Contexte. Les attitudes et comportements alimentaires développés pendant la petite enfance influencent grandement la relation future du mangeur à l’égard des aliments. Le parent s’avère en ce sens des plus déterminants. Objectifs. Cette étude vise à connaître les pratiques et attitudes employées par les parents québécois d’enfants d’âge préscolaire pour favoriser le « bien manger » ainsi que le repas familial agréable. Méthodes. La collecte de données a été effectuée à l’aide de cartes postales qui étaient distribuées dans six cent cinquante Centres de la Petite Enfance parmi les mille retrouvés dans la province de Québec pour ensuite être remises aux parents. L’analyse des réponses aux deux questions ouvertes figurant sur la carte postale a été effectuée en se basant sur le modèle des prises alimentaires de Jean-Pierre Poulain. Résultats. Plus de mille (1257) cartes postales ont été retenues aux fins d’analyses. Les données recueillies permettent dans un premier temps de connaître la perception des parents quant à la notion de manger bien. Les notions de qualité et de quantité, diamétralement opposées se démarquent. Dans le cas du repas familial, la discussion, la présence de tous les membres à table ainsi que l’absence de télévision sont abondamment mentionnées. Conclusion. Cette étude permet de constater la variété des stratégies employées par les parents québécois au moment du repas. Ces données d’une grande pertinence pour la santé publique permettront de cibler les messages clés à promouvoir auprès de ces acteurs d’influence dans le développement des habitudes alimentaires des tout petits.

Ce mémoire en recherche-création explore l’enfermement volontaire et les différents types de tensions qu’il provoque. Court roman prenant la forme du journal intime, La Parenthèse met en scène un jeune homme qui décide de s’enfermer chez lui une semaine durant et s’interdit tout contact avec l’extérieur – autant pour prendre un congé temporaire de la vie qu’il mène que pour examiner les raisons de sa détresse quotidienne. Le monologue intérieur se transforme rapidement en dialogue, dès lors qu’un double vindicatif, interrompant la voix principale par des « répliques » entre parenthèses, fait son apparition. Une relation houleuse – sous tension – se tisse entre ces deux facettes du personnage tout au long des sept jours de la réclusion, les passages de dispute alternant avec des récits de souvenirs. En somme, le roman tente de dramatiser la question de l’emprisonnement de soi-même et de la limitation de l’écriture, cette limitation pouvant être à la fois malsaine et libératrice. Quant à l’essai, Tensions et enfermement dans les Cent Vingt Journées de Sodome du marquis de Sade, il part du thème de l’enfermement (en l’occurrence, celui des quatre amis qui exécutent le projet de passer quatre mois dans un château isolé) pour postuler une « architecture du désir » dans Les Cent Vingt Journées de Sodome. L’essai mobilise les ressources de la narratologie en prenant en compte les effets du texte sur le lecteur ; sont ainsi mises en évidence les tensions – sexuelle pour les protagonistes, narrative pour le lecteur – élaborées par cette écriture de l’enfermement et de la contrainte, dans laquelle le désir est toujours maintenu mais rarement satisfait.

A complete census of planetary systems around a volume-limited sample of solar-type stars (FGK dwarfs) in the Solar neighborhood (d a parts per thousand currency signaEuro parts per thousand 15 pc) with uniform sensitivity down to Earth-mass planets within their Habitable Zones out to several AUs would be a major milestone in extrasolar planets astrophysics. This fundamental goal can be achieved with a mission concept such as NEAT-the Nearby Earth Astrometric Telescope. NEAT is designed to carry out space-borne extremely-high-precision astrometric measurements at the 0.05 mu as (1 sigma) accuracy level, sufficient to detect dynamical effects due to orbiting planets of mass even lower than Earth's around the nearest stars. Such a survey mission would provide the actual planetary masses and the full orbital geometry for all the components of the detected planetary systems down to the Earth-mass limit. The NEAT performance limits can be achieved by carrying out differential astrometry between the targets and a set of suitable reference stars in the field. The NEAT instrument design consists of an off-axis parabola single-mirror telescope (D = 1 m), a detector with a large field of view located 40 m away from the telescope and made of 8 small movable CCDs located around a fixed central CCD, and an interferometric calibration system monitoring dynamical Young's fringes originating from metrology fibers located at the primary mirror. The mission profile is driven by the fact that the two main modules of the payload, the telescope and the focal plane, must be located 40 m away leading to the choice of a formation flying option as the reference mission, and of a deployable boom option as an alternative choice. The proposed mission architecture relies on the use of two satellites, of about 700 kg each, operating at L2 for 5 years, flying in formation and offering a capability of more than 20,000 reconfigurations. The two satellites will be launched in a stacked configuration using a Soyuz ST launch vehicle. The NEAT primary science program will encompass an astrometric survey of our 200 closest F-, G- and K-type stellar neighbors, with an average of 50 visits each distributed over the nominal mission duration. The main survey operation will use approximately 70% of the mission lifetime. The remaining 30% of NEAT observing time might be allocated, for example, to improve the characterization of the architecture of selected planetary systems around nearby targets of specific interest (low-mass stars, young stars, etc.) discovered by Gaia, ground-based high-precision radial-velocity surveys, and other programs. With its exquisite, surgical astrometric precision, NEAT holds the promise to provide the first thorough census for Earth-mass planets around stars in the immediate vicinity of our Sun.

Centronuclear myopathy (CNM) is a genetically heterogeneous disorder associated with general skeletal muscle weakness, type I fiber predominance and atrophy, and abnormally centralized nuclei. Autosomal dominant CNM is due to mutations in the large GTPase dynamin 2 (DNM2), a mechanochemical enzyme regulating cytoskeleton and membrane trafficking in cells. To date, 40 families with CNM-related DNM2 mutations have been described, and here we report 60 additional families encompassing a broad genotypic and phenotypic spectrum. In total, 18 different mutations are reported in 100 families and our cohort harbors nine known and four new mutations, including the first splice-site mutation. Genotype-phenotype correlation hypotheses are drawn from the published and new data, and allow an efficient screening strategy for molecular diagnosis. In addition to CNM, dissimilar DNM2 mutations are associated with Charcot-Marie-Tooth (CMT) peripheral neuropathy (CMTD1B and CMT2M), suggesting a tissue-specific impact of the mutations. In this study, we discuss the possible clinical overlap of CNM and CMT, and the biological significance of the respective mutations based on the known functions of dynamin 2 and its protein structure. Background Repetitive behaviours (RB) in patients with Gilles de la Tourette syndrome (GTS) are frequent. However, a controversy persists whether they are manifestations of obssessive-compulsive disorder (OCD) or correspond to complex tics. Methods 166 consecutive patients with GTS aged 15–68 years were recruited and submitted to extensive neurological, psychiatric and psychological evaluations. RB were evaluated by the YBOCS symptom checklist and Mini International Neuropsychiatric Interview (M.I.N.I), and classified on the basis of a semi-directive psychiatric interview as compulsions or tics. Results RB were present in 64.4% of patients with GTS (107/166) and categorised into 3 major groups: a ‘tic-like’ group (24.3%–40/166) characterised by RB such as touching, counting, ‘just right’ and symmetry searching; an ‘OCD-like’ group (20.5%–34/166) with washing and checking rituals; and a ‘mixed’ group (13.2%–22/166) with both ‘tics-like’ and ‘OCD-like’ types of RB present in the same patient. In 6.3% of patients, RB could not be classified into any of these groups and were thus considered ‘undetermined’. Conclusions The results confirm the phenomenological heterogeneity of RB in GTS patients and allows to distinguish two types: tic-like behaviours which are very likely an integral part of GTS; and OCD-like behaviours, which can be considered as a comorbid condition of GTS and were correlated with higher score of complex tics, neuroleptic and SSRIs treatment frequency and less successful socio-professional adaptation. We suggest that a meticulous semiological analysis of RB in GTS patients will help to tailor treatment and allow to better classify patients for future pathophysiologic studies. Contrast-enhanced ultrasonography (CEUS) is a novel imaging technique that is safe and applicable on the bedside. Recent developments seem to enable CEUS to quantify organ perfusion. We performed an exploratory study to determine the ability of CEUS to detect changes in renal perfusion and to correlate them with effective renal plasma flow.

Diskutiert werden die Möglichkeiten architektonischen Einfügens anhand der Position, die der französische Architekt Jean Nouvel in Theorie wie Praxis einnimmt. Im Rückblick auf sein Werk zeigt sich dabei, dass er sich von Anbeginn mit der Reflexion über die Möglichkeiten und die Grenzen architektonischen Einfügens in bereits bestehende bauliche Ensembles auseinandergesetzt und eine Reihe von Konzepten entwickelt hat, deren Begrifflichkeit sich an Philosophen wie Félix Guattari, Gilles Deleuze und Michel Foucault orientiert. Die konkrete Anwendung dieser Begriffe wird anhand von zwei Projekten erörtert, die einmal die Einfügung eines kompletten Gebäudeinnern in bereits bestehende Außenwände eines historischen Baus (Oper Lyon), ein anderes Mal die Einfügung eines Neubaus in ein prägnantes, städtebauliches Ensemble (Hotel "Sofitel Vienna Stephansdom“) betreffen.

OBJECTIVES In resource-constrained settings, tuberculosis (TB) is a common opportunistic infection and cause of death in HIV-infected persons. TB may be present at the start of antiretroviral therapy (ART), but it is often under-diagnosed. We describe approaches to TB diagnosis and screening of TB in ART programs in low- and middle-income countries. METHODS AND FINDINGS We surveyed ART programs treating HIV-infected adults in sub-Saharan Africa, Asia and Latin America in 2012 using online questionnaires to collect program-level and patient-level data. Forty-seven sites from 26 countries participated. Patient-level data were collected on 987 adult TB patients from 40 sites (median age 34.7 years; 54% female). Sputum smear microscopy and chest radiograph were available in 47 (100%) sites, TB culture in 44 (94%), and Xpert MTB/RIF in 23 (49%). Xpert MTB/RIF was rarely available in Central Africa and South America. In sites with access to these diagnostics, microscopy was used in 745 (76%) patients diagnosed with TB, culture in 220 (24%), and chest X-ray in 688 (70%) patients. When free of charge culture was done in 27% of patients, compared to 21% when there was a fee (p = 0.033). Corresponding percentages for Xpert MTB/RIF were 26% and 15% of patients (p = 0.001). Screening practices for active disease before starting ART included symptom screening (46 sites, 98%), chest X-ray (38, 81%), sputum microscopy (37, 79%), culture (16, 34%), and Xpert MTB/RIF (5, 11%). CONCLUSIONS Mycobacterial culture was infrequently used despite its availability at most sites, while Xpert MTB/RIF was not generally available. Use of available diagnostics was higher when offered free of charge.
