed concomitantly in all the conditions tested. Nuclear fusion and heterochromatin decondensation were not found relevant for the presumed acquisition of the cold-hardening response in T. infestans. The decreased frequency of apoptosis and necrosis following sequential cold shocks including under fasting conditions, indicated that tolerance to sequential cold shocks occurred in T. infestans of the mentioned origin.
