From July 1984 to September 1986, 105 cases of American cutaneous leishmaniasis were studied in a locality closely situated to an urbanized area of the city of Rio de Janeiro, Brazil. Settement in this area was established at least 20 years ago but the first cases were noted six months prior to the beginning of this study. Cases were almost exlusively cutaneous and ulcerated, with one to six months of evolution. Montenegro's skin tests were positive in all cases and anti-Leishmania antibodies were detected by indirect immunofluorescence test in 74.3% of the patients. Parasites were demonstrated in 69.5% of cases. Domestic animals were easily found infected; 32% of the examined dogs and 30.8% of the examined equines were positive to the presence of Leishmania in cutaneous ulcerated lesions. Parasite isolates from human, dog andequines were immunologically characterized and identified as L. b. braziliensis. 73,0% of the sandfly population were Lutzomyia intermedia mainly caught on human baits and on domestic animals. Our observations suggest that this is an area of recent established L. b. braziliensis infection and that transmission probably occurs indoors or outdoors close to the houses.

In view of recent studies incriminating several species of anophelines, besides Anopheles darlingi, as malaria vectors in the Brazilian Amazon, we performed an anopheline survey in four localities - Ariquemes, Cujubim, Machadinho and Itapoã do Oeste - in Rondônia, the most malarious State in the Country. Twenty species were found. An. darlingi was, by far, the dominant species and the only one whose density coincided with that of malaria. On human baits it was more numerous in the immediate vincinity of houses than indoors whre, however, it was almost the only species encountered. On both situations it fed mostly at sunset and during the first half of the night. It was less numerous far from houses and scarce inside the forest. Other species (An. triannulatus, An. evansae, An. albitarsis, An. strodei) appeared in appreciable numbers only in Ariquemes, both in areas with and without malaria. The remaining species were scanty. An. darlingi was confirmed as the primary local vector.

In paralel with several other epidemiologic and entomologic data of 19 Municipalities of Espírito Santo State, Brazil, the feeding pattern of 222 Triatoma vitticeps is studied through precipitin tests. Very high levels of natural infection with Trypanosoma cruzi are observed in adult insects, in contrast with the abscence or minimum degrees of infection among nymphs and human individuals. The precipitin tests showed the contact of the insects with multiple blood sources, chiefly human and birds, followed by rodents and marsupials. The data suggest that T. vitticeps in spite of being highly antropophilic, become infected by T. cruzi in sylvatic ambient and occasionally invade houses. The species doesn't seem to be - at least until now - a good vector in the domestic cycle of Chagas' disease. Several factors seem to be involved in this conclusion, mainly the low density of the insect in the houses, its hardness to coloniza them, its slowness concerning to suction and defecation and possibly its low susceptibility to different T. cruzi strains.

Along 24 consecutive months, from January 1984 to December 1986, mosquito captures were performed in a rural area of said municipality. Aiming to evaluate the comparative frequency of the mosquito species inside houses and in the immediate vicinity and far from houses, the captures were made in two types of domiciles - one permanently and the other only sporadically inhabited - as well as in surrounding woods. Variations in temperature, relative humidity and rainfall were locally recorded. A tendency for domiciliation became evident by the presence of Aedes scapularis in the human domiciles, mainly in those permantely inhabited. Culex quinquefasciatus showed to be adapted to live with humans also in rural areas, in which, in some instances, insecticides had to be used to keep in under control. Such observations, mainly in terms of Ae. scapularis, reinforce the possibility of those mosquitoes, under favourable conditions, becoming carries of arboviroses to humans in rural environments.

We sprayed, in three groups of Domiciliary Units, 10 and 15 g of alphacypermethrin and 31.3 g of cypermethrin per pump of 10 liters, equivaling, if applied on 250 [squared meters), respectiveliy, to 60 and 40 mg of a alphacypermethrin [per squared meter] and 125,2 mg of cypermethrin [per squared meter]. The invasion, colonization and cumulative indices in the houses and the positivity index in shelters near houses were very low and similar in the three groups, in the 15 months of evaluation, indicating that alphacypermethrin can replace cypermethrin. Triatoma infestans predominated in the Domiciliary Units before treatment and T. brasiliensis and T. pseudomaculata predominated after it.

Lutzomyia intermedia was the predominant species. It was collected indoors but in much higher numbers outdoors, in the close vicinity of houses. In comparative captures it was found to feed more frequently on equines, and less frequently on man and dogs. Studies, using man and animals as baits, showed that blood-feeding started in the evening, and remained stable during the night, until early morning. June, August and October were the months of highest density.

The occurence of acute cutaneous leishmaniasis among inhabitants of 10 farms within 10 Km of the hamlet of Corte de Pedra, Bahia, Brazil was studied prospectively from 1984-l989. A mean population of 1,056 inhabitants living in 146 houses were visited every 6 months and the number of sKin ulcers recorded. A leishmanin skin test survey was done people with suggestive skin scars or active disease in l984. The incidence of skin ulcers due to Leishmania (Viannia) brasiliensis (Vlb) reached 83/1,000 inhabitants but declined sharply in the subsequent 2 years. Retrospective data shows that leishamiasis is a sporadic endemic disease. Although the reasons for this epidemic are unclear some possible aetiological factors are discussed.

The effect of deltamethrin of the sandfly population in a focus of American Cutaneous Leishmaniasis in Viana, Espírito Santo State is desvribed. The phlebotomine population density was determined inside and outside houses during a one year period in a treated and untreated area. The results showed a significant reduction in the number of sandflies inside houses in the treated area compared with both the untreated area and the same area before sparying. Despite having a residual action for 12 months after spraying the insecticide was ineffective outside houses. It is concluded therefore that deltamethrin is an efficient insecticide for reducing the number of phlebotomine sandflies inside houses.

A study of peridomestic man-biting culicines in the Amazon Basin was conducted from January through December, 1987. Fifteen species of mosquitoes from six genera were collected by volunters in all-night human-bait indoor and outdoor collections at five hoses in and near the town of Costa Marques, Rondônia, Brazil. Culex quinquefasciatus and members of the Mansonia titillans/indubitans Group comprised 61 and 33%, respectively, of all culcines collected from human-bait outside houses and 62 and 35%, respectively, of those collected from volunteers inside houses in the town. In rural areas, Cx. quinquefasciatus was less abundant and only comprised 2 and 5% of the culcines, respectively, collected inside and outside houses. Mansonia titillans/indubitans Group comprised 75% and 86% of the culicines collected inside and outside houses, respectively, from rural residences. Culex quinquefasciatus and members of Mn. titillans/indubitans Group were more endophilic than other culcines collected. Nocturnal and seasonal biting rhythms for the more common culcines are described

To identify wild hosts of Trypanosoma cruzi, surveys were conducted in the subandean valleys of Jujuy Province, Argentina, between June 1986 and March 1987. Seventy two mammals from 13 different species were examined by xenodiagnosis. Fifty two of them were mostly roedents trapped at the localities of Maimará, León and Tilcara, and the remainder had been kept in captivity at the Estación Biológica Experimental, in Jujuy. Trypanosoma cruzi infection was detected only in 2 Octodontomys gliroides (2 pos./8 exam. 25%) from all 72 examined mammals. Isolates were called Octodontomys Argentina 1 and 2 (OA1 and OA2). Both infected animals were caught at the archaelogical ruin of Pucará, at Tilcara. Repeated searches for triatomines in the ruin itself and in neighbour houses rendered negative results. Groups of mice inoculated with either OA1 or OA2 isolates became infected between 7 (OA1) to 12 days (OA2) postinoculation PI. Parasitemia peaks were observed between day 12th - 14th PI. Scarce amastigote nests were found in myocardium and skeletal muscle. Mortality was observed only for mice inoculated with OA1. Isoenzyme patterns of OA1 and OA2 were identical to one found in dogs and slightly different from that of human parasites in Argentina. Bones from Octodontomys sp., were recently found in a cave, dated 10200-8600 BC, in Pumamarca, near Tilcara, Jujuy. There are evidences that O. gliroides cohabited with man in ancient times and was associated to the domestic cycle of T. cruzi transmission, playing a role like that of domestic cavies. in Bolivia.

In the Epidemiological Surveillance Program in the county of Bambuí, Minas Gerais, between August 1986 and December 1988, 154 Panstrongylus megistus were captured by the local population in both peridomicile and intradomicile environments. Fifteen (9.8%) of the P. megistus harboured Trypanosoma cruzi. Preciptin tests showed that the most frequent triatomine blood meal sources were birds, but other sources were dogs, men and cats. The isoenzyme characterization of 13 T. cruzi strains showed that six belonged to zymodeme Z1, corresponding to the wild parasites, and seven belonged to zymodeme Z2, corresponding to parasites isolated from chronic chagasic patients (domestic cycle). As P. megistus were found to be naturally infected by parasites from both cycles, they are cleary able to transmit T. cruzi from the wild cycle to the domestic cycle. Furthermore the capacity of P. megistus in colonizing houses was observed in one residence, vacant for several years, in wich 153 triatomines were captured. The data show the possibility of P. megistus reintroducing transmission of Chaga's disease in the county if Epidemiological Surveillance is interrupted.

The Kilombero Malaria Project (KMP) attemps to define opperationally useful indicators of levels of transmission and disease and health system relevant monitoring indicators to evaluate the impact of disease control at the community or health facility level. The KMP is longitudinal community based study (N = 1024) in rural Southern Tanzania, investigating risk factors for malarial morbidity and developing household based malaria control strategies. Biweekly morbidity and bimonthly serological, parasitological and drug consumption surveys are carried out in all study households. Mosquito densities are measured biweekly in 50 sentinel houses by timed light traps. Determinants of transmission and indicators of exposure were not strongly aggregated within households. Subjective morbidity (recalled fever), objective morbidity (elevated body temperature and high parasitaemia) and chloroquine consumption were strongly aggregated within a few households. Nested analysis of anti-NANP40 antibody suggest that only approximately 30% of the titer variance can explained by household clustering and that the largest proportion of antibody titer variability must be explained by non-measured behavioral determinants relating to an individual's level of exposure within a household. Indicators for evaluation and monitoring and outcome measures are described within the context of health service management to describe control measure output in terms of community effectiveness.
