The saphenous nerve (SaN) innervates the region from the upper medial thigh to the medial aspect of the foot and ankle. A femoral nerve block (FNB) is effective for blockade of the SaN but this causes quadriceps weekness and reduced patient mobility that is unsuitable in an ambulatory surgical setting.

This report summarizes the data, observations, methods, assumptions, and decisions for the design of the Relief Well Rehabilitation Project in the Right Abutment Drainage Tunnel at Chief Joseph Dam. Chief Joseph Dam (CJD) is a dam on the Columbia River and is owned and operated by the U.S. Army Corps of Engineers (USACE). It is the second only to Grand Coulee dam as the largest producer of hydropower in the United States. The right abutment drainage tunnel contains wooden stave relief wells. Water flows from these wells which reduces the hydrostatic pressure in the right abutment of the dam. The 22 wells in the floor of the tunnel are 60 years old and are in need of rehabilitation. The objective of this project is to control the groundwater gradient, prevent the movement of sediment, stop total screen collapse, and prevent initiation of backwards erosion and piping in the abutment. The rehabilitation solution is to install new stainless steel screens into the existing wells, backfill the annular space between the old wooden screen and the new stainless steel screens with a 3/8-inch pea gravel filter pack, and install a new top cap to hold the new screen in place. This report documents the data, observations, and methods used to complete the final design. During tunnel inspections USACE geologists observed dislodged end plugs and evidence of sediment movement out of the formation. The relief wells have historically high flows between 6,000 gallons per minute (gpm) to 9,000 gpm. New screens are designed based on as-built data and historic tunnel flow. The new screens are 8-in diameter, 100 slot (0.10-inch) screens. We found that screen diameter and slot size would provide adequate transmitting capacity for most of the relief wells. The filter pack gradation is based on descriptions from foundation construction reports. I found that 3/8-inch pea gravel is appropriate for the abutment material. During design, I also considered an option to install the screens into the relief wells without filter pack. I eliminated this option because it did not meet our rehabilitation objective to prevent total failure of the wooden screens.

and human capital externalities. Because of such externalities, education investment is too low and fertility is too high. While education subsidies are the conventional means to deal with these problems, we show that the optimal policy also comprises debt even when distortionary taxes are used. The reason is that debt tips the usual trade-off between children's quantity and quality in favor of the latter by increasing the bequest cost of children. There has been a growth of interest in the role of humour in organizations from both practitioner and academic perspectives. Various claims for the functionality of humour have been made, ranging from stress reduction to helping form and cement corporate cultures. Latching on to these presumed benefits, businesses and consultants have begun to employ humour and comedy in a direct and explicit manner. However, there is a counterpoint, which suggests that humour cannot always be managed and in fact has subversive qualities. This article addresses the issue of the subversive potential of comedy in organizational contexts. It draws illustratively on the case of a successful corporate comedian to do so. The article argues, through an analysis of the case, the history and philosophy of comedy, and theories of the comedic, that while comedy has inherent subversive potential, it most often is contained. Indeed, it suggests that comedy works by intruding as a potential threat to mundane reality, but offers comic relief when it is apparent that the threat will not be actualized and the status quo ante prevails. Objective: To ascertain the extent to which neonatal analgesia was used in Australia for minor invasive procedures as an indicator of evidence-based practice in neonatology. Methods: A cross-sectional telephone survey of hospitals in all Australian states and territories with more than 200 deliveries per year was carried out. Questions were asked regarding awareness of the benefits and the use of analgesia for minor invasive procedures in term and near term neonates. Analysis was undertaken according to state and territory, annual birth numbers and the level of neonatal nursery care available. Results: Data were available from 212 of 214 eligible hospitals. Of the total respondents, 51% and 70% respectively were aware of the benefits of sucrose and breast-feeding for neonatal analgesia. Eleven per cent of units administered sucrose before venepuncture and 25% of units used breast-feeding. Ten per cent of units used sucrose before heel prick with 49% utilizing breast-feeding. Expressed breast milk was used in 10% of units. Analgesia was given less frequently before intravenous cannulation compared to venepuncture and heel prick. Awareness and implementation of neonatal analgesia varied widely in the states and territories. There was a trend for hospitals providing a higher level of neonatal care to have a greater awareness of sucrose as an analgesic (P < 0.0001) and the use of sucrose for venepuncture (P = 0.029), heel prick (P = 0.025) and intravenous catheter insertion (P = 0.013). Similar trends were found on analysis according to birth number of the maternity units. Smaller units had a greater usage of breast-feeding as an analgesic for heel prick (P = 0.017). Conclusion: Despite good evidence for the administration of sucrose and breast milk in providing effective analgesia for newborn infants, it is not widely used in Australia. It is imperative that the gap between research findings and clinical practice with regard to neonatal analgesia be addressed.

This paper studies optinnal public debt in a dynastic model with human capital externalities that cause human capital investment (fertility) to be below (above) its socially optimal level. By reducing fertility and raising human capital investment, the optimal debt can exceed 10% of output for plausible parameterizations.
