Die exzitatorische Neurotransmission erfolgt über ionotrope Glutamat-Rezeptoren von denen dem NMDA-(N-Methyl-D-aspartat)-Rezeptor durch seine hohe Leitfähigkeit für Ca2+-Ionen eine besondere Rolle zugesprochen wird. Bei seiner Überaktivierung kommt es zu exzitotoxischen Prozessen, die direkt mit neurodegenerativen Erkrankungen einhergehen und nach einem Schlaganfall, bei akuten Epilepsien, Morbus Parkinson, Alzheimer Demenz aber auch im Bereich der neuropathischen Schmerzentstehung eine wichtige Rolle spielen.rnDurch das Eingreifen in die glutamatvermittelten pathologischen Prozesse verspricht man sich daher die Möglichkeit einer Neuroprotektion bei der Therapie verschiedener neurodegenerativer Erkrankungen, die primär auf völlig unterschiedliche Ursachen zurückzuführen sind.rnAusgehend von in früheren Arbeiten synthetisierten Hydantoin-substituierten Dichlor-indol-2-carbonsäure-Derivaten, die hochaffine Eigenschaften zur Glycin-Bindungsstelle des NMDA-Rezeptors aufweisen, sollten neue Derivate entwickelt und untersucht werden, die hinsichtlich ihrer Affinität zur Glycin-Bindungsstelle des NMDA-Rezeptors, ihrer Pharmakokinetik sowie physikochemischen Parameter in präparativ-organischen, radiopharmazeutischen und zell- bzw. tierexperimentellen Studien in vitro sowie in vivo charakterisiert werden sollten. Von besonderem Interesse war dabei die Evaluierung der synthetisierten Verbindungen in einem Verdrängungsassay mit dem Radioliganden [3H]MDL105,519 mit dem der Einfluss der strukturellen Modifikationen auf die Affinität zur Glycin-Bindungsstelle des Rezeptors untersucht wurde, sowie die Selektivität und die Potenz der Liganden abgeschätzt wurde.rnIm Rahmen der Struktur-Wirkungs-Untersuchungen mit Hilfe der Bindungsexperimente konnten bestimmte Strukturmerkmale als essentiell herausgestellt bzw. bekräftigt werden. Die Testverbindungen zeigten dabei IC50-Werte im Bereich von 0,0028 bis 51,8 μM. Die entsprechenden Ester dagegen IC50-Werte von 23,04 bis >3000 μM. Als vielversprechende Strukturen mit Affinitäten im niedrigen nanomolaren Bereich stellten sich Derivate mit einer 4,6-Dichlor-oder Difluor-Substitution am Indolgrundgerüst (2,8 bis 4,6 nM) heraus. Auch die Substitution des Phenylhydantoin-Teils durch das bioisostere Thienylhydantoin führte zu einer gleichbleibenden ausgeprägten Affinität (3,1 nM). rnZur Abschätzung der Bioverfügbarkeit, insbesondere der Fähigkeit zur Überwindung der Blut-Hirn-Schranke, wurden die Lipophilien bei einer Auswahl der Testverbindungen durch Bestimmung ihrer log P-Werte ermittelt. Neben dem Verfahren der potentiometrischen Titration wurde eine HPLC-Methode an einer RP-Phase verwendet.rnUm das Zytotoxizitätsprofil der synthetisierten Strukturen frühzeitig abschätzen zu können, wurde ein schnell durchführbares, zellbasiertes in vitro-Testsystem, der kommerziell erhältliche „Cell Proliferation Kit II (XTT-Test)“, eingesetzt. rnIm Rahmen von Positronen-Emissions-Tomographie-Experimenten an Ratten wurde eine Aussage bezüglich der Aufnahme und Verteilung eines radioaktiv markierten, hochaffinen Liganden an der Glycinbindungsstelle des NMDA-Rezeptors im Gehirn getroffen. Dabei wurden sowohl ein Carbonsäure-Derivat sowie der korrespondierende Ethylester dieser Testung unterworfen.rn

Die Pharmazeutische Betreuung erfolgt mit dem Ziel der Identifikation, Vermeidung und Lösung von Arzneimittelbezogenen Problemen (AbP). In einer prospektiven, randomisierten Studie sollte geprüft werden, ob durch klinisch-pharmazeutische Betreuung bei multimorbiden Patienten mit Polymedikation die Dauer eines AbP verkürzt und ob durch einen individuellen, bebilderten Medikationsplan das Patientenwissen zur aktuellen stationären Medikation verbessert werden kann. In die Studie wurden 244 herzchirurgische Patienten eingeschlossen (≥65 Jahre, ≥5 Arzneimittel). Die Patienten wurden in vier Gruppen randomisiert. Es gab keine statistischen Unterschiede bezüglich der Patientenmerkmale Alter, Geschlecht, Aufnahmediagnose, Anzahl an Arzneimitteln bei der Aufnahme oder stationäre Verweildauer. Das Patientenwissen zur stationären Medikation wurde mittels Fragebogen bei 188 Patienten ermittelt (Rücklaufquote 78%). Zur objektiven Evaluation des Patientenwissens wurden die Patientenangaben zu Einnahmegründen, Arzneimittelbezeichnung, Stärke und Dosierung und zur Therapiedauer gerinnungshemmender Arzneimittel mit der tatsächlichen Medikation verglichen. Insgesamt wurden 1335 AbP bei 235 Patienten detektiert. Durch klinisch-pharmazeutische Betreuung konnten mehr AbP erkannt, frühzeitig gelöst und deren Dauer im Median von 4 auf 2 Tage signifikant (p-Wert<0,001) reduziert werden. Ein bebilderter Medikationsplan verbessert das Patientenwissen über die stationäre Medikation subjektiv und objektiv und erhöht die Motivation der Patienten, die Arzneimittel entsprechend den Einnahmehinweisen auf dem Medikationsplan einzunehmen.

This dissertation explores how diseases contributed to shape historical institutions and how health and diseases are still affecting modern comparative development. The overarching goal of this investigation is to identify the channels linking geographic suitability to diseases and the emergence of historical and modern insitutions, while tackling the endogenenity problems that traditionally undermine this literature. I attempt to do so by taking advantage of the vast amount of newly available historical data and of the richness of data accessible through the geographic information system (GIS). The first chapter of my thesis, 'Side Effects of Immunities: The African Slave Trade', proposes and test a novel explanation for the origins of slavery in the tropical regions of the Americas. I argue that Africans were especially attractive for employment in tropical areas because they were immune to many of the diseases that were ravaging those regions. In particular, Africans' resistance to malaria increased the profitability of slaves coming from the most malarial parts of Africa. In the second chapter of my thesis, 'Caste Systems and Technology in Pre-Modern Societies', I advance and test the hypothesis that caste systems, generally viewed as a hindrance to social mobility and development, had been comparatively advantageous at an early stage of economic development. In the third chapter, 'Malaria as Determinant of Modern Ethnolinguistic Diversity', I conjecture that in highly malarious areas the necessity to adapt and develop immunities specific to the local disease environment historically reduced mobility and increased isolation, thus leading to the formation of a higher number of different ethnolinguistic groups. In the final chapter, 'Malaria Risk and Civil Violence: A Disaggregated Analysis for Africa', I explore the relationship between malaria and violent conflicts. Using georeferenced data for Africa, the article shows that violent events are more frequent in areas where malaria risk is higher.

Gut microbial acquisition during the early stage of life is an extremely important event since it affects the health status of the host. In this contest the healthy properties of the genus Bifidobacterium have a central function in newborns. The aim of this thesis was to explore the dynamics of the gut microbial colonization in newborns and to suggest possible strategies to maintain or restore a correct balance of gut bacterial population in infants. The first step of this work was to review the most recent studies on the use of probiotics and prebiotics in infants. Secondly, in order to prevent or treat intestinal disorders that may affect newborns, the capability of selected Bifidobacterium strains to reduce the amount of Enterobacteriaceae and against the infant pathogen Streptococcus agalactiae was evaluated in vitro. Furthermore, the ability of several commercial fibers to stimulate selectively the growth of bifidobacterial strains was checked. Finally, the gut microbial composition in the early stage of life in response to the intrapartum antibiotic prophylaxis (IAP) against group B Streptococcus was studied using q-PCR, DGGE and next generation sequencing. The results globally showed that Bifidobacterium breve B632 strain is the best candidate for the use in a synbiotic product coupled to a mixture of two selected prebiotic fibers (galactooligosaccharides and fructooligosaccharides) for gastrointestinal disorders in infants. Moreover, the early gut microbial composition was affected by IAP treatment with infants showing lower counts of Bifidobacterium spp. and Bacteroides spp. coupled to a decrement of biodiversity of bacteria, compared to control infants. These studies have shown that IAP could affect the early intestinal balance in infants and they have paved the way to the definition of new strategies alternative to antibiotic treatment to control GBS infection in pregnant women.

Acute myeloid leukemia (AML) is a very aggressive cancer of the hematopoietic system. Chemotherapy and immunotherapeutical approaches including hematopoietic stem cell transplantation (HSCT) and donor lymphocyte infusion (DLI) are the only curative options available. The beneficial graft-versus-leukemia (GVL) effect of cellular immunotherapy is mostly mediated by donor-derived CD8+ T lymphocytes that recognize minor histocompatibility antigens (mHags) and leukemia-associated antigens (LAAs) presented on the surface of AML blasts (Falkenburg et al. 2008; Kolb 2008). A main complication is graft-versus-host disease (GVHD) that can be induced when cytotoxic T lymphocytes (CTLs) recognize broadly expressed antigens. To reduce the risk of GVHD, specific allogeneic T-cell therapy inducing selective GVL responses could be an option (Barrett & Le Blanc 2010; Parmar et al. 2011; Smits et al. 2011). This requires efficient in vitro strategies to generate AML-reactive T cells with an early differentiation phenotype as well as vigorous effector functions and humanized mouse models to analyze the anti-leukemic potential of adoptively transferred T cells in vivo. In this study, AML-reactive CTL clones and oligoclonal T-cell lines could be reliably generated from the naive subset of healthy HLA-class I-identical donors by stimulation with primary AML blasts in mini-mixed-lymphocyte / leukemia cultures (MLLCs) in eight different patient / donor pairs. These CTLs were promising candidates for cellular immunotherapy because of their relatively early differentiation phenotype and strong proliferative and lytic capabilities. The addition of the common γ-chain cytokine IL-21 to the stimulation protocol enabled more precursors to develop into potent leukemia-reactive CTLs, presumably by its beneficial effects on cell survival and antigen-specific proliferation during the first weeks of cultures. It also strengthened the early-stage phenotype. Three long-term cultured CTLs exemplarily transferred into leukemia-engrafted immunodeficient NSG mice mediated a significant reduction of the leukemic burden after a single transfusion. These results demonstrate that CTL clones with reactivity to patient-derived AML blasts can be isolated from the naive compartment of healthy donors and show potent anti-leukemic effects in vivo. The herein described allo-MLLC approach with in vitro “programmed” naive CTL precursors independent of a HSCT setting is a valuable alternative to the conventional method of isolating in vivo primed donor CTLs out of patients after transplantation (Kloosterboer et al. 2004; Warren et al. 2010). This would make leukemia-reactive CTLs already available at the time point of HSCT, when residual leukemia disease is minimal and the chances for complete leukemia eradication are high. Furthermore, leukemia-reactive CTLs effectively expanded by this in vitro protocol can be used as screening populations to identify novel candidate LAAs and mHags for antigen-specific immunotherapy.

Introduzione e scopo: la rapida diffusione delle malattie dismetaboliche sta modificando l’epidemiologia dell’epatocarcinoma (HCC). Scopo della tesi è, attraverso quattro studi, analizzare l’impatto di questi cambiamenti nella gestione clinica del paziente affetto da HCC. Materiali e metodi: quattro studi di coorte, condotti con analisi retrospettiva del database ITA.LI.CA. Studio 1:3658 pazienti arruolati tra il 01-01-2001 ed il 31-12-2012 suddivisi in base alla data di diagnosi:2001-2004 (954 pazienti), 2005-2008 (1122 pazienti), 2009-2012 (1582 pazienti). Studio 2:analisi comparativa tra 756 pazienti con HCC-NAFLD e 611 pazienti con HCC-HCV. Studio 3:proposta di quattro modelli alternativi al BCLC originale con validazione di una proposta di sottostadiazione dell’intermedio, considerando 2606 pazienti arruolati tra il 01-01-2000 e il 31-12-2012 e riallocati secondo gradi diversi di perfomance status (PS). Studio 4:analisi di 696 pazienti con HCC in stadio intermedio diagnosticato dopo il 1999 stratificati per trattamento. Risultati: studio 1:progressivo aumento dell’età alla diagnosi e delle eziologie dismetaboliche; più frequente esordio dell’HCC in stadio precoce e con funzione epatica più conservata; aumento della sopravvivenza dopo il 2008. Studio 2:i pazienti con HCC-NAFLD mostrano più frequentemente un tumore infiltrativo diagnosticato fuori dai programmi di sorveglianza, con prognosi peggiore rispetto ai pazienti HCC-HCV. Questa differenza di sopravvivenza si elimina rimuovendo i fattori di confondimento attraverso propensity analysis. Studio 3:il PS1 non è un predittore indipendente di sopravvivenza. Il modello 4 (considerando PS1=PS0 e con la sottostadiazione proposta), ha la migliore capacità discriminativa. Studio 4:i trattamenti curativi riducono la mortalità più della TACE, anche dopo propensity analysis. Conclusioni: l’aumento delle patologie dismetaboliche comporterà diagnosi di malattia ad uno stadio più avanzato, quando sintomatica, rendendo necessario stabilire un programma di sorveglianza. Inoltre per una migliore stratificazione e gestione dei pazienti, bisogna riconsiderare il ruolo del PS ed offrire un ventaglio di opzioni terapeutiche anche per il pazienti in stadio intermedio.

Nel tumore combinato epatocolangiocellulare (CHC) le componenti epatocitarie e colangiocitarie sono entrambe presenti. Obiettivo: valutare gli aspetti diagnostici radiologici e caratteristiche clinico-demografiche del CHC su cirrosi. Raccolti pazienti con CHC su cirrosi afferenti a due centri del Nord Italia (Bologna, S. Orsola-Malpighi e Milano,IRCCS Ca’ Granda Maggiore Hospital) tra 2003-2013, con diagnosi istologica di CHC. FASE 1:confronto tra ecografia con mdc (CEUS), TC cmdc e RM cmdc nella diagnosi e caratterizzazione dei noduli di CHC. Casistica di 35 pazienti e 37 noduli (due recidive CHC incluse). Mediana delle dimensioni: 25 mm. Non si è identificato un pattern contrastografico patognomonico per CHC. Pattern di enhancement arterioso ad anello periferico, suggestivo per forma colangiocitaria, atipico per HCC, presente nel 26%,50%,29% dei noduli a CEUS,TC,RM. La CEUS avrebbe portato a una errata diagnosi di HCC tipico in un numero maggiore di casi (48%) vs TC(15%,p=0.005), e RM(18%,p=0.080).L’indicazione della malignità del nodulo (presenza di wash-out dopo enhancement arterioso), era fornita con maggiore accuratezza da parte della CEUS(78%), vs TC (24%,p<0.0001) e RM(29%,p=0.002). FASE 2:analisi degli aspetti clinico-laboratoristici e prognostici del CHC e confronto tramite match 1:2 con HCC su cirrosi (36 CHC,72 HCC). Nel CHC correlano positivamente con sopravvivenza le terapie “curative” (trapianto, resezione chirurgica, terapie ablative percutanee a radiofrequenza/ alcolizzazione), stadio precoce alla diagnosi, dimensioni e essere in sorveglianza per diagnosi precoce di HCC. Correlano indipendentemente con sopravvivenza stadio precoce di malattia (unifocale, ≤ 2 cm) e essere in programma di sorveglianza(multivariata). Sopravvivenze del CHC sovrapponibili al gruppo HCC a 1 anno, e lievemente inferiori a 3/5 anni (81%, 39%, 21% vs 83%, 59% e 40%,p=0.78,p=0.080 e p=0.14). Sopravvivenza mediana per CHC (2.36 anni) inferiore vs HCC (4.09 anni) pur senza significatività statistica.

Therapeutic RNAs, especially siRNAs, are a promising approach for treating diseases like cancer, neurodegenerative disorders and viral infections. Their application, however, is limited due to a lack of safe and efficient delivery systems. Nanosized carriers with the ability to either complex or entrap RNA species are a promising option. rn rn rnSuch a carrier has to meet a lot of requirements, some of which are even partly contradictive. Understanding and controlling the interplay between the different demands would advance a strategic design at an early stage of therapeutic development. rn rn This work is centered around a systematic evaluation of polyplexes, such carriers that are able to complex siRNA due to electrostatic interactions. Six structurally and chemically diverse candidates, poly-L-lysine brushes, block copolymers, cationic peptides, cationic lipids, nanohydrogels, and manganese oxide particles, were tested in a simultaneous fashion. The assays, mostly based on fluorescently labeled siRNA, ranged from the evaluation of polyplex formation and stability to in vitro parameters like cellular uptake and knockdown capability. The analysis from several perspectives offered insight into the interplay between the specifications of one polyplex. Assessing the different carriers under exactly the same experimental conditions also allowed conclusions about favourable traits and starting points for further optimization. This comparative approach also revealed weaknesses of some of the conventional protocols, which were therefore contrasted with alternative methods. In addition, in vitro knockdown assays were optimized and the impact of fluorescently labeled siRNA on knockdown efficiency was assessed. rn rn rn A second class of carriers, which share the ability to entrap siRNA inside their matrix, are briefly addressed. Nanocapsules, dextran particles and liposomes were assessed for basic features like siRNA encapsulation and knockdown capability. rn rn rn rn In an approach towards targeted delivery of RNA, liposomes were endowed with mitochondriotropic tags. Despite successful functionalization, no colocalization between the liposomal cargo and mitochondria was so far observed, which makes further optimization necessary.

Der Fokus dieser Doktorarbeit liegt auf der kontrollierten Benetzung von festen Oberflächen, die in vielen Bereichen, wie zum Beispiel in der Mikrofluidik, für Beschichtungen und in biologischen Studien von Zellen oder Bakterien, von großer Bedeutung ist.rnDer erste Teil dieser Arbeit widmet sich der Frage, wie Nanorauigkeit das Benetzungsverhalten, d.h. die Kontaktwinkel und die Pinningstärke, von hydrophoben und superhydrophoben Beschichtungen beeinflusst. Hierfür wird eine neue Methode entwickelt, um eine nanoraue Silika-Beschichtung über die Gasphase auf eine superhydrophobe Oberfläche, die aus rauen Polystyrol-Silika-Kern-Schale-Partikeln besteht, aufzubringen. Es wird gezeigt, dass die Topographie und Dichte der Nanorauigkeiten bestimmt, ob sich die Superhydrophobizität verringert oder erhöht, d.h. ob sich ein Flüssigkeitstropfen im Nano-Wenzel- oder Nano-Cassie-Zustand befindet. Das verstärkte Pinning im Nano-Wenzel-Zustand beruht auf dem Eindringen von Flüssigkeitsmolekülen in die Nanoporen der Beschichtung. Im Nano-Cassie-Zustand dagegen sitzt der Tropfen auf den Nanorauigkeiten, was das Pinning vermindert. Die experimentellen Ergebnisse werden mit molekulardynamischen Simulationen in Bezug gesetzt, die den Einfluss der Oberflächenbeschichtungsdichte und der Länge von fluorinierten Silanen auf die Hydrophobizität einer Oberfläche untersuchen. rnEs wurden bereits verschiedenste Techniken zur Herstellung von transparenten superhydrophoben, d.h. extrem flüssigkeitsabweisenden, Oberflächen entwickelt. Eine aktuelle Herausforderung liegt darin, Funktionalitäten einzuführen, ohne die superhydrophoben Eigenschaften einer Oberfläche zu verändern. Dies ist extrem anspruchsvoll, da funktionelle Gruppen in der Regel hydrophil sind. In dieser Arbeit wird eine innovative Methode zur Herstellung von transparenten superhydrophoben Oberflächen aus Janus-Mikrosäulen mit variierenden Dimensionen und Topographien entwickelt. Die Janus-Säulen haben hydrophobe Seitenwände und hydrophile Silika-Oberseiten, die anschließend selektiv und ohne Verlust der superhydrophoben Eigenschaften der Oberfläche funktionalisiert werden können. Diese selektive Oberflächenfunktionalisierung wird mittels konfokaler Mikroskopie und durch das chemische Anbinden von fluoreszenten Molekülen an die Säulenoberseiten sichtbar gemacht. Außerdem wird gezeigt, dass das Benetzungsverhalten durch Wechselwirkungen zwischen Flüssigkeit und Festkörper in der Nähe der Benetzungslinie bestimmt wird. Diese Beobachtung widerlegt das allgemein akzeptierte Modell von Cassie und Baxter und beinhaltet, dass hydrophile Flächen, die durch mechanischen Abrieb freigelegt werden, nicht zu einem Verlust der Superhydrophobizität führen müssen, wie allgemein angenommen.rnBenetzung kann auch durch eine räumliche Beschränkung von Flüssigkeiten kontrolliert werden, z.B. in mikrofluidischen Systemen. Hier wird eine modifizierte Stöber-Synthese verwendet, um künstliche und natürliche Faser-Template mit einer Silika-Schicht zu ummanteln. Nach der thermischen Zersetzung des organischen Templat-Materials entstehen wohldefinierte Silika-Kanäle und Kanalkreuzungen mit gleichmäßigen Durchmessern im Nano- und Mikrometerbereich. Auf Grund ihrer Transparenz, mechanischen Stabilität und des großen Länge-zu-Durchmesser-Verhältnisses sind die Kanäle sehr gut geeignet, um die Füllgeschwindigkeiten von Flüssigkeiten mit variierenden Oberflächenspannungen und Viskositäten zu untersuchen. Konfokale Mikroskopie ermöglicht es hierbei, die Füllgeschwindigkeiten über eine Länge von mehreren Millimetern, sowie direkt am Kanaleingang zu messen. Das späte Füllstadium kann sehr gut mit der Lucas-Washburn-Gleichung beschrieben werden. Die anfänglichen Füllgeschwindigkeiten sind jedoch niedriger als theoretisch vorhergesagt. Wohingegen die vorhergehenden Abschnitte dieser Arbeit sich mit der quasistatischen Benetzung beschäftigen, spielt hier die Dynamik der Benetzung eine wichtige Rolle. Tatsächlich lassen sich die beobachteten Abweichungen durch einen geschwindigkeitsabhängigen Fortschreitkontaktwinkel erklären und durch dynamische Benetzungstheorien modellieren. Somit löst diese Arbeit das seit langem diskutierte Problem der Abweichungen von der Lucas-Washburn-Gleichung bei kleinen Füllgeschwindigkeiten.

High altitude constitutes an exciting natural laboratory for medical research. Although initially, the aim of high-altitude research was to understand the adaption of the organism to hypoxia and find treatments for altitude-related diseases, during the past decade or so, the scope of this research has broadened considerably. Two important observations led the foundation for the broadening of the scientific scope of high-altitude research. First, high-altitude pulmonary edema represents a unique model that allows studying fundamental mechanisms of pulmonary hypertension and lung edema in humans. Second, the ambient hypoxia associated with high-altitude exposure facilitates the detection of pulmonary and systemic vascular dysfunction at an early stage. Here, we will review studies that, by capitalizing on these observations, have led to the description of novel mechanisms underpinning lung edema and pulmonary hypertension and to the first direct demonstration of fetal programming of vascular dysfunction in humans.

Daytime sleepiness is a complaint of about 5-10% in a normal population. The consequences, such as impaired performance and accidents at the workplace and while driving, have major impact on the affected and on society. According to Swiss federal statistics only 1-3% of all motor vehicle accidents are due to excessive daytime sleepiness, which is in great contrast to a figure of 10 to 20% of all accidents derived from scientific studies. Due to the inadequate statistical representation of the problem, insufficient countermeasures have been realized, and the state of drivers breaching traffic regulations is not adequately investigated in this respect. The most prevalent cause of microsleep induced accidents is certainly lack of sleep due to social or professional reasons. A treating physician must also consider sedating drugs and various diseases. The typical characteristics of accidents due to falling asleep at the wheel and the risk factors involved are well established, so that informing the general public, taking prophylactic countermeasures and a targeted investigation in this respect of drivers who have breached the law are all feasible. Since symptoms of sleepiness can be recognized well before any impairment of performance occurs, the most important countermeasure is information of the drivers on the risk factors and on efficient countermeasures against sleepiness at the wheel. Besides correct diagnosis and treatment, the primary goal of physicians treating patients with pathological daytime sleepiness is to inform them at an early stage about the risks of sleepiness and the large responsibility they bear while driving. This information should be written down in the patients' records. Professional drivers suffering from daytime sleepiness, drivers who have already had an accident due to microsleep and unreasonable drivers should be referred to a centre of sleep disorders for objective measurements of sleepiness.

BACKGROUND: Most people experience low back pain (LBP) at least once in their lifetime. Only a minority of them go on to develop persistent LBP. However, the socioeconomic costs of persistent LBP significantly exceed the costs of the initial acute LBP episode. AIMS: To identify factors that influence the progression of acute LBP to the persistent state at an early stage. METHODS: Prospective inception cohort study of patients attending a health practitioner for their first episode of acute LBP or recurrent LBP after a pain free period of at least 6 months. Patients were assessed at baseline addressing occupational and psychological factors as well as pain, disability, quality of life and physical activity and followed up at 3, 6, 12 weeks and 6 months. Variables were combined to the three indices 'working condition', 'depression and maladaptive cognitions' and 'pain and quality of life'. RESULTS: The index 'depression and maladaptive cognitions' was found to be a significant baseline predictor for persistent LBP up to 6 months (OR 5.1; 95% CI: 1.04-25.1). Overall predictive accuracy of the model was 81%. CONCLUSIONS: In this study of patients with acute LBP in a primary care setting psychological factors at baseline correlated with a progression to persistent LBP up to 6 months. The benefit of including factors such as 'depression and maladaptive cognition' in screening tools is that these factors can be addressed in primary and secondary prevention.

Bullous pemphigoid (BP), the most common autoimmune subepidermal bullous disease, is associated with an autoantibody response to BP180 and BP230, two components of junctional adhesion complexes in human skin promoting dermo-epidermal cohesion. Retrospective analyses demonstrated that these autoantigens harbor several epitopes targeted by autoaggressive B and T cells. The aim of this prospective multicenter study was to assess the evolution of IgG autoantibodies in 35 BP patients over a 12-month observation period. Epitope-spreading (ES) events were detected in 17 of 35 BP patients (49%). They preferentially occurred in an early stage of the disease and were significantly related to disease severity at diagnosis. Moreover, in three patients, spreading of IgG reactivity to intracellular epitopes of BP180 and BP230 was preceded by recognition of the BP180 ectodomain. Finally, IgG reactivity with extracellular epitopes of BP180 and intracellular epitopes of BP230 correlated with the severity of BP in disease course. These findings support the idea that IgG recognition of the BP180 ectodomain is an early and crucial event in BP disease, followed by variable intra- and intermolecular ES events, which likely shape the individual course of BP.
