El pie plano astrágalo vertical congénito es una deformidad poco frecuente que se puede encontrar de forma aislada, dentro de un cuadro polimalformativo o en relación con afectaciones neurológicas. Se caracteriza por una luxación del escafoides por encima del astrágalo,lo que provoca un pie rígido o de dificil manipulación. En este artículo los autores describen un caso clínico de un paciente que acude a consulta por presentar una inflamación dolorosa en la zona de la cabeza del astrágalo del pie derecho. Una vez realizado todo el protocolo exploratorio, se plantea la resolución mediante un plan de tratamiento en dos fases: una primera fase consistente en la realización de paracentesis evacuadora o terapéutica-curativa; y una segunda fase en la que se diseña tratamiento ortopodológlco de descarga.

The biodistribution of simultaneous intra-arterial and intravenous injections of a radiolabelled anti-CEA MAb F(ab')2 fragment was studied in three patients with liver metastases from colorectal cancer. Identical MAb fragments, labelled with either 125I or 131I, were injected over a period of 30 min into the hepatic artery and into a peripheral vein. After 1 or 2 days, biodistribution was measured in the surgically removed metastases, normal tissue samples and blood. By tissue radioactivity counting, tumour uptake in the range 6.3-9.1% of injected dose per gram was found. Superimposable metastasis-to-blood and metastasis-to-normal liver ratios were obtained for both iodine isotopes in all three patients. The results indicate that the intra-arterial injection of MAb F(ab')2 fragments gives no measurable advantage over more convenient injections into a peripheral vein.

Ensaios de competição intra-específica foram conduzidos sobre Amaranthus viridis L. e A. hybridus L., a última espécie com dois biótipos distintos, tipo verde e tipo roxo. Os três ensaios, conduzidos separadamente, utilizaram cinco densidades de sementes por vaso. Em termos gerais, as sementes germinadas prontamente foram a razão do surgimento da próxima geração, visto que estabeleceram plântulas rapidamente e estas venceram em competição. A partir de certo número de sementes semeadas (20 por vaso), o número de plantas adultas obtidas revelou-se independente do número de sementes semeadas, o que indica que a capacidade de suporte do ambiente, ao invés do tamanho do banco de sementes no solo, é a razão da regulação do tamanho populacional; a mesma observação aplica-se à produção de matéria seca. Sob condições de competição, as espécies A. hybridus - tipo verde e A. viridis produziram maior quantidade de biomassa para reprodução (22% a 34%) do que A. hybridus - tipo roxo (15% a 18%). As primeiras, portanto, revelaram-se mais estrategistas-r que a última.

PURPOSE: Thirty to forty percent of patients with recurrent gastrointestinal perforation/anastomotic leakage or acute necrotizing pancreatitis develop intra-abdominal invasive candidiasis (IC). A corrected Candida colonization index (CCI) > or =0.4 is a powerful predictor of IC. Fluconazole prevents intra-abdominal IC in this setting, but azole-resistant Candida species are emerging. The aim of this study was to explore the efficacy and safety of caspofungin for prevention of intra-abdominal IC in high-risk surgical patients. METHODS: Prospective non-comparative single-center study in consecutive adult surgical patients with recurrent gastrointestinal perforation/anastomotic leakage or acute necrotizing pancreatitis. Preventive caspofungin therapy (70 mg, then 50 mg/day) was given until resolution of the surgical condition. Candida colonization index and CCI, occurrence of intra-abdominal IC and adverse events were monitored. RESULTS: Nineteen patients were studied: 16 (84%) had recurrent gastrointestinal perforation/anastomotic leakage and 3 (16%) acute necrotizing pancreatitis. The median duration of preventive caspofungin therapy was 16 days (range 4-46). The colonization index decreased significantly during study therapy, and the CCI remained <0.4 in all patients. Caspofungin was successful for prevention of intra-abdominal IC in 18/19 patients (95%, 1 breakthrough IC 5 days after inclusion). No drug-related adverse event requiring caspofungin discontinuation occurred. CONCLUSION: Caspofungin may be efficacious and safe for prevention of intra-abdominal candidiasis in high-risk surgical patients. This needs to be further investigated in randomized trials.

Living with infertility and its emotional impact in couples has been frequently investigated. The objective of the present study was to establish in couples that initiate an IVF treatment:(1) their coping profile, anxiety level (State-anxiety; Trait-anxiety) and depression symptomatology;(2) the explanatory power of coping strategies over anxiety and depression levels, depending on gender. Method: we used a transversal study with 92 couples that begun an IVF treatment in the Assisted Reproduction Unit of the Hospital Clinic. All of them respond to the Spanish adaptation of the CRI-A, the STAI and the BDI-I. Results: the IVF population in comparison to the normal sample use less coping strategies and presents lower levels of anxiety and depression. Women IVF in comparison with man employ further avoidance strategies and present a higher score in depression and state-anxiety. Even when some exceptions exist, anxiety and depression levels are not well explained by coping strategies. Conclusions: It has been shown that the use of coping strategies on the IVF population are lower than in normative sample. Most of the couples present low anxiety (state / trait) and depression levels, and just a low percentage present scores that can require clinical attention. Preoperative imaging for resection of chest wall malignancies is generally performed by computed tomography (CT). We evaluated the role of (18)F-fluorodeoxyglucose (FDG) positron emission tomography (PET) in planning full-thickness chest wall resections for malignancies. We retrospectively included 18 consecutive patients operated from 2004 to 2006 at our institution. Tumor extent was measured by CT and PET, using the two largest perpendicular tumor extensions in the chest wall plane to compute the tumor surface assuming an elliptical shape. Imaging measurements were compared to histopathology assessment of tumor borders. CT assessment consistently overestimated the tumor size as compared to PET (+64% vs. +1%, P<0.001). Moreover, PET was significantly better than CT at defining the size of lesions >24 cm(2) corresponding to a mean diameter >5.5 cm or an ellipse of >4 cm x 7.6 cm (positive predictive value 80% vs. 44% and specificity 93% vs. 64%, respectively). Metabolic PET imaging was superior to CT for defining the extent of chest wall tumors, particularly for tumors with a diameter >5.5 cm. PET can complement CT in planning full-thickness chest wall resection for malignancies, but its true value remains to be determined in larger, prospective studies.

Cuando se realiza una encuesta social en un amplio territorio queda siempre el deseo de aplicar análisis similares a los realizados en la encuesta a poblaciones o territorios más reducidos, evidentemente utilizando los propios datos de la encuesta. El objetivo de este articulo consiste en mostrar cómo cada estrato de una muestra estratificada puede constituir una base muestral para llevar a cabo dichos análisis con todas las garantías de precisión o, al menos, con garantías calculables y aceptables sin aumentar el número muestral para la encuesta general.

PURPOSE: (18)F-Fluorocholine (FCH) and (11)C-acetate (ACE) PET are widely used for detection of recurrent prostate cancer (PC). We present the first results of a comparative, prospective PET/CT study of both tracers evaluated in the same patients presenting with recurrence and low PSA to compare the diagnostic information provided by the two tracers. METHODS: The study group comprised 23 patients studied for a rising PSA level after radical prostatectomy (RP, 7 patients, PSA ≤ 3 ng/ml), curative radiotherapy (RT, 7 patients, PSA ≤ 5 ng/ml) or RP and salvage RT (9 patients, PSA ≤ 5 ng/ml). Both FCH and ACE PET/CT scans were performed in a random sequence a median of 4 days (range 0 to 11 days) apart. FCH PET/CT was started at injection (307 ± 16 MBq) with a 10-min dynamic acquisition of the prostate bed, followed by a whole-body PET scan and late (45 min) imaging of the pelvis. ACE PET/CT was performed as a double whole-body PET scan starting 5 and 22 min after injection (994 ± 72 MBq), and a late view (45 min) of the prostate bed. PET/CT scans were blindly reviewed by two independent pairs of two experienced nuclear medicine physicians, discordant subgroup results being discussed to reach a consensus for positive, negative end equivocal results. RESULTS: PET results were concordant in 88 out of 92 local, regional and distant findings (Cohen's kappa 0.929). In particular, results were concordant in all patients concerning local status, bone metastases and distant findings. Lymph-node results were concordant in 19 patients and different in 4 patients. On a per-patient basis results were concordant in 22 of 23 patients (14 positive, 5 negative and 3 equivocal). In only one patient was ACE PET/CT positive for nodal metastases while FCH PET/CT was overall negative; interestingly, the ACE-positive and FCH-negative lymph nodes became positive in a second FCH PET/CT scan performed a few months later. CONCLUSION: Overall, ACE and FCH PET/CT showed excellent concordance, on both a per-lesion and a per-patient basis, suggesting that both tracers perform equally for recurrent prostate cancer staging.

Para demostrar que los parámetros periodontales no se alteran por la fenestración con colgajo de reposición apical de los caninos incluidos, se seleccionaron 12 pacientes con canino incluido por vestibular y el canino contralateral correctamente erupcionado. El tratamiento fue ortodóncico-quirúrgico, con colgajo de reposición apical (9 casos) o de reposición apical lateral (3 casos). Tres años después de finalizar el tratamiento ortodóncico, se valoró el estado periodontal de los caninos mediante: índice de plata, índice gingival, sangrado al sondaje, profundidad de sondaje y anchura de encía queratinizada. No se hallaron diferencias estadísticamente significativas entre el grupo experimental (caninos fenestrados) y el control (caninos contralaterales).

El embarazo conlleva una serie de cambios orgánicos, fisiológicos (del sistema cardiovascular y gastrointestinal, de la función pulmonar, hematológicos...) y de conducta que pueden repercutir en la cavidad bucal. El granuloma gravídico se considera una entidad patológica propia del embarazo. Podremos observar también en las pacientes gestantes un aumento en la incidencia de caries, asociado fundamentalmente a un incremento de Jos factores locales cariogénicos (descuido de la higiene bucal, cambio de Jos hábitos dietéticos y horarios...), y un posible empeoramiento de la patología gingival y periodontal asociado a las variaciones hormonales que acompañan al embarazo y a factores locales irritativos. El segundo trimestre de gestación será el momento ideal para llevar a cabo cualquier tratamiento dental rutinario. Deberemos ir con sumo cuidado en lo que se refiere a la administración de fármacos (penicilina, eritromicina, cefalosporinas de primera generación, paracetamol y lidocaína con vasoconstrictor se consideran fármacos de prescripción segura durante el embarazo) y la realización de radiografías dentales (es imprescindible colocar un delantal plomado a la paciente).

La xerostomía o disminución de la cantidad de saliva en la cavidad bucal suele tener una serie de consecuencias: unas directas, tales como el aumento en la incidencia de caries y enfermedad periodontal, y otras indirectas, como la dificultad en la masticación o la mala adaptación de las prótesis. De ahí la importancia de una correcta lubrificación salival de la cavidad bucal. En el presente trabajo se realiza una revisión de los problemas ocasionados por la xerostomía, así como de su diagnóstico y tratamiento, aspectos éstos un tanto conflictivos, debido a la dificultad de objetivar la sintomatología y de conseguir resultados duraderos.
